目的:从非洲视角,探究中非卫生合作的态度、需求及主观影响因素,为深化中非卫生合作提供参考。方法:采用问卷调查法,对参加2018年中非卫生合作高级别会议的非洲政府官员、专家学者、医药企业代表等进行调查。结果:共回收有效问卷134份,接受调查对象来自35个非洲国家。超过90%的非洲代表对中非卫生合作表示肯定,81.5%的非洲代表所在单位具有合作意向,意向合作的重点领域是疾病防控(56.2%)、医学人才培养(55.2%)等;认为最有效的合作途径是通过政府协议或接洽(89.4%),影响中非卫生合作的主要因素是伙伴关系(87.3%);最容易开展卫生合作的国家依次为其他非洲国家、中国等(P<0.001);影响合作难易程度的因素是政府和民众的支持(P=0.001)、安全风险(P=0.029)。结论:中非卫生合作影响因素多元化,政府作用占主导,可通过加强顶层设计,增进伙伴关系,畅通合作渠道,加强产品质量与认证,提升合作有效性。

目的:分析和探索手法复位小夹板外固定结合中药治疗中老年桡骨远端骨折疗效。方法:收集2016年1月-2018年1月本院诊治的40例中老年桡骨远端骨折患者,分别采取手法复位及手法复位联合中药治疗,分析比较两者之间的疗效差异。结果:实验组水肿消退时间及骨折愈合时间明显低于对照组,P<0.05,差异有统计学意义。实验组的尺偏角平均变化低于对照组,P<0.05,差异有统计学意义。实验组术后6个月的腕关节优良率明显高于对照组,P<0.05,差异有统计学意义。结论:手法复位联合中药对桡骨远端骨折治疗具有显著的治疗效果,促进骨折愈合,恢复腕关节功能,具有重要的临床意义。

Background Transanal total mesorectal excision(taTME)or intersphincteric resection(ISR)has recently proven to be a valid and safe surgical procedure for low rectal cancer.However,studies focusing on the combination of these two technologies are limited.This study aimed to evaluate perioperative results,long-termoncologic outcomes,and anorectal functions of patients with low rectal cancer undergoing taTME combined with ISR,by comparing with those of patients undergoing laparoscopic abdominoperineal resection(laAPR).Methods After 1:1 propensity score matching,200 patients with low rectal cancer who underwent laAPR(n=100)or taTME combined with ISR(n=100)between September 2013 and November 2019 were included.Patient demographics,clinicopathological characteristics,oncological outcomes,and anal functional results were analysed.Results Patients in the taTME-combined-with-ISR group had less intraoperative blood loss(79.6672.6 vs 107.3665.1 mL,P=0.005)and a lower rate of post-operative complications(22.0%vs 44.0%,P<0.001)than those in the laAPR group.The overall local recurrence rates were 7.0%in both groups within 3 years after surgery.The 3-year disease-free survival rates were 86.3%in the taTME-combined-with-ISR group and 75.1%in the laAPR group(P=0.056),while the 3-year overall survival rates were 96.7%and 94.2%,respectively(P=0.319).There were 39 patients(45.3%)in the taTME-combined-with-ISR group who developed major low anterior resection syndrome,whereas 61 patients(70.9%)had good post-operative anal function(Wexner incontinence score≤10).Conclusion We found similar long-term oncological outcomes for patients with low rectal cancer undergoing laAPR and those undergoing taTME combined with ISR.Patients receiving taTME combined with ISR had acceptable post-operative anorectal function.

Lead halide perovskite quantum dots(PQDs)display remarkable photoelectric performance.However,defects such as weak stability in air and water environments limit the development of lead halide PQDs in solid-state light applications.Herein,centrifugal spinning is used for the fabrication of stable luminous CsPbBr3 PQD nanofibers.After immersion in water for 11 months,the PQD fibers still maintained considerable photoluminescence quantum yield,showing high stability in hostile environments.The water-stability mechanism of the fibers can be explained by the changing defect density,crystal growth of PQDs,and the molecular transformation at the fiber surface.The white LED based on the CsPbBr3 fibers exhibits satisfying color gamut performance(128%of National Television System Committee).Due to the short photoluminescence lifetime of CsPbBr3 PQDs,the communication potential is also considered.The CsPbBr3 fibers obtained by centrifugal spinning present a bandwidth of 11.2 MHz,showing promising performance for solid-state light and visible light communication applications.

Earth surface longwave radiation(SLR),including downward(DLR),upward(ULR),and net longwave radiation(NLR),significantly impacts the surface radiation budget and global climate evolution.However,the spatiotemporal variation in SLR remains poorly understood.In this study,three satellite products(GLASS-MODIS V40,GLASS-AVHRR,and CERES-SYN)and three reanalysis datasets(ERA5,MERRA-2,and GLDAS)were validated using ground measurements from 288 sites at seven observation networks.The mean biases and root mean square errors of the monthly DLR(ULR,NLR)estimates from the six products were-6.36(-3.56,-2.86)Wm^(-2) and 16.63(14.33,13.38)Wm^(-2),respectively.Large differences in the spatial distribution of the SLR were mainly observed at high-latitude,high-altitude and desert/barren-covered regions.Large interannual variability was detected at high latitudes.GLASS-AVHRR and ERA5 better captured the long-term variability in DLR and ULR,whereas GLASS-AVHRR and MERRA-2 better detected trends in NLR.An increasing trend in DLR and ULR was observed between 1982 and 2015,followed by a decreasing trend from 2016 to 2021;the NLR flux did not exhibit a significant trend.Overall,the GLASS-AVHRR and ERA5 SLR estimates were more accurate and stable than those of the,other products in accuracy,spatiotemporal distribution,and trend analysis.

