This study aimed to exploit a new virtual block method to spare normal lung tissue in VMAT planning for patients with locally advanced non-small cell lung cancer(LA-NSCLC).The previous method was used to manually restrict the angle of the beam passing through,which ignored the location and shape of large targets that varied between different slices and did not block the beamlets precisely.Unlike the previous method,this new virtual block method was used to block the beamlets when necessary by closing the multi-leaf collimator(MLC)at prerequisite angles.The algorithm for closing the MLC depended on the thickness of the beamlets passing through the lungs and avoided only the entrance radiation beamlet.Moreover,this block can be automatically contoured.A retrospective study was performed to compare the VMAT plans with and without the virtual block method for 17 LANSCLC patients,named the block plan(B-plan)/non-block plan(N-plan).All cases were selected in this study because of the large tumor size and unmet dose constraints of the lungs.In addition to the maximum dose constraint for the virtual block,B-plans adopted identical optimization parameters to N-plans for each patient.These two types of plans were compared in terms of dosimetric indices and plan scores.The results were statistically analyzed using the Wilcoxon nonparametric signed-rank test.B-plans have advantages in the following dosimetric metrics that have statistical significance(p<0.05):(1)lower V_(5)/V_(10)/D_(mean)/normal tissue complication probability(NTCP)of total lungs;(2)reductions in V_(5)/V_(10)for the contralateral lung;(3)decrease in Dmean/V_(40)of the heart;(4)decrease in esophagus V_(40);(5)reductions in Dmean,V_(5)/V_(10) of normal tissue.B-plans(82.51±7.07)achieved higher-quality scores than N-plans(80.74±7.22).The new virtual block spared the lungs as well as other normal structures in VMAT planning for LA-NSCLC.Thus,the block method may decrease the risk of radiation-related toxicity in patients.

BACKGROUND Hepatitis B cirrhosis(HBC)is a chronic disease characterized by irreversible diffuse liver damage and aggravated by intestinal microbial imbalance and metabolic dysfunction.Although the relationship between certain single probiotics and HBC has been explored,the impact of the complex ready-to-eat Lactobacillus paracasei N1115(LP N1115)supplement on patients with HBC has not been determined.AIM To compare the changes in the microbiota,inflammatory factor levels,and liver function before and after probiotic treatment in HBC patients.METHODS This study included 160 HBC patients diagnosed at the General Hospital of Ningxia Medical University between October 2018 and December 2020.Patients were randomly divided into an intervention group that received LP N1115 supplementation and routine treatment and a control group that received routine treatment only.Fecal samples were collected at the onset and conclusion of the 12-wk intervention period.The structure of the intestinal microbiota and the levels of serological indicators,such as liver function and inflammatory factors,were assessed.RESULTS Following LP N1115 intervention,the intestinal microbial diversity significantly increased in the intervention group(Pnal microbiota was characterized by an increase in the proportions of probiotic microbes and a reduction in harmful bacteria.Additionally,the intervention group demonstrated notable improvements in liver function indices and significantly lower levels of inflammatory factors(Pnal microbial imbalance in HBC patients by modulating the structure of the intestinal microbiota,improving liver function,and reducing inflammatory factor levels.

Developing new drugs to treat Parkinson's disease efficiently is challenging. Here we report that chitosan nanoparticles(APPDNs) could serve as novel candidates for the design of anti-PD drugs. In this study, we investigated the effects of chitosan poly ethyleneglycol-poly lactic acid(PEG-PLA) nanoparticles conjugated with nerve growth factor(NGF), acteoside(ACT) and plasmid Dna(p Dna) for PD therapy using in vitro and in vivo models. Using PD cell models, we demonstrated that APPDN had good neuroprotective effects. More significantly, experiments using mouse PD models demonstrated that APPDNs could ameliorate the behavioral disorders of sick mice. Immunohistochemical and western blot(WB) analyses demonstrated that APPDNs could significantly reverse dopaminergic(DA) neuron loss in the substantia nigra and striatum of sick mice. This study opens up a novel avenue to develop anti-PD drugs.

The pandemic COVID-19 is certainly one of the most severe infectious diseases in human history.In the last 2 years,the COVID-19 pandemic has caused over 418.6 million confirmed cases and 5.8 million deaths world-wide.Young people make up the majority of all infected COVID-19 cases,but the mortality rate is relatively lower compared to older age groups.Currently,about 55.04%individuals have been fully vaccinated rapidly approaching to herd immunity globally.The challenge is that new SARS-CoV-2 variants with potential to evade immunity from natural infection or vaccine continue to emerge.Breakthrough infections have occurred in both SARS-CoV-2 naturally infected and vaccinated individuals,but breakthrough infections tended to exhibit mild or asymptomatic symptoms and lower mortality rates.Therefore,immunity from natural infection or vaccination can reduce SARS-CoV-2 pathogenicity,but neither can completely prevent SARS-CoV-2 infection/reinfection.Fortunately,the morbidity and mortality of COVID-19 continue to decline.The 7-day average cumulative case fatality of COVID-19 has decreased from 12.3%on the February 25,2020,to 0.27%on January 09,2022,which could be related to a decreased SARS-CoV-2 variant virulence,vaccine immunization,and/or better treatment of patients.In conclusion,elimination of SARS-CoV-2 in the world could be impossible or at least an arduous task with a long way to go.The best strategy to prevent COVID-19 pandemic is to expand inoculation rate of effective vaccines.As the population reaches herd immunity,the mortality rate of COVID-19 may continue to decrease,and COVID-19 could eventually become another common cold.

