Achieving nanoconfinement-controlled synthesis of nanoplatelets(NPLs)via solution process under ambient condition remains a challenge.In this work,we developed a general ligand-induced strategy to synthesize colloidal stable all-inorganic semiconductor NPLs with controllable lateral dimensions.By introducing certain metal salts(cations:Zn^(2+)and In^(3+),anions:NO_(3)^(−),BF_(4)^(−),or triflate OTf−),wurtzite-structured(WZ-)CdS,CdSe,CdTe,and alloy Cd1−xZnxSe NPLs were directly synthesized in solution through the controlled diffusion of magic-size clusters(MSCs)at room temperature.Mechanism studies revealed that destabilization of MSCs and nanoconfined growth in templates facilitated the formation of NPLs.The present study not only provides a new synthetic route for the preparation of NPLs but also helps to provide insight into their probable formation mechanism and presents an important advance toward the rational design of functional nanomaterials.

BACKGROUND:The ion shift index(ISI) as a prognostic indicator that can show the severity of hypoxic-ischemic injury.We aimed to evaluate the performance of the ISI in predicting unfavorable neurological outcomes at hospital discharge in cardiac arrest(CA) patients following extracorporeal cardiopulmonary resuscitation(ECPR) and to compare its performance to other prognostic predictors.METHODS:This was a retrospective observational study including adult CA patients treated with ECPR between January 2018 and December 2022 in a tertiary hospital.Data regarding clinical characteristics and laboratory parameters were collected from medical records.The ISI was determined based on the first available serum electrolyte levels after ECPR.The primary outcome was unfavorable neurological status at hospital discharge,defined as Cerebral Performance Categories 3–5.Comparisons of the characteristics between the two groups were made using the χ2 test for categorical variables and the t-test or non-parametric Mann-Whitney U-test for continuous variables,as appropriate.Correlation analysis was performed using Spearman’s rank correlation coeficient.A two-tailed P-value 3.12 predicted unfavorable outcomes with a sensitivity and specificity of 74.6% and 85.2%,respectively(P<0.001).The prognostic performance of ISI(area under the curve [AUC]=0.887) was similar to that of other predictors,such as gray-to-white matter ratio(AUC=0.850,P=0.433) and neuronspecific enolase(AUC=0.925,P=0.394).CONCLUSION:ISI may be used as a prognostic biomarker to predict neurological outcomes in CA patients

Introduction:Plasma circulating tumor DNA(ctDNA)is an ideal approach to detecting the epidermal growth factor receptor(EGFR)T790M mutation,which is a major mechanism of resistance to first-generation EGFR-tyrosine kinase inhibitor(TKI)therapy.The present study aimed to explore the association of ctDNA-identified T790M mutation with disease failure sites and clinical prognosis in non-small cell lung cancer(NSCLC)patients.Methods:Patients who progressed on first-generation TKIs were categorized into failure site groups of chest limited(CF),brain limited(BF)and other(OF).Amplification refractory mutation system(ARMS)and droplet digital PCR(ddPCR)were used to identify the T790M mutation in ctDNA.Prognosis was analyzed with Kaplan-Meier methods.Results:Overall concordance between the two methods was 78.3%.According to both ARMS and ddPCR,patients in the OF group had a significantly higher rate of T790M mutation than did patients in the BF and CF groups(P<0.001),and a significantly higher T790M mutation rate was also observed in OF-group patients than in those in the CF and BF groups(P<0.001).AZD9291 was found to be an excellent treatment option and yielded the longest survival for T790M+patients in all groups who had progressed on EGFR-TKIs;for other treatments,the prognosis of T790M−patient subgroups varied.Conclusions:The present study demonstrates that T790M mutation in ctDNA is associated with failure sites for NSCLC patients after EGFR-TKI therapy and indicates that both failure site and T790M mutational status greatly influ-ence treatment selection and prognosis.

Circulating cell-free DNAs(cfDNAs)are fragmented DNA molecules released into the blood by cells.Previous studies have suggested that mitochondria-originated cfDNA fragments(mt-cfDNAs)in cancer patients are more fragmented than those from healthy controls.However,it is still unknown where these short mtcfDNAs originate,and whether the length of mt-cfDNAs can be correlated with tumor burden and cancer progression.In this study,we first performed whole-genome sequencing analysis(WGS)of cfDNAs from a human tumor cell line-xenotransplantation mouse model and found that mt-cfDNAs released from transplanted tumor cells were shorter than the mouse counterpart.We next analyzed blood cfDNA samples from hepatocellular carcinoma and prostate cancer patients and found that mt-cfDNA lengths were inversely related to tumor size as well as the concentration of circulating tumor DNA.Our study suggested that monitoring the size of mt-cfDNAs in cancer patients would be a useful way to estimate tumor burden and cancer progression.

