Trocar opening: silicone oil removal with phacoemulsification and intraocular lens implantation

AIM: To evaluate the efficacy and safety of a modified technique [trocar opening(TO)] for silicone oil removal(SOR) in combination with phacoemulsification and intraocular lens(IOL) ***: A total of 60 eyes of 60 patients with cataract and silicone oil-filled eyes were enrolled in this study. The patients were divided into two groups: the patients in the control group underwent 23-gauge pars plana active SOR surgery with phacoemulsification and IOL implantation, while the patients in the TO group underwent TO methods during surgery. Best corrected visual acuity(BCVA), surgery time, intraocular pressure, and operative complications were observed 6 mo after ***: There was no significant difference between the two groups in terms of age, gender, preoperative, intraocular pressure, or time of silicone oil stay. Prior to surgery, the mean BCVA for the control and TO groups was 1.34±0.44 and 1.36±0.42. At 6 mo following surgery, the mean BCVA improved to 0.74±0.36 and 0.77±0.32, respectively(P<0.001). There was no significant difference between the two groups. The mean SOR time was 6.9±2.3 min and 4.8±1.2 min in the control and TO groups(P=0.008). The total operation time was 28.2±8.5 min and 24.6±6.4 min, respectively(P=0.035). Posterior capsule rupture occurred in four eyes of control and none of TO group(P<0.01). Late recurrent retinal detachment occurred in one eye in the control group(2 mo after surgery) and in one eye in the TO group(4 mo after surgery). CONCLUSION: TO is a simple, effective, time-saving, and safe method for SOR combined with phacoemulsification and IOL implantation.

Additive manufacturing technology based on laser powder bed fusion(LPBF)offers a novel approach for fabricating bulk metallic glass(BMG)products without restriction in size and ***,the BMGs prepared by LPBF usually suffered from less plasticity and poorer fracture toughness as compared to their cast counterparts due to partial crystallization in heat-affected zones(HAZs).Since crystallization in HAZs is hard to avoid completely in LPBF BMGs,it is desirable to design a suitable alloy system,in which only ductile crystalline phase,instead of brittle intermetallics,is formed in *** unique structure could effectively increase the toughness/plasticity of the LPBF *** achieve this goal,a quaternary BMG system with a composition of Zr_(47.5)Cu_(45.5)Al_(5)Co_(2)is adopted and subjected to *** is found that nearly a single phase of B_(2)-ZrCu is precipitated in HAZs,while a fully amorphous phase is formed in molten pools(MPs).This B_(2)phase reinforced BMG composite exhibits excellent mechanical properties with enhanced plasticity and ***,it is easy to modulate the mechanical properties by altering the amount of the B_(2)phase via adjusting the laser energy ***,the best combination of strength,plasticity,and notch toughness is obtained in the BMG composite containing 27.4%B_(2)phase and 72.6%amorphous phase,which exhibits yield strength(σ_(s))of 1423 MPa,plastic strain(ε_(p))of 4.65%,and notch toughness(K_(q))of 53.9 MPa m 1/***,a notable strain-hardening is also *** improvement of plasticity/toughness and appearance of strain-hardening behavior are mainly due to the martensite phase transformation from the B_(2)phase to the Cm phase during plastic deformation(i.e.,the phase transformation-induced plasticity effect).The current work provides a guide for making advanced BMGs and BMG composites by additive manufacturing.

We used 11 years of census data from 450 seedling quadrats established in a 20-ha forest dynamics plot to study seedling dynamics in tree species of a tropical seasonal rainforest in Xishuangbanna,southwestern *** found that overall seedling recruitment rate and relative growth rate were higher in the rainy season than in the dry *** the recruitment rate of seedlings from canopy tree species(two species)and the relative growth rate of seedlings from understory species(nine species)were higher in the rainy season than in the dry ***,in the rainy season,the recruitment rate of seedlings was higher for canopy tree species than for understory tree *** addition,relative growth rate of seedlings was higher in the canopy species than in understory seedlings in the dry *** also observed that,in both rainy and dry seasons,mortality rate of seedlings was higher for canopy species than for understory ***,canopy tree species appear to have evolved a flexible strategy to adapt to the seasonal changes of a monsoon *** contrast,understory tree species seem to have adopted a conservative ***,these species mainly release seedlings in the rainy season and maintain relatively stable populations with a lower mortality rate and recruitment rate in both dry and rainy *** study suggests that canopy and understory seedling populations growing in forest understory may respond to future climate change scenarios with distinct regeneration strategies.

Hepatocellular carcinoma (HCC) is one of the most common malignancies, ranking the sixth in the world, with 55% of cases occurring in China. Usually, patients with HCC did not present until the late stage of the disease, thus limiting their therapeutic options. Although surgical resection is a potentially curative modality for HCC, most patients with intermediate-advanced HCC are not suitable candidates. The current therapeutic modalities for intermediate-advanced HCC include: (1) surgical procedures, such as radical resection, palliative resection, intraoperative radiofrequency ablation or cryosurgical ablation, intraoperative hepatic artery and portal vein chemotherapeutic pump placement, two-stage hepatectomy and liver transplantation; (2) interventional treatment, such as transcatheter arterial chemoembolization, portal vein embolization and image-guided locoregional therapies; and (3) molecularly targeted therapies. So far, how to choose the therapeutic modalities remains controversial. Surgeons are faced with the challenge of providing the most appropriate treatment for patients with intermediate-advanced HCC. This review focuses on the optional therapeutic modalities for intermediateadvanced HCC.
