Role of social organization engagement in high-risk groups intervention against HIV/AIDS:a case study from 176 cities of China

Background: A high-risk prevention strategy is an effective way to fight against human immunodeficiency virus (HIV) and acquired immunodeficiency syndrome (AIDS). The China AIDS Fund for Non-Governmental Organizations (CAFNGO) was established in 2015 to help social organizations intervene to protect high-risk populations in 176 cities. This study aimed to evaluate the role of social organizations in high-risk population interventions against HIV/AIDS.Methods: This study was based on the CAFNGO program from 2016 to 2020. The collected data included the number and types of social organizations participating in high-risk group interventions and the amount of funds obtained by these organizations each year. We explored the factors influencing the number of newly diagnosed AIDS cases using a spatial econometric model. Furthermore, we evaluated the effectiveness of intervention activities by comparing the percentages of the individuals who initially tested positive, and the individuals who took the confirmatory test, as well as those who retested positive and underwent the treatment.Results: Overall, from 2016 to 2020, the number of social organizations involved in interventions to protect HIV/AIDS high-risk populations increased from 441 to 532, and the invested fund increased from $3.98 to $10.58 million. The number of newly diagnosed cases decreased from 9128 to 8546 during the same period. Although the number of cities with overall spatial correlations decreased, the spatial agglomeration effect persisted in the large cities. Citywise, the number of social organizations (direct effect 19.13), the permanent resident population (direct effect 0.12), GDP per capita (direct effect 17.58;indirect effect -15.38), and passenger turnover volume (direct effect 5.50;indirect effect -8.64) were the major factors influencing new positive cases confirmed through the testing interventions performed by the social organizations. 目前,中国DRG(Diagnosis-Related Group)支付方式改革已初显成效,公立医院如何在这一制度背景下迈向高质量发展道路备受关注。本文采用诠释主义案例研究范式,深描在DRG支付制度背景下公立医院“谋生行义”的过程,归纳出公立医院“义生耦合”的高质量发展机制:正式制度嵌入设计激励约束机制,而政治嵌入若发挥纠正作用将促使组织激励结构趋同;医院通过非正式制度嵌入传导激励约束机制,实现组织生存和制度落地;制度嵌入性与政治嵌入性嵌合形成公立医院高质量发展助力,有利于实现利益主体间激励相容。DRG支付制度背景下公立医院变革及公共治理的实践已十分丰富,本文界定了制度嵌入性和政治嵌入性的概念,为上述情境提供了新的理论解释框架,并通过质性案例研究进行学理归纳;另外,文章拓展了新经济社会学理论在中国行政事业单位的应用。

目的:对浙江省县域中医药服务能力进行综合评价,为优化县域中医药卫生资源配置提供参考。方法:计算浙江省71家县级中医院中医药服务能力莫兰指数,采用LISA聚类分析探究空间自相关关系,构建空间回归模型,分析影响县域中医药服务能力的影响因素。结果:浙江省县域中医药服务能力莫兰指数为0.193(P<0.05),存在显著的空间正相关关系;人均生产总值、农村居民人均可支配收入和60岁以上人口占比等因素对县域中医药服务能力具有显著影响。结论:健全中医药管理体制、考核机制和综合评价体系,加大财政对中医药的投入与政策倾斜,推进优质医疗资源有效扩容和均衡布局,提升县域中医药服务能力。

目的:分析DRG高倍率病例特征及住院费用结构,为医院精细化管理和科学控费提供参考依据。方法:选取某三甲医院2021年度高倍率病例1 618例,正常倍率病例37 738例。描述高倍率病例特征,比较高倍率组与正常倍率组的差异,采用灰色关联度分析高倍率病例住院费用的关联结构。结果:与正常倍率组相比,高倍率组在住院天数、性别、次诊断数量、出院情况以及除其他类费用外的所有住院费用均存在显著差异(P<0.05)。住院费用结构中,多组诊断费与住院总费用关联最大。总体灰色关联度排序中诊断费关联最高,治疗费次之,西药费对住院总费用的影响高于中药费。结论:应推动特病单议和专家评审制度,鼓励新技术及重点专科建设;科学制定高倍率病例认定标准,落实医保办专人包保联动制;加强高倍率病例审核,创新智能监管手段;优化临床路径管理,促进中西医结合治疗。

