An easier method for performing a pancreaticojejunostomy.for the soft pancreas using a fast-absorbable suture

AIM: To clarify.the usefulness of a new method for performing a pancreaticojejunostomy.by.using a fast-absorbable suture material irradiated poly.lactin 910, and a temporary.stent tube for a narrow pancreatic duct with a soft pancreatic ***: Among 63 consecutive patients with soft pancreas undergoing a pancreaticoduodenectomy.from 2003 to 2006, 35 patients were treated with a new reconstructive method. Briefly. after the pancreatic transaction, a stent tube was inserted into the lumen of the pancreatic duct and ligated with it by.a fast-absorbable suture. Another tip of the stent tube was introduced into the intestinal lumen at the jejunal limb, where a purse-string suture was made by.another fast-absorbable suture to roughly.fix the tube. The pancreaticojejunostomy.was completed by.ligating two fast-absorbable sutures to approximate the ductal end and the jejunal mucosa, and by.adding a rough anastomosis between the pancreatic parenchy.a and the seromuscular lay.r of the jejunum. The initial surgical results with this method were retrospectively.compared with those of the 28 patients treated with conventional duct-to-mucosa ***: The incidences of postoperative morbidity.including pancreatic fistula were comparable between the two groups (new; 3%-17% vs conventional; 7%-14% according to the definitions). There was no mortality.and re-admission. Late complications were also rarely.***: A pancreaticojejunostomy.using an irradiated poly.lactin 910 suture material and a temporary.stent is easy.to perform and is feasible even in cases with a narrow pancreatic duct and a normal soft pancreas.

This building site lies in the northwest of the Guigong Palace, about 160m east of Tiesuo village, Liucunbao township, Weiy.ng district, Xi'an city. Shaanxi province. It consists of two large-sized house-foundations arranged from north to south, seven small house-foundations between them, and six ruined walls. The unearthed terra-cotta and pottery.objects include bricks, tiles, tile-ends, supports, spindle whorls, jars and lamps. In addition, there are irons, stone tools and copper coins. The houses seem to have not been suitable to human living and activities and must have been a complex of storage rooms built in the Guigong Palace in the mid and late Western Han period, the discovery.of which provides important material for study.ng the structure and lay.ut of the Guigong Palace.

Recoil Momentum of Target Ions in Collisions of Ar^(6+)+CO_2 at Energies Below 300 eV/u

The collision dy.amics and fragmentation process of molecule by.highly.charged ion impact for single electron capture processes at the low energies below 1 keV/u were studied. The collision energy.dependence of the recoil momentum was obtained experimentally.and compared it with those calculated by.a theoretical model using a deflection function with polarization potential. A fairly.good agreement between the measured and calculated results was reached. This suggests that the polarization potential play. a crucial role in the low-energy.region.

Des-gamma-carboxy.prothrombin as an important prognostic indicator in patients with small hepatocellular carcinoma

AIM:To clarify.the effect of a high des-gamma-carboxy.prothrombin (DCP) level on the invasiveness and prognosis of small hepatocellular carcinoma. METHODS: Among 142 consecutive patients with known DCP levels, who underwent hepatectomy.because of hepatocellular carcinoma, 85 patients met the criteria for small hepatocellular carcinoma, i.e. one ≤ 5 cm sized single tumor or no more than three ≤ 3 cm sized tumors. RESULTS: The overall survival rate of the 142 patients was 92.1% for 1 y.ar, 69.6% for 3 y.ars, and 56.9% for 5 y.ars. Multivariate analy.is showed that microscopic vascular invasion (P = 0.03) and serum DCP ≥ 400 mAU/mL (P = 0.02) were independent prognostic factors. In the group of patients who met the criteria for small hepatocellular carcinoma, DCP ≥ 400 mAU/mL was found to be an independent prognostic factor for recurrence-free (P = 0.02) and overall survival (P = 0.0005). In patients who did not meet the criteria, the presence of vascular invasion was an independent factor for recurrence-free (P = 0.02) and overall survivals (P = 0.01). In 75% of patients with small hepatocellular carcinoma and high DCP levels, recurrence occurred extrahepatically. CONCLUSION: For small hepatocellular carcinoma, a high preoperative DCP level appears indicative fortumor recurrence. Because many.patients with a high preoperative DCP level develop extrahepatic recurrence, it is necessary.to screen the whole body.

Dosimetric Analy.is of Three Different Radiotherapy.Techniques in Patients with Breast Cancer and Their Impact on Organs at Risk (OAR)

Aim: Dosimetric analy.is of three different Radiotherapy.techniques in patients with Breast Cancer and their impact on OAR’s. Materials and Methods: 12 patients of Carcinoma Breast who received breast radiotherapy.were selected for analy.is. Computed tomography.(CT) simulation image data sets were retrieved. Planning target Volume (PTV), heart and ipsilateral lung were contoured for planning and analy.is of doses. Three different plans using conventional bi-tangential fields were prepared, Plan 1 with SAD full beam with wedge (SAD-FBW), Plan 2 with SSD half beam with wedge (SSD-HBW) and plan 3 with SSD half beam without wedge techniques (SSD-HBO) in CMS Xio TPS. Statistical analy.is was done using SPSS version 16.0. Results: The PTV coverage was significantly.better in SAD-FBW techniques when compared with the other two techniques i.e. SSD-HBW (mean = 92.33, SD = 4.69, p = 0.005) and SSD-HBO (mean = 75.05, SD = 11.92, p = 0.002). The mean heart doses were significantly.better in SAD-FBW compared to SSD-HBW (mean = 3.75, SD = 2.27, p = 0.017) but in SSD-HBO technique mean heart doses were better than SAD-FBW technique (mean = 3.27, SD = 1.94, p = 0.004). Similarly. the left lung V20 values were significantly.better in SSD-HBO technique than SAD-FBW technique (mean = 11.75, SD = 4.34, p = 0.004) but there was statistically.insignificant difference between the SAD-FBW and SSD-HBW techniques. The treatment monitor units were significantly.less in SAD-FBW compared to SSD-HBW but SSD-HBO has less MU compared to SAD-FBW technique. Conclusion: SAD Full Beam with wedge technique is practically.better than SSD Half Beam technique with or without wedge without compromising PTV coverage considering other dosimetric parameters. We also recommend that centers treating patient with half beam SSD techniques should shift immediately.to SAD techniques.
