Off-pump sequential bilateral internal mammary artery grafting combined with selective arterialization of the coronary venous system

Background Off-pump coronary artery bypass surgery （OPCAB） has been widely applied in recent years as a less invasive method of myocardial revascularization. This study evaluated the sequential bilateral internal mammary artery grafting combined with selective arterialization of the coronary venous system during *** From April 2004 to August 2010, patients with diffuse right coronary lesions were studied retrospectively and divided into two groups. Group 1 included seventeen patients who underwent this surgery while group 2 included twenty-one patients without right coronary artery surgical therapy. All patients presented with symptoms of angina. Blood flow of bridged vessels was measured. The perioperative ventricular parameters including left ventricular ejection fraction and end diastolic diameter were compared. During follow-up, myocardial nuclide imaging and coronary angiography were carried *** Off-pump coronary artery bypass was performed with an average of 3.6 grafts per patient. Hospital mortality was zero. At the time of follow-up, the patients in group 1 recovered better than in group 2 （P〈0.05）. In both groups, the mean New York Heart Association （NYHA） class and ejection fraction increased significantly （P〈0.001） and the mean left ventricular end-diastolic diameter decreased significantly （P 〈0.05）. Myocardial blood supply of inferior wall in group 1 was obviously improved by myocardial nuclide imaging. Sulfur doped carbonaceous materials are promising anodes for potassium-ion batteries because of their ability to bridge active sites and induce C/S electron coupling,resulting in increased ion storage capacitance However,the large potassium ions could cause significant volume expansion and structure collapse during operation in sulfur doped carbonaceous anodes,which lead to rapidly capacity sacrifice during long-term *** design for anchoring sulfur atom in carbon skeleton is a novel way to alleviate the structure collapse and maintain the cycling ***,this study developed a controlled nanopore and sulfur doped carbon sphere structure(S-NPHCSs).In potassium-ion batteries S-NPHCSs anode demonstrated exceptional performance with a high reversible capacity of 247 mAh·g^(–1)after 50 cycles at 0.2 A·g^(–1)and delivered a long cycle stability of 600 cycles at a high current density of 1.0 A·g^(–1).Interconnected nanopores and doped sulfur structure not only expand the accumulation space and offer ample active sites for diffusion and adsorption of potassium ions,but also build stable channels through nanopore structure to ensure the cyclic *** finding provides a fundamental theory for designing nanopore structures and introducing sulfur doped carbonaceous materials to enhance capacitive potassium storage and long cycle stability.

Anteroapical aneurysm plication improves mechanical intraventricular dyssynchrony in patients with anterior myocardial infarction

Background Left ventricular （LV） dyssynchrony has been described to occur in patients with myocardial infarction. Dyssynchrony of left ventricular mechanical contraction produces adverse hemodynamic consequences. This study aimed to test the capacity of geometric rebuilding by aneurysm plication to restore a more synchronous contractile pattern after a mechanical, rather than electrical, intervention. Methods A total of sixty patients with anterior myocardial infarction, QRS duration 〈120 ms, electively undergoing operation between January 2008 and January 2010 were included for analysis. Real-time 3-dimensional echocardiography was performed to assess LV function, LV systolic and diastolic dyssynchrony by measuring ejection fraction （EF）, peak ejection rate （PER）, peak filling rate （PFR） and LV dyssynchrony. LV dyssynchrony was defined as the systolic dyssynchrony of the time to reach the minimum systolic volume for 16 LV segments, expressed in percent cardiac cycle, systolic dyssynchrony index （SDI）. We compared changes of LV dyssynchrony at different interval times. Results LV contraction was significantly asynchronous because preoperative SDI was higher, EF, PER and PFR were lowered. Compared with function after operation, LV mechanical intraventricular resynchronization was improved with decreased SDI （（8.7_＋0.5） % vs. （14.3_＋1.6） %, P=0.01）; LV function was improved with EF increasing （（43_＋9）% vs. （37_＋7）%, P=0.001）, and LV systolic and diastolic dyssynchrony was improved with more rapid PFR （199.4_＋15.6 vs. 148.4_＋21.2, P=0.002） and PER （212.4_＋14.5 vs. 156.3_＋_26.2, P=0.001）. Conclusions Systolic and diastolic dyssynchrony was highly prevalent in patients with aneurysm, irrespective of QRS duration. Aneurvsm olication oroduces a mechanical *** rP.~***.

