Application of superb microvascular imaging in focal liver lesions

AIM To explore the ability of superb microvascular imaging(SMI) in differential diagnosis of focal liver lesions(FLLs) and to compare SMI morphology findings to those of color Doppler ultrasound and enhanced *** Twenty-four patients with 31 FLLs were included in our study,with diagnoses of hemangioma(HE)(n = 17),hepatocellular carcinoma(HCC)(n = 5),metastatic lesions(n = 5),primary hepatic lymphoma(n = 1),focal nodular hyperplasia(FNH)(n = 2),and adenoma(n = 1). Nine lesions were pathologically diagnosed,and 22 lesions were radiologically confirmed,all of which were evaluated by at least two types of enhanced imaging techniques. All patients had undergone SMI. Patients were divided into subgroups based on pathological and radiological diagnoses to analyze SMI manifestations. We also compared the SMI manifestations of the most common malignant FLLs(HCCs and metastatic lesions) with those of the most common benign FLLs(HEs).RESULTS HEs were classified into three SMI subgroups: diffuse dot-like type(n = 6),strip rim type(n = 8),and nodular rim type(n = 3). The sizes of the three types of HEs were significantly different(P = 0.00,< 0.05). HCCs were classified into two subgroups: diffuse honeycomb type(n = 2) and non-specific type(n = 3). Four of the metastatic lesions were the strip rim type,and the other metastatic lesion was the thick rim type,which is the same as that of lymphoma. FNH was described as a spoke-wheel type,and adenoma as a diffuse honeycomb type. The SMI types of HCCs and metastatic lesions were significantly different from those of HEs(P = 0.048,< 0.05).CONCLUSION SMI technology enables microvascular evaluation of FLLs without using any contrast agent. For HEs,lesion size may affect SMI performance. SMI is able to provide useful information for differential diagnosis of HCCs and metastatic lesions from HEs.

Application of ultrasound in aggressive angiomyxoma: Eight case reports and review of literature

Aggressive angiomyxoma （AAM） is a rare tumour that often occurs in soft tissues of the female genital tract. Eight cases of AAM are reported in this article, and the clinical features and ultrasound and magnetic resonance imaging （MRI） results of the eight cases are reviewed and summarized. The main complaints of all the patients were palpable and painless masses in the vulva or scrotum. The lesions were mainly located in the vulva, pelvis, and perineal region, with a large scope of involvement. The sonographic features of AAM were characteristic. On sonography, all of the masses were of irregular shape and showed hypoechogenicity, with a heterogeneous inner echotexture. Intratumoural and peritumoural blood fows were detected by colour Doppler imaging. On real-time ultrasonic imaging, prominent deformation of the lesions was observed bycompressing the masses with the probe. Some special imaging features were also revealed, including a la-minated or swirled appearance of inner echogenicity, and a fnger-like or tongue-like growth pattern. On MRI imaging, the lesions showed intermediate-intensity signals and intermediate to high-intensity signals on TI-weighted and T2-weighted sequences. A rapid and uneven enhancement pattern was demonstrated. After the comparison of sonographic features with MRIand pathological findings, we found the relevance of the ultrasonographic characteristics with MRI and his-tological features of AAM. Ultrasound can be a valuable imaging method for the preoperative diagnosis, eva-luation of scope, and follow-up of AAM.

A Gas-Kinetic Unified Algorithm for Non-Equilibrium Polyatomic Gas Flows Covering Various Flow Regimes

In this paper,a gas-kinetic unified algorithm(GKUA)is developed to investigate the non-equilibrium polyatomic gas flows covering various *** on the ellipsoidal statistical model with rotational energy excitation,the computable modelling equation is presented by unifying expressions on the molecular collision relaxing parameter and the local equilibrium distribution *** constructing the corresponding conservative discrete velocity ordinate method for this model,the conservative properties during the collision procedure are preserved at the discrete level by the numerical method,decreasing the computational storage and *** and implicit lower-upper symmetric Gauss-Seidel schemes are constructed to solve the discrete hyperbolic conservation equations *** the new GKUA,some numerical examples are simulated,including the Sod Riemann problem,homogeneous flow rotational relaxation,normal shock structure,Fourier and Couette flows,supersonic flows past a circular cylinder,and hypersonic flow around a plate placed *** results obtained by the analytic,experimental,direct simulation Monte Carlo method,and other measurements in references are compared with the GKUA results,which are in good agreement,demonstrating the high accuracy of the present ***,some polyatomic gas non-equilibrium phenomena are observed and analysed by solving the Boltzmann-type velocity distribution function equation covering various flow regimes.

