Bullous pemphigoid associated with acquired hemophilia A: A case report

BACKGROUND Acquired hemophilia A (AHA) is a rare and potentially severe bleeding disordercaused by circulating autoantibodies against factor Ⅷ (FⅧ). In approximately50% of the patients, the condition is associated with autoimmune diseases,cancers, medication use, pregnancy, and the post-partum period. Bullous pemphigoid(BP) is a chronic autoimmune subepidermal blistering disease associatedwith tissue-bound and circulating autoantibodies against BP antigens 180 (BP180)and 230 (BP230). AHA-associated BP has a high mortality rate;hence, the understandingof this disease must *** SUMMARY A 69-year-old man presented with erythema, blisters, blood blisters, and crustsaccompanied by severe pruritus for more than 20 days, and ecchymosis andswelling on his left upper arm for 3 days. Pathological examination revealed asubepidermal blister that contained eosinophils. Laboratory tests showed that theBP180 autoantibody levels had increased, isolated activated partial thromboplastintime was notably prolonged (115.6 s), and coagulation FⅧ activity wasextremely low (< 1.0%). Furthermore, the FⅧ inhibitor titer had greatlyincreased (59.2 Bethesda units). Therefore, the patient was diagnosed as having BP associated with AHA, prescribed 0.05% topical halometasone cream, and transferred to a higher-level hospitalfor effective treatment;however, he died after 2 *** AHA associated BP is rare, dangerous, and has a high mortality rate. Therefore, its timely diagnosis and effectivetreatment are necessary.

Probabilistic Response and Short-Term Extreme Load Estimation of Offshore Monopile Wind Turbine Towers by Probability Density Evolution Method

A new analysis framework based on probability density evolution method(PDEM)and its Chebyshev collocation solution are introduced to predict the dynamic response and short-term extreme load of offshore wind turbine(OWT)towers subjected to random sea *** regard to the stochastic responses,random function method is employed to generate samples of sea elevation,the probability density evolution equation(PDEE)is solved to calculate time-variant probability density functions of structural *** the probabilistic load estimation,a FAST model of NREL 5MW offshore turbine is established to obtain samples of bending moment at the tower *** equivalent extreme event theory is used to construct a virtual stochastic process(VSP)to assess the short-term extreme *** results indicate that the proposed approach can predict time-variant probability density functions of the structural responses,and shows good agreement with Monte Carlo ***,the predicted short-term extreme load can capture the fluctuation at the tail of the extreme value distribution,thus is more rational than results from the typical distribution ***,the proposed method shows good adaptation,precision and efficiency for the dynamic response analysis and load estimation of OWT towers.

Pollution characteristics of organic and elemental carbon in PM_(2.5) in Xiamen,China

Xiamen, located on the southeastern coastal line of China, is undergoing rapid urbanization and industrialization, so its air quality has a trend of degradation. However, studies on level, temporal and spatial changes of fine particles （PM2.5） and their carbonaceous fractions are scarce. In this article, abundance, sources, seasonal and spatial variations, distribution of organic carbon （OC） and elemental carbon （EC） in PM2.5, were studied at suburban, urban and industrial sites in Xiamen during four season-representative months in 2009-2010. PM2.5 samples were collected with middle volume sampler and were analyzed for OC and EC with thermal optical transmittance （TOT） method. Results showed that the annual average PM2.5 concentrations were 63.88-74.80 Ixg/m3 at three sites. While OC and EC concentrations were in the range of 15.81-19.73 [xg/m3 and 2.74-3.49 ~tg/m3, respectively, and clearly presented the summer minima and winter maxima in this study. The carbonaceous aerosol accounted for 42.8%-47.3% of the mass of PMzs. The annual average of secondary organic carbon （SOC） concentrations in Xiamen were 9.23-11.36 ~g/m3, accounting for approximately 56% of OC. Strong correlations between OC and EC was found in spring （R2 = 0.50） and autumn （R2 = 0.73）, suggesting that there were similar emission and transport processes for carbonaceous aerosols in these two seasons, while weak correlations were found in summer （R2 = 0.33） and winter （R2 = 0.41）. The OCI＇EC ratios in PM2.5 varied from 2.1 to 8.7 with an annual average of 5.7, indicating that vehicle exhaust, coal smoke and biomass burning were main source apportionments of carbonaceous fractions in Xiamen.

