Large Basilar Trunk Aneurysm Discovered Incidentally in a Patient with Ischaemic Stroke: Case Report

Summary: Many anomalies can be observed in the basilar trunk, such as aneurysms. Basilar trunk aneurysms are rare with a low reported prevalence. In this study, we describe a case of ischaemic stroke revealed by a basilar trunk aneurysm associated with venous thromboembolic disease. The patient was 60 years old and had no specific pathological history. She was seen for a sudden onset of right hemiplegia associated with dysarthria that had been present for 12 hours. The physical examination revealed a pyramidal syndrome and an altered general condition. An emergency cerebral computed tomography (CT) scan showed evidence of a cerebrovascular accident associated with a saccular aneurysm. We adopted the diagnosis of a deep left sylvian stroke associated with a superficial right sylvian stroke in the setting of an incidentally discovered basilar trunk aneurysm. On the 9ème day of hospitalisation, the patient experienced sudden onset of respiratory distress in conjunction with a warm, painful swelling of the right leg. TTE revealed signs of pulmonary embolism, with thrombi in the inferior vena cava and right atrium. Pulmonary embolism was highly probable, with a modified Geneva score of 15. We adopted the diagnosis of DVA complicated by pulmonary embolism in the setting of a basilar trunk aneurysm. The patient was transferred to the cardiology department, where she received oxygen therapy and thrombolysis with streptokinase. The immediate outcome 6 hours later was the sudden death of the patient due to the onset of cardiogenic shock.

Several studies have shown the importance of therapeutic education in the management of diabetes. Its implementation requires a prior assessment of the level of patients’ knowledge. The objective of this study was to assess the level of diabetics’ knowledge about their disease. Methods: This was a cross-sectional study conducted between 2013 and 2015;consenting diabetics, followed for at least one year in medicine department of Souro Sanou Hospital, have been surveyed. The data were collected using a 51-item questionnaire, adapted from “Audit of Diabetes Knowledge” and administered by an interviewer. The level of knowledge was satisfactory/good if at least 80% of the set items or a topic were known. Results: A total of 268 diabetics were surveyed. Their mean age was 54.3 ± 1.4 years and the sex ratio 0.8. The majority (80.9%) lived in urban areas and half (50.4%) were uneducated. Type 2 diabetics accounted for 93.3% of the population;the average seniority was 6.0 ± 0.7 years. The level of knowledge was generally insufficient, with an average total score of 25.4 ± 1.0/51. The topics which were not well mastered related to diet (0.8%), glycated haemoglobin (4.5%), and signs of hypoglycemia (7.8%). The main relatively well-known topics related to the effect of physical exercise on glycemia = 37.7%, the effect of food on glycemia = 36.9% and glycemia target = 32.8%. Conclusion: This study revealed a lack of knowledge from diabetic patients who are followed in the medicine department of the Teaching Hospital Souro SANON (CHUSS). The implementation of a therapeutic education program for diabetics is necessary and urgent.

Objective: To evaluate the quality of second trimester ultrasound examinations in patients consulting in the gynecology and obstetrics department at the University Teaching Hospital Yalgado Ouedraogo (UTH-YO) of Ouagadougou. Method: Our study took place from January 1st to March 31st 2017, in the department of Obstetrics and Gynecology of the University Teaching Hospital Yalgado Ouedraogo of Ouagadougou. This was a descriptive study on the records of the second trimester ultrasound of pregnancy, with analysis of the iconography. We used the criteria of the Technical Committee of Fetal Echography (TCFE) of France to analyse all the ultrasound reports brought by the patients. Results: Three hundred ultrasound reports were collected over three months. It revealed that, in terms of patient identification, 100% of the reports bore the surname and first name of the pregnant woman;88% and 94.67% of the ultrasound scans bore the surname and first name of the ultrasound specialist respectively. Only 2% presented the brand and type of ultrasound scanner. The study of the quality of the appointment showed that 52% of the ultrasounds had been performed at the right time, between 20 Gestational Age (GA) and 25 GA. For the quality of the iconography, referring to the recommendations of the Technical Committee of Fetal Echography (TCFE) of France and taking into account the data confirmed by the literature, we found that the quality was slightly acceptable either respectively for biometric and morphological sections. Conclusion: The quality of the ultrasound reports of the second trimester ultrasounds in patients consulting in the obstetrics and gynecology department of the University Hospital Yalgado OUEDRAOGO was not totally satisfactory.

