Immediate Effect of Electro-acupuncture on Endometrial Blood Flow in Patients with Recurrent Implantation Failure:A Randomized Controlled Trial

Objective To investigate the immediate effects of electro-acupuncture(EA)on endometrial blood flow among recurrent implantation failure(RIF)patients.Methods Eighty RIF patients,enrolled from March 2022 to December 2022,were randomly allocated into either the EA group(40 cases)or the waiting-list(WL)group(40 cases)by using a random number table.The EA group underwent acupuncture at points of Shenting(GV 24),Baihui(GV 4),Benshen(GB 13),bilateral Zigong(EX-CA 1),Huangshu(KI 16),Sanyinjiao(SP 6)and Xuehai(SP10),and electric acupuncture apparatus was connected to EX-CA 1,KI 16,SP 6,and SP 10 with disperse-dense waves at 4/20 Hz frequencies for 30 min after transvaginal ultrasound,while the WL group received no intervention.The primary outcome measured was the endometrial volume blood flow.The secondary outcomes included the bilateral uterine artery index,endometrial volume,endometrial blood flow type,vascular distribution index(VI^(MV))for endometrial and ovary,clinical pregnancy rate,and embryo implantation rate.Results In the EA group,there was a notable decrease in the bilateral pulsatility index and a significant improvement in the endometrial blood flow type post-EA(Pboth the endometrial blood flow type and VI^(MV) for the endometrium and right ovary were markedly higher in the EA group compared to the WL group post-treatment(P0.05).Besides,no adverse events related to EA were observed.Conclusions EA can promptly ameliorate VI^(MV) for the endometrial and right ovary,and endometrial blood flow type.Future randomized controlled trials are warranted to investigate the long-term effects of EA on blood flow of RIF patients and its implications for pregnancy outcomes.(Trial registration No.ChiCTR2200057377).

Rapidly growing primordial black holes as seeds of the massive high-redshift JWST Galaxies

A group of massive galaxies at redshifts of z 7 have been recently detected by the James Webb Space Telescope (JWST),which were unexpected to form at such an early time within the standard Big Bang cosmology.In this work,we propose that this puzzle can be explained by the presence of some primordial black holes (PBHs) with a mass of～1000M⊙.These PBHs act as seeds for early galaxy formation with masses of～108-1010M⊙at high redshift,hence accounting for the JWST observations.We use a hierarchical Bayesian inference framework to constrain the PBH mass distribution models,and find that the Lognormal model with the Mc～750M⊙is preferred over other hypotheses.These rapidly growing BHs are expected to have strong radiation and may appear as high-redshift compact objects,similar to those recently discovered by JWST.Although we focused on PBHs in this work,the bound on the initial mass of the seed black holes remains robust even if they were formed through astrophysical channels.

A Lightweight Network with Dual Encoder and Cross Feature Fusion for Cement Pavement Crack Detection

Automatic crack detection of cement pavement chiefly benefits from the rapid development of deep learning,with convolutional neural networks(CNN)playing an important role in this field.However,as the performance of crack detection in cement pavement improves,the depth and width of the network structure are significantly increased,which necessitates more computing power and storage space.This limitation hampers the practical implementation of crack detection models on various platforms,particularly portable devices like small mobile devices.To solve these problems,we propose a dual-encoder-based network architecture that focuses on extracting more comprehensive fracture feature information and combines cross-fusion modules and coordinated attention mechanisms formore efficient feature fusion.Firstly,we use small channel convolution to construct shallow feature extractionmodule(SFEM)to extract low-level feature information of cracks in cement pavement images,in order to obtainmore information about cracks in the shallowfeatures of images.In addition,we construct large kernel atrous convolution(LKAC)to enhance crack information,which incorporates coordination attention mechanism for non-crack information filtering,and large kernel atrous convolution with different cores,using different receptive fields to extract more detailed edge and context information.Finally,the three-stage feature map outputs from the shallow feature extraction module is cross-fused with the two-stage feature map outputs from the large kernel atrous convolution module,and the shallow feature and detailed edge feature are fully fused to obtain the final crack prediction map.We evaluate our method on three public crack datasets:DeepCrack,CFD,and Crack500.Experimental results on theDeepCrack dataset demonstrate the effectiveness of our proposed method compared to state-of-the-art crack detection methods,which achieves Precision(P)87.2%,Recall(R)87.7%,and F-score(F1)87.4%.Thanks to our lightweight crack detectionmodel,the parameter count of the mo

