BACKGROUND Thymolipoma is a rare benign tumor arising from the anterior mediastinal thymus and is composed of mature fatty tissue and interspersed nonneoplastic thymic tissue. This tumor accounts for only a small percentage of mediastinal masses, and the majority of them are asymptomatic and found incidentally. To date, fewer than 200 cases have been published in the world literature, of which most excised tumors weighed less than 0.5 kg and the largest weighed 6 *** SUMMARY A 23-year-old man presented with a complaint of progressive breathlessness for 6mo. His forced vital capacity was only 23.6% of the predicted capacity, and his arterial partial pressure of oxygen and carbon dioxide were 51 and 60 mmHg,respectively, without oxygen inhalation. Chest computed tomography revealed a large fat-containing mass in the anterior mediastinum that measured 26 cm × 20cm × 30 cm in size and occupied most of the thoracic cavity. Percutaneous mass biopsy revealed only thymic tissue without signs of malignancy. A right posterolateral thoracotomy was successfully performed to remove the tumor along with the capsule, and the excised tumor weighed 7.5 kg, which to our knowledge, was the largest surgically removed tumor of thymic origin. Postoperatively, the patient’s shortness of breath was resolved, and the histopathological diagnosis was thymolipoma. No signs of recurrence were observed at the 6-mo *** Giant thymolipoma causing respiratory failure is rare and dangerous. Despite the high risks, surgical resection is feasible and effective.

Protein O-GlcNAcylation is a post-translational modification that links environmental stimuli with changes in intracellular signal pathways,and its disturbance has been found in neurodegenerative diseases and metabolic ***,its role in the mesolimbic dopamine(DA)system,especially in the ventral tegmental area(VTA),needs to be ***,we found that injection of Thiamet G,an O-GlcNAcase(OGA)inhibitor,in the VTA and nucleus accumbens(NAc)of mice,facilitated neuronal O-GlcNAcylation and decreased the operant response to sucrose as well as the latency to fall in rotarod *** with DAergic neuron-specific knockout of O-GlcNAc transferase(OGT)displayed severe metabolic abnormalities and died within 4–8 weeks after ***,mice specifically overexpressing OGT in DAergic neurons in the VTA had learning defects in the operant response to sucrose,and impaired motor learning in the rotarod ***,overexpression of OGT in GABAergic neurons in the VTA had no effect on these *** results suggest that protein O-GlcNAcylation of DAergic neurons in the VTA plays an important role in regulating the response to natural reward and motor learning in mice.

AIM: To study the mechanism underlying carbon tetrachloride (CCl4)-induced alterations of protein synthesis in liver. METHODS: Male Sprague-Dawley rats were given CCl4 (1 mL/100 g body weight) and 3H-leucine incorporation. Malondialdehyde (MDA) level in the liver, in vitro response of hepatocyte nuclei nucleotide triphosphatase (NTPase) to free radicals, and nuclear export of total mRNA with 3'-poly A+ were measured respectively. Survival response of HepG2 cells to CCl4 treatment was assessed by methyl thiazolyl tetrazolium. Km and Vmax values of nuclear envelope NTPase activity in liver of rats treated with CCl4 were assayed by a double-reciprocal plot. RESULTS: The protein synthesis was inhibited while the MDA level was signif icantly increased in liver of rats treated with CCl4. In addition, CCl4 decreased the NTPase binding capacity of nuclear envelope (Km value) in cultured HepG2 cells. Moreover, in vitro ferrous radicals from Fenton's system suppressed the NTPase activity of liver nuclear envelope in a dose-dependent manner. Down-regulation of the nuclear envelope NTPase activity indicated a lower energy provision for nucleocytoplasmic transport of mRNA molecules, an evidence in CCl4-treated HepG2 cells correspondingly supported by the nuclear sequestration of poly (A)+ mRNA molecules in morphological hybridization research. CONCLUSION: Inhibition of mRNA transport, suggestive of decreased NTPase activity of the nuclear envelope, may be involved in carbon tetrachloride-inhibited protein synthesis in liver.