Role of methoxy and C_(α)-based substituents in electrochemical oxidation mechanisms and bond cleavage selectivity of β-O-4 lignin model compounds

In order to better understand the specific substituent effects on the electrochemical oxidation process of β-O-4 bond, a series of methoxyphenyl type β-O-4 dimer model compounds with different localized methoxyl groups, including 2-(2-methoxyphenoxy)-1-phenylethanone, 2-(2-methoxyphenoxy)-1-phenylethanol, 2-(2-methoxyphenoxy)-1-(4-methoxyphenyl)ethanone, 2-(2-methoxyphenoxy)-1-(4-methoxyphenyl)ethanol, 2-(2,6-dimethoxyphenoxy)-1-(4-methoxyphenyl)ethanone, 2-(2,6-dimethoxyphenoxy)-1-(4-methoxyphenyl)ethanol have been selected and their electrochemical properties have been studied experimentally by cyclic voltammetry, and FT-IR spectroelectrochemistry. Combining with electrolysis products distribution analysis and density functional theory calculations, oxidation mechanisms of all six model dimers have been explored. In particular, a total effect from substituents of both para-methoxy(on the aryl ring closing to Cα) and Cα-OH on the oxidation mechanisms has been clearly observed, showing a significant selectivity on the Cα-Cβbond cleavage induced by electrochemical oxidations.

Big Data Access Control Mechanism Based on Two-Layer Permission Decision Structure

Big data resources are characterized by large scale, wide sources, and strong dynamics. Existing access controlmechanisms based on manual policy formulation by security experts suffer from drawbacks such as low policymanagement efficiency and difficulty in accurately describing the access control policy. To overcome theseproblems, this paper proposes a big data access control mechanism based on a two-layer permission decisionstructure. This mechanism extends the attribute-based access control (ABAC) model. Business attributes areintroduced in the ABAC model as business constraints between entities. The proposed mechanism implementsa two-layer permission decision structure composed of the inherent attributes of access control entities and thebusiness attributes, which constitute the general permission decision algorithm based on logical calculation andthe business permission decision algorithm based on a bi-directional long short-term memory (BiLSTM) neuralnetwork, respectively. The general permission decision algorithm is used to implement accurate policy decisions,while the business permission decision algorithm implements fuzzy decisions based on the business constraints.The BiLSTM neural network is used to calculate the similarity of the business attributes to realize intelligent,adaptive, and efficient access control permission decisions. Through the two-layer permission decision structure,the complex and diverse big data access control management requirements can be satisfied by considering thesecurity and availability of resources. Experimental results show that the proposed mechanism is effective andreliable. In summary, it can efficiently support the secure sharing of big data resources.

FKF1b controls reproductive transition associated with adaptation to geographical distribution in maize

Maize(Zea mays subspecies mays)is an important commercial crop across the world,and its flowering time is closely related to grain yield,plant cycle and latitude adaptation.FKF1 is an essential clock-regulated blue-light receptor with distinct functions on flowering time in plants,and its function in maize remains unclear.In this study,we identified two FKF1 homologs in the maize genome,named ZmFKF1a and ZmFKF1b,and indicated that ZmFKF1a and ZmFKF1b independently regulate reproductive transition through interacting with ZmCONZ1 and ZmGI1 to increase the transcription levels of ZmCONZ1 and ZCN8.We demonstrated that ZmFKF1b underwent artificial selection during modern breeding in China probably due to its role in geographical adaptation.Furthermore,our data suggested that ZmFKF1bHap_C7 may be an elite allele,which increases the abundance of ZmCONZ1 mRNA more efficiently and adapt to a wider range of temperature zone than that of ZmFKF1bHap_Z58 to promote maize floral transition.It extends our understanding of the genetic diversity of maize flowering.This allele is expected to be introduced into tropical maize germplasm to enrich breeding resources and may improve the adaptability of maize at different climate zones,especially at temperate region.