The continuing increase in human activities is causing global changes such as increased deposition of atmospheric nitrogen. There is considerable interest in understanding the effects of increasing atmospheric nitrogen deposition on soil enzyme activities, specifically in terms of global nitrogen cycling and its potential future contribution to global climate change. This paper summarizes the ecological effects of atmospheric nitrogen deposition on soil enzyme activities, including size-effects, stage-effects, site-effects, and the effects of different levels and forms of atmospheric nitrogen deposition. We discuss needs for further research on the relationship between atmospheric nitrogen deposition and soil enzymes.

An unprecedented enzyme-catalyzed asymmetric domino aza-Michael/aldol reaction of 2-aminobenzaldehyde and a,b-unsaturated aldehydes is achieved. Pepsin from porcine gastric mucosa provided mild and efficient access to diverse substituted 1,2-dihydroquinolines in yields of 38%–97% with 6%–24%enantiomeric excess（ee）. This work not only provides a novel method for the synthesis of dihydroquinoline derivatives, but also promotes the development of enzyme catalytic promiscuity.

Hybrid-panel-based new design idea for large reflector antenna

Large reflector antennas are widely used as radio telescopes and active main reflectors are generally applied to improve the surface accuracy. Considering that the high cost has been one important problem in engineering, it is worth discussing whether it is necessary to install actuators on all the panels. Thus, in this paper, a hybrid-panel-based new design idea for large reflector antenna is proposed. Assuming that the actuators are installed only in the region of the reflector with large deformations and there are no actuators in other region to reduce the actuator number, the surface accuracies and the corresponding electromagnetic(EM) performances calculated by three different panel adjustment strategies are compared. The most effective method is that the deformed reflector should be first preadjusted to reduce the gravity deformation and then the panels equipped with actuators should be adjusted to the locations determined by the best fitting reflector(BFR) derived by the deformed reflector with no actuators. A 35 m reflector antenna is adopted as an example to calculate the surface accuracy and EM performance when parts of the panels are equipped with actuators. The simulation results show that there is no need to install actuators on all panels and the presented method can greatly reduce the number of actuators with guaranteed surface accuracy. Thus, during the antenna structural design phase, once the surface accuracy requirement is given, the number of actuators can be minimized to reduce the manufacturing and maintenance costs as much as possible. This paper can provide valuable guidance for the design of an active main reflector with hybrid panels.

The influence of thermal deformation on the performance of reflector antennas has become increasingly significant with the increasing aperture and working frequency.The use of a thermal compensation database is an efficient method to compensate for the deformation caused by the non-uniform temperature distribution.However,how to efficiently and accurately match and call the database remains as one of the tough challenges for the antenna thermal compensation system to achieve real time compensation.Therefore,this study proposes a data match and call method for the thermal compensation database of the reflector antenna,matching the database from three aspects:the overall rms match of temperature data,the similarity area match of each data sample,and the key area match of key structural positions.The validation of this method is demonstrated in an example.The difference between the pointing adjustment amount calculated by the matched data and the collected data was found to be less than 1",which satisfied the requirements of practical engineering,thus achieving real-time thermal compensation of the antenna.

The present protocol describes an observational cohort study that was designed to propose a therapeutic scheme and formulate an individualized treatment strategy for frail elderly patients diagnosed with multiple diseases in a Chinese,multicenter setting.Over a 3-year period,we will recruit 30,000 patients from 10 hospitals and collect baseline data including patient demographic information,comorbidity characteristic,FRAIL scale,age-adjusted Charlson comorbidity index(aCCI),relevant blood tests,the results of imaging examination,prescription of drugs,length of hospital stay,number of overall re-hospitalizations and death.Elderly patients(≥65 years old)with multimorbidity and receiving hospital care are eligible for this study.Data collection is being performed at baseline and 3,6,9 and 12 months after discharge.Our primary analysis was all-cause death,readmission rate and clinical events(including emergency visits,stroke,heart failure,myocardial infarction,tumor,acute chronic obstructive pulmonary disease,etc).The study is approved by the national Key R&D Program of China(2020YFC2004800).Data will be disseminated in manuscripts submitted to medical journals and in abstracts submitted to international geriatric conferences.Clinical Trial Registration:[www.ClinicalTrials.gov],identifier[ChiCTR2200056070].
