Effectivenss of electroacupuncture for skeletal muscle pain in Parkinson's disease:a Clinical randomized controlled trial

OBJECTIVE:To explore the effect of electroacupuncture on skeletal muscle pain in Parkinson's disease(PD).METHODS:A single-center randomized controlled trial was conducted with sixty patients with Parkinson's disease with skeletal muscle pain were randomly divided into electroacupuncture group and sham acupuncture control group with 30 patients each.The electric acupuncture group was treated with electric acupuncture,while the control group was treated with Park needle pseudoacupuncture.Both groups were treated 5 times a week for a total of 4 weeks,and both groups completed 20 treatments.King's Parkinson's Pain Scale(KPPS)and visual analog scale(VAS)were used before and after treatment to evaluate the pain degree of patients.Realtime shear wave elastography(SWE)and modified Ashworth score(MAS)were used to evaluate the changes of muscle tone.Parkinson's comprehensive Score Scale(MDS-UPDRS,including UPDRS Ⅱ and UPDRS Ⅲ)was used to evaluate exercise ability.Hamilton Depression Scale(HAMD)score was used to evaluate the emotional changes of patients.Spearman correlation analysis was used to explore the correlation between pain degree and muscle tone,exercise ability and emotion.RESULTS:During the study,one case fell off in the control group,and 30 cases were eventually included in the analysis and treatment group and 29 cases in the control group.After treatment,Young's modulus of biceps and quadriceps and shear wave velocity of biceps were decreased in electroacupuncture group compared with before treatment,while KPPS score,VAS score,UPDRS Ⅱ,UPDRS Ⅲ and modified Ashworth score were decreased,with statistical significance(P0.05).After treatment,KPPS score,VAS score,UPDRS Ⅱ and UPDRS Ⅲ,MAS,HAMD score,Young's modulus of biceps and shear wave velocity in electroacupuncture group were significantly lower than those in control group(P<0.05).Spearman correlation analysis showed that KPPS score was positively correlated with UPDRS Ⅲ(r=0.414,P<0.05).KPPS score was

Experimental investigation on the gravity driven discharge of cohesive particles from a silo with two outlets

An experimental study on the gravity driven discharge of cohesive particles from a silo with two outlets was performed.The discharge behaviors under the conditions that a single outlet was open and two outlets were open were investigated by varying the moisture content of the particles and the filling height of the particles in the silo.The results show that the discharge rate of the cohesive particles increases gradually at the beginning,then almost keeps constant,and finally drops obviously.The discharge rate in case of two openings is around 1.1–1.6 times that in case of a single opening.Larger filling height leads to lower discharge rate in case of a single opening but results in higher discharge rate in case of two openings.Furthermore,the avalanche dynamics in case of a single opening was examined,and the mixing behavior of the cohesive particles was evaluated.It is observed that the discharge flow is promoted by the avalanche phenomenon in the silo,generating a general trend that the normalized mass of discharge increases with the filling height at higher moisture contents.In case of a single opening,the transition from mass flow to funnel flow favors the particle mixing,resulting in an increasing mixing index as the moisture content increases.In general,a better performance of mixing can be achieved in case of a single opening compared with in case of two openings.This study provides vital information for fundamental understanding of the gravity driven discharge of cohesive particles from the silo with multiple outlets.

Latewood width(LWW)indices of trees are considered a reliable proxy of summer precipitation in the Northern Hemisphere.However,the strong coupling and high correlation between earlywood width(EWW)and LWW indices often prevent registration of climate signals of the LWW index.In this study,328-year-long earlywood width and latewood width chronologies were developed from Chinese pine at two sites in the Hasi Mountains,north central China.The climate responses of these chronologies were analyzed and the LWW index used to derive sum-mer precipitation signals.Correlation analyses showed that LWW was particularly influenced by earlywood growth and recorded stronger climate signals of the previous year as EWW,rather than those of the current year with infrequent summer climate signals.However,after removing the effect of earlywood growth using a simple regression model,the adjusted LWW chronology(LWW_(adj))showed a strong relationship with July precipitation in dry years.This suggests that the LWW_(adj) chronology has the potential to be used to investigate long-term variability in summer precipitation in drought-limited regions.

