Clinical efficacy and pathological outcomes of transanal endoscopic intersphincteric resection for low rectal cancer

BACKGROUND Transanal endoscopic intersphincteric resection(ISR)surgery currently lacks sufficient clinical research and reporting.AIM To investigate the clinical effectiveness of transanal endoscopic ISR,in order to promote the clinical application and development of this technique.METHODS This study utilized a retrospective case series design.Clinical and pathological data of patients with lower rectal cancer who underwent transanal endoscopic ISR at the First Affiliated Hospital of Xiamen University between May 2018 and May 2023 were included.All patients underwent transanal endoscopic ISR as the surgical approach.We conducted this study to determine the perioperative recovery status,postoperative complications,and pathological specimen charac-teristics of this group of patients.RESULTS This study included 45 eligible patients,with no perioperative mortalities.The overall incidence of early complications was 22.22%,with a rate of 4.44%for Clavien-Dindo grade≥III events.Two patients(4.4%)developed anastomotic leakage after surgery,including one case of grade A and one case of grade B.Postoperative pathological examination confirmed negative circumferential resection margins and distal resection margins in all patients.The mean distance between the tumor lower margin and distal resection margin was found to be 2.30±0.62 cm.The transanal endoscopic ISR procedure consistently yielded high quality pathological specimens.CONCLUSION Transanal endoscopic ISR is safe,feasible,and provides a clear anatomical view.It is associated with a low incidence of postoperative complications and favorable pathological outcomes,making it worth further research and application.

Cuproptosis,a novel mechanism of programmed cell death,has not been fully explored in the context of spermatogenic cells.Thisstudy investigated the potential involvement of cuproptosis in spermatogenic cell death using a mouse model of copper overload.Sixty male Institute of Cancer Research(ICR)mice were randomly divided into four groups that received daily oral gavage withsodium chloride(control)or copper sulfate(CuSO4)at 50 mg kg−1,100 mg kg−1,or 200 mg kg−1,for 42 consecutive days.Micesubjected to copper overload exhibited a disruption in copper homeostasis.Additionally,significant upregulated expression of keycuproptosis factors was accompanied by a significant rise in the rates of testicular tissue cell apoptosis.Immunohistochemicalanalysis revealed the presence of ferredoxin 1(Fdx1)in Sertoli cells,Leydig cells,and spermatogenic cells at various stages oftesticular development,and the Fdx1-positive staining area was significantly increased in copper-overloaded mice.Mitochondrialdysfunction and decreased adenosine triphosphate levels were also observed,further implicating mitochondrial damage undercuproptosis.Further analyses revealed pathological lesions and blood−testis barrier destruction in the testicular tissue,accompaniedby decreased sperm concentration and motility,in copper-overloaded mice.In summary,our results indicate that copper-overloadedmice exhibit copper homeostasis disorder in the testicular tissue and that cuproptosis participates in spermatogenic cell death.These findings provide novel insights into the pathogenic mechanisms underlying spermatogenic cell death and provide initialexperimental evidence for the occurrence of cuproptosis in the testis.

First Digit Distributions of Gamma-Ray Bursts

The occurrence of the first significant digits from real world sources is usually not equally distributed,but is consistent with a logarithmic distribution instead,known as Benford’s law.In this work,we perform a comprehensive investigation on the first digit distributions of the duration,fluence,and energy flux of gamma-ray bursts (GRBs) for the first time.For a complete GRB sample detected by the Fermi satellite,we find that the first digits of the duration and fluence adhere to Benford’s law.However,the energy flux shows a significant departure from this law,which may be due to the fact that a considerable part of the energy flux measurements is restricted by lack of spectral information.Based on the conventional duration classification scheme,we also check if the durations and fluences of long and short GRBs (with duration T_(90)>2 s and T_(90)≤2 s,respectively) obey Benford’s law.We find that the fluences of both long and short GRBs still agree with the Benford distribution,but their durations do not follow Benford’s law.Our results hint that the long–short GRB classification scheme does not directly represent the intrinsic physical classification scheme.

In this paper,we consider the derivatives of intersection local time for two independent d-dimensional symmetricα-stable processes X^(α) and X^(α)with respective indices α and α.We first study the sufficient condition for the existence of the derivatives,which makes us obtain the exponential integrability and H?lder continuity.Then we show that this condition is also necessary for the existence of derivatives of intersection local time at the origin.Moreover,we also study the power variation of the derivatives.

