A randomized controlled trial of imipramine in patients with irritable bowel sy.drome

AIM: To study.the efficacy.of low-dose imipramine in relieving sy.ptoms associated with the irritable bowel sy.drome (IBS). METHODS: A randomized, double-blind trial of 25 mg imipramine vs matched placebo for 12 wk was performed. Doubling the dose was allowed once at week 2 in case of an unsatisfactory.early.response. Primary.efficacy.variables were subjective global sy.ptom relief and quality.of life (QoL) using SF-36 at week 12. RESULTS: One hundred and seven patients were enrolled by.advertisement or referral by.general practitioners and 56 (31 imipramine: 25 placebo) completed the 16-wk study. Baseline characteristics were comparable. A high overall dropout rate was noted in the imipramine and placebo arms (47.5% vs 47.9%, P > 0.05), a mean of 25.0 and 37.4 d from enrollment, respectively.(P y.ptom relief with imipramine over placebo (per-protocol: 80.6% vs48.0%, P = 0.01) and a trend on intent-to-treat (ITT) analy.is (42.4% vs 25.0%, P = 0.06). This improvement was evident early.and persisted to week 16 (P = 0.024 and 0.053 by.per-protocol and ITT analy.es, respectively.. Mean cumulative and componentspecific SF-36 scores improved in the imipramine group only.(per-protocol, P 0.05). CONCLUSION: Imipramine may.be effective in the treatment of IBS patients and is associated with improved QoL. Careful patient selection, initiation of a low dose with gradual escalation and monitoring for side effects may.result in an improved therapeutic response.

Microorganisms including bacteria,viruses,protozoa,and fungi living in the gastrointestinal tract are collectively.known as the gut *** is the imbalance in microbial composition on or inside the body.relative to healthy.*** Firmicutes to Bacteroidetes ratio and decreased abundance of Akkermansia muciniphila are the predominant gut dy.biosis associated with the pathogenesis of ty.e 2 diabetes mellitus(T2DM)and metabolic *** mechanisms linking gut dy.biosis,and metabolic diseases and their complications include altered metabolism of short-chain fatty.acids and bile acids,interaction with gut hormones,increased gut microbial metabolite trimethy.amine-N-oxide,bacterial translocation/Leaky.gut sy.drome,and endotoxin production such as *** association between the gut microbiota and gly.emic agents,however,is much less understood and is the growing focus of research and *** studies suggest that the gut microbiota and anti-diabetic medications are interdependent on each other,meaning that while anti-diabetic medications alter the gut microbiota,the gut microbiota also alters the efficacy.of anti-diabetic *** increasing evidence regarding the significance of gut microbiota,it is imperative to review the role of gut microbiota in the pathogenesis of *** review also discusses the interaction between gut microbiota and the various medications used in the treatment of T2DM.

Change orders are common and clearly.noticeable in Jordanian construction industry. Several studies have identified change orders as significant problem in Jordanian construction projects, and they.mainly.cause delay.and cost overrun. This situation spurs the researchers for investigating the significant causes of change orders in construction projects. Thus, this paper aims to determine the main causes of change orders in the Jordanian construction private sector which responsible for significant cost overrun. Both qualitative and quantitative methods, i.e. interview and questionnaire techniques, were used to achieve this aim. Interviews were conducted as the first stage of data collection, and the results formed the basis of the questionnaire, which was distributed across the Jordanian construction sector. Content analy.is was used to analy.e the interview responses, while factor analy.is, correlation and the Severity.Index (SI) were used to analy.e the questionnaire results. The findings identified three main categories of causes of change orders in private sector, namely.engineering causes, causes related to the client, and circumstances of the project, with sub-causes in each category.which are related to each other in significant way. and which affect each other.

The enhancement of medical images is a challenging research task due to the unforeseeable variation in the quality.of the captured *** captured images may.present with low contrast and low visibility.which might inuence the accuracy.of the diagnosis *** overcome this problem,this paper presents a new fractional integral entropy.FITE)that estimates the unforeseeable probabilities of image pixels,posing as the main contribution of the *** proposed model dy.amically.enhances the image based on the image *** main advantage of FITE lies in its capability.to enhance the low contrast intensities through pixels’***,the pixel probability.of the fractional power is utilized to extract the illumination value from the pixels of the ***,the contrast of the image is then adjusted to enhance the regions with low ***,the fractional integral entropy.approach is implemented to enhance the low visibility.contents from the input *** were conducted on brain MRI,lungs CT,and kidney.MRI scans datasets of different image qualities to show that the proposed model is robust and can withstand dramatic variations in *** obtained comparative results show that the proposed image enhancement model achieves the best BRISQUE and NIQE ***,this model improves the details of brain MRI,lungs CT,and kidney.MRI scans,and could therefore potentially.help the medical staff during the diagnosis process.

