To the Editor:Hematopoietic cell transplantation(HCT)is a vital modality to cure severe hematological disorders.[1]Global or regional summarized data of HCT are annually reported by the Chinese Blood and Marrow Transplantation Registry Group(CBMTRG),European Society for Blood and Marrow Transplantation(EBMT)and Center for International Blood and Marrow Transplant Research(CIBMTR).It is shown that the number of transplants is continuously increasing,particularly in china in recent years.[2]Since the donor accessibility is significantly addressed by the well-established protocol of haplo-identical HCT(haplo-HCT).

Background:The initial randomized,double-blinded,actively controlled,phase III ANEAS study(NCT03849768)demonstrated that aumolertinib showed superior efficacy relative to gefitinib as first-line therapy in epidermal growth factor receptor(EGFR)-mutated advanced non-small cell lung cancer(NSCLC).Metastatic disease in the central nervous system(CNS)remains a challenge in the management of *** study aimed to compare the efficacy of aumolertinib versus gefitinib among patients with baseline CNS metastases in the ANEAS ***:Eligible patients were enrolled and randomly assigned in a 1:1 ratio to orally receive either aumolertinib or gefitinib in a double-blinded *** with asymptomatic,stable CNS metastases were ***-up imaging of the same modality as the initial CNS imaging was performed every 6 weeks for 15 months,then every *** responsewas assessed by a neuroradiological blinded,independent central review(neuroradiological-BICR).The primary endpoint for this subgroup analysis was CNS progression-free survival(PFS).Results:Of the 429 patients enrolled and randomized in the ANEAS study,106 patients were found to have CNS metastases(CNS Full Analysis Set,cFAS)at baseline by neuroradiological-BICR,and 60 of them had CNS target lesions(CNS Evaluable for Response,cEFR).Treatment with aumolertinib significantly prolonged median CNS PFS compared with gefitinib in both cFAS(29.0 vs.8.3 months;hazard ratio[HR]=0.31;95%confidence interval[CI],0.17-0.56;P<0.001)and cEFR(29.0 vs.8.3 months;HR=0.26;95%CI,0.11-0.57;P<0.001).The confirmed CNS overall response rate in cEFRwas 85.7%and 75.0%in patients treated with aumolertinib and gefitinib,*** risk analysis showed that the estimated probability of CNS progression without prior non-CNS progression or death was consistently lower with aumolertinib than with gefitinib in patients with and without CNSmetastases at *** new safety findings were ***:The

目的探讨老年HBV相关慢加急性肝衰竭(HBV-ACLF)患者的临床特点,以及影响患者近期预后的危险因素。方法选取2015年1月—2023年1月在西部战区总医院收治的417例HBV-ACLF患者进行回顾性研究。收集患者一般情况、血常规、生化指标、肝硬化及失代偿事件情况(腹水及分级,肝性脑病及分级)等临床资料。随访患者90天生存情况。根据年龄将患者分为老年组(≥60岁,n=106)和非老年组(<60岁,n=311),老年组中根据90天生存情况分为生存组(n=41)和死亡或移植组(n=65)。定量资料两组间比较采用成组t检验或Mann-Whitney U检验,定性资料两组间比较采用χ^(2)检验。采用二元Logistic回归分析确定老年HBV-ACLF患者90天死亡风险的独立影响因素,构建老年HBV-ACLF患者死亡风险预测列线图模型。使用受试者工作特征曲线(ROC曲线)分别评价训练集与验证集中模型对于HBV-ACLF患者转归预测价值。绘制训练集与验证集中所构建模型的校准曲线与决策曲线,判断模型拟合程度与预测收益。结果老年患者的90天病死率显著高于非老年患者(P<0.05),其中老年组女性发生率、肝硬化基础发生率、肝性脑病发生率及分级、腹水发生率、肝纤维化指标水平(APRI、FIB-4)显著高于非老年组(P值均<0.05);而总胆固醇、高密度脂蛋白、白蛋白、甲胎蛋白、淋巴细胞等方面显著低于非老年组(P值均<0.05)。在老年HBV-ACLF患者中,生存组与死亡或移植组在总胆固醇、TBil、国际化标准比值(INR)、甲胎蛋白、血小板、肌酐、血清钠、单核细胞数量、肝性脑病发生率及分级等方面存在显著性差异(P值均<0.05)。此外,多因素Logistic回归分析提示INR(OR=11.351,95%CI:1.942~66.362)、单核细胞(OR=23.636,95%CI:1.388~402.529)、TBil(OR=1.007,95%CI:1.001~1.013)、血小板(OR=0.968,95%CI:0.945~0.993)为HBV-ACLF老年患者90天预后的独立影响因素(P值均<0.05),由此构建的列线图模型具有较高预测价值(ROC曲线下面积为0.915,敏感度为88.0%,特异度为86.7%),且在验证集中同样具有较高效能与拟合度,决策曲线提示获益较好,该模型与常用预测模型(MELD评分、COSSHACLFⅡ评分等)相比,仍具有较高预测效能。结论老年HBV-ACLF患者可能因肝脏合成、储备功能、再生能力下降、免疫功能紊乱等原因而表现为短期高病死率。INR、单核细胞、TBil、血小板在预测老年HBV-ACLF患者死亡风险方面具有较高价值,由此构建的列线图模型具有较高预测效能。

