Relationship between the Stratospheric Arctic Vortex and Surface Air Temperature in the Midlatitudes of the Northern Hemisphere

It is known that different relationships exist between the strength and displacement of the stratospheric polar vortex(SPV),and the surface air temperature(SAT)patterns in Eurasia and North America,but the mechanisms behind these relationships remain unclear,especially on an interannual timescale.Based on empirical orthogonal function(EOF)analysis using NCEP reanalysis data over 1958–2018,this study attempts to ascertain the relationship between the SPV intensity and displacement over the Arctic and the SATs in the midlatitudes of the Northern Hemisphere.Our results indicate that a strengthened SPV corresponds to an SAT increase in Eurasia and a decrease in eastern North America and Greenland.When the SPV is shifted towards Eurasia,however,a corresponding SAT increase occurs in both North America and Eurasia,with a larger increase in North America than in Eurasia.Specifically,a strengthened SPV tends to correspond to a positive North Atlantic Oscillation-like circulation in the troposphere with negative geopotential height(GH)anomalies in Greenland and eastern North American continent and positive GH anomalies to the north of 45°N in Eurasia,which corresponds to lower SATs in North America than in Eurasia.However,when the SPV shifted towards Eurasia,it was accompanied by a positive Pacific/North American-like pattern with a deepened Aleutian low,which corresponds to the increasing SATs in North America.These tropospheric circulation changes are related to the response of tropospheric planetary wave activity to the SPV.A strengthened SPV corresponds to the weakening of tropospheric planetary wave-1 waves,which is accompanied by a negative GH in North America but a positive GH in Eurasia.If the SPV shifted towards Eurasia,the tropospheric planetary wave-1(-2)waves strengthened(weakened),and the combined effects of the planetary wave-1 and wave-2 waves would cause positive GH anomalies in both Eurasia and North America.

Refractory hypotension under neuraxial anesthesia for cesarean delivery

Aortocaval compression syndrome is also known as a supine hypotensive syndrome, it is very common in late pregnancy especially in supine positioning under neuraxial anesthesia. It is occurred when the inferior vena cava and the aorta were compressed by the uterus, and often manifested as hypotension, tachycardia, nausea, dizziness, and syncope. Besides, reducing of uteroplacental perfusion may result in morbidity or even mortality to the fetus. It is critical to recognize the cause of hypotension in the pregnant patient, and it is necessary to alleviate the compressing pressure of uterus on the inferior vena cava immediately if suspected of aortocaval compression. We report a pregnant woman with rheumatoid arthritis (RA) who experienced refractory hypotension resistant to large doses of vasopressor therapy when undergoing cesarean delivery with combined spinal-epidural anesthesia.

Multifunctional role of oral bacteria in the progression of non-alcoholic fatty liver disease

Non-alcoholic fatty liver disease (NAFLD) encompasses a spectrum of liver disorders of varying severity, ultimately leading to fibrosis. This spectrum primarily consists of NAFL and non-alcoholic steatohepatitis. The pathogenesis of NAFLD is closely associated with disturbances in the gut microbiota and impairment of the intestinal barrier. Non-gut commensal flora, particularly bacteria, play a pivotal role in the progression of NAFLD. Notably, Porphyromonas gingivalis, a principal bacterium involved in periodontitis, is known to facilitate lipid accumulation, augment immune responses, and induce insulin resistance, thereby exacerbating fibrosis in cases of periodontitis-associated NAFLD. The influence of oral microbiota on NAFLD via the "oral-gut-liver" axis is gaining recognition, offering a novel perspective for NAFLD management through microbial imbalance correction. This review endeavors to encapsulate the intricate roles of oral bacteria in NAFLD and explore underlying mechanisms, emphasizing microbial control strategies as a viable therapeutic avenue for NAFLD.