V_p and V_p/V_s structures in the crust and upper mantle of the Taiwan region, China

A tomographic study of the Vp and Vp/Vs structures in the crust and upper mantle beneath the Taiwan region of China is conducted by simultaneous inversion of P and S arrival times. Compared with the previous tomographic results, the spherical finite difference technique is suitable for the strong heterogeneous velocity structure, and may improve the accuracy in the travel time and three-dimensional ray tracing calculations. The Vp and Vp/Vs structures derived from joint inversion and the relocated earthquakes can provide better constraints for analyzing the lateral heterogeneity and deep tectonic characters in the crust and upper mantle. Our tomographic results reveal significant relations between the seismic wavespeed structure and the tectonic characters. In the shallow depth, sedimentary basins and orogen show distinct wavespeed anomalies, with low Vp, high Vp/Vs in basins and high Vp, low Vp/Vs in orogen. As the suture zone of Eurasian Plate and Philippine Sea Plate, Longitudinal Valley is characterized by a significant high Vp/Vs anomaly extending to the middle-lower crust and upper mantle, which reflects the impact of rock cracking, partial melting, and the presence of fluids. In the northeast Taiwan, the Vp, Vp/Vs anomalies and relocated earthquakes depict the subducting Philippine Sea Plate under the Eurasian Plate. The high Vp of oceanic plate and the low Vp, high Vp/Vs atop the subducted oceanic plate extend to 80 km depth. Along the east-west profiles, the thickness of crust reaches 60 km at the east of Central Range with eastward dipping trend, which reveals the eastward subduction of the thickened and deformed crust of the Eurasian continental plate.

The advection-dominated accretion flow+thin accretion disk model for two low-luminosity active galactic nuclei: M81 and NGC 4579

It was found that advection-dominated accretion flow （ADAF）＋thin disk model calculations can reproduce the observed spectral energy distributions （SEDs） of two low-luminosity active galactic nuclei （AGNs）, provided they are accreting at -0.01-0.03 Eddington rates and the thin disks are truncated to ADAFs at -100Rs （Rs is the Schwarzschild radius） for M81 and NGC 4579 （Quataert et al. 1999）. However, the black hole masses adopted in their work are about one order of magnitude lower than recent measurements on these two sources. Adopting the well estimated black hole masses, our ADAF＋thin disk model calculations can reproduce the observed SEDs of these two low- luminosity AGNs, if the black hole is accreting at 2.5×10^-4 Eddington rates with the thin disk truncated at Rtr=120Rs for M81（m= 3.3×10^-3 and Rtr=80Rs are required for NGC 4579）. The transition zones with temperature from the thin disk with ,-10^4- 10^5 to ,-10^9-10^10K in the ADAF will inevitably emit thermal X-ray lines, which provides a useful diagnosis of their physical properties. The observed widths of the thermal X-ray iron lines at -6.8 keV are consistent with Doppler broadening by Keplerian motion of the gases in the transition zones at ,-100Rs. We use the structure of the transition zone between the ADAF and the thin disk derived by assuming the turbulent diffusive heat mechanism to calculate their thermal X-ray line emission with the standard software package Astrophysical Plasma Emission Code （APEC）. Comparing them with the equivalent widths of the observed thermal X-ray iron lines in these two sources, we find that the turbulent diffusive heat mechanism seems to be unable to reproduce the observed thermal X-ray line emission. The test of the evaporation model for the accretion mode transition with the observed thermal X-ray line emission is briefly discussed.

Characterization of polycyclic aromatic hydrocarbons and gas/particle partitioning in a coastal city, Xiamen, Southeast China

An intensive sampling program had been undertaken in autumn （October, 2008） and winter （December, 2008 and January, 2009） at urban （Xiamen University and Xianyue residential area）, suburban （Institute of Urban Environment）, industrial area （Lulian Hotel） and background （Tingxi Reservoir） in Xiamen, Fujian Province, to characterize the atmospheric concentration and gas-particle phase partitioning of PAHs. The average concentration of total PAHs in winter was almost 1.7 times higher than those in autumn. The log scale plot of Kp versus sub-cooled liquid vapor pressure （PL^0） for all the data of autumn and winter season samples gave significantly different slopes. The slope for the winter samples （-0.72） was steeper than that for the autumn samples （-0.58）. The partitioning results indicated that slope values varied depending on characteristics of specific site, source region and meteorological conditions which play important roles in the partitioning of PAHs. In addition, local emission sources had a stronger effect on partitioning results than long-transported polluted plume. The sources of PAHs in five sampling sites in Xiamen also have been discussed initially. Diagnostic ratios showed that the primary source of PAHs in urban, suburban and industrial area was from vehicle exhausts. While emission from petrochemical factory and power plant was another main contributor to industrial area.

With the development of color Doppler and laparoscopic ultrasound, now intraoperative ultrasound(IOUS) plays an important role in liver surgery. Compared to percutaneous ultrasound(US), IOUS is conducted directly on the liver surface, with no blind spots or dead ends, thus improves the detection, localization and characterization of lesions without influencing factors, such as obesity.

Uppermost mantle structure of the North China Craton: Constraints from interstation Pn travel time difference tomography

The uppermost mantle is the key area for exchange of heat flux and material convection between the crust and lithospheric mantle.