断层永久位移预测及断层悬索桥地震响应研究

断层错动诱导的地表破裂永久位移对跨断层桥梁结构会造成严重破坏,准确预测断层错位的永久位移对跨断层桥梁结构抗震性能评估具有重要意义。首先结合反向传播神经网络(BPNN)提出具有较高精度的断层错位量预测模型,然后通过低频成分叠加高频成分方法人工合成断层面两侧地震动(简称跨断层地震动),最后以某主跨为538 m的跨断层大跨钢桁架悬索桥为研究对象,利用有限元软件ABAQUS建立考虑桩−土相互作用(m法)的全桥计算模型,探究断层位置和跨越角度对悬索桥地震响应的影响。结果表明:基于BPNN提出的方法能精确预测断层位错永久位移,对比已有的回归公式具有更高的精度,为人工合成跨断层地震动的准确性提供了保障;断层相对位置会显著影响桥梁结构的地震响应,与近断层地震动作用相比,在跨断层地震动作用下,悬索桥的塔底剪力、弯矩、相对位移以及扭矩响应增加了22.79%、154.1%、139.36%和265.48%;断层跨越角度对悬索桥动力响应影响明显,其响应均以跨越角度θ=90°呈规律对称分布,塔顶纵向位移、塔底纵向弯矩、塔底纵向剪力以及塔底扭矩最大值均出现在θ=90°,分别为0.81 m、282.45 MN∙m、3.96 MN和6.14 MN∙m。研究成果可为跨断层悬索桥抗震分析提供一套简便、可靠的人工地震动合成方法,也揭示了跨越角度和断层位置对跨断层悬索桥地震响应的影响。

为了研究漏风影响下的集中排烟系统排烟量富裕系数,建立了集中排烟系统风压及风量解算模型,通过与全尺寸试验数据对比,验证了该模型的准确性。并依据该模型,分析了集中排烟系统在不同设计热释放速率、隧道长度、排烟阀间距及排烟阀面积下的排烟道内风压及风量分布,研究了各参数下的排烟量富裕系数。结果表明:随着热释放速率的增大,排烟系统设计排烟量逐渐增大,但排烟量富裕系数与热释放速率无明显关联。对于使用高密闭排烟阀的情况,当隧道长度在10 km以内时,满足排烟需求的排烟量富裕系数均在10%以下;对于使用普通排烟阀的情况,无论是特长隧道还是超长隧道,排烟量富裕系数几乎都在20%以上,当隧道长度超过9 km时,排烟量富裕系数甚至能超过100%。随着排烟阀间距的增大,排烟风机的风量富裕系数和排烟系统的设计排烟量均表现出递减趋势。随着排烟阀面积增大,设计排烟量逐渐增大,排烟量富裕系数也有相似的趋势,尤其是普通排烟阀更为明显。因此,在满足排烟要求的前提下,提高排烟阀质量、扩大排烟阀间距和缩小排烟阀面积,可以起到降低排烟量富裕系数的效果。

卡维地洛、普萘洛尔治疗肝硬化食管静脉曲张出血患者再出血的临床研究

目的探讨卡维地洛、普萘洛尔对肝硬化食管静脉曲张出血(EVB)患者再出血的预防价值。方法将肝硬化EVB患者分为试验组和对照组。试验组给予卡维地洛分散片,起始剂量6.25mg,bid,口服;对照组给予盐酸普萘洛尔片,起始剂量20mg,bid,口服,2组疗程均为1年。比较2组患者的临床疗效,2组患者治疗6个月、治疗1年的再出血率、血流动力学,观察治疗期间安全性。结果试验组入组64例,对照组入组61例。治疗后,试验组和对照组的总有效率分别为93.75%(60例/64例)和91.80%(56例/61例),治疗6个月的再出血率分别为4.69%(3例/64例)和9.84%(6例/61例),在统计学上差异均无统计学意义(均P>0.05)。试验组和对照组治疗1年的再出血率分别为10.94%(7例/64例)和24.59%(15例/61例),在统计学上差异有统计学意义(P<0.05)。治疗6个月,试验组和对照组的门静脉内径(PVD)分别为(12.39±2.41)和(13.88±1.76)mm,脾静脉内径(SVD)分别为(7.56±1.52)和(8.35±1.69)mm,门静脉平均血流速度(Vp)分别为(35.26±7.04)和(38.12±7.60)cm·s^(-1),脾静脉平均血流速度(Vs)分别为(20.03±4.11)和(22.34±4.69)cm·s^(-1),试验组的上述指标与对照组比较,在统计学上差异均有统计学意义(均P<0.05)。治疗1年,试验组和对照组PVD分别为(11.87±2.52)和(13.15±2.04)mm,SVD分别为(7.33±1.48)和(8.22±1.55)mm,Vp分别为(33.96±6.75)和(37.46±6.83)cm·s^(-1),Vs分别为(19.26±4.33)和(21.55±4.47)cm·s^(-1),试验组的上述指标与对照组比较,在统计学上差异均有统计学意义(均P<0.05)。试验组的药物不良反应有心动过缓、低血压、头晕、恶心呕吐,对照组的药物不良反应有心动过缓、低血压、头晕、嗜睡、恶心呕吐、皮疹。试验组和对照组的总药物不良反应发生率分别为12.50%和27.87%,在统计学上差异有统计学意义(P<0.05)。结论卡维地洛相比普萘洛尔,预防再出血的效果更好,更能减轻食管静脉曲张。