Infliximab therapy and outcomes in patients with polyarticular juvenile idiopathic arthritis:a single-center study in China

Background Juvenile idiopathic arthritis(JIA)is a chronic inflammatory disease that includes seven heterogeneous subgroups with different prognoses.In particular,polyarticular JIA(pJIA)has a longer period of active disease and a poorer prognosis.Tumor necrosis factor(TNF)-alpha inhibitors are effective in patients with pJIA,but the therapeutic regimen remains controversial.Here,we performed a single-center study to determine the potential correlation between TNF-alpha inhibitor(infliximab)therapy and outcomes in these patients.Methods Clinical data of 40 pJIA patients were collected at our center from January 1,2010 to January 1,2018,and patients were grouped according to the timing of infliximab therapy.The erythrocyte sedimentation rate(ESR),the number of joints with active disease,and the 27-point juvenile arthritis disease activity score(JADAS-27)were analyzed.Results The ESR,the active joint count,and the JADAS-27 decreased significantly in all groups after 3 months(P=0.041/0.415/0.008,0.022/0.030/<0.001,and 0.05/0.012/<0.001,respectively)and 6 months(P=0.036/0.045/0.041,0.076/0.037/<0.001,and 0.096/0.006/<0.001,respectively)of infliximab treatment,although the rates of change of these parameters were similar.However,after 12 months,only patients treated with infliximab within 3 months of disease onset had a stable ESR,active joint count,and JADAS-27,while these parameters increased sharply when infliximab was administered 3 months and especially 1 year after disease onset.Conclusions TNF-alpha is a pleiotropic pro-inflammatory cytokine of crucial importance in the pathogenesis of JIA.Infliximab can improve the outcomes of patients with pJIA significantly,and should be introduced early during the clinical course.

As an important barrier,the pulmonary alveolar epithelium plays an important role in lung gas exchange and host defense.The alveolar epithelium is mainly comprised of alveolar type I(AT1)and type II(AT2)cells that are tightly connected to maintain the integrity of the alveolar epithelium.There are several kinds of stem/progenitor cells capable of repairing the adult alveolar epithelium in response to various injuries.Kim and colleagues identified bronchioalveolar stem cells(BASCs)that are capable of differentiating into alveolar cells and airway cells.[1]In addition,a subset of alveolar cells expressing the laminin receptorα6β4 but not surfactant protein C(SPC)was able to regenerate into AT2 cells upon bleomycin-induced lung injury.[2]Rare keratin 5-positive basal cells give rise to alveolar epithelial lineages after influenza infection.[3,4]Sex-determining region Y-box 2(Sox2)is a transcription factor that maintains and regulates self-renewal and pluripotency in embryonic stem cells.[5]The functional roles of Sox2 in the repair of alveolar epithelium injuries remain unknown.This study aimed to investigate whether the alveolar cells expressing Sox2 are involved in alveolar epithelial repair.

Objective:Several studies indicated that tonsillectomy can improve the prognosis of patients with immunoglobulin A nephropathy(IgAN).However,the relationship between tonsillar immunity and IgAN is still unclear.Methods:A total of 14 IgAN patients were recruited in the current study from May 2015 to April 2016 in Tongji Hospital.B cells,dendritic cells(DCs),and IgAl positive cells in human tonsils were detected using immunofluorescence and immunohistochemistry.Correlations between these cells and clinicopathologic features were evaluated.

Achieving very high cycle fatigue performance of Au thin films for flexible electronic applications

The fatigue damage behavior of the nanocrystalline Au films on polyimide substrates was investigated.It was found that the very high-cycle fatigue damage resistance of the Au film was significantly enhanced by at least a factor of~2 in supported loading through adding an ultrathin Ti interlayer at the Au film/polyimide interface.Such a better fatigue damage resistance is mainly ascribed to the effective suppression of voiding at the Au film/polyimide interface through modulation of the Au/Ti interface,and thus the propensity of the cyclic strain localization and grain boundary cracking is reduced.The finding may provide a potential strategy for the design of flexible devices with ultra-long fatigue life.