Zircon Hf Isotope Mapping for Understanding Crustal Architecture and Its Controls on Mineralization during Early Cretaceous in the Southern Great Xing'an Range,NE China

Voluminous Early Cretaceous granitoids and associated large-scale ore deposits are distributed within the southern Great Xing'an Range(SGXR),NE China.Based on previously published geochronology and zircon Hf-isotope data,Hf isotope mapping is undertaken to improve our understanding of crustal architecture and its controls on ore deposits.The ore-related Early Cretaceous granitoids were sourced predominantly from juvenile crust,with the involvement of variable proportions of ancient crustal materials.The crustal architecture,as inferred from Hf isotopic contour maps,indicates two distinct Hf isotopic domains in SGXR,including(1)a higher-ε_(Hf)(+7 to+11)juvenile crust containing minor ancient crustal material,and(2)a lower-ε_(Hf)(+2 to+6)juvenile crust containing a greater proportion of ancient crustal materials.The Hf isotopic maps identify links between crustal architecture and regional metallogeny.Copper deposits and other deposits with significant Cu production are restricted mainly to the higher-ε_(Hf)juvenile crustal regions in the northern and eastern SGXR.Deposits dominated by other metals(e.g.,Mo,Sn,W,Pb,Zn,and Ag)occur mainly in the lower-ε_(Hf)juvenile crustal regions in the southern and western SGXR.Interaction between juvenile crust-derived melts and ancient crustal components played an important role on the distribution of various ore metals.

Adenosine exerts its dual functions of homeostasis and neuromodulation in the brain by acting at mainly 2 G-protein coupled receptors,called A1 and A2A receptors.The adenosine A2A receptor(A2AR)antagonists have been clinically pursued for the last 2 decades,leading to final approval of the istradefylline,an A2AR antagonist,for the treatment of OFF-Parkinson's disease(PD)patients.The approval paves the way to develop novel therapeutic methods for A2AR antagonists to address 2 major unmet medical needs in PD and traumatic brain injury(TBI),namely neuroprotection or improving cognition.In this review,we first consider the evidence for aberrantly increased adenosine signaling in PD and TBI and the sufficiency of the increased A2AR signaling to trigger neurotoxicity and cognitive impairment.We further discuss the increasing preclinical data on the reversal of cognitive deficits in PD and TBI by A2AR antagonists through control of degenerative proteins and synaptotoxicity,and on protection against TBI and PD pathologies by A2AR antagonists through control of neuroinflammation.Moreover,we provide the supporting evidence from multiple human prospective epidemiological studies which revealed an inverse relation between the consumption of caffeine and the risk of developing PD and cognitive decline in aging population and Alzheimer's disease patients.Collectively,the convergence of clinical,epidemiological and experimental evidence supports the validity of A2AR as a new therapeutic target and facilitates the design of A2AR antagonists in clinical trials for disease-modifying and cognitive benefit in PD and TBI patients.

Association between the cumulative triglyceride-glucose index and the recurrence of atrial fibrillation after radiofrequency catheter ablation

BACKGROUND Triglyceride-glucose(TyG)index values are a new surrogate marker for insulin resistance.This study aimed to explore the relationship between cumulative TyG index values and atrial fibrillation(AF)recurrence after radiofrequency catheter ablation(RFCA).METHODS A total of 576 patients with AF who underwent RFCA at the Second Affiliated Hospital of Xi'an Jiaotong University were included in this study.The participants were grouped based on cumulative TyG index values tertiles within 3 months after ablation.Cox regression and restricted cubic spline analyses were used to determine the relationship between cumulative TyG index values and AF recurrence.The predictive value of all risk factors was assessed by receiver operating curve analysis.RESULTS There were 375 patients completed the study(age:63.23±10.73 years,64.27%male).The risk of AF recurrence increased with increasing cumulative TyG index values tertiles.After adjusting for potential confounders,patients in the medium cumulative TyG index group[hazard ratio(HR)=4.949,95%CI:1.778–13.778,P=0.002]and the high cumulative TyG index group(HR=8.716,95%CI:3.371–22.536,P<0.001)had a higher risk of AF recurrence than those in the low cumulative TyG index group.The restricted cubic spline regression model also showed an increased risk of AF recurrence with increasing cumulative TyG index values.When considering cumulative TyG index values,left atrial diameter,and lactate dehydrogenase levels as a comprehensive factor,the model could effectively predict AF recurrence after RFCA[area under the curve(AUC)=0.847,95%CI:0.797–0.897,P<0.001].CONCLUSIONS Cumulative TyG index values were a risk factor for AF recurrence after RFCA.Monitoring longitudinal TyG index values may assist with optimized for risk stratification and outcome prediction for AF recurrence.