AIM:To report preliminary results of the efficacy and safety of sunitinib in the management of Taiwan Residents gastrointestinal stromal tumors (GIST) patients facing imatinib mesylate (IM) intolerance or ***:Between 2001 and May 2010,199 Taiwan Residents patients with metastatic GIST were treated at Chang Gung Memorial *** them,23 (11.6%) patients receiving sunitinib were ***:Sixteen male and 7 female patients with a median age of 59 years (range:24-83 years) received ***-two GIST patients changed to sunitinib because of IM failure and 1 because of *** median duration of sunitinib administration was 6.0 mo (range:2-29 mo).The clinical benefit was 65.2% [2 complete response (CR),4 partial response (PR),and 9 stationary disease (SD);15/23].In 12 patients harboring mutations of the kit gene at exon 11,the clinical benefit rate (CR,PR,and SD) was 75.0% and 6 patients with tumors containing kit exon 9 mutations had a clinical benefit of 50.0% (not significant,P=0.344).The progression free survival (PFS) and overall survival (OS) did not differ between patients whose GISTs had wild type,KIT exon 9,or KIT exon 11 ***-foot syndrome was the most common cause of grade Ⅲ adverse effect (26.1%),followed by anemia (17.4%),and neutropenia (13.0%).During the median 7.5-mo follow-up after sunitinib use,the median PFS and OS of these 23 GIST patients after sunitinib treatment were 8.4 and 14.1 mo,***:Sunitinib appears to be an effective treatment for Taiwan Residents with IM-resistant/intolerant GISTs and induced a sustained clinical benefit in more than 50% of Taiwan Residents advanced GIST patients.

The variability of the East Asian winter monsoon （EAWM） can be divided into an ENSO-related part （EAWMEN） and an ENSO-unrelated part （EAWMres）.The influence of EAWMres on the ENSO-East Asian summer monsoon （EASM） relationship in the decaying stages of ENSO is investigated in the present *** achieve this,ENSO is divided into four groups based on the EAWMres：（1） weak EAWMres-E1Ni（n）o （WEAWMres-EN）; （2） strong EAWMres-E1Ni（n）o （SEAWMresEN）; （3） weak EAWMres-La Ni（n）a （WEAWMres-LN）; （4） strong EAWMres-La Ni（n）a （SEAWMres-LN）.Composite results demonstrate that the EAWMres may enhance the atmospheric responses over East Asia to ENSO for WEAWMres-EN and *** corresponding low-level anticyclonic （cyclonic） anomalies over the western North Pacific （WNP） associated with El Ni（n）o （La Ni（n）a） tend to be ***,this feature may persist into the following summer,causing abundant rainfall in northern China for WEAWMres-EN cases and in southwestern China for SEAWMres-LN *** contrast,for the SEAWMres-EN and WEAWMres-LN groups,the EAWMres tends to weaken the atmospheric circulation anomalies associated with E1 Ni（n）o or La Ni（n）*** these cases,the anomalous WNP anticyclone or cyclone tend to be reduced and confined to lower latitudes,which results in deficient summer rainfall in northern China for SEAWMres-EN and in southwestern China for *** study suggests that anomalous EAWMres may have an effect on the extra-tropical sea surface temperature anomaly,which persists into the ensuing summer and may interfere with the influences of ENSO.

Background There is a paucity of studies conducted in China on the outcomes of all live-birth extremely premature infants(EPIs)and there is no unifed recommendation on the active treatment of the minimum gestational age in the feld of perinatal medicine in *** aimed to investigate the current treatment situation of EPIs and to provide evidence for formulating reasonable treatment *** We established a real-world ambispective cohort study of all live births in delivery rooms with gestational age(GA)between 24+0 and 27+6 weeks from 2010 to *** Of the 1163 EPIs included in our study,241(20.7%)survived,while 849(73.0%)died in the delivery room and 73(6.3%)died in the neonatal intensive care *** all included EPIs,862(74.1%)died from withholding or withdrawal of *** of stratifcation according to GA or birth weight,the proportion of total mortality attributable to withdrawal of care is *** infants with the GA of 24 weeks,active treatment did not extend their survival time(P=0.224).The survival time without severe morbidity of the active treatment was signifcantly longer than that of withdrawing care for infants older than 25 weeks(P<0.001).Over time,the survival rate improved,and the withdrawal of care caused by socioeconomic factors and primary nonintervention were reduced signifcantly(P<0.001).Conclusions The mortality rate of EPIs is still *** of care is common for EPIs with smaller GA,especially in the delivery *** is necessary to use a multi-center,large sample of real-world data to fnd the survival limit of active treatment based on our treatment capabilities.

A linear fluorinated benzocyclobutene-type monomer(4F-bis-BCB) was facilely synthesized by a one-step copper-catalyzed etherification reaction and a simple precipitation post-purification ***,a series of BCB-based polymeric low-dielectric(low-k)materials were obtained by the thermal-induced ring-opening copolymerization of 4F-bis-BCB with divinyl tetramethyl disiloxanebisbenzocyclobutene(DVS-BCB) monomer and further simple thermal curing at high temperature(200-300℃).The resultant fully cured materials demonstrated excellent low dielectric properties at high frequency of 10 GHz(dielectric constant(D_(k))116°),ultra-low water absorption(<0.19% after soaked in water at room temperature for 60 h) and excellent planarization ability(surface roughness<0.56 nm of 3 μm-thick film).Overall,this new fluorinated BCB-type monomer provides us an alternative for the facile preparation of low-k polymeric materials and exhibits great potential for future applications in high-frequency communication and three-dimensional high-density packaging technologies.
