Establishment and evaluation of a prognostic model for patients with unresectable gastric cancer liver metastases

BACKGROUND Liver metastases(LM)is the primary factor contributing to unfavorable outcomes in patients diagnosed with gastric cancer(GC).The objective of this study is to analyze significant prognostic risk factors for patients with GCLM and develop a reliable nomogram model that can accurately predict individualized prognosis,thereby enhancing the ability to evaluate patient outcomes.AIM To analyze prognostic risk factors for GCLM and develop a reliable nomogram model to accurately predict individualized prognosis,thereby enhancing patient outcome assessment.METHODS Retrospective analysis was conducted on clinical data pertaining to GCLM(type III),admitted to the Department of General Surgery across multiple centers of the Chinese PLA General Hospital from January 2010 to January 2018.The dataset was divided into a development cohort and validation cohort in a ratio of 2:1.In the development cohort,we utilized univariate and multivariate Cox regression analyses to identify independent risk factors associated with overall survival in GCLM patients.Subsequently,we established a prediction model based on these findings and evaluated its performance using receiver operator characteristic curve analysis,calibration curves,and clinical decision curves.A nomogram was created to visually represent the prediction model,which was then externally validated using the validation cohort.RESULTS A total of 372 patients were included in this study,comprising 248 individuals in the development cohort and 124 individuals in the validation cohort.Based on Cox analysis results,our final prediction model incorporated five independent risk factors including albumin levels,primary tumor size,presence of extrahepatic metastases,surgical treatment status,and chemotherapy administration.The 1-,3-,and 5-years Area Under the Curve values in the development cohort are 0.753,0.859,and 0.909,respectively;whereas in the validation cohort,they are observed to be 0.772,0.848,and 0.923.Furthermore,the calibration curves demonstrated excellent cons

Efficient capture,safe storage and release of tritium from the international thermonuclear experimental reactor(ITER) reaction exhaust gas is a perplexing problem,and the development of an efficient tritium-getter material with ultra-low hydrogenation equilibrium pressure is considered as a reliable way.In this work,Zr_(2)Co alloy was selected as a tritium-getter material and prepared through induction levitation melting.Fundamental performance test results show that Zr_(2)Co exhibits an ultra-low hydrogenation equilibrium pressure of 3.22 × 10^(-6) Pa at 25℃ and excellent hydriding kinetics under a low hydrogen pressure of 0.005 MPa.Interestingly,unique phase transition behaviors were presented in Zr_(2)Co-H system.Specifically,Zr_(2)CoH_(5) formed by Zr_(2)Co hydrogenated at room temperature is initially decomposed into ZrH_(2) and ZrCoH_(3) at200 ℃.With the temperature increasing to 350 ℃,ZrCoH_(3)is dehydrogenated to ZrCo,and then ZrCo further reacts with ZrH_(2) at 650 ℃ to reform Zr_(2)Co and hydrogen.Among the staged phase transition pathways during dehydrogenation,the decomposition of Zr_(2)CoH_(5) occurs preferentially,which is well accordance with both the smallest reaction energy barrier and the maximum reaction spontaneity that are determined respectively from kinetics activation energy and thermodynamics Gibbs free energy.Furthermore,first principles calculation results indicate that the stronger binding of hydrogen in interstitial environments of ZrCoH_(3)and ZrH_(2) triggers the hydrogen-stabilized phase transformation of Zr_(2)CoH_(5).The unique phase transition mechanisms in Zr_(2)Co-H system can shed light on the further exploration and regulation of analogous staged phase transition of hydrogen storage materials.

Macrocycle confinement induced guest near-infrared(NIR)luminescence was research hotspot currently.Here in,we reported a cucurbit[7]uril(CB[7])confined 3,7-bis((E)-2-(pyridin-4-yl)vinyl)-10-Hphenothiazine bridged bis(4-(4-bromophenyl)pyridine)(G),which not only boosted its NIR luminescence but also realized detection of HClO/ClO^(-)in living cells and lysosome imaging.Fluorescence spectroscopy experiments were performed to calculate the detection ability of probe G to HClO/ClO^(-)up to 147 nmol/L.As compared with G,supramolecular probe G■CB[7]formed after encapsulated by CB[7],the detection ability towards HClO/ClO^(-)was improved to 24 nmol/L which was ascribe to the macrocycle CB[7]confinement increasing the fluorescence intensity to 103 folds.Accompanying the excitation wavelength changing,the fluorescence red-shifted to 820 nm when excited by 570 nm light,which was used to NIR lysosome imaging.Meanwhile,the supramolecular assembly G■CB[7]was also successfully used to highly sense to exogenous HClO/ClO^(-)in RAW 264.7 cells and live animal.