Soil is a huge terrestrial carbon pool, which has higher carbon storage than the sum of atmospheric and terrestrial vegetation carbon. Small fluctuations in soil carbon pool can affect regional carbon flux and global climate change. As soil organic carbon plays key roles in soil carbon storage and sequestration, studying its composition, sources and stability mechanism is a key to deeply understand the functions of terrestrial ecosystem and how it will respond to climate changes. The recently-proposed concept of soil Microbial Carbon Pump(MCP) emphasizes the importance of soil microbial anabolism and its contributions to soil carbon formation and stabilization, which can be applied for elucidating the source, formation and sequestration of soil organic carbon. This article elaborates MCP-mediated soil carbon sequestration mechanism and its influencing factors, as well as representative scientific questions we may explore with the soil MCP conceptual framework.

Background:This study aimed to determine the reasons for conversion and elucidate the safety and efficacy of transition to tenofovir alafenamide/emtricitabine/bictegravir sodium(TAF/FTC/BIC)in highly active antiretroviral therapy(HAART)-experienced HIV-infected patients in real-world settings.Methods:We conducted a retrospective cohort study.The treatment conversion rationales,safety,and effectiveness in 1684 HIV-infected patients with previous HAART experience who switched to TAF/FTC/BIC were evaluated at Beijing Ditan Hospital from September 2021 to Auguest 2022.Results:Regimen simplification(990/1684,58.79%)was the most common reason for switching,followed by osteoporosis or osteopenia(375/1684,22.27%),liver dysfunction(231/1684,13.72%),decline in tenofovir alafenamide/emtricitabine/elvitegravir/cobicistat(TAF/FTC/EVG/c)with food restriction(215/1684,12.77%),virological failure(116/1684,6.89%),and renal dysfunction(90/1684,5.34%).In patients receiving non-nucleotide reverse transcriptase inhibitors(NNRTI)-containing regimens,lipid panel changes 1 year after switching indicated a difference of 3.27±1.10 mmol/L vs.3.40±1.59 mmol/L in triglyceride(P=0.014),4.82±0.74 mmol/L vs.4.88±0.72 mmol/L in total cholesterol(P=0.038),3.09±0.70 mmol/L vs.3.18±0.66 mmol/L in low-density lipoprotein(P<0.001),and 0.99±0.11 mmol/L vs.0.95±0.10 mmol/L in high-density lipoprotein(P<0.001).Conversely,among patients receiving booster-containing regimens,including TAF/FTC/EVG/c and lopinavir/ritonavir(LPV/r),lipid panel changes presented decreased trends.We also observed an improved trend in viral load suppression,and alanine transaminase(ALT),aspartate transaminase(AST),estimated glomerular filtration rate(eGFR),and serum creatinine levels after the transition(P<0.001).Conclusion:The transition to TAF/FTC/BIC demonstrated good treatment potency.Furthermore,this study elucidates the motivations behind the adoption of TAF/FTC/BIC in real-world scenarios,providing clinical evidence supporting the stable conversion to TAF/FTC/BIC for HAA

Mechanical Stimulus Inhibits the Growth of a Bone Tissue Model Cultured In Vitro

Objectives To construct the cancellous bone explant model and a method of culturing these bone tissues in vitro,and to investigate the effect of mechanical load on growth of cancellous bone tissue in vitro.Methods Cancellous bone were extracted from rabbit femoral head and cut into 1-mm-thick and8-mm-diameter slices under sterile conditions.HE staining and scanning electron microscopy were employed to identify the histomorphology of the model after being cultured with a new dynamic load and circulating perfusion bioreactor system for 0,3,5,and 7 days,respectively.We built a three-dimensional model using microCT and analyzed the loading effects using finite element analysis.The model was subjected to mechanical load of 1000,2000,3000,and 4000μεrespectively for 30 minutes per day.After 5 days of continuous stimuli,the activities of alkaline phosphatase(AKP)and tartrate-resistant acid phosphatase(TRAP)were detected.Apoptosis was analyzed by DNA ladder detection and caspase-3/8/9 activity detection.Results After being cultured for 3,5,and 7 days,the bone explant model grew well.HE staining showed the apparent nucleus in cells at the each indicated time,and electron microscope revealed the living cells in the bone tissue.The activities of AKP and TRAP in the bone explant model under mechanical load of 3000 and 4000μεwere significantly lower than those in the unstressed bone tissues(all P<0.05).DNA ladders were seen in the bone tissue under 3000 and 4000μεmechanical load.Moreover,there was significant enhancement in the activities of caspase-3/8/9 in the mechanical stress group of 3000 and 4000με(all P<0.05).Conclusions The cancellous bone explant model extracted from the rabbit femoral head could be alive at least for 7 days in the dynamic load and circulating perfusion bioreactor system,however,pathological mechanical load could affect the bone tissue growth by apoptosis in vitro.The differentiation of osteoblasts and osteoclasts might be inhibited after the model is stimulated by mechanical load of 3000 and 4000με.