Diagnosis and treatment of small intestinal bleeding:Retrospective analysis of 76 cases

AIM: To investigate the causes of small intestinal bleed- ing as well as its diagnosis and therapeutic approaches. METHODS: A retrospective analysis was conducted ac- cording to the clinical records of 76 patients with small intestinal bleeding admitted to our hospital in the past 5 years. RESULTS: In these patients, tumor was the most fre- quent cause of small intestinal bleeding (37/76), fol- lowed by Meckel’s diverticulum (21/76), angiopathy (15/76) and ectopic pancreas (3/76). Of the 76 patients, 21 were diagnosed by digital subtraction angiography, 13 by barium and air double contrast X-ray examination of the small intestine, 11 by 99mTc-sestamibi scintigraphy of the abdominal cavity, 6 by enteroscopy of the small intestine, 21 by laparoscopic laparotomy, and 4 by ex- ploratory laparotomy. Although all the patients received surgical treatment, most of them (68/76) received part enterectomy covering the diseased segment and entero- anastomosis. The follow-up time ranged from 1 year to 5 years. No case had recurrent alimentary tract bleeding or other complications. CONCLUSION: Tumor is the major cause of small in- testinal bleeding followed by Meckel’s diverticulum and angiopathy. The main approaches to definite diagnosis of small intestinal bleeding include digital subtraction an- giography, 99mTc-sestamibi scintigraphy of the abdominal cavity, barium and air double contrast X-ray examina- tion of the small intestine, laparoscopic laparotomy or exploratory laparotomy. Part enterectomy covering the diseased segment and enteroanastomosis are the most effective treatment modalities for small intestinal bleed- ing.

Toward High-Performance Delta-Based Iterative Processing with a Group-Based Approach

Many systems have been built to employ the delta-based iterative execution model to support iterative algorithms on distributed platforms by exploiting the sparse computational dependencies between data items of these iterative algorithms in a synchronous or asynchronous approach. However, for large-scale iterative algorithms, existing synchronous solutions suffer from slow convergence speed and load imbalance, because of the strict barrier between iterations;while existing asynchronous approaches induce excessive redundant communication and computation cost as a result of being barrier-free. In view of the performance trade-off between these two approaches, this paper designs an efficient execution manager, called Aiter-R, which can be integrated into existing delta-based iterative processing systems to efficiently support the execution of delta-based iterative algorithms, by using our proposed group-based iterative execution approach. It can efficiently and correctly explore the middle ground of the two extremes. A heuristic scheduling algorithm is further proposed to allow an iterative algorithm to adaptively choose its trade-off point so as to achieve the maximum efficiency. Experimental results show that Aiter-R strikes a good balance between the synchronous and asynchronous policies and outperforms state-of-the-art solutions. It reduces the execution time by up to 54.1% and 84.6% in comparison with existing asynchronous and the synchronous models, respectively.

Application of laparoscopy in diagnosis and treatment of massive small intestinal bleeding: Report of 22 cases

