Current treatment status in patients with severe aortic valve stenosis and outcome of long term follow-up at advanced age： a Chinese single center study

Background Surgical aortic valve replacement is the standard treatment for patients with severe aortic stenosis, but some registries have indicated that 30% to 60% of these patients are not treated surgically, usually due to advanced age and/or comorbidities. This single center study in China investigated the current treatment status in the patients with severe aortic stenosis and evaluated the long term clinical outcome in advanced age patients whether or not undergoing aortic valve replacement. Methods Clinical data of 867 consecutive patients with severe aortic stenosis between January 2000 and December 2006 were retrospectively analyzed. The patients 〉65 years old were followed up by telephone or information from medical records. The primary end-point was all-cause mortality. Results The patients＇ average age was （52±19） years （range, 1-91 years）, and 34% were women. The percentages of the patients aged 〈15 years, between 15 and 34 years, between 35 and 54 years, between 55 and 64 years, between 65 and 74 years, and 〉75 years who underwent surgical aortic valve replacement were 82.3%, 87.2%, 88.8%, 78.2%, 65.3% and 22.2% respectively. In the patients （n=256）〉65 years old, 43.4% had New York Heart Association class III and IV symptoms, 39.1% had hypertension, 33.2% had coronary heart disease, and 3.1% had stroke. In the patients not undergoing aortic valve replacement, 1.6% had renal insufficiency, 4.7% had chronic obstructive pulmonary disease, 2.0% had critical hematopathy, and 0.4% had mammary cancer. A total of 186 （72.7%） patients finished the follow-up, and the average duration of the follow-up was （60±2.6） months. In the patients between 65 and 74 years old, the total deaths and cardiac deaths in the patients undergoing aortic valve replacement decreased significantly compared with those with conservative treatment （10.3% vs. 53.7%, P 〈0.001 and 6.3% vs. 50.7%, P〈0.001）. COMPARISON OF SHORT- AND MID-TERM OUTCOMES BETWEEN CYPHER AND TAXUS STENTS IN PATIENTS WITH COMPLEX LESIONS OF THE CORONARY ARTERIES

Background Drug-eluting stent （DES） could obviously reduce in-stent restenosis, which has been proved by international multi-center clinical trials. However, the types of the lesions for stenting were highly selected in these trials. Up to now, there has been no large scale study on the effect of DES in treating complex lesions in real world. Although REALITY trial was just reported during American College of Cardiology Congress 2005, the entry criteria for lesions were limited to one or two de novo lesions. This study was conducted to compare the short- and mid-term clinical outcomes between sirolimus-eluting stent （CYPHER stent） and paclitaxel-eluting stent （TAXUS stent） in patients with complex lesion. Methods This is a retrospective study. From April 2002 to June 2004, a total of 1061 patients were treated with DES in Fu Wai Hospital, of which, 611 patients （642 lesions with 698 CYPHER stents） were in CYPHER group, and 450 patients （534 lesions with 600 TAXUS stents） were in TAXUS group. There was no significant difference in clinical data and lesion types between CYPHER group and TAXUS group. Results Success rates of stent implantation were 99.2% and 98.8% in CYPHER and TAXUS stent groups respectively. The major adverse cardiac events （MACE） during in-hospital and 6-8-month follow-up were 0.7% and 2.3% in CYPHER stent group versus 1.3% and 3.2% in TAXUS stent group. There was no significant difference in MACE rate between these two groups. Restenosis rate was a little higher in TAXUS stent group than that in CYPHER stent group （14.0% vs 7.3%）, but there was no significant difference. The incidence of acute occlusion of side branch after implanting DES in main vessel was 6.9% in CYPHER group and 11.9% in TAXUS group （P〈0.05） . Conclusions CYPHER and TAXUS DES were safe and effective in patients with complex lesion. Clinical outcomes of CYPHER stent were better than TAXUS stent in bifurcation lesions. ST-elevated acute myocardial infarction happening 1 month post stent implantation： late thrombosis in-stents or new lesions？

