●AIM:To introduce the macular hole(MH)hydromassage technique as a potentially beneficial approach for the treatment of large or persistent MH.●METHODS:This retrospective observational case series comprised 16 consecutive patients(17 eyes)diagnosed with *** criteria involved a hole aperture diameter larger than 600μm or the presence of an unclosed MH larger than 600μm following the previous *** MH repair procedures were administered in all cases,involving the manipulation and aspiration of the hole margin through the application of water flow with a soft-tip flute needle.A comprehensive assessment was conducted for each case before and after surgery,and optical coherence tomography(OCT)images were captured at every follow-up point.●RESULTS:The mean preoperative aperture diameter was 747±156μm(range 611-1180μm),with a mean base diameter of 1390±435μm(range 578-2.2.μm).Following surgery,all cases achieved complete anatomical closure of MH,with 13 cases(76.5%)exhibiting type 1 closure and 4 cases(2..5%)demonstrating type 2.*** significant differences were observed in the preoperative OCT variables between the two closure *** with type 1 closure showed a significantly improved visual acuity(0.70±0.10,range 0.50-0.80)compared to those with type 2.closure(0.90±0.12.range 0.80-1.00,P=0.014).●CONCLUSION:The MH hydromassage technique demonstrates promising results,achieving acceptable closure rates in cases of large or persistent *** technique may serve as an effective adjunctive maneuver during challenging MH surgery.

Sequence-length variation of mtDNA HVS-I C-stretch in Chinese ethnic groups

The purpose of this study was to investigate mitochondrial DNA (mtDNA) hypervariable segment-I (HVS-I) C-stretch variations and explore the significance of these variations in forensic and population genetics studies. The C-stretch sequence variation was studied in 919 unrelated individuals from 8 Chinese ethnic groups using both direct and clone sequencing ap-proaches. Thirty eight C-stretch haplotypes were identified, and some novel and population specific haplotypes were also detected. The C-stretch genetic diversity (GD) values were relatively high, and probability (P) values were low. Additionally, C-stretch length heteroplasmy was observed in approximately 9% of individuals studied. There was a significant correlation (r=-0.961, P1) between the expansion of the cytosine sequence length in the C-stretch of HVS-I and a reduction in the number of up-stream adenines. These results indicate that the C-stretch could be a useful genetic maker in forensic identification of Chinese populations. The results from the Fst and dA genetic distance matrix, neighbor-joining tree, and principal component map also suggest that C-stretch could be used as a reliable genetic marker in population genetics.

The ambient air quality standard(AAQS)is a vital policy instrument for protecting the environment and human *** Province is at the forefront of china's efforts to protect its ecological environment,with an official goal to achieve world-leading air quality by ***,neither the national AAQS nor the World Health Organization guideline offers sufficient guidance for improving air quality in Hainan because Hainan has well met the former while the latter is excessively ***,the establishment of Hainan's local AAQS becomes ***,research regarding the development of local AAQS is scarce,especially in comparatively more polluted countries such as *** relatively high background values and significant interannual fluctuations in air pollutant concentrations in Hainan present challenges in the development of local *** research proposes a world-class local AAQS of Hainan Province by reviewing the AAQS in major countries or regions worldwide,analyzing the influence of different statistical forms,and carefully evaluating the attainability of the *** the proposed AAQS,the annual mean concentration limit for PM2.5,the annual 95th percentile of daily maximum 8-h mean(MDA8)concentration limit for O_(3),and the peak season concentration limit for O_(3) are set at 10,12.,and 85μg/m^(3),*** study indicates that,with effective control policies,Hainan is projected to achieve compliance with the new standard by *** implementation of the local AAQS is estimated to avoid 1,52.(1,2.3–1,789)and 2.9(132.501)premature deaths attributable to longterm exposure to PM2.5 and O_(3) in Hainan in 2.35,respectively.

Objective: To evaluate the effectiveness and safety of Chinese medicine(CM) in the treatment of coronavirus disease 2.19(COVID-19) in china. Methods: A multi-center retrospective cohort study was carried out, with cumulative CM treatment period of 3 days during hospitalization as exposure. Data came from consecutive inpatients from December 19, 2.19 to May 16, 2.2. in 4 medical centers in Wuhan, china. After data extraction, verification and cleaning, confounding factors were adjusted by inverse probability of treatment weighting(IPTW), and the Cox proportional hazards regression model was used for statistical analysis. Results:A total of 2.2.2.COVID-19 patients were included. There were 1,684 patients in the CM group and 588 patients in the control group. Compared with the control group, the hazard ratio(HR) for the deterioration rate in the CM group was 0.52.[95% confidence interval(CI): 0.41 to 0.64, P2.(95% CI: 0.19 to 0.44, P2.00062.17)

Pulmonary hypertension (PH) is a fatal disorder characterized by pulmonary vascular remodeling and obstruction. The phosphodiesterase 4 (PDE4) family hydrolyzes cyclic AMP (cAMP) and is comprised of four subtypes (PDE4A–D). Previous studies have shown the beneficial effects of pan-PDE4 inhibitors in rodent PH;however, this class of drugs is associated with side effects owing to the broad inhibition of all four PDE4 isozymes. Here, we demonstrate that PDE4B is the predominant PDE isozyme in lungs and that it was upregulated in rodent and human PH lung tissues. We also confirmed that PDE4B is mainly expressed in the lung endothelial cells (ECs). Evaluation of PH in Pde4b wild type and knockout mice confirmed that Pde4b is important for the vascular remodeling associated with PH. In vivo EC lineage tracing demonstrated that Pde4b induces PH development by driving endothelial-to-mesenchymal transition (EndMT), and mechanistic studies showed that Pde4b regulates EndMT by antagonizing the cAMP-dependent PKA–CREB–BMPRII axis. Finally, treating PH rats with a PDE4B-specific inhibitor validated that PDE4B inhibition has a significant pharmacological effect in the alleviation of PH. Collectively, our findings indicate a critical role for PDE4B in EndMT and PH, prompting further studies of PDE4B-specific inhibitors as a therapeutic strategy for PH.