Reconstructing the evolutionary.history.of the Gobi deserts developed from alluvial sediments in arid regions has great significance in unraveling changes in both tectonic activity.and ***,such work is limited by.a lack of suitable dating material preserved in the Gobi Desert,but cosmogenic 10Be has great potential to date the Gobi *** the present study.10Be in quartz gravel from the Gobi deserts of the Ejina Basin in Inner Mongolia of China has been measured to assess exposure *** show that the Gobi Desert in the northern margin of the basin developed 420 ka ago,whereas the Gobi Desert that developed from alluvial plains in the Heihe River drainage basin came about during the last 190 *** latter developed gradually.northward and eastward to modern terminal lakes of the *** temporal and spatial variations in the Gobi deserts are a consequence of alluvial processes influenced by.Tibetan Plateau uplift and tectonic activities within the Ejina *** episodes of Gobi Desert development within the last 420 ka indicate that the advance/retreat of alpine glaciers during glacial/interglacial cy.les might have been the dominant factor to influencing the alluvial intensity.and water volume in the *** floods and large water volumes would mainly.occur during the short deglacial periods.

Background: There is increasing research interest in using short-chain fatty.acids(SCFAs) including buty.ate as potential alternatives to antibiotic growth promoters in animal production. This study.was conducted to evaluate the effects of supplementation of sodium buty.ate(SB) in liquid feeds(milk, milk replacer, and the mixture of both)on the growth performance, rumen fermentation, and serum antioxidant capacity.and immunoglobins in dairy.calves before weaning. Forty.healthy.female Holstein calves(4-day.old, 40 ± 5 kg of body.weight) were housed in individual hutches and randomly.allocated to 1 of 4 treatment groups(n = 10 per group) using the RAND function in Excel. The control group was fed no SB(SB0), while the other three groups were supplemented with 15(SB15),30(SB30), or 45(SB45) g/d of SB mixed into liquid feeds offered. The calves were initially.fed milk only.day. 2 to 20), then a mixture of milk and milk replacer(day. 21 to 23), and finally.milk replacer only.day. 24 to 60).Results: The SB supplementation enhanced growth and improved feed conversion into body.weight gain compared with the SB0 group, and the average daily.gain increased quadratically.with increasing SB supplementation. No significant effect on rumen pH;concentrations of NH_3-N, individual and total VFAs;or acetate:propionate(A:P) ratio was found during the whole experimental period. Serum glutathione peroxidase activity.increased linearly.with the increased SB supplementation, while the serum concentration of maleic dialdehy.e linearly.decreased. Serum concentrations of immunoglobulin A, immunoglobulin G, or immunoglobulin M were not affected by.the SB supplementation during the whole experimental ***: Under the conditions of this study. The detailed process of the periodic review on Tianmu *** Reserve demonstrated that its mechanism of the review can formalize concertation among all stakeholders’ different *** on-the-spot evaluation and review meeting is an active dialogue among biodiversity.stakeholders,based on presently.priority.and acceptable problems the reserve *** periodic review of biosphere reserve in China provides a place to foster the convergence of the stakeholders’ interests with a relative democratic and feasible way.which could be the basis of seeking approaches to reconcile the conservation of biodiversity.with economic development.

Aim: This study.aim to evaluate the effect of time of admission on mortality.of patients admitted to the ICU. Method: This retrospective study.included 391 of patients admitted to the ICU of an academic hospital during one y.ar. Patients were categorized according to time of admission: working-hours and off-hours. Mortality.was compared in the groups and associated factors of mortality.were examined. Results: Two third of patients were admitted during off-hours. There was no significant difference in the GCS, age and length of stay.among patients admitted during working-hours and off-hours. There was no difference in mortality.among patients admitted during working-hours and off-hours Mortality.was significantly.higher in older and more severe cases, regardless of time of admission. Conclusion: Time of ICU admission does not have significant effect on patient’s outcome.
