Background Infantile epileptic spasms syndrome(IESS)is a serious disease in infants,and it usually evolves to other epilepsy types or syndromes,especially refractory or super-refractory focal *** adrenocorticotropic hormone(ACTH)is one of the first-line and effective treatment plans for IESS,it has serious side effects and is not sufficiently *** A retrospective study of the clinical outcomes of ACTH combined with magnesium sulfate(MgSO4)therapy for IESS in two hospital centers was *** major outcome of the single and combined treatment was evaluated by changes in seizure frequency and improvements in hypsarrhythmia electroencephalography(EEG).To reduce the confounding bias between the two groups,we used SPSS for the propensity score matching(PSM)*** We initially recruited 1205 IESS patients from two Chinese hospitals and treated them with ACTH combined with MgSO4 and ACTH *** 1005 patients were enrolled in the treatment(ACTH combined with MgSO4:744,ACTH:261),and both treatment plans had a more than 55%response ***,compared to patients treated with ACTH alone,those patients treated with ACTH combined with MgSO4 had better performance in terms of the seizure frequency and hypsarrhythmia *** PSM,the two groups also showed significant differences in responder rate[70.8%(95%confidence interval,CI)=66.7%–74.8%)vs.53.8%(95%CI=47.4%–60.2%),P3 months(P<0.05).In addition,the overall incidence of adverse reactions in the ACTH combined with MgSO4 group was much lower than that in the ACTH group(31.4%vs.63.1%,P<0.001).During the treatment,only

BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-Meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

Transequatorial Filament Eruption and Its Link to a Coronal Mass Ejection

We revisit the Bastille Day flare/CME Event of 2000 July 14, and demonstrate that this flare/CME event is not related to only one single active region （AR）. Activation and eruption of a huge transequatorial filament are seen to precede the simultaneous filament eruption and flare in the source active region, NOAA AR 9077, and the full halo-CME in the high corona. Evidence of reconfiguration of large-scale magnetic structures related to the event is illustrated by SOHO EIT and Yohkoh SXT observations, as well as, the reconstructed 3D magnetic lines of force based on the force-free assumption. We suggest that the AR filament in AR 9077 was connected to the transequatorial filament. The large-scale magnetic composition related to the transequatorial filament and its sheared magnetic arcade appears to be an essential part of the CME parent magnetic structure. Estimations show that the filament- arcade system has enough magnetic helicity to account for the helicity carried by the related CMEs. In addition, rather global magnetic connectivity, covering almost all the visible range in longitude and a huge span in latitude on the Sun, is implied by the Nan^ay Radioheliograph （NRH） observations. The analysis of the Bastille Day event suggests that although the triggering of a global CME might take place in an AR, a much larger scale magnetic composition seems to be the source of the ejected magnetic flux, helicity and plasma. The Bastille Day event is the first described ex- ample in the literature, in which a transequatorial filament activity appears to play a key role in a global CME. Many tens of halo-CME are found to be associated with transequatorial filaments and their magnetic environment.

类钙调素(Calmodulin-like,CML)是植物特有的钙信号感受器蛋白之一,参与植物生长发育和响应外界环境信号转导。甘蔗(Saccharum ***)中CML应答甘蔗花叶病毒(Sugarcane mosaic virus,SCMV)侵染尚未见报道。本研究从甘蔗热带种Badila(***)中克隆了一个CML基因,命名为ScCML13。该基因开放读码框(open reading frame,ORF)长度为519 bp,编码172 aa。生物信息学分析表明,ScCML13属于稳定的亲水脂溶蛋白,无跨膜结构域,有4个结合Ca2+的EF-hand结构域;系统进化树分析表明,该蛋白在单子叶和双子叶植物中及单子叶C3和C_(4)植物中具有明显分化;酵母双杂交(yeast two-hybrid,Y2H)和双分子荧光互补(bimolecular fluorescence complementation,BiFC)试验表明,ScCML13与SCMV的运动蛋白P3N-PIPO互作。亚细胞定位试验表明ScCML13定位于内质网和细胞核。共定位试验发现ScCML13会干扰SCMV-P3N-PIPO的质膜(plasma membrane)或胞间连丝(plasmodesmata)定位。实时荧光定量PCR(Real Time Quantitative PCR,RT-qPCR)分析发现,ScCML13基因主要在甘蔗叶片中表达,在节间和根中的相对表达量低;ScCML13基因在感染SCMV后2 h显著上调,随后下调至与对照组相比的水平,而在感染后期上调。
