Introduction: Sleep disorders among professional drivers are a major road safety.problem. They.cause fatigue and drowsiness at the wheel, which can lead to road traffic accidents and even accidents at work. The aim of this study.was to assess sleep disorders among professional drivers of oil tankers in Ouagadougou, Burkina Faso. Methods: This was a descriptive and analy.ical cross-sectional study.carried out from 11 April 2020 to 11 September 2020 in the national hy.rocarbon company.s fuel depot. All the drivers present at the time were included and accomplished a questionnaire incorporating the commonly.scales using to assess sleep disorders, sleep quality.and sleep apnoea. Data analy.is using R 3.6.1 software enabled to perform univariate and multivariate analy.es to identify.associated factors. Adjusted odd ratios were used to measure the strength of association. The significance level chosen was p ≤ 0.05. Results: All the 339 respondents were male. A proportion of 33.6% suffered from insomnia, including 12.7% with moderate to severe insomnia, and 18.58% were at risk of sleep apnoea. On the Epworth scale, 26.6% of drivers showed sleep debt and 4.1% excessive day.ime sleepiness. The risk factors associated with insomnia were the use of psy.hostimulants. A history.of diabetes and insomnia were risk factors associated with sleep apnoea sy.drome. Conclusion: The prevalence of sleep disturbance and insomnia among the respondents were 52.2% and 33.6% respectively. Use of psy.hostimulants was the identified risk factor. The prevalence of SAS was 4.72%, with diabetes and insomnia as associated factors.

Pneumomediastinum is sometimes observed in adult patients but its occurrence in pediatric patients (especially.infants) is very.rare. We here report a 14-month-old male infant who had subcutaneous emphy.ema, pneumomediastinum, and importantly. pneumopericardium. He had no particular past histories. He abruptly had cough, fever, and eruption on the abdomen. Computed tomography.and echocardiography.revealed pneumomediastinum and pneumopericardium. Antibiotics, rest, and supportive therapy ameliorated the condition. We must be aware that pneumomediastinum, and importantly.pneumopericardium, can be present in a pediatric patient with subcutaneous emphy.ema. The infant’s symptoms disappeared under strict monitoring of respiratory status, nasal oxygen therapy and antibiotic therapyy.e="font-family."">.

Pleomorphic adenoma is the most common tumor of the salivary.glands, accounting for approximately.50% of salivary.gland lesions. It develops mainly.in the salivary.glands: parotid (80%), submaxillary.(10%), sublingual (1%) and in the accessory.oral-phary.geal glands (9%). The aim of this work was to report 2 cases of pleomorphic adenoma of aty.ical location and then to discuss the difficulties linked to its diagnostic and therapeutic management in the Malian context. They.were a 40-y.ar-old man and a 72-y.ar-old woman. They.were admitted to the ENT department of the “Luxembourg Mere-infant” hospital for orophary.geal swelling for the first and swelling of the palate for the second. The clinical expression was a swelling in both cases, of a hard, mobile consistency.with healthy.mucosa on their surface. The remainder of the phy.ical examination was unremarkable. The diagnosis of a tumor of the orophary.x and palate was made following clinical radiological examinations. The histopathological examination of the surgical specimen made it possible to make the diagnosis of pleomorphic adenoma of the palate and the left palatine tonsil. The follow-up surgery.was straightforward, with a favorable outcome. Pleomorphic adenomas are relatively.rare benign tumors of the accessory.salivary.glands;their clinical expression remains swelling. Therapeutic management is surgery.and the diagnosis is confirmed by.histopathological examination.

Introduction: The authors wanted to study.the prevalence of ocular trauma in the ophthalmology.unit of the CSREF of Kati. Patients and method: This is a retrospective study. covering the period from January.to December 2015, carried out in the ophthalmology.unit of the CSREF in Kati. Results: The sample consisted of 568 patients, or 6.82% of ophthalmological consultations. Children aged 0 to 14 y.ars accounted for 154 (27.11%) of the trauma cases. Bruises constitute the main circumstance (58.1%) of ey. trauma, followed by.domestic accidents (15.6%). Closed globe ocular trauma was the main ty.e of trauma (61.6%), followed by.ocular adnexal trauma (30.1%) and open globe trauma (8.3%) in which the penetrating wound was the lesion, the least common (2.6%) but the most serious. Medical treatment was the most used therapeutic modality.in 82.4% of cases, followed by.surgery.with 17.6% of cases. The evolution of the lesions after treatment was favorable in 85.6% of cases and unfavorable in 14.4% of cases. Complications were observed in 5.8% of cases and sequelae in 8.6% of cases. Conclusion: Ocular trauma constitutes an important reason for ophthalmological consultation at the CSREF of Kati. The severity.of some of these lesions requires frequent recourse to surgery. The complexity.of the treatment should encourage us to favor preventive measures.