Background ST-elevated acute myocardial infarction （STEAMI） happening in the first month post percutaneous coronary intervention （PCI） is almost related to acute thrombosis or subacute thrombosis in-stents. This study aimed to investigate the possible causes of myocardial infarction one month later. Methods Patients who had a history of successful PCI, and received coronary angiography or re-PCI due to STEAMI were included in this study. The AMI-related lesions and previous angiographic findings such as the number of lesions, the degree of the stenosis, the type of stents and acute results of last PCI were recorded. If the AMI-related lesion was localized in-stents or at the edge of stents （distance apart from the edge 〈5 mm）, it was defined to be late thrombosis; otherwise as a new-lesion induced AMI. Results One hundred and ninety-two patients aged 40-79 years were included in this study. New lesions, as the cause of STEAMI, were found in 144 patients （Group A, 75%）, and late thrombosis in 48 patients （Group B, 25%）. Almost all newly built thromboses were found at the sites of previous insignificant lesions （diameter stenosis 〈50%）. There was a significant difference in the average time from previous PCI to AMI （（30.1±12.4） vs （20.3±11.9） months） between the two groups. Diabetes mellitus （DM） and drug-eluting stent （DES） utilization were associated with markedly higher morbidity of late thrombosis in adjusted Logistic regression （hazard ratio （HR） 3.4, 95% confidence interval （C/） 1.1-10.9 and 5.3, 95% Cl 1.1-26.5）. Conclusions STEAMIs happening 1 month after PCI are more likely to develop from previous insignificant lesion rupture than from late thrombosis in-stents. Moreover, DM and DES are associated with the high incidence of late thrombosis, which may indicate that intensive antiplatelet therapy should be considered in patients with diabetes.

Comparison of immediate and followup results between transradial and transfemoral approach for percutaneous coronary intervention in true bifurcational lesions

Background A comparison of efficacy and safety between transradial and transfemoral approach for percutaneous coronary intervention （PCI） in bifurcations has not been done. This study evaluated feasibility of transradial PCI （TRI） and compared the immediate and followup results with transfemoral PCI （TFI） in bifurcations. Methods One hundred and thirty-four consecutive patients with bifurcations were treated with PCI in our hospital from April 2004 to October 2005. Of these, there were 60 patients （88 lesions） in TRI group and 74 patients （101 lesions） in TFI group. Bifurcations type was classified according to the Institut Cardiovasculaire Paris Sud Classification. Results TRI group had smaller stent diameter （（3.06±0.37） mm vs （3.18±0.35） mm, P=0.023） and postprocedural in-stent minimum lumen diameter （（2.62±0.37） mm vs （2.74±0.41） mm, P=0.029） than TFI, but there were not significant differences in in-stent subacute thrombosis rate （0% vs 1.0%, P=0.349）, target lesion revascularization （TLR）（0% vs 1.0%, P=0.349） following procedure and thrombosis （2.3% vs 1.0%, P=0.482）, in-stent restenosis （12.5% vs 10.9%, P=0.731）, in-segment restenosis （17.0% vs 14.9%, P=0.681）, TLR （10.2% vs 13.9%, P=0.446） and TLR-free cumulative survival rate （89.8% vs 86.1%, P=0.787） at seven months followup. No death was reported in the two groups. Conclusion Transradial intervention is feasible and appears to be as effective and safe as transfemoral PCI in treatment of true bifurcational lesions.

The perturbation in the magnetic field generated by the rotation or oscillation of magnetic domains in magnetic materials can emit low-frequency electromagnetic waves,which are expected to be used in low-frequency ***,the magnetic emission intensity,defined by the perturbation ability,of current commercially applied amorphous alloys,such as Metglas,cannot meet the application requirements for low-frequency antennas due to the domain motion energy ***,a multi-phase Metglas amorphous alloy was constructed by incorporatingα-Fe nanocrystals using rapid annealing to manipulate the domain *** was found that 3.89 times higher magnetic emission intensity is obtained compared to the pristine due to the synergism of the deformation and displacement ***,the low-frequency magnetic emission performance verification was carried out by preparing magnetoelectric composites as the antenna vibrator by assembling the alloy and macro piezoelectric fiber composites(MFC).Enhancements of magnetic emission intensity are found at 93.3%and 49.2%at the first and second harmonic frequencies compared with the unmodified alloy ***,the approach leads to the development of high-performance communication with a novel standard for evaluation.

