Value of magnetic resonance diffusion combined with perfusion imaging techniques for diagnosing potentially malignant breast lesions

BACKGROUND Lesions of breast imaging reporting and data system(BI-RADS)4 at mammography vary from benign to malignant,leading to difficulties for clinicians to distinguish between them.The specificity of magnetic resonance imaging(MRI)in detecting breast is relatively low,leading to many false-positive results and high rates of re-examination or biopsy.Diffusion-weighted imaging(DWI),combined with perfusion-weighted imaging(PWI),might help to distinguish between benign and malignant BI-RADS 4 breast lesions at mammography.AIM To evaluate the value of DWI and PWI in diagnosing BI-RADS 4 breast lesions.METHODS This is a retrospective study which included patients who underwent breast MRI between May 2017 and May 2019 in the hospital.The lesions were divided into benign and malignant groups according to the classification of histopathological results.The diagnostic efficacy of DWI and PWI were analyzed respectively and combinedly.The 95 lesions were divided according to histopathological diagnosis,with 46 benign and 49 malignant.The main statistical methods used included the Student t-test,the Mann-Whitney U-test,the chi-square test or Fisher’s exact test.RESULTS The mean apparent diffusion coefficient(ADC)values in the parenchyma and lesion area of the normal mammary gland were 1.82±0.22×10^(-3)mm^(2)/s and 1.24±0.16×10^(-3)mm^(2)/s,respectively(P=0.021).The mean ADC value of the malignant group was 1.09±0.23×10^(-3)mm^(2)/s,which was lower than that of the benign group(1.42±0.68×10^(-3)mm^(2)/s)(P=0.016).The volume transfer constant(Ktrans)and rate constant(Kep)values were higher in malignant lesions than in benign ones(all P<0.001),but there were no significant statistical differences regarding volume fraction(V_(e))(P=0.866).The sensitivity and specificity of PWI combined with DWI(91.7%and 89.3%,respectively)were higher than that of PWI or DWI alone.The accuracy of PWI combined with DWI in predicting pathological results was significantly higher than that predicted by PWI or DWI alone.CONCLUSION DWI,combined wit

Suppressing oxygen redox in layered oxide cathode of sodium-ion batteries with ribbon superstructure and solid-solution behavior

Sodium-ion batteries(SIBs)are proved as one of the most acceptable candidates for replacing lithium-ion batteries in some fields by virtue of a similar“rocking chair”mechanism and the abundance of sodium.The voltage,rate performance,and energy density of these batteries are mainly determined by the cath-odes.Hence,a Li-Ni-Co co-substituted P2-Na_(0.67)[Li_(0.1)(Mn_(0.7)Ni_(0.2)Co_(0.1))_(0.9)]O_(2)(NLMNC)with ribbon super-structure is prepared with the aim of multi-ion synergistic modification.Owing to the addition of Ni and Co,the Jahn-Teller distortion of Mn can be suppressed corresponding with the improved structural stability,and a little bit of oxygen redox activities is triggered.When with the substitution of 10%Li,the X-ray diffraction(XRD)peaks of NLMNC show the ribbon superstructure at about 21°and 22°.The smooth charge/discharge profiles of the NLMNC cathode exhibit the solid-solution reaction.In addition,the platform at high voltage disappears corresponding with the existing oxygen redox activities being suppressed which may be related to the ribbon superstructure and the promotion of the Ni redox.Such NLMNC cathode can deliver a reversible discharge capacity of 123.5 mA h g^(-1)at 10 mA g^(-1).Even if the current density increases to 500 mA g^(-1),a reversible discharge capacity of 112.8 mA h g^(-1)still can be ob-tained.The distinguished cycling stability is related to the reversible migration of Li+between the metal oxide layer and the interlayer and low volume change during cycling.It is also needing to be mentioned that the capacity retention of NLMNC cathode is about 94.4%(based on the highest discharge capacity)after 100 cycles.This work presents an effective route to develop high-performance cathodes for SIBs.