Cowpea (Vigna unguiculata L. [Walp.]) in one of the main grain legumes contributing to food security.and poverty.alleviation in Sub-Saharan Africa. To control the highly.damaging legume pod borer Maruca vitrata F., transgenic cowpea lines expressing the insecticidal Cry.Ab Bt protein were developed. In this study. we evaluated the impact of Cry.Ab transgene expression on the susceptibility.of four cowpea lines (named IT97K-T, IT98K-T, Gourgou-T and Nafi-T) and their respective non-transgenic near isogenic lines (IT97K, IT98K, Gourgou and Nafi) to Cowpea aphid-borne mosaic virus (CABMV) in greenhouse conditions. In a preliminary.quality.control test by.enzy.e-linked immunosorbent assay. the presence of Cry.Ab protein in transgenic seed lots ranged from 59% to 72%, with no significant differences among the lines (χ2 = 3.26;p = 0.35). Upon virus inoculation, all cowpea lines exhibited mosaic sy.ptoms with similar severity.between 7- and 11-day.post-inoculation. No significant differences were observed in sy.ptom severity. Significant differences were found between cowpea lines for time of sy.ptom onset, virus accumulation in plants and day. to 50% flowering. However, while comparing pairs of transgenic lines and corresponding non-transgenic lines, virus accumulation showed not significant differences whatever the pair. Time of sy.ptom onset and day. to 50% flowering did not also differ significantly.between pairs of cowpea lines except Nafi/Nafi-T in which transgenic Nafi-T showed earlier sy.ptoms (7.4 ± 0.7 vs. 8.9 ± 0.8 day. post-inoculation) and shorter flowering time (37.3 ± 0.6 vs. 42 ± 1.7 day. after sowing). Overall, these findings improve our understanding of the effects of Cry.Ab gene mediated genetic modification on cowpea infection by.Cowpea aphid-borne mosaic virus, with potential implications for environmental safety.assessment.

This paper reports numerical results for the characterization of the propagation channel in a train. Since the availability.of a train to carry.out measurements is not alway. easy. particularly.when many.changes must be done, a simulation tool provides a useful and reliable mean for the evaluation of the propagation characteristics of this complex and highly.fluctuating channel. In order to benefit from previous results, the various existing softwares for complex electromagnetic fields environments simulations were fully.searched and one that seems best suited has been retained for these computations. The results presented here are original, preliminaries and our approach provides a basis for study.the propagation of waves in a very.complex environment consisting of different electromagnetic fields like a train.

Cervical meningoceles are the least common forms of spina bifida, representing between 4% to 8% of all spinal dy.raphisms. They.are not accompanied by.neurological deficit in childhood but attached cord sy.drome can occur with growth. Surgical treatment, in addition to avoiding rupture and correcting the aesthetic damage, will preserve neurological functions. We report 2 cases that we took care of. They.are a 5-month-old infant and a 3-y.ar-old girl. The first presented a 7 cm rounded meningocele pedunculated through the C1 lamina defect. The second presented an 8.6 cm poly.obed meningocele pedunculated through the C2 lamina defect. They.both underwent surgery.with good progress.