Macroautophagy(referred to as autophagy hereafter)is a major intracellular lysosomal degradation pathway that is responsible for the degradation of misfolded/damaged proteins and organelles.Previous studies showed that autophagy protects against acetaminophen(APAP)-induced injury(AILI)via selective removal of damaged mitochondria and APAP protein adducts.The lysosome is a critical organelle sitting at the end stage of autophagy for autophagic degradation via fusion with autophagosomes.In the present study,we showed that transcription factor EB(TFEB),a master transcription factor for lysosomal biogenesis,was impaired by APAP resulting in decreased lysosomal biogenesis in mouse livers.Genetic loss-of and gain-of function of hepatic TFEB exacerbated or protected against AILI,respectively.Mechanistically,overexpression of TFEB increased clearance of APAP protein adducts and mitochondria biogenesis as well as SQSTM1/p62-dependent non-canonical nuclear factor erythroid 2-related factor 2(NRF2)activation to protect against AILI.We also performed an unbiased cell-based imaging high-throughput chemical screening on TFEB and identified a group of TFEB agonists.Among these agonists,salinomycin,an anticoccidial and antibacterial agent,activated TFEB and protected against AILI in mice.In conclusion,genetic and pharmacological activating TFEB may be a promising approach for protecting against AILI.

Portable fluorogenic probe for monitoring of volatile amine vapour and food spoilage

There are urgent needs of volatile amine gas sensors with high-performance in food quality control,disease monitoring and environmental pollution.Thin-film fluorescent probe is suitable for amine vapour sensing due to its high sensitivity,high selectivity,and no polluting analyte.Herein,a novel fluorescent probe based on indacenodithiophene structure withπconjugated system was designed and synthesized.The experimental results show that the films prepared by this material exhibit rapid and distinct fluorescence quenching after being exposed to saturated vapours of primary amine,secondary amine and tertiary amine represented by n-propylamine,diethylamine and trimethylamine,respectively.The quenching of fluorescence is 84%,87%and 96%,respectively,within 10 s.The detection mechanism of probe for primary amine is based on specific chemical reaction,while the detection mechanism for secondary amine and tertiary amine is intramolecular charge transfer.Further experiments show that the detection limit of the fluorescent probe for trimethylamine,an important marker of food spoilage,could reach 4.610 ppt.On-site detection based on spoilage of small yellow croaker suggests the material possesses the potential for food freshness detection.This simple fluorogenic probe is an original approach to simplify real-time visual monitoring of volatile amine vapour.

Currently,with the advent of high-repetition-rate laser-plasma experiments,the demand for online diagnosis for the X-ray spectrum is increasing because the laser-plasma-generated X-ray spectrum is very important for characterizing electron dynamics and applications.In this study,scintillators and silicon PIN(P-type–intrinsic-N-type semiconductor)diodes were used to construct a wideband online filter stack spectrometer.The X-ray sensor and filter arrangement was optimized using a genetic algorithm to minimize the condition number of the response matrix.Consequently,the unfolding error was significantly reduced based on numerical experiments.The detector responses were quantitatively calibrated by irradiating the scintillator and PIN diode with various nuclides and comparing the measuredγ-ray peaks.A prototype 15-channel spectrometer was developed by integrating an X-ray detector with front-and back-end electronics.The prototype spectrometer could record X-ray pulse signals at a repetition rate of 1 kHz.Furthermore,an optimized spectrometer was employed to record the real-time spectra of laser-driven bremsstrahlung sources.This optimized spectrometer offers a compact solution for spectrum diagnostics of ultrashort X-ray pulses,exhibiting improved accuracy in terms of spectrum measurements and repetition rates,and could be widely used in next-generation high-repetition-rate high-power laser facilities.