Treatment of adult congenital anal atresia with rectovestibular fistula:A rare case report

BACKGROUND Female anorectal malformation is a correctable congenital defect.Delayed manifestations in patients with anal deformities are uncommon,especially after adolescence.CASE SUMMARY The clinical data of a 19-year-old adult female patient with congenital anal atresia accompanied by rectovestibular fistula as the main manifestation was retrospectively analyzed.Diagnosis was made based on the patient's clinical symptoms,signs,imaging showing the fistula,X-ray and magnetic resonance imaging results.The preoperative examination was improved.Anorectoplasty was performed.The patient exhibited an improvement in quality of life and presented no evidence of fecal incontinence during the 6-mo follow-up.CONCLUSION Transfistula anorectoplasty is a reasonable and reliable surgical method for the treatment of adult congenital anal atresia and rectovestibular fistula.

Background The coronavirus disease 2019(COVID-19)continues to spread worldwide.Integrated Chinese and Western medicine have had some successes in treating COVID-19.Objective This study aims to evaluate the efficacy and safety of three traditional Chinese medicine drugs and three herbal formulas(3-drugs-3-formulas)in patients with COVID-19.Search strategy Relevant studies were identified from 12 electronic databases searched from their establishment to April 7,2022.Inclusion criteria Randomized controlled trials(RCTs),non-RCTs and cohort studies that evaluated the effects of 3-drugs-3-formulas for COVID-19.The treatment group was treated with one of the 3-drugs-3-formulas plus conventional treatment.The control group was treated with conventional treatment.Data extraction and analysis Two evaluators screened and selected literature independently,then extracted basic information and assessed risk of bias.The treatment outcome measures were duration of main symptoms,hospitalization time,aggravation rate and mortality.RevMan 5.4 was used to analyze the pooled results reported as mean difference(MD)with 95%confidence interval(CI)for continuous data and risk ratio(RR)with 95%CI for dichotomous data.Results Forty-one studies with a total of 13,260 participants were identified.Our analysis suggests that compared with conventional treatment,the combination of 3-drugs-3-formulas might shorten duration of fever(MD=–1.39;95%CI:–2.19 to–0.59;P<0.05),cough(MD=–1.57;95%CI:–2.16 to–0.98;P<0.05)and fatigue(MD=–1.36;95%CI:–2.21 to–0.51;P<0.05),decrease length of hospital stay(MD=–2.62;95%CI–3.52 to–1.72;P<0.05),the time for nucleic acid conversion(MD=–2.92;95%CI:–4.26 to–1.59;P<0.05),aggravation rate(RR=0.49;95%CI:0.38 to 0.64;P<0.05)and mortality(RR=0.34;95%CI:0.19 to 0.62;P<0.05),and increase the recovery rate of chest computerized tomography manifestations(RR=1.22;95%CI:1.14 to 1.3;P<0.05)and total effectiveness(RR=1.24;95%CI:1.09 to 1.42;P<0.05).Conclusion The 3-drugs-3-formulas can play an active role in treating all stages o

BACKGROUND The dynamic characteristics of glucose metabolism and its risk factors in patients living with human immunodeficiency virus(PLWH)who accepted primary treatment with the efavirenz(EFV)plus lamivudine(3TC)plus tenofovir(TDF)(EFV+3TC+TDF)regimen are unclear and warrant investigation.AIM To study the long-term dynamic characteristics of glucose metabolism and its contributing factors in male PLWH who accepted primary treatment with the EFV+3TC+TDF regimen for 156 wk.METHODS This study was designed using a follow-up design.Sixty-one male treatmentnaive PLWH,including 50 cases with normal glucose tolerance and 11 cases with prediabetes,were treated with the EFV+3TC+TDF regimen for 156 wk.The glucose metabolism dynamic characteristics,the main risk factors and the differences among the three CD4+count groups were analyzed.RESULTS In treatment-naive male PLWH,regardless of whether glucose metabolism disorder was present at baseline,who accepted treatment with the EFV+3TC+TDF regimen for 156 wk,a continuous increase in the fasting plasma glucose(FPG)level,the rate of impaired fasting glucose(IFG)and the glycosylated hemoglobin(HbA1c)level were found.These changes were not due to insulin resistance but rather to significantly reduced isletβcell function,according to the homeostasis model assessment ofβcell function(HOMA-β).Moreover,the lower the baseline CD4+T-cell count was,the higher the FPG level and the lower the HOMA-βvalue.Furthermore,the main risk factors for the FPG levels were the CD3+CD8+cell count and viral load(VL),and the factors contributing to the HOMA-βvalues were the alanine aminotransferase level,VL and CD3+CD8+cell count.CONCLUSION These findings provide guidance to clinicians who are monitoring FPG levels closely and are concerned about IFG and decreased isletβcell function during antiretroviral therapy with the EFV+3TC+TDF regimen for long-term application.