Non-alcoholic steatohepatitis(NASH)is a primary cause of cirrhosis and hepatocellular carcinoma.Unfortunately,there is no approved drug treatment for NASH.AMP-activated kinase(AMPK)is an important metabolic sensor and whole-body regulator.It has been proposed that AMPK activators could be used for treating metabolic diseases such as obesity,type 2 diabetes and NASH.In this study,we screened a marine natural compound library by monitoring AMPK activity and found a potent AMPK activator,candidusin A(CHNQD-0803).Further studies showed that CHNQD-0803 directly binds recombinant AMPK with a K_(D) value of 4.728×10^(-8) M and activates AMPK at both molecular and intracellular levels.We then investigated the roles and mechanisms of CHNQD-0803 in PA-induced fat deposition,LPS-stimulated infammation,TGF-β-induced fbrosis cell models and the MCD-induced mouse model of NASH.The results showed that CHNQD-0803 inhibited the expression of adipogenesis genes and reduced fat deposition,negatively regulated the NF-κB-TNFαinfammatory axis to suppress infammation,and ameliorated liver injury and fbrosis.These data indicate that CHNQD-0803 as an AMPK activator is a novel potential therapeutic candidate for NASH treatment.

Molecular Weight Dependence of Associative Behavior in Polyimide/DMF Solutions

Eight 6FDA-TFDB polyimide(PI)samples with absolute molecular weights ranging from 1.25 × 10^5 g mol^-1 to 3.11 × 10^5 g mol^-1 are obtained by precipitation fractionation.Rheological experiments are conducted to determine the influence of molecular weight on the associating behavior of PI in A/X-dimethylformamide(DMF)solutions in a broad volume fraction,including abnormal steady shear flow,solution heterogeneity,and scaling behavior.Abnormal flow behaviors,i.e.,multi-region shear thinning and weak shear thickening,are studied,and these behaviors have not been reported in literature.The heterogeneity of PI/DMF solutions is examined by dynamic rheological test.By plotting qsp versus four concentration regions of l-IV can be distinguished for all PI samples with various molecular weights.The scaling results in different concentration regions are in good agreement with the associative polymer theory proposed by Rubinstein and Semenov.The scaling exponents do not show molecular weight dependence in concentration regions I and II.In concentration regions Ⅲ and Ⅳ,the scaling exponents change little when the molecular weight is below 242 k but increase when the molecular weight increases from 242 k to 311 k.This work can help us to understand polyimide solution properties from dilute to semidilute entangled solutions,and will guide the polyimide solution preparation for different processing.

Renal Doppler and Novel Biomarkers to Assess Acute Kidney Injury in a Swine Model of Ventricular Fibrillation Cardiac Arrest

Background： Majority of the research on cardiac arrest （CA） have focused on post-CA brain injury and myocardial dysfunction, the renal dysfunction and acute kidney injury （AKI） in other critical illnesses after CA have not been well described. This study was designed to assess AKI with renal Doppler and novel AK1 biomarkers in a swine model of ventricular fibrillation cardiac arrest （VFCA）. Methods： Thirty healthy piglets were divided into VFCA group （17 = 22） and Sham group （n = 8） in a blinded manner. Mean arterial pressure, heart rate. and cardiac output were recorded continuously. Cardiac arrest （CA） was induced by programmed electric stimulation in the VFCA group, and then cardiopulmonary resuscitation was performed. Twenty piglets returned of spontaneous circulation （ROSC） and received intensive care. Blood and urine samples were collected for AKI biomarkers testing, and Color Doppler flow imaging was performed at baseline, 6 h, 12 h, and 24 11. respectively after ROSC. At ROSC 24 h, the animals were sacrificed and a semi-quantitative evaluation of pathologic kidney injury was performed. Results： In the VFCA group, corrected resistive index （cRl） increased from 0.47 ± 0.03 to 0.64 ± 0.06, and pulsatility index （PI） decreased from 0.82 ± 0.03 to 0.68 ± 0.04 after ROSC. Cystatin C （CysC） in both serum and urine samples increased at ROSC 6 h, but neutrophil gelatinase-associated lipocalin （NGAL） in serum increased to 5.34 ± 1.68 ng/ml at ROSC 6 h, and then decreased to 3.16 ± 0.69 ng/ml at ROSC 24 h while CysC increasing constantly. According to the renal histopathology, 18 of 20 animals suffered from kidney injury. The grade of renal injury was highly correlated with RI, cRI, NGAL, and CysC. Linear regression equation was established： Grade of renal injury = 0.002× serum CysC ＋ 6.489 × PI ＋ 4.544 × cRI - 8.358 （r^2 = 0.698, F = 18.506, P 〈 0.001）.
