Results of Ponseti Method in the Management of the Congenital Clubfoot

Background: Clubfoot is a debilitating pathology and a source of abnormal gait in children. Treatment with the Ponseti method is the treatment used in the management of congenital clubfoot in children under 9 years old. This study aims to identify the proportion of children with clubfoot supported by the Ponseti method and to evaluate their effectiveness. Materials and Method: A multicenter retrospective descriptive study for six (06) months from June 1st to November 30th, 2019 concerning the evaluation of the Ponseti method in the management of clubfoot in Madagascar was conducted. Results: One hundred ninety-one (191) children with club feet were screened during the study period. One hundred and fifty children, or 226 congenital equine varus clubfoot were treated with the Ponseti method during the study period. The male gender was the most affected compared to the female gender with a sex ratio of 1.75. The clubfoot was bilateral in 50.66% of cases and idiopathic in 92.66%. The initial Pirani score was on average (±SD) 5.04/4.98 (±1.22/1.20). A proportion of 23.33% of cases of recurrence was recorded at 3 months of use of brace abduction splint. Conclusion: In Madagascar, the incidence of congenital clubfoot remains relatively low but clubfoot is still the main limitation of walking and a lack of footwear in children. Ponseti method is the gold standard in the management of this pathology. However, it requires a long-term follow-up.

The automation of plant phenotyping using 3D imaging techniques is ***,conventional methods for reconstructing the leaf surface from 3D point clouds have a trade-off between the accuracy of leaf surface reconstruction and the method’s robustness against noise and missing *** mitigate this trade-off,we developed a leaf surface reconstruction method that reduces the effects of noise and missing points while maintaining surface reconstruction accuracy by capturing two components of the leaf(the shape and distortion of that shape)separately using leaf-specific *** separation simplifies leaf surface reconstruction compared with conventional methods while increasing the robustness against noise and missing *** evaluate the proposed method,we reconstructed the leaf surfaces from 3D point clouds of leaves acquired from two crop species(soybean and sugar beet)and compared the results with those of conventional *** result showed that the proposed method robustly reconstructed the leaf surfaces,despite the noise and missing points for two different leaf *** evaluate the stability of the leaf surface reconstructions,we also calculated the leaf surface areas for 14 consecutive days of the target *** result derived from the proposed method showed less variation of values and fewer outliers compared with the conventional methods.

AIM To elucidate the effect of antithrombotic therapy(ATT) on bleeding and thromboembolic complications during or after laparoscopic digestive *** Published articles or internationally accepted abstracts between 2000 and 2017 were searched from PubMed, Cochrane Database, and Google Scholar, and studies involving laparoscopic digestive surgery and antiplatelet therapy(APT) and/or anticoagulation therapy(ACT) were included after careful review of each study. Data such as study design, type of surgical procedures, antithrombotic drugs used, and surgical outcome(both bleeding and thromboembolic complications) were extracted from each *** Thirteen published articles and two internationally accepted abstracts were eligible for inclusion in the systematic review. Only one study concerning elective laparoscopic cholecystectomy in patients with peri-operative heparin bridging for ACT showed that the risk of postoperative bleeding was higher compared with those without ACT. The remaining 14 studies reported no significant differences in the incidence of bleeding complications between the ATT group and the group without ATT. The risk of thromboembolic events(TE) associated with laparoscopic digestive surgery in patients receiving ATT was not significantly higher than those with no ATT or interrupted *** Laparoscopic digestive surgery in ATT-burdened patients for prevention of bleeding and TE showed satisfactory results. The risk of hemorrhagic complication during or after these procedures in patients with continued APT or heparin bridging was not significantly higher than in patients with no ATT or interrupted APT.

In this paper, the portfolio selection problem under Cumulative Prospect Theory （CPT） is investigated and a model of portfolio optimization is presented. This model is solved by coupling scenario generation techniques with a genetic algorithm. Moreover, an Adaptive Real-Coded Genetic Algorithm （ARCGA） is developed to find the optimal solution for the proposed model. Computational results show that the proposed method solves the portfolio selection model and that ARCGA is an effective and stable algorithm. We compare the portfolio choices of CPT investors based on various bootstrap techniques for scenario generation and empirically examine the effect of reference points on investment behavior.

On the Effects of Central Japan Expressway’s Commuter Toll Discount Policy in Nagoya Area

Road pricing policies have two equally important objectives that have to be considered： the optimal use of the road network and the fiscal sustainability for the management of the network. The aim of this work is to examine the effects on these two objectives of a recent pricing policy implemented in Japan＇s expressway networks. Recently the Nippon （Japan） Expressway Companies （NEXCO, operators of Japan＇s nation wide expressway network） have implemented several toll discount policies for users equipped with electronic toll collectors （ETCs）. One of the policies is the commuter discount policy that discounts 50% for a journey on the NEXCO expressway no longer than 100 km during the commuting peak hours. This paper examines its effect on congestion reduction in the road network of Nagoya area and its influence on the use of expressways operated by another major local public expressway company in Nagoya. The congestion easing effect of this policy is compared with a potential marginal cost pricing scheme. These observations suggest that policy coordination regarding toll levels is needed for expressway corporations and transportation authorities for improving transportation efficiency of utilization of the whole network.

Key cytotoxic drugs of chemotherapy for gastroesophageal cancer include fluoropyrimidine, platinum, taxanes and irinotecan. Concurrent chemoradiotherapy is one of the main treatment strategies, especially for esophageal cancer. As molecular target agents, the anti-HER2 antibody trastuzumab for HER2-positive gastric cancer and the anti-angiogenesis agent ramucirumab combined with paclitaxel have been proven to improve the survival of gastric cancer patients. Recently, anti-PD-1 antibodies have become available as second- or later-line chemotherapy. Microsatellite instability is also useful as a biomarker to select patients suitable for immunotherapy. Furthermore, genome-wide analysis has improved our understanding of the biological features and molecular mechanisms of gastroesophageal cancer and will provide optimized treatment selection.