目的比较急诊行交锁髓内钉内固定术与微创经皮钢板固定术(mininally invasive percutaneous plate osteosynthesis,MIPPO)治疗Al型胫骨中下段骨折的临床疗效。方法选取2017年1月至2020年12月于武汉市汉口医院治疗的58例A1型胫骨中下段骨折患者为研究对象,根据手术方法不同,将纳入患者分为髓内钉组(n=30)和MIPPO组(n=28)。比较两组患者的手术时间、术中出血量、术中透视次数、切口长度、切口感染率、术后完全负重时间、骨折愈合时间和疗效。结果髓内钉组患者的手术时间长于MIPPO组,术中出血量、透视次数均多于MIPPO组(P0.05)。结论MIPPO具有手术时间短、出血量少、透视次数少等优势,交锁髓内钉术后完全负重时间更早,有利于患者的康复锻炼,但二者在治疗A1型胫骨中下段骨折的远期疗效方面并无差别。

为了探究‘丹霞2号’红茶在鲜叶采摘、萎凋、揉捻、发酵和干燥等5个加工阶段次生代谢产物动态变化特征及其对茶品质形成的影响,利用液相色谱串联四极杆—静电场轨道阱—线性离子阱质谱系统(LC-Q-LIT-OT-MS)对不同阶段茶叶进行了代谢组学分析。在‘丹霞2号’茶叶中共鉴定到124种代谢物,包括22种儿茶素类、19种儿茶素聚合物类和32种黄酮及黄酮糖苷等。多维统计数据分析显示,在由鲜叶采摘到萎凋、萎凋到揉捻、揉捻到发酵、发酵到干燥的转变中分别涉及162、412、308、126种代谢物的显著变化。其中红茶滋味品质特征成分种类和含量在揉捻和发酵两个阶段变化最为显著。儿茶素类物质的含量随着加工过程整体呈现下降趋势;儿茶素聚合物在揉捻期含量显著升高,随后在发酵和干燥过程中逐渐降低。黄酮类及黄酮糖苷类物质的含量整体呈现增加趋势,在揉捻或发酵阶段升高最为明显。儿茶素类、儿茶素聚合物、黄酮及其糖苷类的含量与天冬氨酸、谷氨酰胺、苯丙氨酸及可溶性糖的含量呈现正相关。

Crassulacean acid metabolism(CAM)has high water-use efficiency(WUE)and is widely recognized to have evolved from C3 photosynthesis.Different plant lineages have convergently evolved CAM,but the molecular mechanism that underlies C3-to-CAM evolution remains to be clarified.Platycerium bifurcatum(elkhorn fern)provides an opportunity to study the molecular changes underlying the transition from C3 to CAM photosynthesis because both modes of photosynthesis occur in this species,with sporotrophophyll leaves(SLs)and cover leaves(CLs)performing C3 and weak CAM photosynthesis,respectively.Here,we report that the physiological and biochemical attributes of CAM in weak CAM-performing CLs differed from those in strong CAM species.We investigated the diel dynamics of the metabolome,proteome,and transcriptome in these dimorphic leaves within the same genetic background and under identical environmental conditions.We found that multi-omic diel dynamics in P.bifurcatum exhibit both tissue and diel effects.Our analysis revealed temporal rewiring of biochemistry relevant to the energy-producing pathway(TCA cycle),CAM pathway,and stomatal movement in CLs compared with SLs.We also confirmed that PHOSPHOENOLPYRUVATE CARBOXYLASE KINASE(PPCK)exhibits convergence in gene expression among highly divergent CAM lineages.Gene regulatory network analysis identified candidate transcription factors regulating the CAM pathway and stomatal movement.Taken together,our results provide new insights into weak CAM photosynthesis and new avenues for CAM bioengineering.