High Mountain Asia(HMA)shows a remarkable warming tendency and divergent trend of regional precipitation with enhanced meteorological *** rapid thawing of the HMA cryosphere may alter the magnitude and frequency of nature *** reviewed the influence of climate change on various types of nature hazards in HMA region,including their phenomena,mechanisms and *** reveals that:1)the occurrences of extreme rainfall,heavy snowfall,and drifting snow hazards are escalating;accelerated ice and snow melting have advanced the onset and increased the magnitude of snowmelt floods;2.due to elevating trigger factors,such as glacier debuttressing and the rapid shift of thermal and hydrological regime of bedrock/snow/ice interface or subsurface,the mass flow hazards including bedrock landslide,snow avalanche,ice-rock avalanches or glacier detachment,and debris flow will become more severe;3)increased active-layer detachment and retrogressive thaw slumps slope failures,thaw settlement and thermokarst lake will damage many important engineering structures and infrastructure in permafrost region;4)multi-hazards cascading hazard in HMA,such as the glacial lake outburst flood(GLOF)and avalanche-induced mass flow may greatly enlarge the destructive power of the primary hazard by amplifying its volume,mobility,and impact force;and 5)enhanced slope instability and sediment supply in the highland areas could impose remote catastrophic impacts upon lowland regions,and threat hydropower security and future water *** future,ongoing thawing of HMA will profoundly weaken the multiple-phase material of bedrock,ice,water,and soil,and enhance activities of nature *** and cascading hazards of high magnitude will prevail in *** the glacier runoff overpasses the peak water,low flow or droughts in lowland areas downstream of glacierized mountain regions will became more frequent and *** escalating hazards in the HMA region requires tackling s

Objective To understand the genetic load in the Chinese population for improvement in diagnosis, prevention and rehabilitation of deafness. Methods DNA samples, immortalized cell lines as well as detailed clinical and audiometric data were collected through a national genetic resources collecting network. Two conventional genetic approaches were used in the studies. Linkage analysis in X chromosome and autosomes with microsatellite markers were performed in large families for gene mapping and positional cloning of novel genes. Candidate gene approach was used for screening the mtDNA 12.rRNA, GJB2.and SLC2.A4 mutations in population -based samples. Results A total of 2.572.Chinese hearing loss families or sporadic cases were characterized in the reported studies, including seven X-linked, one Y-linked, 2. large and multiplex autosomal dominant hearing loss families, 607 simplex autosomal recessive hereditary hearing loss families, 100 mitochondrial inheritance families, 147 GJB2.induced hearing loss cases, 2.0 cases with enlarged vestibular aqueduct (EVA) syndrome, 169 sporadic cases with auditory neuropathy, and 1,2.3 sporadic sensorineural hearing loss cases. Through linkage analysis or sequence analysis, two X-linked families were found transmitting two novel mutations in the POU3F4 gene, while another X -linked family was mapped onto a novel locus, nominated as AUNX1 (auditory neuropathy, X-linked locus 1). The only Y-linked family was mapped onto the DFNY1 locus(Y-linked locus 1, DFNY1). Eight of the 2. autosomal dominant families were linked to various autosomal loci. In population genetics studies, 2.567 familial cases and sporadic patients were subjected to mutation screening for three common hearing loss genes: mtDNA 12. rRNA 1555G, GJB2.and SLC2.A4. The auditory neuropathy cases in our samples were screened for OTOF gene mutations. Conclusions These data show that the Chinese population has a genetic load on hereditary hearing loss. The Lamiaceae family is renowned for its terpenoid-based medicinal components,but Leonurus,which has traditional medicinal uses,stands out for its alkaloid-rich ***,the principal active compound found in Leonurus,has demonstrated promising effects in reducing blood lipids and treating ***,the biosynthetic pathway of leonurine remains largely ***,we present the chromosome-level genome sequence assemblies of Leonurus japonicus,known for its high leonurine production,and Leonurus sibiricus,characterized by very limited leonurine *** integrating genomics,RNA sequencing,metabolomics,and enzyme activity assay data,we constructed the leonurine biosynthesis pathway and identified the arginine decarboxylase(ADC),uridine diphosphate glucosyltransferase(UGT),and serine carboxypeptidase-like(SCPL)acyltransferase enzymes that catalyze key reactions in this *** analyses revealed that the UGT–SCPL gene cluster evolved by gene duplication in the ancestor of Leonurus and neofunctionalization of SCPL in ***,which contributed to the accumulation of leonurine specifically in ***,our comprehensive study illuminates leonurine biosynthesis and its evolution in Leonurus.

首先对机枪刚柔耦合系统进行了分析,采用一定的假设和简化,计及自动机的大位移运动和土壤抗力的影响,分析了弹性脚架机枪的结构与受力情况。采用模态综合法,将枪管和脚架处理为柔性体,建立了6刚体4柔体的机枪刚柔耦合仿真模型,确定了各部件之间的关系。对机枪刚柔耦合模型进行参数化设计,抽象出部件结构、机构位置、载荷等可变参数。给出了某机枪的刚柔耦合仿真应用实例,分析了其运动学特性及脚架结构参数对枪口精度的影响。
