Clinical Outcome of Autologous Hematopoietic Stem Cell Infusion via Hepatic Artery or Portal Vein in Patients with End-stage Liver Diseases

Objective To investigate the efficacy of hematopoietic stem cell （HSC） transplantation via the hepatic artery vs. the portal vein for end-stage liver disease （ESLD）. Methods Patients with hepatic decompensation were prospectively recruited from September 2010 to September 2012 to receive HSC transplantation via the hepatic artery or the portal vein. Liver function was examined at 3, 6, and 12 months after transplantation. Liver biopsy results were analyzed using the Knodell score. Results Eighty patients （58 males and 22 females） were enrolled in the study. The Child-Pugh score was grade B in 69 cases, and grade C in the remaining 11 cases. HSC transplantation was performed via the portal vein in 36 patients and via the hepatic artery in 44 patients. ALT levels decreased while serum albumin levels increased significantly in both groups at 6 and 12 months after HSC transplantation （P〈0.05 compared with pre-transplantation levels）. Total biliruhin levels decreased significandy in both groups at 3, 6, and 12 months after HSC transplantation （P〈0.05 compared with pre-transplantation levels）. Additionally, prothromhin time decreased in both groups at 12 months after HSC transplantation （P〈0.05 compared with pre-transplantation level）. There were no significant differences in ALT, total bilirubin and prothromhin time between the two groups either before or after transplantation. Moreover, Knodell score decreased significantly at 6 and 12 months. Histological examination showed that liver cell edema, degeneration, necrosis, and inflammation were significantly relieved at 3, 6, and 12 months after transplantation. The incidence of portal vein thrombosis, upper gastrointestinal bleeding, and hepatic encephalopathy were 1.25%, 3.75%, and 2.5% respectively. The one-year survival rate was 100%. 为了探究熔铸炸药内部的微观结晶组织形貌,提高炸药柱的成型质量,采用ProCAST铸造模拟软件中的CA?FE模块对TNT熔铸炸药内部结晶组织进行了数值模拟。利用CA?FE模块中的元胞自动机,预测了不同工艺条件与TNT形核参数下药柱内部晶粒的形貌和尺寸,并通过调整熔体的形核过冷度、浇注温度和冷却速度,得出了晶粒细化工艺改进措施。结果表明:随着熔体的体形核过冷度的减小,药柱内的等轴晶区逐渐增大,晶粒平均尺寸由715.5μm减小到458.5μm;随着熔体的面形核过冷度减小,药柱内部晶粒形貌没有明显变化,平均尺寸由715.5μm增加到719.2μm;随着熔体浇注温度的降低,药柱内部等轴晶区逐渐增大,晶粒平均尺寸由1114.5μm减小到729.2μm;随着冷却速度的降低,药柱内部中心等轴晶区逐渐扩大,外层等轴晶区逐渐消失,晶粒平均尺寸由719.4μm增大到1149.1μm。工艺改进后,药柱内全部由细密等轴晶组成,晶粒平均尺寸为516.9μm,尺寸分布较为集中,药柱内部微观质量有较大改善。

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

针对射频识别(RFID)标签应用,采用SMIC.18 RF CMOS工艺设计了一种温度传感器。利用MOS管迁移率和阈值电压与温度的关系设计了延迟时间与温度相关的延迟单元,进而产生与温度相关的脉冲信号,并通过后续电路对脉冲采样计数,将温度信息转换成数字信号供RFID标签数字模块使用。仿真测试结果表明:当温度为-20～80℃时,温度传感器精度为1℃。标签模拟电路的总功耗电流为6μA。

基于IBM 0.18μm SiGe BICMOS工艺,采用温度脉冲转换方式设计了一种应用于无源RFID标签的温度传感器。与绝对温度呈正比(PTAT)的电流源和电流饥饿环型振荡器产生频率与温度呈正相关的振荡信号,作为计数器的时钟信号;用数字模块对接收的帧头代码进行处理得到一个宽度为200μs的脉冲信号,作为计数器的使能信号;利用时域数字量化方式就可以得到不同温度下的数字信号。温度传感器总面积为0.03 mm2,温度在-100~120℃范围内变化时,振荡器输出频率范围由800 kHz^1.8 MHz。在1.8 V电源电压下,温度传感器平均输出电流约为13μA,芯片测试结果的有效分辨率可以达到0.864 LSB/℃。