Scaffold protein RACK1A positively regulates leaf senescence by coordinating the EIN3-miR164-ORE1 transcriptional cascade in Arabidopsis

Plants have adopted versatile scaffold proteins to facilitate the crosstalk between multiple signaling pathways. Leaf senescence is a well-programmed developmental stage that is coordinated by various external and internal signals. However,the functions of plant scaffold proteins in response to senescence signals are not well understood. Here, we report that the scaffold protein RACK1A(RECEPTOR FOR ACTIVATED C KINASE1A) participates in leaf senescence mediated by ethylene signaling via the coordination of the EIN3-miR164-ORE1 transcriptional regulatory cascade. RACK1A is a novel positive regulator of ethylene-mediated leaf senescence. The rack1a mutant exhibits delayed leaf senescence, while transgenic lines overexpressing RACK1A display early leaf senescence. Moreover, RACK1A promotes EIN3(ETHYLENE INSENSITIVE 3) protein accumulation, and directly interacts with EIN3 to enhance its DNA-binding activity. Together, they then associate with the miR164 promoter to inhibit its transcription, leading to the release of the inhibition on downstream ORE1(ORESARA 1) transcription and the promotion of leaf senescence.This study reveals a mechanistic framework by which RACK1A promotes leaf senescence via the EIN3-miR164-ORE1 transcriptional cascade, and provides a paradigm for how scaffold proteins finely tune phytohormone signaling to control plant development.

Improvements in Long-Lead Prediction of Early-Summer Subtropical Frontal Rainfall Based on Arctic Sea Ice

Seasonal prediction of East Asia(EA) summer rainfall, especially with a longer-lead time, is in great demand, but still very challenging. The present study aims to make long-lead prediction of EA subtropical frontal rainfall(SFR) during early summer(May-June mean, MJ) by considering Arctic sea ice(ASI) variability as a new potential predictor. A MJ SFR index(SFRI), the leading principle component of the empirical orthogonal function(EOF) analysis applied to the MJ precipitation anomaly over EA, is defined as the predictand. Analysis of 38-year observations(1979-2016) revealed three physically consequential predictors. A stronger SFRI is preceded by dipolar ASI anomaly in the previous autumn, a sea level pressure(SLP) dipole in the Eurasian continent, and a sea surface temperature anomaly tripole pattern in the tropical Pacific in the previous winter. These precursors foreshadow an enhanced Okhotsk High, lower local SLP over EA, and a strengthened western Pacific subtropical high. These factors are controlling circulation features for a positive SFRI. A physical-empirical model was established to predict SFRI by combining the three predictors. Hindcasting was performed for the 1979-2016 period, which showed a hindcast prediction skill that was, unexpectedly, substantially higher than that of a four-dynamical models’ ensemble prediction for the 1979-2010 period(0.72 versus 0.47). Note that ASI variation is a new predictor compared with signals originating from the tropics to mid-latitudes. The long-lead hindcast skill was notably lower without the ASI signals included, implying the high practical value of ASI variation in terms of long-lead seasonal prediction of MJ EA rainfall.

BACKGROUND The midpoint transverse process to pleura(MTP)block,a novel technique for thoracic paravertebral block(TPVB),was first employed in laparoscopic renal cyst decortication.CASE SUMMARY Thoracic paravertebral nerve block is frequently employed for perioperative analgesia during laparoscopic cyst decortication.To address safety concerns associated with TPVBs,we administered MTP blocks in two patients prior to administering general anesthesia for laparoscopic cyst decortication.The MTP block was performed at the T9 level under ultrasound guidance,with 20 mL of 0.5%ropivacaine injected.Reduced sensation to cold and pinprick was observed from the T8 to T11 dermatome levels.Immediately postoperative Numeric Pain Rating Scale scores were 0/10 at rest and on movement,with none exceeding a mean 24 h numeric rating scale>3.CONCLUSION MTP block was effective technique for providing postoperative analgesia for patients undergoing laparoscopic renal cyst decortication.

Simultaneous guidance of electromagnetic and elastic waves via glide symmetry phoxonic crystal waveguides

A phoxonic crystal waveguide with the glide symmetry is designed,in which both electromagnetic and elastic waves can propagate along the glide plane at the same time.Due to the glide symmetry,the bands of the phoxonic crystal super-cell degenerate in pairs at the boundary of the Brillouin zone.This is the so-called band-sticking effect and it causes the appearance of gapless guided-modes.By adjusting the magnitude of the glide dislocation the edge bandgaps,the bandgap of the guided-modes at the boundary of the Brillouin zone,can be further adjusted.The photonic and phononic guided-modes can then possess only one mode for a certain frequency with relatively low group velocities,achieving single-mode guided-bands with relatively flat dispersion relationship.In addition,there exists acousto-optic interaction in the cavity constructed by the glide plane.The proposed waveguide has potential applications in the design of novel optomechanical devices.
