Association of fluid-attenuated inversion recovery vascular hyperintensity with ischaemic events in internal carotid artery or middle cerebral artery occlusion

Background and purpose Individuals with intracranial artery occlusion have high rates of ischaemic events and *** has been challenging to identify patients who had high-risk stroke using a simple,valid and non-invasive screening *** study aimed to investigate whether fluid-attenuated inversion recovery(FLAIR)vascular hyperintensity(FVH),a specific imaging sign on the FLAIR sequence,could be a predictor of ischaemic events in a population with internal carotid artery(ICA)or middle cerebral artery(MCA)*** We retrospectively analysed 147 patients(mean 60.43±12.83 years)with 149 lesions,including 37 asymptomatic and 112 symptomatic cases of ICA or MCA *** occlusion was considered if ischaemic events were present in the relevant territory within 90 *** Alberta Stroke Program Early Computed Tomography Score(FVH-ASPECTS:0-7,with 0 indicating absence of FVH and 7 suggesting prominent FVH)and collateral circulation grade were assessed for each *** logistic regression analysis was performed to detect independent markers associated with symptomatic *** A lower FVH-ASPECTS was associated with a more favourable collateral circulation grade(rho=−0.464,p<0.0001).The FVH-ASPECTS was significantly lower in the asymptomatic occlusion group than in the symptomatic occlusion group(p<0.0001).FVH-ASPECTS(Odd ratio,2.973;95%confidence interval,1.849 to 4.781;p<0.0001)was independently associated with symptomatic status after adjustment for age,sex,lesion location and collateral circulation grade in the multivariate logistic *** area under the curve was 0.861 for the use of FVH-ASPECTS to identify symptomatic *** The ability to discriminate symptomatic from asymptomatic occlusion suggests that FVH may be a predictor of *** a simple imaging sign,FVH may serve as a surrogate for haemodynamic impairments and can be used to identify high-risk stroke cases early in ICA or

Emergence of the colistin resistance gene,mcr-1,has attracted worldwide *** the prevalence of mcr-1-positive Escherichia coli(MCRPEC)strains in human carriage showing a significant decrease between 2016 and 2019,genetic differences in MCRPEC strains remain largely *** therefore conducted a comparative genomic study on MCRPEC strains from fecal samples of healthy human subjects in 2016 and *** identified three major differences in MCRPEC strains between these two time ***,the insertion sequence ISApl1 was often deleted and the percentage of mcr-1-carrying IncI2 plasmids was increased in MCRPEC strains in ***,the antibiotic resistance genes(ARGs),aac(3)-IVa and blaCTX-M-1,emerged and coexisted with mcr-1 in ***,MCRPEC strains in 2019 contained more virulence genes,resulting in an increased proportion of extraintestinal pathogenic ***(ExPEC)strains(36.1%)in MCRPEC strains in 2019 compared to that in 2016(10.5%),implying that these strains could occupy intestinal ecological niches by competing with other commensal *** results suggest that despite the significant reduction in the prevalence of MCRPEC strains in humans from 2016 to 2019,MCRPEC exhibits increased resistance to other clinically important ARGs and contains more virulence genes,which may pose a potential public health threat.

Although carbapenem use is prohibited in animals in China,carbapenem-resistant Escherichia coli(CREC),especially New Delhi metallo-β-lactamase(NDM)-producing strains,are widely prevalent in foodproducing *** present,the impact of livestock-associated CREC strains on human populations at the national level is ***,we conduct a retrospective cross-sectional study to investigate the prevalence of CREC from clinical settings across 22 Chinese provinces or municipalities and analyze anthropogenic factors associated with their *** also ascertain the blaNDMand blaKPCabundance among pig and chicken farms and present a detailed genomic framework for CREC of animal and human ***,631/29799(2.1%)clinical Escherichia coli(***)isolates were identified as *** analysis revealed that being male,an age below 1,an age between 13 and 18,provinces with greater chicken production,and provinces with higher pig production were associated with higher odds of CREC *** general,73.8%(n=45/61)of pig farms and 62.2%(n=28/45)of chicken farms had a blaNDMabundance of 1×10^(-5)to 1×10^(-3)and 1×10^(-3)to 1×10^(-2),*** all the Chinese NDM-positive ***(n=463)available at the National Center for Biotechnology Information(NCBI),the genomic analysis revealed that blaNDM-5and Inc X3 were the predominant carbapenemase gene-plasmid combination,while a highly homogeneous relationship between NDM-positive isolates from humans and animals was demonstrated at the plasmid and core genome *** the findings suggest frequent CREC transmission between humans and animals,indicating that further discussions on the use of antibiotics in animals and humans are needed,both in China and across the globe.