A novel polyurethane polymer containing perylene diimide(PDI)unit and aniline segment,denoted as PU-PDI,was designed and synthesized.Their tensile property and photophysical characteristic were investigated using various experimental and theoretical techniques.It is found that the elongation of PU-PDIs can reach more than 1000%,indicating good tensile performance.Moreover,based on the photoinduced electron transfer mechanism,the aniline segments in polyurethane can quench the auto-fluorescence of PDI unit to a negligible value,which overcomes the intractable fluorescence drawbacks of conventional colour filters.The non-emissive characteristic,in combination with the good thermal stability and tensile property of PU-PDI,provides a feasible design strategy to fabricate optical filters with high display quality.

AIM:To investigate the role of autophagy inhibitor 3-methyladenine(3-MA)on a diabetic mice model(DM)and the potential mechanism.METHODS:Male C57BL/6J mice were randomly divided into a normal control group(NC group)and an DM group.DM were induced by multiple low-dose intraperitoneal injection of streptozotocin(STZ)60 mg/kgd for 5 consecutive days.DM mice were randomly subdivided into untreated group(DM group),3-MA(10 mg/kgd by gavage)treated group(DM+3-MA group)and chloroquine(CQ;50 mg/kg by intraperitoneal injection)treated group(DM+CQ group).The fasting blood glucose(FBG)levels were recorded every week.At the end of experiment,retinal samples were collected.The expression levels of pro-apoptotic proteins cleaved caspase-3,cleaved poly ADP-ribose polymerase 1(PARP1)and Bax,anti-apoptotic protein Bcl-2,fibrosisassociated proteins Fibronectin and type 1 collagenα1 chain(COL1A1),vascular endothelial growth factor(VEGF),inflammatory factors interleukin(IL)-1βand tumor necrosis factor(TNF)-α,as well as autophagy related proteins LC3,Beclin-1 and P62 were determined by Western blotting.The oxidative stress indicators 8-hydroxydeoxyguanosine(8-OHdG)and malondialdehyde(MDA)were detected by commercial kits.RESULTS:Both 3-MA and CQ had shor t-term hypoglycemic effect on FBG and reduced the expression of VEGF and inflammatory factors IL-1βand TNF-αin DM mice.3-MA also significantly alleviated oxidative stress indicators 8-OHdG and MDA,decreased the expression of fibrosisrelated proteins Fibronectin and COL1A1,pro-apoptotic proteins cleaved caspase-3,cleaved PARP1,as well as the ratio of Bax/Bcl-2.CQ had no significant impact on the oxidative stress indicators,fibrosis,and apoptosis related proteins.The results of Western blotting for autophagy related proteins showed that the ratio of LC3 II/LC3 I and the expression of Beclin-1 in the retina of DM mice were decreased by 3-MA treatment,and the expression of P62 was further increased by CQ treatment.CONCLUSION:3-MA has anti-apoptotic and anti-fibrotic effects on the retina of