Acute Toxicity and Antipyretic Activities of a Methanolic Extract of Alchornea cordifolia Leaves

Alchornea cordifolia (Euphorbiaceae) is a very prized plant among traditional healers in Africa. Its leaves are used for its antipyretic properties in traditional areas. The aim of our study is to determine the acute toxicity and the antipyretic activity of a methanolic extract of Alchornea cordifolia leaves. Acute toxicity was assessed by measuring mortality, changes in body weight, spontaneous movements, and normal rectal temperature in mice. Antipyretic activity was evaluated by brewer’s yeast-induced hyperpyrexia in rats according to Teotino method (1963). The antipyretic effect of methanolicextract of Alchornea cordifolia leaves was compared with paracetamol (100 mg/kg bw) orally. Groups of mice treated with doses of 6500;3250;1625 and 812.5mg/kg of the extract did not show any mortality, nor significant alteration of body weight, nor alteration of spontaneous movements. However, incomplete reversed dose-dependent hypothermic activity was observed with doses of 50.78;101.56;203.12;406.25;and 812.5 mg/kg p.o. of the extract, showing acute toxicity of this plant. In the antipyretic assay, the extract with doses of 50.78;101.56;203.12;406.25;and 812.5 mg/kg p.o. exhibited a significant dose-dependent antipyretic activity similar to paracetamol (100 mg/kg bw) in rats. Thus Alchornea cordifolia may inhibit prostaglandins-biosynthesis from hypothalamus. Our results support claims on its traditional uses in management of fever. However Alchornea cordifolia may affect hypothalamus not only during fever but also when body temperature is normal.

Determinants of Fruit and Vegetables Intake among Secondary School Pupils in the City of Bobo-Dioulasso (Burkina Faso): A Cross-Sectional Study

According to WHO, eating at least five portions of fruit and vegetables a day at an early age is protective against cancers, cardiovascular diseases and diabetes. The purpose of this study was to determine among secondary school pupils in the city of Bobo-Dioulasso, the level of observation of this recommendation and the associated factors. Methods: This was a cross-sectional study conducted from March to April 2016, among secondary school pupils in 1st Form, 4th Form and 7th Form of public and private secondary schools. The diet survey methods were the recalling of the last 24 hours associated with intake frequency questionnaire. An intake of less than one fruit (or a slice of a big fruit) per day and for vegetables, less than one portion of raw or cooked vegetables per day was considered insufficient. Results: A total of 1993 pupils were surveyed. The mean age was 17.5 ± 3.6 years and the sex ratio 0.7;75.4% and 90.0% of pupils had insufficient daily intake of fruit and vegetables respectively. The determinants of this consumption deficiency were: Sex (male, OR = 1.7 (1.3 - 2.2)), secondary school (public, OR = 1.6 (1.3 - 2.0)), class (4th Form, OR = 2.5 (1.5 - 4.2)/7th Form, OR = 3.7 (2.7 - 5.1)), the mother (thin build, OR = 3.2 (2.0 - 5.3)/overweight/obesity, OR = 2.8 (1.6 - 4.7)) and physical inactivity (OR = 1.5 (1.2 - 2.0)). Conclusion: The promotion of a healthy diet among secondary school pupils should take into account the type of school, sex, educational level, physical activity and mothers’ influence.

Upper gastrointestinal endoscopy is an excellent way for the diagnosis of high digestive pathology. We report the results of 1022 upper gastrointestinal endoscopy performed at Bobo-Dioulasso CHU-SS, in western Burkina Faso (West Africa). The aim of this study was to document the indications and lesions found in high endoscopy at the CHUSS. Patients and Methods: It was a cross-sectional study, prospective to describe the results of gastroscopy performed from 1st January 2015 to 30th June 2016 at the digestive endoscopy unit CHU-Souro Sanou Bobo-Dioulasso. Results: In the study period, 1022 upper gastrointestinal endoscopies were performed. The main indications were: The epigastric pain (48.6%), portal hypertension (10.7%) and gastroesophageal reflux disease (9.9%). The pathologies observed were dominated by gastropathies (48.11%), peptic ulcer (27%) and oesophageal varices (9.68%). Epigastralgias were the main circumstance for the discovery of: 52.3% of esophagitis, 49.17% of gastropathies and 46.12% of peptic ulcers. Histopathologically, the results of the 236 biopsies were dominated by gastritis (88.56%), stomach cancers (7.63%) and esophagus (3.81%). Conclusion: The main indication of the UGIE at the CHU-Souro Sanou in Bobo-Dioulasso was epigastralgia. The pathologies observed were dominated by gastropathy, esophagitis and PUD.

Obstetrical Vacuum Deliveries: Clinical Aspects and Fetal Prognosis in Regional Teaching Hospital of Ouahigouya

Objective: To determine the frequency of obstetrical vacuum deliveries in the service of obstetrics and gynecology of regional teaching hospital of Ouahigouya and then to assess fetal outcomes. Patients and Methods: It was about a cross sectional and descriptive retrospective data collection from 1st January 2014 to 31st December 2016 in the service of obstetrics and gynecology of regional teaching hospital of Ouahigouya. Patients who had vacuum delivered in the service with single pregnancy, at least 34 weeks gestation age and summit presentation and had a useful medical folder were included in our study. The data were collected and the analysis used epi-info software 7.***.1.0. version, 2010 Word and excel. The results were presented in percentage for qualitative variables and in means standard deviation for quantitative variables. Results: We have collected 6233 deliveries from 1st January 2014 to 31st December 2016, out of which 312 were done by using obstetrical vacuum giving a frequency of 5.0%. The mean age of patients was 23.5 ± 6.4 years. The average parity was 1.3 ± 0.7 women per delivery and 73.1% of our patients were pauciparous. 90.1% of patients were referred. The main indications were maternal weakness (43%), fetal distress (36.5%), prolonged expulsive phase (6.1%), stopping progression (4.8%) and the scar uterus (4.5%). Fetal prognosis was dominated by the caput in 13 cases (4.2%) and excoriations of the scalp in 5 cases (1.7%). Conclusion: Vacuum extractions are very limited in low setting countries. Its popularization is essential to reduce maternal and fetal mortality.