Chemoradiotherapy plus tislelizumab for mismatch repair proficient rectal cancer with supraclavicular lymph node metastasis:A case report

BACKGROUND According to the latest report,colorectal cancer is still one of the most prevalent cancers,with the third highest incidence and mortality worldwide.Treatment of advanced rectal cancer with distant metastases is usually unsatisfactory,especially for mismatch repair proficient(pMMR)rectal cancer,which leads to poor prognosis and recurrence.CASE SUMMARY We report a case of a pMMR rectal adenocarcinoma with metastases of multiple lymph nodes,including the left supraclavicular lymph node,before treatment in a 70-year-old man.He received full courses of chemoradiotherapy(CRT)followed by 4 cycles of programmed death 1 inhibitor Tislelizumab,and a pathologic complete response(pCR)was achieved,and the lesion of the left supraclavicular lymph node also disappeared.CONCLUSION pMMR advanced rectal cancer with preserved intact distant metastatic lymph nodes may benefit from full-course CRT combined with immunotherapy.

Efficacy and safety of targeted therapy plus immunotherapy combined with hepatic artery infusion chemotherapy (FOLFOX) for unresectable hepatocarcinoma

BACKGROUND The advent of cutting-edge systemic therapies has driven advances in the treatment of hepatocellular carcinoma(HCC),and therapeutic strategies with multiple modes of delivery have been shown to be more efficacious than mono-therapy.However,the mechanisms underlying this innovative treatment modality have not been elucidated.AIM To evaluate the clinical efficacy of targeted therapy plus immunotherapy combined with hepatic arterial infusion chemotherapy(HAIC)of FOLFOX in patients with unresectable HCC.METHODS We enrolled 53 patients with unresectable HCC who received a combination of targeted therapy,immunotherapy,and HAIC of FOLFOX between December 2020 and June 2021 and assessed the efficacy and safety of the treatment regimen.RESULTS The objective response rate was 60.4%(32/53),complete response was 24.5%(13/53),partial response was 35.9%(19/53),and stable disease was 39.6%(21/53).The median duration of response and median progression-free survival were 9.1 and 13.9 months,respectively.The surgical conversion rate was 34.0%(18/53),and 1-year overall survival was 83.0%without critical complicating diseases or adverse events(AEs).CONCLUSION The regimen of HAIC of FOLFOX,targeted therapy,and immunotherapy was curative for patients with unresectable HCC,with no serious AEs and a high rate of surgical conversion.

Effect of wall-disruption on nutrient composition and in vitro digestion of camellia and lotus bee pollens

The nutrient digestion,absorption and biological activity of bee pollen may be limited due to the complex pollen wall.Here,the effect of superfine grinding technology on the release of nutrients from bee pollen were investigated,and their antioxidant activities and in vitro digestion were explored in this study.Results showed that the content of nutrients in bee pollen increased after wall disruption.Among them,fat content increased by 22.55%-8.31%,protein content increased by 0.54%-4.91%,starch content increased by 36.31%-48.64%,soluble sugar content increased by 20.57%-29.67%,total phenolic acid content increased by 11.73%-86.98%and total flavonoids content increased by 14.29%-24.79%.At the same time,the antioxidant activity increased by 14.84%-46.00%.Furthermore,the active components such as phenolic compounds in the wall-disruption bee pollen were more readily to be released during the in vitro digestion,and easier to be absorbed because of their higher bioaccessibility.Antioxidant activities during in vitro digestion were also improved in walldisruption bee pollen.These findings provide evidence that bee pollen wall disruption was suggested,thus,it is more conducive to exerting the value of bee pollen in functional foods.

Microstructure and properties of coarse-grained WC-10Co cemented carbides with different carbon contents during heat treatments

The effects of cryogenic treatment（CT） and tempering-cryogenic treatment（TCT） on the microstructure and properties of coarse-grained WC-10Co cemented carbides with different carbon contents were researched.The binder phase,WC mean grain sizes,W solubility in the binder,relative magnetic s aturation,densities,hardness,wear resistance and second phase precipitation of cemented carbides with different heat treatments were discussed.The results show that there are significant changes of microstructure and properties in the samples with CT and TCT,especially due to the precipitation of metastable nanoparticles WxCoyCzin the binder during the heat treatments of CT and TCT.With the simultaneous combination of microstructure and nanoparticle-reinforced binder,a dramatically improved combination of hardness and wear resistance of the samples after TCT has been achieved.