Effect of TiO_(2) addition on microstructures and properties of Mgo-Cao refractory aggregates

The removal of a failed implant with high torque causes significant damage to the surrounding tissue,compromising bone regeneration and subsequent osseointegration in the defect ***,we report a case of carrier screw fracture followed by immediate implant removal,bone grafting and delayed reimplantation.A dental implant with a fractured central carrier screw was removed using the bur-forceps *** resulting three-wall bone defect was filled with granular surface demineralized freeze-dried bone allograft(SD-FDBA).Cone-beam computerized tomography was performed at 1week,6months and 15months postoperatively and standardized for quantitative *** alveolar bone width and height at 15months post-surgery were about 91%of the original values,with a slightly lower bone density,calculated using the gray value *** graft site was reopened and was found to be completely healed with dense and vascularized bone along with some residual bone *** followed by restoration was performed 8months *** quality of regenerated bone following SD-FDBA grafting was adequate for osseointegration and longterm implant *** excellent osteogenic properties of SD-FDBA are attributed to its human origin,cortical bone-like structure,partly demineralized surfaces and bone morphogenetic protein-2-containing *** investigation with more cases and longer follow-up was required to confirm the final clinical effect.

Extreme waves have a profound impact on coastal infrastructure;thus,understanding the variation law of risky analysis and disaster prevention in coastal zones is *** paper analyzed the spatiotemporal characteristics of extreme wave heights adjacent to China from 1979 to 2018 based on the ERA5 *** extreme value analysis is undertaken in eight repre-sentative points to investigate the trends in the values of 50-and 100-year wave *** show that the mean value of extreme waves is the largest in the eastern part of Taiwan Island and the smallest in the Bohai Sea from 1979 to *** the extreme wave height in the northeastern part of Taiwan Island shows a significant increase trend in the study *** analysis shows remarkable variations in the values of 50-and 100-year significant wave heights in eight *** the annual mean change,E1,E2,S1,and S2 present an increasing trend,while S3 shows a decreasing *** points for the seasonal mean change demon-strate an increasing trend in spring and winter,while other points show a decreasing trend in summer and ***,the E1 point growth rate is large in autumn,which is related to the change in typhoon intensity and the northward movement of the typhoon path.

Melting geodynamics reveals a subduction origin for the Purang ophiolite,Tibet,China

The debate regarding whether the Yarlung-Zangbo ophiolite(YZO)on the south of the Qinghai-Tibet Plateau,formed in a mid-ocean ridge(MOR)or a supra-subduction zone(SSZ)setting has remained *** we present petrological,mineralogical,and geochemical data associated with modeling melting geodynamics of the mantle peridotites from the Purang ophiolite in the western segment of the Yarlung-Zangbo Suture Zone(YZSZ)to explore its tectonic *** Purang lherzolites are characterized by the protogranular texture and have abyssal-peridotite-like mineral compositions,including low Cr^(#)(20-30)and TiO_(2) contents(<0.1wt%)in spinel,high Al_(2)O_(3)(2.9wt%-4.4wt%)and CaO(1.9wt%-3.7wt%)contents in orthopyroxene and LREE-depletion in *** of these lherzolites can be modeled by~11%dynamic melting of the DMM source with a small fraction of melt(~0.5%)entrapped within the source,a similar melting process to typical abyssal *** Purang harzburgites are characterized by the porphyroclastic texture and exhibit highly refractory mineral compositions such as high spinel Cr^(#)(40-68),low orthopyroxene Al_(2)O_(3)(<2.2wt%)and CaO(<1.1wt%)*** in these harzburgites are enriched in Sr(up to 6.0 ppm)and LREE[(Ce)N=0.02-0.4],but depleted in Ti(200 ppm,on average)and HREE[(Yb)N<2].Importantly,the more depleted samples tend to have higher clinopyroxene Sr and LREE *** observations indicate an open-system hydrous melting with a continuous influx of slab fluid at a subduction *** modeled results show that these harzburgites could be formed by 19%-23%hydrous melting with the supply rate of slab fluid at 0.1%-1%.The lower clinopyroxene V/Sc ratios in harzburgites than those in lherzolites suggest a high oxidation stage of the melting system of harzburgites,which is consistent with a hydrous melting environment for these *** is therefore concluded that the Purang ophiolite has experienced a transformat

Background In-stent restenosis （ISR） remains a challenge for interventional cardiologists. Some data suggest that drug-eluting stents （DES） represent a promising new option for the treatment of patients with ISR. Currently, 2 DES platforms are available [sirolimus-eluting stents （SES） and paclitaxel-eluting stents （PES） ], but the superiority of either approach for treating ISR has not been convincingly demonstrated. The aim of the present study was to retrospectively compare angiographic and clinical outcomes after treatment of ISR with SES or PES in a series of consecutive patients with ISR.