Game theory studies the mathematical models for self-interested individuals. Nash equilibrium is arguably the most central solution in game theory. While finding the Nash equilibrium in general is known as polynomial parity arguments on directed graphs(PPAD)-complete, learning in games provides an alternative to approximate Nash equilibrium, which iteratively updates the player's strategy through interactions with other players. Rules and models have been developed for learning in games, such as fictitious play and no-regret learning. Particularly, with recent advances in online learning and deep reinforcement learning,techniques from these fields greatly boost the breakthroughs in learning in games from theory to application.As a result, we have witnessed many superhuman game AI systems. The techniques used in these systems evolve from conventional search and learning to purely reinforcement learning(RL)-style learning methods,gradually getting rid of the domain knowledge. In this article, we systematically review the above techniques,discuss the trend of basic learning rules towards a unified framework, and recap applications in large games.Finally, we discuss some future directions and make the prospect of future game AI systems. We hope this article will give some insights into designing novel approaches.

Although lithium-sulfur batteries are one of the favorable candidates for next-generation energy storage devices,a few key challenges that have not been addressed have limited its commercialization.These challenges include lithium dendrite growth in the anode side,volume change of the active material,poor electrical conductivity,dissolution and migration of poly sulfides,and slow rate of solid-state reactions in the cathode side.Since the electrochemical performance of lithium-sulfur batteries is greatly affected by the design of the cathode host material,it has also been widely discussed in addressing the abovementioned issues.In this paper,three design ideas of cathode host materials in terms of microstructure,crystal structure and electronic structure are introduced and summarized.Crucially,the current progress of these three structural design strategies and their effects on the electrochemical performance of lithium-sulfur batteries are discussed in detail.Finally,future directions in the structural design of cathode materials for lithium-sulfur batteries are discussed and further perspectives are provided.

Selective adsorption of tetracycline by b-CD-immobilized sodium alginate aerogel coupled with ultrafiltration for reclaimed water

In this work,a novel composite material based on b-cyclodextrin-immobilized sodium alginate aerogel(b-CD/NaAlg)was developed utilizing cross-linker of epichlorohydrin and applied as an adsorbent to remove tetracycline antibiotics from reclaimed wastewater.A series of characterizations were utilized to confirm the successful synthesis of the adsorbent and this b-CD/NaAlg presented a three-dimensional network at the nanoscale or microscale.Under optimal conditions(pH=4,t=8 h,b-CD:NaAlg=9,adsorbent dosage=1.5 g·L^(-1)),the maximum removal rate of b-CD/NaAlg to tetracycline was 70%.The adsorption behavior of tetracycline on b-CD/NaAlg conformed to the Freundlich isotherm model(R^(2)=0.9977)and the pseudo-second-order kinetic model(R^(2)=0.9993).Moreover,the adsorbent still removed 55.3%of tetracycline after five cycles.Specially,the adsorbent was integrated with ultrafiltration to adsorb tetracycline antibiotics from simulated reclaimed wastewater,and the removal rate of tetracycline reached 78.9%within 2 h.The existence of Cr(VI)had a negligible impact on tetracycline removal,while the presence of humic acid exhibited a promoting effect.The possible adsorption mechanisms were also elucidated through X-ray photoelectron spectroscopy and density functional theory analysis.In summary,b-CD/NaAlg represents an environmentally friendly,efficient,and sustainable adsorbent for removing tetracycline antibiotics from reclaimed water.

BACKGROUND We report a low-birth-weight child(1.8 kg)with neonatal type III congenital esophageal atresia(CEA)combined with symptomatic patent ductus arteriosus(PDA).After comprehensive evaluation,esophageal anastomosis was performed on postnatal day 11 after excluding surgical contraindications,and arterial catheter ligation was performed at the same time.Concurrent surgery for CEA combined with PDA has not been clearly reported in the literature.CASE SUMMARY We report a 6-day-old female child with type III CEA and PDA.The patient presented with foam at the mouth after birth,cough and shortness of breath after feeding.At another hospital,she was considered to have neonatal pneumonia,neonatal jaundice and congenital heart disease and transferred to our hospital.After iodine oil radiography of the esophagus and echocardiography we con-firmed diagnosis of CEA and PDA.The diameter of the PDA was 8 mm,with obvious left to right shunting.We performed right rear extrapleural orificium fistula ligation and esophageal anastomosis,and ligation of PDA via left axilla straight incision after 5 d of hospitalization.The operations were successful,and the incision healed after 12 d,and the patient was discharged.We re-examined the patient 1 mo after surgery.She did not vomit when she ate rice flour.Esophageal angiography showed no stricture of the anastomotic stoma.The patient weighed 3.2 kg.CONCLUSION For CEA patients with multiple risk factors,comprehensive,timely and accurate diagnosis and evaluation,and early treatment may improve prognosis.
