Early Versus Delayed Laparoscopic Cholecystectomy for Acute Cholecystitis with Mild Pancreatitis

The safety and feasibility of early laparoscopic cholecystectomy(LC)for acute cholecystitis with mild pancreatitis were explored.A total of 973 patients with acute pancreatitis,including 651 mild cases and 322 moderate or severe cases were retrospectively studied from July 2014 to December 2018 in our *** 426 mild pancreatitis cases with acute cholecystitis were enrolled in this study,of which 328 patients underwent LC during the same-admission(early LC group),and 98 patients underwent LC a period of time after conservative treatment(delayed LC group).Clinical characteristics,operative findings and complications were recorded and followed *** two groups were comparable in age,gender,the grade of American Society of Anesthesiologist(ASA),biochemical findings and Balthazar computer tomography(CT)rating(P>0.05).The operation interval and hospital stay in early LC group were significantly shorter than in delayed LC group(5.83+1.62 vs.41.3618.44 days;11.38+2.43 vs.16.49+3.48 days,P<0.01).There was no significant difference in the average operation time between the two *** preoperative biliary related events recurred in early LC group but there were 21 cases of preoperative biliary related events in delayed LC group(P<0.01).There was no significant difference in conversion rate(3.85 vs.5.10%,P=0.41)and surgical complication rate(3.95 vs.4.08%,P-0.95)between early LC group and delayed LC *** the postoperative follow-up period of 375 cases,biliary related events recurred in 4 cases in early LC group and 3 cases in delayed LC group(P=0.37).The effect of early LC during the same-admission is better than delayed LC for acute cholecystitis with mild pancreatitis.

Occult pancreaticobiliary reflux is a pathogenic factor of some benign biliary diseases and gallbladder cancer

Background:Pancreaticobiliary maljunction(PBM)is a well-known high-risk factor for biliary malignant tumors because of constant pancreaticobiliary reflux(PBR).However,the impact of occult pancreaticobiliary reflux(OPR),which is characterized by high bile amylase levels in individuals with anatomically normal pancreaticobiliary junction,on biliary diseases remains *** aim of this study was to assess the correlation between OPR and biliary ***:We enrolled 94 consecutive patients with normal pancreaticobiliary junction and primary biliary diseases confirmed by magnetic resonance *** prospectively collected patients’bile samples and measured bile amylase *** investigated the incidence of OPR and the difference in bile amylase levels among these patients and assessed the correlation between high bile amylase levels(HBAL)and benign or malignant biliary diseases,as well as the OPR risk ***:The incidence of OPR was 36.6%in patients with benign biliary diseases,26.7%in those with cholangiocarcinoma and 62.5%in those with gallbladder *** median bile amylase level tended to be higher in patients with gallbladder cancer than in those with benign biliary diseases,but there was no significant difference(165.5 IU/L vs.23.0 IU/L,P=0.212).The prevalence of an HBAL with bile amylase levels of 1000-7500 IU/L was similar in patients with gallbladder cancer and benign biliary ***,the incidence of HBAL with bile amylase levels greater than 7500 IU/L was significantly higher in patients with gallbladder cancer than in those with benign biliary diseases(37.5%vs.4.2%,P=0.012).Multivariate logistic regression analysis revealed that choledocholithiasis was an independent risk factor for ***:OPR can occur in benign and malignant biliary diseases,and it may be a pathogenic factor for some benign biliary diseases and a high-risk factor for gallbladder *** is a correlation between choledocholit

High-temperature calcination dramatically promotes the activity of Cs/Co/Ce-Sn catalyst for soot oxidation

Catalytic oxidation of soot is of great importance for emission control on diesel *** this work,a highly active Cs/Co/Ce-Sn catalyst was investigated for soot oxidation,and it was unexpectedly found that high-temperature calcination greatly improved the activity of the *** the calcination temperature was increased from 500℃ to 750℃,T_(50) decreased from 456.9℃ to 389.8℃ in a NO/O_(2)/H_(2)O/N_(2) *** results revealed that high-temperature calcination can promote the ability to transfer negative charge density from Cs to other metal cations in Cs/Co/Ce-Sn,which will facilitate the production of more oxygen defects and the generation of more surface-active oxygen *** oxygen species are beneficial to the oxidation of NO to NO_(2),leading to the high yield of NO_(2) ***,the Cs/Co/Ce-Sn catalyst calcined at 750℃ demonstrated higher activity than that calcined at 500℃.This work provides a pathway to prepare high efficiency catalysts for the removal of soot and significant insight into the effects of calcination on soot oxidation catalysts.

Development and application of an intelligent thermal state monitoring system for sintering machine tails based on CNN-LSTM hybrid neural networks

Real-time prediction and precise control of sinter quality are pivotal for energy saving,cost reduction,quality improvement and efficiency enhancement in the ironmaking *** advance,the accuracy and comprehensiveness of sinter quality prediction,an intelligent flare monitoring system for sintering machine tails that combines hybrid neural networks integrating convolutional neural network with long short-term memory(CNN-LSTM)networks was *** system utilized a high-temperature thermal imager for image acquisition at the sintering machine tail and employed a zone-triggered method to accurately capture dynamic feature images under challenging conditions of high-temperature,high dust,and *** feature images were then segmented through a triple-iteration multi-thresholding approach based on the maximum between-class variance method to minimize detail loss during the segmentation *** the advantages of CNN and LSTM networks in capturing temporal and spatial information,a comprehensive model for sinter quality prediction was constructed,with inputs including the proportion of combustion layer,porosity rate,temperature distribution,and image features obtained from the convolutional neural network,and outputs comprising quality indicators such as underburning index,uniformity index,and FeO content of the *** accuracy is notably increased,achieving a 95.8%hit rate within an error margin of±*** the system is applied,the average qualified rate of FeO content increases from 87.24%to 89.99%,representing an improvement of 2.75%.The average monthly solid fuel consumption is reduced from 49.75 to 46.44 kg/t,leading to a 6.65%reduction and underscoring significant energy saving and cost reduction effects.

Charge polarity inversion and zincophilicity improvement for chitosan separator towards durable aqueous zinc-ion batteries