Effect of surfactant frequently used in soil flushing on oxygen mass transfer in micro-nano-bubble aeration system

In-site soil flushing and aeration are the typical synergetic remediation technology for contaminated sites.The surfactant present in flushing solutions is bound to affect the aeration efficiency.The purpose of this study is to evaluate the effect of surfactant frequently used in soil flushing on the oxygen mass transfer in micro-nano-bubble(MNB)aeration system.Firstly,bio-surfactants and chemical surfactants were used to investigate their effects on Sauter mean diameter of bubble(dBS),gas holdup(ε),volumetric mass-transfer coefficient(kLa)and liquid-side mass-transfer coefficient(kL)in the MNB aeration system.Then,based upon the experimental results,the Sardeing's and Frossling's models were modified to describe the effect of surfactant on kL in the MNB aeration.The results showed that,for the twenty aqueous surfactant solutions,with the increase in surfactant concentration,the value of dBS,kLa and kL decreased,while the value ofεand gas-liquid interfacial area(a)increased.These phenomena were mainly attributed to the synergistic effects of immobile bubble surface and the suppression of coalescence in the surfactant solutions.In addition,with the presence of electric charge,MNBs in anionic surfactant solutions were smaller and higher in number than in non-ionic surfactant solutions.Furthermore,the accumulation of surfactant on the gas-liquid interface was more conspicuous for small MNB,so the reduction of kL in anionic surfactant solutions was larger than that in non-ionic surfactant solutions.Besides,the modified Frossling's model predicted the effect of surfactant on kL in MNB aeration system with reasonable accuracy.

Analysis of the influencing factors and clinical related characteristics of pulmonary tuberculosis in patients with type 2 diabetes mellitus

BACKGROUND In China,the prevalence of type 2 diabetes mellitus(T2DM)among diabetic patients is estimated to be between 90%-95%.Additionally,China is among the 22 countries burdened by a high number of tuberculosis cases,with approximately 4.5 million individuals affected by active tuberculosis.Notably,T2DM poses a significant risk factor for the development of tuberculosis,as evidenced by the increased incidence of T2DM coexisting with pulmonary tuberculosis(T2DMPTB),which has risen from 19.3%to 24.1%.It is evident that these two diseases are intricately interconnected and mutually reinforcing in nature.AIM To elucidate the clinical features of individuals diagnosed with both T2DM and tuberculosis(T2DM-PTB),as well as to investigate the potential risk factors associated with active tuberculosis in patients with T2DM.METHODS T2DM-PTB patients who visited our hospital between January 2020 and January 2023 were selected as the observation group,Simple DM patients presenting to our hospital in the same period were the control group,Controls and case groups were matched 1:2 according to the principle of the same sex,age difference(±3)years and disease duration difference(±5)years,patients were investigated for general demographic characteristics,diabetes-related characteristics,body immune status,lifestyle and behavioral habits,univariate and multivariate analysis of the data using conditional logistic regression,calculate the odds ratio(OR)values and 95%CI of OR values.RESULTS A total of 315 study subjects were included in this study,including 105 subjects in the observation group and 210 subjects in the control group.Comparison of the results of both anthropometric and biochemical measures showed that the constitution index,systolic blood pressure,diastolic blood pressure and lymphocyte count were significantly lower in the case group,while fasting blood glucose and high-density lipoprotein cholesterol levels were significantly higher than those in the control group.The results of univariate analysis showed that poo

Examining the characteristics between time and distance gaps of secondary crashes

Understanding the characteristics of time and distance gaps between the primary(PC)and secondary crashes(SC)is crucial for preventing SC ccurrences and improving road safety.Although previous studies have tried to analyse the variation of gaps,there is limited evidence in quantifying the relationships between different gaps and various influential factors.This study proposed a two-layer stacking framework to discuss the time and distance gaps.Specifically,the framework took random forests(RF),gradient boosting decision tree(GBDT)and eXtreme gradient boosting as the base classifiers in the first layer and applied logistic regression(LR)as a combiner in the second layer.On this basis,the local interpretable model-agnostic explanations(LIME)technology was used to interpret the output of the stacking model from both local and global perspectives.Through SC dentification and feature selection,346 SCs and 22 crash-related factors were collected from California interstate freeways.The results showed that the stacking model outperformed base models evaluated by accuracy,precision,and recall indicators.The explanations based on LIME suggest that collision type,distance,speed and volume are the critical features that affect the time and distance gaps.Higher volume can prolong queue length and increase the distance gap from the SCs to PCs.And collision types,peak periods,workday,truck involved and tow away likely induce a long-distance gap.Conversely,there is a shorter distance gap when secondary roads run in the same direction and are close to the primary roads.Lower speed is a significant factor resulting in a long-time gap,while the higher speed is correlated with a short-time gap.These results are expected to provide insights into how contributory features affect the time and distance gaps and help decision-makers develop accurate decisions to prevent SCs.