Sex development anomalies represent a group of congenital pathologies in which chromosomal, gonadal and anatomical sex differentiation is atypical. The aim of our study was to use molecular biology techniques to elucidate sex in cases of anomalies of sexual differentiation in Burkina Faso. This cross-sectional study took place from March 2023 to June 2023. Oral and blood samples were collected respectively using sterile swabs and stored on Swab Kits and NUCLEIcardTM (https://***/product-ranges/nucleic-card/) were used to determine gonosome profiles. Extraction was carried out using the DNA Swap solution Kit and the DNA IQ System Kit, and a PowerPlex? 21 kit (Promega) for amplification. The Applied Biosystems 9700 thermal cycler was used for PCR followed by 36 cm capillary electrophoresis in the Applied Biosystems 3130 prism sequencer. Sequence files were analyzed using GeneMapper IDX v. 3.2 software. Seven (07) patients were registered during the study period. There were 4 cases of XX DSD or 46, XX DSD and 3 cases of XY DSD or 46, XY DSD. The median age of our patients was 16 years. Civilian sex was male in 4 cases and female in 3. The most frequent reason for consultation was micropenis in 3 cases, followed by primary amenorrhea and sex ambiguity. There were 03 cases of discrepancy between genetic sex and civil sex. The accessibility of molecular diagnosis is little known to clinicians. XX DSDs or 46, XX DSDs were the most frequent (4/7) in our study. The problem facing this situation is early diagnosis to help prevent complications in Burkina Faso.

Over the past three decades, genomic and epigenetic sciences have identified more than 70 genes involved in the molecular pathophysiology of Alzheimer’s disease (AD). DNA methylation, abnormal histone and chromatin regulation and the action of various miRNAs induce AD. The identification of mutated genes has paved the way for the development of diagnostic kits and the initiation of gene therapy trials. However, despite major advances in neuroscience research, there is yet no suitable treatment for AD. Therefore, the early diagnosis of this neurodegenerative disease raises several ethical questions, including the balance between the principle of non-maleficence and the principle of beneficence. The aims of this research were to present the genomic and ethical aspects of AD, and to highlight the ethical principles involved in its presymptomatic diagnosis and therapy. A systematic review of the literature in PubMed, Google Scholar and Science Direct was carried out to outline the genomic aspects and ethical principles relating not only to the presymptomatic diagnosis of AD, but also to its gene therapy. A total of 16 publications were selected. AD is a multifactorial disease that can be genetically classified into Sporadic Alzheimer’s Disease and Familial Alzheimer’s Disease based on family history. Gene therapy targeting specific disease-causing genes is a promising therapeutic strategy. Advancements in artificial intelligence applications may enable the prediction of AD onset several years in advance. While early diagnosis of AD may empower patients with full decision competence for early decision-making, it also carries implications for the patient’s family members, who are at risk of developing the disease, potentially becoming a source of confusion or anxiety. AD has a significant impact on the life of individuals at risk and their families. Given the absence of disease modifying therapy, genetic screening and early diagnosis for this condition raise ethical issue

Background: Parkinson’s disease (PD) is a complex, multifactorial neurodegenerative disorder with a pathophysiology deriving from the synergy of abnormal aggregation of neuroinflammation, synuclein and dysfunction of lysosomes, mitochondria and synaptic transport difficulties influenced by genetic and idiopathic factors. Worldwide, PD has a prevalence of 2-3% in people over the age of 65. To date, there is no certified, effective treatment for PD. Aim: The aims of this research were: (i) to present, on the basis of recent advances in molecular genetics and epigenetics, the genomic aspects and challenges of gene therapy trials for PD;(ii) to outline the ethical principles applicable to therapeutic trials for PD. Method: A systematic literature review was carried out to identify relevant articles reporting on genomic aspects and gene therapy in PD from 2001 to October 2023. The search was conducted in French and/or English in three databases: PubMed, Google Scholar and Science Direct. PRISMA guidelines were used in this systematic review. Results: A total of thirty-three publications were selected. An inductive thematic analysis revealed that numerous genetic mutations (SNCA, Parkin, PINK1, DJ-1, LRRK2, ATP13A2, VPS35, Parkin/PRKN, PINK1, DJ1/PARK7) and epigenetic events such as the action of certain miRNAs (miR-7, miR-153, miR-133b, miR-124, miR-137) are responsible for the onset of PD, and that genetic therapy for this pathology raises ethical questions that need to be elucidated in the light of the bioethical principles of autonomy, beneficence, non-maleficence and justice. Conclusion: There is no zero risk in biotechnology. Then, it will be necessary to assess all the potential risks of Parkinson disease’s gene therapy to make the right decision.