Prevalence of prolonged grief disorder and its symptoms among bereaved individuals in China:a systematic review and meta-analysis

Background The prevalence of prolonged grief disorder(PGD)and its symptoms among the bereaved population in China vary considerably.Aims This meta-analysis aims to estimate the prevalence of PGD and its symptoms among bereaved individuals in China.Methods We conducted a literature search in major Chinese and English databases from their inception to 4 October 2023,for cross-sectional studies on the prevalence of PGD or its symptoms in bereaved Chinese individuals.The risk of bias of the included studies and certainty of the evidence were assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Studies Reporting Prevalence Data(‘JBI checklist’)and the Grading of Recommendations,Assessment,Development and Evaluations(GRADE),respectively.The‘metaprop’package in R V.4.1.2 was used to synthesise the prevalence.Results A total of 28 studies involving 10994 bereaved individuals were included in the analysis,with JBI checklist scores between 3 and 7.The combined prevalence(95%confidence interval)of PGD and its symptoms was 8.9%(4.2%to 17.6%)and 32.4%(18.2%to 50.8%),respectively.PGD and its symptoms were most prevalent among those who had lost their only child(22.7%)and those bereaved by earthquakes(80.4%),respectively.The GRADE system assigned a very low certainty level to the evidence for the pooled prevalence of PGD and its symptoms.Conclusions The pooled prevalence of PGD and its symptoms indicate a potential high need for grief counselling services among bereaved individuals in China.This need is particularly pronounced in those who have lost their only child and those bereaved due to earthquakes.Further methodologically rigorous studies are needed to provide more accurate prevalence estimates.PROSPERO registration number CRD42023432553.

Numerical simulation of transient characteristics in a bulb turbine during the load rejection process

To evaluate the safety of the bulb tubular turbine,the dynamic hydraulic characteristics of a hydropower station system during the load rejection process are studied through numerical simulations and a prototype test.In the developed model,a dynamic grid technology(DGT)controls the closure of the guide vane and the blade,whilst the moment balance equation and the user-defined function(UDF)provide the runner’s rotation speed.The 3-D transient simulation method can well predict the rotation speed and mass flow curves in the state of load rejection.The simulation outcomes of the system performance are basically consistent with the measurement data of the prototype.As observed,the runner is subjected to the reversely increased torque and axial force,the system is in a braking phase,and the maximum speed peaks at 144.6%of the rated speed.Moreover,the internal flow of the runner is greatly affected by the closure of the guide vane,and the draft tube forms an eccentric spiral vortex rope.It breaks downstream,aggravating the instability of the draft tube.Overall,the transient characteristics span for the first five seconds,demonstrating the importance of establishing an efficient governing controller.The obtained results are useful for designing the turbine’s flow channel with a double regulating function and comprehending the turbine’s transient characteristics.

Intelligent overheating fault diagnosis for overhead transmission line using semantic segmentation

The strain clamps and leading wires are important components that connect conductors on overhead transmission lines and conduct current.During operation,poor contact between these components can cause abnormal overheating,leading to electric failures and threatening power system reliability.Recently,the use of unmanned aerial vehicles equipped with infrared thermal imagers for strain clamp and leading wire maintenance has become increasingly popular.Deep learning-based image recognition shows promising prospects for intelligent fault diagnosis of overheating faults.A pre-treatment method is proposed based on dynamic histogram equalisation to enhance the contrast of infrared images.The DeepLab v3+network,loss function,and existing networks with different backbones are compared.The DeepLab v3+network with ResNet101 and convolutional block attention module added,and the Focal Loss function achieved the highest performance with an average pixel accuracy of 0.614,an average intersection over union(AIoU)of 0.567,an F1 score of 0.644,and a frequency weighted intersection over union of 0.594 on the test set.The optimised Atrous rates has increased the AIoU by 12.91%.Moreover,an intelligent diagnosis scheme for evaluating the defect state of the strain clamps and leading wires is proposed and which achieves a diagnostic accuracy of 91.0%.