目的调查体外膜肺氧合辅助心肺复苏(extracorporeal cardiopulmonary resuscitation,ECPR)治疗急性心肺梗死(acute myocardial infarction,AMI)的90 d主要肾脏不良事件(major adverse kidney events,MAKE)。方法回顾性分析南京医科大学第一附属医院急诊中心2015年4月至2023年4月75例AMI-ECPR的资料,以90 d存活/死亡、有/无RRT(renal replacement therapy)、是否AKI(acute kidney injury)原因启动RRT进行分组,比较年龄、性别、查尔森合并症指数、IHCA/OHCA(out-of-hospital/in-hospital cardiac arrest)、初始心律、Gensini评分、ECPR初始血气pH和乳酸值、无灌流时间、心脏骤停至ECMO(extracorporeal membrane oxygenation)转流时间(CA-Pump On时间)、ECMO和RRT治疗时间、90 d存活率等的差异,并随访幸存者的肾功能情况。结果①68例AMI-ECPR患者纳入分析,90 d存活22人(32.4%),共有54人(79.4%)联合RRT,90 d MAKE有48人(70.6%)。②与死亡组相比,90 d存活组初始可电击心律的比例高,Gensini评分低,且ECPR初始血气pH值高、乳酸值低。③RRT组冠状动脉病变、ECPR初始酸中毒和高乳酸血症的严重程度显著高于非RRT组,非RRT组悉数存活。④AKI-RRT组与非AKI-RRT组之间差异无统计学意义,21人AKI 1期启动RRT,存活5人,其中1人90 d仍需要RRT,7人AKI 2~3期启动RRT均死亡。结论AMI-ECPR患者90 d MAKE的发生率高达70.6%,合并AKI的AMI-ECPR幸存者90 d肾功能恢复不良率高达20.0%,积极启动RRT避免发生AKI或在AKI早期启动RRT,可能改善AMI-ECPR患者的预后。

目的总结体外膜肺氧合辅助心肺复苏(extracorporeal cardiopulmonary resuscitation,ECPR)治疗心脏性猝死(sudden cardiac death,SCD)的经验和效果。方法回顾性分析南京医科大学第一附属医院急诊中心2015年4月至2023年4月120例成人SCD-ECPR的资料,以90 d存活/死亡、OHCA/IHCA(out-of-hospital/in-hospital cardiac arrest)、是否为急性心肌梗死(acute myocardial infarction,AMI)和心脏骤停至ECMO(extracorporeal membrane oxygenation)转流时间(CA-Pump On时间)60 min为界进行分组,比较年龄、性别、查尔森合并症指数、IHCA/OHCA、初始心律、无灌流时间、CA-Pump On时间、ECMO撤离成功率、90 d存活率、ECMO治疗时间的差异。结果①114例SCD-ECPR患者纳入分析,90 d存活45例(39.5%),其中40例(88.9%)神经功能预后良好。②90 d存活组的年龄和无灌流时间显著低于死亡组,且IHCA和初始可电击心律的比例高。③IHCA组的无灌流时间显著低于OHCA组,且90 d存活率高。④OHCA和院际联动延长CA-Pump On时间,降低90 d存活率。⑤AMI组的年龄大且查尔森合并症指数高,90天存活率显著低于非AMI组。结论ECPR改善SCD患者的预后,健康预期寿命长、IHCA、初始可电击心律、无灌流时间短的患者高获益。流畅的SCD-ECPR生命救治链提高存活率,筛选OHCA、ECPR启动延迟或需要院际联动的高获益人群,尽管CA-Pump On时间>60 min,仍有生存潜力。

目的检索、评价并总结体外心肺复苏院内启动准备流程的相关证据, 以期为临床开展体外心肺复苏提供循证依据。方法采用循证护理研究方法, 根据"6S"证据模型, 自上而下检索美国指南网、苏格兰院际间指南网、英国国家临床医学研究所指南网等网站及UpToDate、The Cochrane Library、PubMed、Embase、中国知网、万方等数据库中与体外心肺复苏院内启动准备相关的指南、临床决策、专家共识、系统评价等文献, 检索日期为建库至2023年5月20日。研究者对检索文献进行筛选、质量评价、证据提取并整合。结果最终纳入11篇文献, 包括2篇指南, 6篇专家共识, 1篇系统评价, 2篇类实验研究, 从医疗条件、团队组建、物品管理、运行机制、启动前救治及启动判断6个方面总结出18条最佳证据。结论本研究总结了体外心肺复苏院内启动准备流程的相关证据, 可为促进体外心肺复苏推广及实施提供参考。后续研究中仍需要着重关注体外心肺复苏的团队建设、人员培训考核以及管理制度优化, 以提高体外心肺复苏的救治效率及准备度。