Optimizing Multi-Dimensional Packet Classification for Multi-Core Systems

Packet classification has been studied for decades; it classifies packets into specific flows based on a given rule set. As software-defined network was proposed, a recent trend of packet classification is to scale the five-tuple model to multi-tuple. In general, packet classification on multiple fields is a complex problem. Although most existing software-based algorithms have been proved extraordinary in practice, they are only suitable for the classic five-tuple model and difficult to be scaled up. Meanwhile, hardware-specific solutions are inflexible and expensive, and some of them are power consuming. In this paper, we propose a universal multi-dimensional packet classification approach for multi-core systems. In our approach, novel data structures and four decomposition-based algorithms are designed to optimize the classification and updating of rules. For multi-field rules, a rule set is cut into several parts according to the number of fields. Each part works independently. In this way, the fields are searched in parallel and all the partial results are merged together at last. To demonstrate the feasibility of our approach, we implement a prototype and evaluate its throughput and latency. Experimental results show that our approach achieves a 40% higher throughput than that of other decomposed-based algorithms and a 43% lower latency of rule incremental update than that of the other algorithms on average. Furthermore, our approach saves 39% memory consumption on average and has a good scalability.

Discrimination of Pb-Zn deposit types using sphalerite geochemistry: New insights from machine learning algorithm

Due to the combined influences such as ore-forming temperature,fluid and metal sources,sphalerite tends to incorporate diverse contents of trace elements during the formation of different types of Lead-zinc(Pb-Zn)deposits.Therefore,trace elements in sphalerite have long been utilized to distinguish Pb-Zn deposit types.However,previous discriminant diagrams usually contain two or three dimensions,which are limited to revealing the complicated interrelations between trace elements of sphalerite and the types of Pb-Zn deposits.In this study,we aim to prove that the sphalerite trace elements can be used to classify the Pb-Zn deposit types and extract key factors from sphalerite trace elements that can dis-criminate Pb-Zn deposit types using machine learning algorithms.A dataset of nearly 3600 sphalerite spot analyses from 95 Pb-Zn deposits worldwide determined by LA-ICP-MS was compiled from peer-reviewed publications,containing 12 elements(Mn,Fe,Co,Cu,Ga,Ge,Ag,Cd,In,Sn,Sb,and Pb)from 5 types,including Sedimentary Exhalative(SEDEX),Mississippi Valley Type(MVT),Volcanic Massive Sulfide(VMS),skarn,and epithermal deposits.Random Forests(RF)is applied to the data processing and the results show that trace elements of sphalerite can successfully discriminate different types of Pb-Zn deposits except for VMS deposits,most of which are falsely distinguished as skarn and epithermal types.To further discriminate VMS deposits,future studies could focus on enlarging the capacity of VMS deposits in datasets and applying other geological factors along with sphalerite trace elements when con-structing the classification model.RF’s feature importance and permutation feature importance were adopted to evaluate the element significance for classification.Besides,a visualized tool,t-distributed stochastic neighbor embedding(t-SNE),was used to verify the results of both classification and evalua-tion.The results presented here show that Mn,Co,and Ge display significant impacts on classification of Pb-Zn deposits and In,Ga,Sn,Cd,and Fe al

Influence of Backbone and Axial Substituent of Catalyst onα-Imino-ketone Nickel Mediated Ethylene(Co)Polymerization

Theα-imino-ketone nickel catalyst is an emerging versatile platform that is easy to prepare and allows for the production of branched high molecular weight functionalized polyethylenes.However,study on this catalyst system is rare thus far.In this contribution,by introducing different backbones,flexible and rigid axial substituents into theα-imino-ketone framework,a family of cationic nickel catalysts were synthesized and fully characterized.Without the addition of any activator,systematic studies on ethylene polymerization and copolymerization with polar monomers were performed to explore the influence of both backbone and axial substituent on catalytic activity,polymer molecular weight,branching density and incorporation.In particular,owing to the unique semi-opening feature of theα-imino-ketone framework,the preferred nickel catalyst exhibited high activity of 175 kg·mol^(-1)·h^(-1)to produce functionalized polyethylene with molecular weight of 13.4 kg·mol^(-1)and comonomer incorporation of 2.9 mol%.