AIM: To investigate the diagnostic and therapeutic value of laparoscopy in patients with massive small intestinal bleeding. METHODS: Twenty-two patients with massive small in- testinal bleeding and hemodynamic alteration underwent laparoscopic laparotomy in our unit from December 2002 to April 2005. Post pathologic sites were found, laparos- copy- or laparoscopy-assisted part small intestinal resec- tion including pathologic intestinal site and enteroanas- tomosis was performed in all these patients. RESULTS: The bleeding sites were successfully detected by laparoscopy in all these 22 patients. Massive small intestinal bleeding was caused by jejunum benign stromal tumor in 8 cases, by jejunum potential malignant stromal tumor in 5 cases, by jejunum malignant stromal tumor in 1 case, by Mechel’s diverticulum in 5 cases, by small intestinal vascular deformity in 2 cases, and by ectopic pancreas in 1 case. A total of 16 patients underwent laparoscopy-assisted enterectomy and enteroanastomosis of small intestine covering the diseased segment and 6 patients received enterectomy of the diseased segment under laparoscope. No surgical complications occurred and the outcome was satisfactory. CONCLUSION: Laparoscopy in diagnosis and treatment of massive small intestinal bleeding is noninvasive with less pain, short recovery time and definite therapeutic efficacy.

COLOR DOPPLER ULTRASONOGRAPHY APPEARANCES OF RENAL VEIN THROMBOSIS AND ITS DIAGNOSTIC VALUE

Objective To evaluate color Doppler ultrasonography (CDU) appearances of renal vein thrombosis (RVT) and its diagnostic *** Ten patients with RVT were analyzed retrospectively. Renal structure, distributions of intrarenal flow signals, echogenicity, and flow fullness in main renal veins were observed with CDU. Resistance index (RI) was recorded from the waveforms of segmental or interlobar renal *** Ten kidneys in nine patients were confirmed to have thrombus within the main renal veins, and one patient was confirmed to have thrombus within the small intrarenal veins. The appearances of the main renal vein thrombosis included full of solid echogenicity or strip echogenicity and complete or partial filling defect within the main renal veins, and absent or a few intrarenal venous flow signals in 70% of kidneys involved. The appearances of intrarenal vein thrombosis included obscure renal structure and no venous flow signal within the involved part of the kidneys. Reverse diastolic flow in the intrarenal artery had only a sensitivity of 36% (4/11); in other 7 kidneys without intrarenal arterial reverse diastolic flow, increased RI (mean, 0.84; range, 0.74-0.96) was *** CDU is helpful for rapid clinical diagnosis and follow-up of RVT, and therefore can be the first imaging modality of choice for RVT.

Ultrasonographic Characteristics of Intraductal Papillary Mucinous Neoplasm of the Pancreas

Objective To analyze the clinical and ultrasonographic imaging features of intraductal papillary mutinous neoplasm （1PMN） of the pancreas. Methods Twelve patients with IPMN underwent surgery between May 2005 and December 2008, including 4 （33.3%） with adenoma and 8 （66.7%） with adenocarcinoma. IPMN was classified preoperatively into 3 types based on sonographic findings of different sites： main duct, branch duct, and combined type. All clinical presentations and ultrasonographic findings of those paticnts were reviewed and the correlation be- tween ultrasonographic hndings and histopathological results was analyzed. Results There were 9 men and 3 women with a mean age of 60.1±9.6 years （range, 32-73）. OF all the 12 patients with IPMN, 9 （75.0%） had experienced some symptoms of epigastric discomfort and/or pain as cell as backache; 7 cases were with medical history of acute pancreatitis, 5 cases with diabetes, 4 cases with elevated CA19-9, and 2 cases with steatorrhea. All lesions of IPMN have been revealed by transabdominal ul- trasonography. The mean diameters of the lesions were 1.4±0.8 cm （range, 0.5-2.0） and 6.3±6.0 cm （range, 2.0-20.0） in adenomas and adenocarcinomas, respectively. And the mean diameters of the main duct in adelnomas and adenocarcinomas were 1.0±0.8 cm and 1.6±1.0 cm, respectively. Among the 4 adnomas, 3 （75.0%） cases were classified as branch type based on sonographic findings, and 2 were demonstrated as mural nodules in which no color signals was detected. Among the 8 adenocarcinomas, 5 （62.5%） cases were classifled as main duct type, and 3 （37.5%） as combined type. In 7 of the 8 adcnocarcinomas, mural nodules were detected within the dilated ducts or cysts of the lesions in which color flow signals were detected. Conclusions Transabdominal ultrasonography can reveal the pancreatic cystic lesions of IPMN as well as dilated pancreatic： ducts.