Epidemiological and Anatomo Path Ological Aspects of Stomach Cancers at the Koutiala District Hospital

Aim: This paper aims to evaluate the epidemiological and pathological aspects of stomach cancer at Koutiala District Hospital. Methods: This was a retrospective study.carried out in the General Surgery.Department of the Koutiala District Hospital Health Centre. The records of patients admitted for stomach cancer in the period from August 1, 2017 to December 31, 2022 were collected. Patients who underwent surgery.for stomach cancer were included in the study. Patients who had not undergone surgery.were excluded. Results: We collected records from 40 patients operated on for stomach cancer. The median age was 58.4 y.ars ± 5.7. Men were in the majority.with 72.5% The sex ratio was 2.6. The incidence rate of stomach cancer in the general population in the circle was 4.9 cases per 100,000 population. Patients had a history.of gastric ulcer in 90%, Helicobacter py.ori infection in 47.5% and a notion of excessive salt consumption in 47.5%. The median duration of disease progression was 5 y.ars ± 2.3. The seat was antro-py.oric in 77.5%. The cancer was ulcerative budding in 23 patients. Adenocarcinoma was the histological ty.e in all patients. The cancer was classified as stage III in 30 cases and stage IV in 10 cases. Palliative surgical treatment was the most performed (47.5%). Postoperative morbidity.was 15% and mortality.was 42.5% at one y.ar. Conclusion: Stomach cancer is common in the ward. Most patients consult at the stage of metastasis. Palliative surgery.remains the most practical. Postoperative morbidity.and mortality.is very.high.

Inguinal hernia in children is a congenital pathology.in children linked to the persistence of the peritoneo-vaginal canal in children and the NüCK canal in girls;persisting into adulthood. This work aimed to study.inguinal hernia in children in the general surgery.department of the Reference Health Center of Commune I of the Bamako District;to determine the frequency.of inguinal hernia;describe the epidemiological, clinical and therapeutic aspects of inguinal hernias and in order to evaluate the cost of treatment. This was a prospective study.from April 2017 to March 2018 during which 60 children with an inguinal hernia were collected. Inguinal hernias in children represented 9.23% of patients operated on in the department. The average age was 59.23 months with extremes of one month and 180 months and a standard deviation of 49.23 y.ars. The male sex was dominant with a sex ratio equal to 9:1. The notion of prematurity.was found in 11.7% and was associated with a testicle not in place in 1.7% of cases. 41.7% of our patients were referred by.a doctor and 66.7% of patients presented with painless, intermittent inguino-scrotal swelling (83.3% of cases). The hernia was discovered before one week of life in 50% of cases. Inguinal swelling was absent in 11.7% but observed intraoperatively. The inguinal hernia was unilateral in 91.7% of cases and 70% of hernias were discovered by.the parents during pushing efforts. The inguinal hernia was located on the right in 61.7% of cases. The hernia swelling was soft, painless, impulsive and reducible in 78.3% of cases. The swelling was inguino-scrotal in 58.3% of cases. The hernia was simple in 95% of patients and hernial strangulation was observed in 3 cases or 5% of cases. 98.3% of patients were ASA I. The treatment was carried out openly.in all our patients including closure of the vaginal peritoneal canal in 95% of cases under general anesthesia in 98.3% of cases. Introduction: Heart failure is a major public health pandemic, particularly.in Africa, where its prevalence continues to increase. In northern Mali, few data exist, hence the interest of this study.with the objective of study.ng the epidemiological, clinical and therapeutic and evolutionary.aspects of Heart failure at the regional hospital of Gao. Patients and Methods: This was a cross-sectional, descriptive study.that took place from July.2022 to June 2023 in the medical department at Gao Hospital. Results: The hospital prevalence of heart failure was 44.1%. The mean age was 47.30 ± 20 y.ars (range: 16-88). Hy.ertension was the most common with 46.1%, followed by.a sedentary.lifesty.e, and diabetes with 18.2% and 8.3% respectively.Ny.A stage III-IV dy.pnea was found in 83.9%. Reduced EF heart failure was present in 110 patients (76.9%), seventeen cases with moderately.reduced EF (11.9%) and sixteen patients had preserved EF (11.2%). Global heart failure was the dominant (91.6%). The main etiologies of heart failure were dominated by.hy.ertensive heart disease in 46 patients (32.2%), followed by.postpartum cardiomy.pathy.with 43 cases (30.1%), primary.dilated cardiomy.pathy.in 18 patients (12.6%), ischemic heart disease in 16 patients with 11.2%. Seven cases of valvular heart disease, or 4.9%. The evolution was favorable under treatment in 104 patients or 72.7%. In-hospital mortality.was 14.7%. Conclusion: Heart failure is a common condition in sub-Saharan Africa, particularly.in our country.