基于IME标准Ge-on-SOI工艺提出了一种全差分光电探测器结构,由定向耦合偏振分束器与锗垂直PIN光电探测器组成,可将一路由单模光纤输入的高速1550nm光信号转换为一对高速差分电信号。分析并设计了具体结构,进行了器件模拟,模拟得到定向耦合偏振分束器TE波与TM波的消光比分别为-16.7dB与-15.7dB,锗垂直PIN光电的响应度与带宽分别为0.42A/W与6.51GHz。

BACKGROUND Whether regional lymphadenectomy(RL)should be routinely performed in patients with T1b gallbladder cancer(GBC)remains a subject of *** To investigate whether RL can improve the prognosis of patients with T1b *** We studied a multicenter cohort of patients with T1b GBC who underwent surgery between 2008 and 2016 at 24 hospitals in 13 provinces in *** logrank test and Cox proportional hazards model were used to compare the overall survival(OS)of patients who underwent cholecystectomy(Ch)+RL and those who underwent Ch *** investigate whether combined hepatectomy(Hep)improved OS in T1b patients,we studied patients who underwent Ch+RL to compare the OS of patients who underwent combined Hep and patients who did *** Of the 121 patients(aged 61.9±10.1 years),77(63.6%)underwent Ch+RL,and 44(36.4%)underwent Ch ***(9.1%)patients in the Ch+RL group had lymph node *** 5-year OS rate was significantly higher in the Ch+RL group than in the Ch group(76.3%vs 56.8%,P=0.036).Multivariate analysis showed that Ch+RL was significantly associated with improved OS(hazard ratio:0.51;95%confidence interval:0.26-0.99).Among the 77 patients who underwent Ch+RL,no survival improvement was found in patients who underwent combined Hep(5-year OS rate:79.5%for combined Hep and 76.1%for no Hep;P=0.50).CONCluSION T1b GBC patients who underwent Ch+RL had a better prognosis than those who underwent ***+Ch showed no improvement in prognosis in T1b GBC *** recommended by both the National Comprehensive Cancer Network and Chinese Medical Association guidelines,RL was only performed in 63.6%of T1b GBC *** Ch+RL should be advised in T1b GBC.

血吸虫性肝病合并戊型肝炎老年患者36例临床分析

目的探讨老年血吸虫性肝病合并戊型肝炎老年患者的临床特点。方法将嘉兴市第一医院36例血吸虫性肝病合并戊型肝炎老年患者(观察组)与38例单纯戊型肝炎老年患者(对照组)的血常规、肝功能、凝血功能和肝纤维化血清学指标检测结果,及B超测量肝门静脉内径和脾门区脾静脉内径的结果,结合其临床表现、病程和转归情况等进行分析。结果 74例老年戊型肝炎均为急性起病。血吸虫性肝病合并感染戊型肝炎患者,病情重,易重症化。观察组患者恶心(91.7%,33/36)、呕吐(88.9%,32/36)及皮肤瘙痒(33.3%,12/36)等症状的发生率较对照组患者高(P<0.05)。入院时观察组的平均总胆红素(TBIL)和丙氨酸转氨酶(ALT)水平分别为(153.22±29.71)μmol/L和(705.14±356.55)U/L均显著高于对照组[分别为(109.89±26.28)μmol/L和(485.74±297.49)U/L](P<0.01或P<0.05),而白蛋白(ALB)、凝血酶原活动度(PTA)、血白细胞、血红蛋白和血小板则均显著低于对照组(P<0.01)。观察组的4个肝纤维化血清学指标,即透明质酸(HA)、层黏连蛋白(LN)、血清Ⅲ型前胶原肽(PⅢ)和Ⅳ型胶原(Ⅳ-C)均显著高于对照组(P<0.01)。此外,在B超下测量观察组患者的肝门静脉内径和脾门区脾静脉内径,分别为(16.56±3.38)mm和(10.28±3.31)mm,均显著大于对照组[分别为(14.82±3.48)mm和(8.26±3.27)mm](P<0.01)。观察组患者的肝功能恢复慢,住院时间[(43.7±7.1)d]较对照组[(29.5±5.1)d]显著延长(P<0.01)。结论血吸虫性肝病合并戊型肝炎的老年患者肝脏损害较单纯戊型肝炎老年患者严重,需加强监护与治疗。