Off-pump anteroapical aneurysm plication for left ventricular post-infarction aneurysm： long-term results

Background The temporal response to off-pump anteroapical aneurysm plication has not been well defined. We have evaluated the long-term clinical and functional results of this technique and compared the efficacy with the patch modeling repair. Methods From March 2005 to May 2010, 163 （115 men and 48 women） consecutive patients were operated on for post-infarction left ventricular aneurysm （LVA）, 54 patients underwent patch remodeling （group A） and 109 patients underwent off-pump anteroapical aneurysm plication repair （group B）. All patients had simultaneous coronary revascularization, other operative procedures included septoplasty in eight and ablation of ventricular tachycardia in six. Follow-up ranged from 1 to 5 years, short-term and mid-term outcomes, including complications, cardiac function, and mortality, were assessed. Results Early mortality was 1.8% for all patients （group A 1 death vs. group B 2 deaths, not significant （NS））. Peri-operative support included intraaortic balloon pumping in 16 （9.8%）, （group A 6 patients vs. group B 10 patients, NS） and inotropic drugs in 84 （51.5%）, （group A 34 vs. group B 50, NS）. During a mean follow-up of （3.7±0.8） years, eight patients died, with four due to cardiac-related causes. No patient required transplantation, and two required use of an implantable cardioverter-defibrillator for ventricular tachycardia. Survival at 1 and 5 years was 95% and 86%, respectively. It did not differ significantly between group A and group B. Functional class improved from 2.90±0.59 to 1.65±0.54 among the mid-term survivors （P 〈0.001）, with no significant difference between the two groups. Pre-operative risk factors for mortality or poor function were ejection fraction （EF）〈0.35 （OR=7.9, 95% CI 1.6-40.0）; congestive heart failure （CHF）（OR=4.4, 95% CI 1.0-19.0）; end-systolic volume index （ESVI）〉80 ml/m2 （OR=3.7, 95% CI 1.0-14.0）; and advanced age 〉70 years （OR=2.4, 95% CI 1.0-12.0）.

To verify the performance of the neutron total cross-sectional spectrometer, the neutron total cross section of carbon is initially measured in the energy range of 1 eV to 20 MeV using the time-of-flight method. The measurement is performed at the Back-n white neutron source with a 76-m time-of-flight path using the China Spallation Neutron Source. A multilayer fast fission chamber with 235U and 238U is employed as the neutron detector. The diameter and thickness of the natural graphite sample are 70 mm and 40 mm, respectively. Signal waveforms are collected using a data acquisition system. Off-line data processing was used to obtain the neutron time-of-flight spectra and transmissions. The uncertainty of the counting statistics is generally approximately 3% for each bin in the energy range of 1–20 MeV. It is determined that the results for the neutron total cross section of carbon obtained using ^235U cells are in good agreement with the results obtained using 238U cells within limits of statistical uncertainty. Moreover, the measured total cross sections show good agreement with the broadening evaluated data.

The ^(232)Th(n,f)cross section is very important in basic nuclear physics and applications based on the Th/U fuel *** the time-of-flight method and a multi-cell fast-fission ionization chamber,a novel measurement of the^(232)Th(n,f)cross sec-tion relative to^(235)U in the 1–200 MeV range was performed at the China Spallation Neutron Source Back-n white neutron source(Back-n).The fission event-neutron energy spectra of^(232)Th and^(235)U fission cells were measured in the single-bunch *** 232Th/235U fission cross-sectional ratios were obtained,and the measurement uncertainties were 2.5–3.7%for energies in the 2–20 MeV range and 3.6–6.2%for energies in the 20–200 MeV ***^(232)Th(n,f)cross section was obtained by introducing the standard cross section of^(235)U(n,f).The results were compared with those of previous theoreti-cal calculations,measurements,and *** measured 232Th fission cross section agreed with the main evaluation results in terms of the experimental uncertainty,and 232Th fission resonances were observed in the 1–3 MeV *** present results provide^(232)Th(n,f)cross-sectional data for the evaluation and design of Th/U cycle nuclear systems.

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P<0.001).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

Back-streaming neutrons from the spallation target of the China Spallation Neutron Source(CSNS)that emit through the incoming proton channel were exploited to build a white neutron beam facility(the so-called Back-n white neutron source),which was completed in March *** Back-n neutron beam is very intense,at approximately 29107 n/cm2/s at 55 m from the target,and has a nominal proton beam with a power of 100 kW in the CSNS-I phase and a kinetic energy of 1.6 GeV and a thick tungsten target in multiple slices with modest moderation from the cooling water through the *** addition,the excellent energy spectrum spanning from 0.5 eV to 200 MeV,and a good time resolution related tothe time-of-flight measurements make it a typical white neutron source for nuclear data measurements;its overall performance is among that of the best white neutron sources in the *** with advanced spectrometers,detectors,and application utilities,the Back-n facility can serve wide applications,with a focus on neutron-induced cross-sectional *** article presents an overview of the neutron beam characteristics,the experimental setups,and the ongoing applications at Backn.