报道1例在杭州市中医院内分泌科就诊的肥胖2型糖尿病肾脏病患者,因血糖控制不佳、蛋白尿,使用司美格鲁肽治疗的过程,并进行文献复习。患者为44岁男性,因“血糖升高8年,加重伴双下肢浮肿1周”就诊,入院后完善各项检查,诊断为糖尿病肾脏病Ⅲ期,使用胰岛素泵强化治疗后改用司美格鲁肽1.0 mg每周1次联合联合达格列净(10 mg,1次/d)口服治疗,血糖控制良好,尿微量白蛋白降低,体重减轻。司美格鲁肽是一种兼顾降糖和保护心血管的胰高糖素样肽-1受体激动剂(GLP-1RA)周制剂,可用于肥胖2型糖尿病肾脏病的治疗。

The molecular mechanism of silkworm resistance to Bombyx mori nucleopolyhedrovirus(BmNPV) infection still remains unclear. The chaperonin containing t-complex polypeptide 1(TCP-1) is essential in the folding of tubulin and actin to produce stable and functionally competent protein conformations, but little is known about this protein in silkworm. In this study, a gene encoding TCP-1 beta protein in silkworm was identified, which has an open reading fragment of 1611 bp, encoding a predicted 536 amino acid residues with a molecular weight of approximately 57.6 k Da, and containing a Cpn60CP1 functional domain, the sequence conservatism is 81.52%. The highest transcription level of Bm TCP-1 beta mR NA was in midgut, while the lowest was in hemolymph. To further study the function of BmT CP-1 beta, the expression of BmTCP-1 beta was knocked down with ds RNA, which could result in the down-regulation of cytoskeletal related genes, including actin and tubulin, which was also confirmed by over-expression of Bm TCP-1 beta in BmN using p IZT/V5-His-Mcherry insect vector. Besides, knock-down of Bm TCP-1 beta could prolong the infection process of BmNPV in BmN, which was also confirmed in BmN that contained p IZT/V5-His-Mcherry-BmTCP-1 beta. Based on the study above, we concluded that BmT CP-1 beta plays an essential role in BmNPV infection by regulating the expression of tubulin and actin. Taken together, our work provides a valuable data support for clarifying the molecular mechanism of silkworm resistance to BmNPV infection.

The emergence and spread of the mobile colistin-resistance gene,mcr-1,and its variants pose achallenge to the use of colistin,a last-resort antibiotic used to treat severe infections caused by extensively drug-resistant(XDR)Gram-negative *** adjuvants are a promising strategy to enhance the efficacy of colistin against colistin-resistant pathogens;however,few studies have considered the effects of adjuvants on limiting resistance-gene *** found that chelerythrine(4 mg·L^(-1))derived from Macleaya cordata extract,which is used as an animal feed additive,reduced the minimal inhibitory concentration(MIC)of colistin against an mcr-1 positive Escherichia coli(***)strain by 16-fold(from 2.000 to 0.125 mg·L^(-1)).eliminated approximately 10^(4) colony-forming units(CFUs)of an mcr-1-carrying strain in a murine intestinal infection model,and inhibited the conjugation of an mcr-1-bearing plasmid in vitro(by>100-fold)and in a mouse model(by up to 5-fold).A detailed analysis revealed that chelery-thrine binds to phospholipids on bacterial membranes and increases cytoplasmic membrane fluidity,thereby impairing respiration,disrupting proton motive force(PMF),generating reactive oxygen species(ROS),and decreasing intracellular adenosine triphosphate(ATP)levels,which subsequently downregu-lates mcr-1 and conjugation-associated *** dual effects of chelerythrine can expand the use of antibiotic adjuvants and may provide a new strategy for circumventing mobile colistin resistance.
