Efficacy and safety of ulinastatin in the treatment of septic shock:a systematic review and meta-analysis

Background:Septic shock is a common systemic inflammatory response syndrome for critical patients in the intensive care *** is currently used for the treatment of septic *** study sought to evaluate the efficacy and safety of ulinastatin in the treatment of septic shock ***:Three English databases(Embase,Medline,and Cochrane Library)and four Chinese databases(China National Knowledge Infrastructure,Wanfang data,SinoMed,and VIP)were searched for published randomized controlled *** 16.0 software was used to conduct the ***:A total of 48 articles were included(Chinese article 47,1 in English).The results show that the treatment of ulinastatin could reduce mortality(risk ratio=0.63,95%confidence interval(CI)(0.55,0.72)),multiple organ dysfunction syndrome(risk ratio=0.6,95%CI(0.53,0.68)),length of intensive care unit stay(mean difference(MD)=-3.92,95%CI(-4.65,-3.18)),length of hospital stay(MD=-4.39,95%CI(-6.63,-2.15))and decrease Acute Physiology and Chronic Health Evaluation II score(MD=-4.55,95%CI(-5.63,-3.47))and Sequential Organ Failure Assessment score(MD=-2.02,95%CI(-2.59,-1.44))with P<***,it lowers TNF-α(standardized mean difference(SMD)=-1.78,95%CI(-2.24,-1.32)),Interleukin-6(SMD=-1.17,95%CI(-1.55,-0.8)),C reactive protein(SMD=-1.49,95%CI(-1.99,-0.99)),hypersensitive C-reactive protein(SMD=-1.9,95%CI(-2.87,-0.94))and procalcitonin(SMD=-0.89,95%CI(-1.12,-0.67))levels in the ***:Available evidence shows that ulinastatin reduces case mortality rate,multiple organ dysfunction syndrome,length of intensive care unit stay,and length of hospital stay and decreases Acute Physiology and Chronic Health Evaluation II score and Sequential Organ Failure Assessment ***,it also lowers TNF-α,Interleukin-6,C reactive protein,hypersensitive C-reactive protein,and procalcitonin levels in the body.

Sheep have been one of the most important groups of animals since ancient ***,the knowledge of their migration routes and genetic relationships is still poorly *** investigate sheep maternal migration histories alongside Eurasian communications routes,in this study,we obtain mitochondrial genomes(mitogenomes)from 17 sheep remains in 6 Chinese sites and 1 Uzbekistan site dated 4429–3100 years before present(BP).By obtaining the mitogenomes from the sheep(4429–3556 BP)found in the Tongtian Cave site in Xinjiang,Altai region of northwest China,our results support the emergence of haplogroup C sheep in Xinjiang as early as 4429–3556 *** combined phylogenetic analyses with extant ancient and modern sheep mitogenomes suggest that the Uzbekistan-Altai region may have been a migration hub for early sheep in eastern *** least two migration events have taken place for sheep crossing Eurasia to China,one passing by Uzbekistan and Northwest China to the middle and lower reaches of the Yellow River at approximately 4000 BP and another following the Altai region to middle Inner Mongolia from 4429 BP to 2500 ***,this study provides further evidence for early sheep utilization and migration patterns in Eastern Asia.

The 2022 yangtze mega-flash drought is characterized by strong intensity and rapid development both in time and space,accompanied by a persistent anticyclonic circulation ***,the causes of the extreme event remain elusive given the multiscale nature of *** we presented a brief overview for the oceanic and terrestrial causes of the megaflash drought during the summer of 2022,and estimated the risk in a changing *** the soil moisture percentile as the drought index,it was found that the drought expanded to the entire yangtze River basin within two months,with 80%of basin under severe drought conditions at the end of *** the intensity and onset speed of the 2022 mega-flash drought were ranked as the first during the past 62 years,with return periods of 86 and 259 years,*** results of composite analysis showed that the spring La Ni?a can facilitate the abrupt change from a wet/normal condition in May–June to drought in July–August over the yangtze River basin,which was beneficial for the increase of flash drought intensity and onset speed *** analysis through the linear regression also indicated that the unprecedented intensity was associated with the negative phase of the Pacific Decadal *** by a coupling strength index for soil moisture and vapor pressure deficit,it was found that there was a strong land-atmosphere coupling over the yangtze River basin during July–August *** attribution by using CMIP6 climate models suggested that land-atmosphere coupling increased the risks of flash drought intensity and onset speed like 2022 by 61%±6%and 64%±7%under natural climate forcings,and the synergy of coupling and anthropogenic climate change would increase the risks by 75%±22%and 85%±12%.Our findings emphasized the role of landatmosphere coupling combined with anthropogenic climate change in intensifying flash droughts.

BACKGROUND Radical surgery combined with systemic chemotherapy offers the possibility of long-term survival or even cure for patients with pancreatic ductal adenocar-cinoma(PDAC),although tumor recurrence,especially locally,still inhibits the treatment *** TRIANGLE technique was introduced as an extended dissection procedure to improve the R0 resection rate of borderline resectable or locally advanced ***,there was a lack of studies concerning postoperative complications and long-term outcomes of this procedure on patients with resectable *** Patients with resectable PDAC eligible for PD from our hospital between June 2018 and December 2021 were enrolled in this retrospective cohort *** the patients were divided into PDstandard and PDTRIANGLE groups according to the surgical *** characteristics,surgical data,and postoperative morbidities were *** of the patients were followed up,and the date and location of tumor recurrence,and death were *** Kaplan-Meier method and log-rank test were used for the survival *** There were 93 patients included in the study and 37 underwent the TRIANGLE *** of operation was longer in the PDTRIANGLE group compared with the PDstandard group[440(410-480)min vs 320(265-427)min](P=0.001).Intraoperative blood loss[700(500-1200)mL vs 500(300-800)mL](P=0.009)and blood transfusion[975(0-1250)mL vs 400(0-800)mL](P=0.009)were higher in the PDTRIANGLE *** was a higher incidence of surgical site infection(43.2%vs 12.5%)(P=0.001)and postoperative diarrhea(54.1%vs 12.5%)(P=0.001)in the PDTRIANGLE *** rates of R0 resection and local recurrence,overall survival,and disease-free survival did not differ significantly between the two *** The TRIANGLE technique is safe,with acceptable postoperative morbidities compared with standardized PD,but it does not improve prognosis for patients with resectable PDAC.

Aiming at the difficult problem of solving the conformation statistics of complex polymers, this study presents a novel and concise conformation statistics theoretical approach based on Monte Carlo and Neural Network method. This method offers a new research idea for investigating the conformation statistics of complex polymers, characterized by its simplicity and practicality. It can be applied to more complex topological structure, more higher degree of freedom polymer systems with higher dimensions, theory research on dynamic self-consistent field theory and polymer field theory, as well as the analysis of scattering experimental data. The conformation statistics of complex polymers determine the structure and response properties of the system. Using the new method proposed in this study, taking the semiflexible ring diblock copolymer as an example, Monte Carlo simulation is used to sample this ring conformation to construct the dataset of polymer. The structure factor describing conformation statistics are expressed as continuous functions of structure parameters by neural network supervised learning. This is the innovation of this work. As an application, the structure factors represented by neural networks were introduced into the random phase approximation theory to study the microphase separation of semiflexible ring diblock copolymers. The influence of the ring's topological properties on the phase transition behavior was pointed out.