Responses of plant diversity and soil microorganism diversity to nitrogen addition in the desert steppe,China

Nitrogen(N)deposition is a significant aspect of global change and poses a threat to terrestrial biodiversity.The impact of plant-soil microbe relationships to N deposition has recently attracted considerable attention.Soil microorganisms have been proven to provide nutrients for specific plant growth,especially in nutrient-poor desert steppe ecosystems.However,the effects of N deposition on plant-soil microbial community interactions in such ecosystems remain poorly understood.To investigate these effects,we conducted a 6-year N-addition field experiment in a Stipa breviflora Griseb.desert steppe in Inner Mongolia Autonomous Region,China.Four N treatment levels(N0,N30,N50,and N100,corresponding to 0,30,50,and 100 kg N/(hm2•a),respectively)were applied to simulate atmospheric N deposition.The results showed that N deposition did not significantly affect the aboveground biomass of desert steppe plants.N deposition did not significantly reduce the alfa-diversity of plant and microbial communities in the desert steppe,and low and mediate N additions(N30 and N50)had a promoting effect on them.The variation pattern of plant Shannon index was consistent with that of the soil bacterial Chao1 index.N deposition significantly affected the beta-diversity of plants and soil bacteria,but did not significantly affect fungal communities.In conclusion,N deposition led to co-evolution between desert steppe plants and soil bacterial communities,while fungal communities exhibited strong stability and did not undergo significant changes.These findings help clarify atmospheric N deposition effects on the ecological health and function of the desert steppe.

High-entropy alloys(HEAs),which were introduced as a pioneering concept in 2004,have captured the keen interest of nu-merous researchers.Entropy,in this context,can be perceived as representing disorder and randomness.By contrast,elemental composi-tions within alloy systems occupy specific structural sites in space,a concept referred to as structure.In accordance with Shannon entropy,structure is analogous to information.Generally,the arrangement of atoms within a material,termed its structure,plays a pivotal role in dictating its properties.In addition to expanding the array of options for alloy composites,HEAs afford ample opportunities for diverse structural designs.The profound influence of distinct structural features on the exceptional behaviors of alloys is underscored by numer-ous examples.These features include remarkably high fracture strength with excellent ductility,antiballistic capability,exceptional radi-ation resistance,and corrosion resistance.In this paper,we delve into various unique material structures and properties while elucidating the intricate relationship between structure and performance.

Endogenous biosynthesis of docosahexaenoic acid(DHA)regulates fish oocyte maturation by promoting pregnenolone production

Omega-3 polyunsaturated fatty acids(n-3 PUFAs),particularly docosahexaenoic acid(22:6n-3,DHA),play crucial roles in the reproductive health of vertebrates,including humans.Nevertheless,the underlying mechanism related to this phenomenon remains largely unknown.In this study,we employed two zebrafish genetic models,i.e.,elovl2^(-/-)mutant as an endogenous DHAdeficient model and fat1(omega-3 desaturase encoding gene)transgenic zebrafish as an endogenous DHA-rich model,to investigate the effects of DHA on oocyte maturation and quality.Results show that the elovl2^(-/-)mutants had much lower fecundity and poorer oocyte quality than the wild-type controls,while the fat1 zebrafish had higher fecundity and better oocyte quality than wildtype controls.DHA deficiency in elovl2^(-/-)embryos led to defects in egg activation,poor microtubule stability,and reduced pregnenolone levels.Further study revealed that DHA promoted pregnenolone synthesis by enhancing transcription of cyp11a1,which encodes the cholesterol side-chain cleavage enzyme,thereby stabilizing microtubule assembly during oogenesis.In turn,the hypothalamic-pituitary-gonadal axis was enhanced by DHA.In conclusion,using two unique genetic models,our findings demonstrate that endogenously synthesized DHA promotes oocyte maturation and quality by promoting pregnenolone production via transcriptional regulation of cyp11a1.
