Relationship between red blood cell distribution width and intermediate-term mortality in elderly patients after percutaneous coronary intervention

Background Large-scale clinical research on the relationship between red blood cell distribution width （RDW） and intermediate-term prognosis in elderly patients with coronary artery disease （CAD） is lacking. Thus, this study investigated the effects of RDW on the intermediate-term mortality of elderly patients who underwent elective percutaneous coronary intervention （PCI）. Methods Data from 1891 patients 〉 65 years old underwent elective PCI from July 2009 to September 2011 were collected. Based on preoperative median RDW （12.3%）, the patients were divided into two groups. The low RDW group （RDW 〈 12.3%） had 899 cases; the high RDW group （RDW 〉 12.3%） had 992 cases. The all-cause mortality rates of the two groups were compared. Results Patients in the high RDW group were more likely to be female and accompanied with diabetes, had lower hemoglobin level. The mean follow-up period was 527 days. during follow-up, 61 patients died （3.2%）. The postoperative mortality of the high RDW group was significantly higher than that of the low RDW group （4.3% vs. 2.0%, P = 0.004）. After adjusting other factors, multivariate Cox regression analysis revealed that preoperative high RDW was significantly associated with postoperative all-cause mortality （hazard ratio： 2.301, 95% confidence interval： 1.106-4.785, P = 0.026）. Conclusions Increased RDW was an independent predictor of the increased intermediate-term all-cause mortality in elderly CAD patients after elective PCI.

Effect of metabolic syndrome on prognosis and clinical characteristics of revascularization in patients with coronary artery disease

Background People with metabolic syndrome are at higher risk for developing coronary artery disease （CAD）. The effect of the metabolic syndrome on outcomes in patients with preexisting CAD has not been well studied. This study was conducted to assess the prevalence, characteristics, in hospital and long term prognosis of CAD with metabolic syndrome and to determine the factors influencing the prognosis of the disease. Methods The DESIRE registry contains data of 3696 patients with CAD between 2001 and 2004. Mean long term followup was （8294±373） days. Diagnosis of metabolic syndrome was based on modified International Diabetes Federation （IDF） Worldwide Definition of the Metabolic Syndrome, using body mass index （BMI） instead of waist circumference. Results Of 2596 patients with complete records of height, weight, and so on, 1280 （49.3%） were identified with metabolic syndrome. The patients with metabolic syndrome had higher level of body mass index, systolic blood pressure, diastolic blood pressure, fasting glucose and disordered blood lipid （all P〈0.0001）, with higher creatinine [（10.5±4.3） mg/L vs （9.9±2.9） mg/L, P〈0.0001] and the number of white blood cells [（7.49±2.86）×10^9/L VS （7.19 ± 2.62） ×10^9/L, P=0.008） compared with those without metabolic syndrome. The patients with metabolic syndrome showed severer coronary angiographic alterations （left main artery and/or ≥2-vessel）（73.6% vs 69.6%, P=0.031）. There were no significant differences of major adverse cardiac and cerebral events （maCCE） or mortality in hospital between the two groups. during followup, the ratio of maCCE in CAD with metabolic syndrome patients increased significantly （11.8% vs 10.0%, P=0.044）. Fasting blood glucose （≥ 1000 mg/L） and triglyceride （TG ≥1500 mg/L） were responsible for most of the increased risk associated with the metabolic syndrome （adjusted OR 1.465, 95% CI 1.037-1.874, P=0.032; OR 1.378, 95% CI 1.01±1.768, P=0.044）. The Moon provides a unique environment for investigating nearby astrophysical events such as *** samples retain valuable information from these events,via detectable long-lived“fingerprint”radionuclides such as^(60)*** this work,we stepped up the development of an accelerator mass spectrometry(AMS)method for detecting^(60)Fe using the HI-13tandem accelerator at the China Institute of Atomic Energy(CIAE).Since interferences could not be sufficiently removed solely with the existing magnetic systems of the tandem accelerator and the following Q3D magnetic spectrograph,a Wien filter with a maximum voltage of±60 kV and a maximum magnetic field of 0.3 T was installed after the accelerator magnetic systems to lower the detection background for the low abundance nuclide^(60)Fe.A 1μm thick Si_(3)N_(4) foil was installed in front of the Q3D as an energy *** particle detection,a multi-anode gas ionization chamber was mounted at the center of the focal plane of the ***,an^(60)Fe sample with an abundance of 1.125×10^(-10)was used to test the new AMS *** results indicate that^(60)Fe can be clearly distinguished from the isobar^(60)*** sensitivity was assessed to be better than 4.3×10^(-14)based on blank sample measurements lasting 5.8 h,and the sensitivity could,in principle,be expected to be approximately 2.5×10^(-15)when the data were accumulated for 100 h,which is feasible for future lunar sample measurements because the main contaminants were sufficiently separated.

Can transthoracic Doppler echocardiography be used to detect coronary slow flow phenomenon？

Background Coronary slow flow phenomenon （CSFP） is an important, angiographic clinical entity but is lacking non-invasive detecting techniques. This study aimed to elucidate the value of transthoracic Doppler echocardiography （TTDE） in the diagnosis and monitoring of coronary slow flow in left anterior descending （LAD） coronary *** We consecutively enrolled 27 patients with CSFP in LAD detected by coronary arteriography from August 2009 to April 2010. Thirty-eight patients with angiographically normal coronary flow served as control. Corrected thrombolysis in myocardial infarction （TIMI） frame count （CTFC） was used to document coronary flow velocities. All subjects underwent TTDE within 24 hours after coronary angiography. LAD flow was detected and the coronary diastolic peak velocities （DPV） and diastolic mean velocities （DMV） were *** Sixty of 65 （92.3%） subjects successfully underwent TTDE. Baseline clinical characteristics were similar between the two groups. Coronary DPV and DMV of LAD were significantly lower in the CSFP group than in the control group （（0.228±0.029） m/s vs. （0.302±0.065） m/s, P=0.000; （0.176±0.028） m/s vs. （0.226±0.052） m/s, P=0.000,respectively）. There was a high inverse correlation between CTFC and coronary DPV and DMV （r=-0.727, P=0.000;r=-0.671, P=0.000, respectively）. Receiver operating characteristic （ROC） curve showed that the area under the curve （AUC） was less than one half for coronary DPV （AUC=0.104） and DMV （AUC=0.204）, *** In patients with CSFP, there is a high inverse correlation between CTFC and coronary diastolic flow velocities in the LAD coronary artery, as measured by TTDE. The value of TTDE in the monitoring and evaluation of coronary flow in patients with CSFP deserves further investigation.

The back-streaming white-neutron beamline(Back-n)of the China Spallation Neutron Source is an essential neutronresearch platform built for the study of nuclear data,neutron physics,and neutron *** types of cross-sectional neutron-reaction measurements have been performed at Back-n since early *** measurements have shown that a significant number of gamma rays can be transmitted to the experimental stations of Back-n along with the neutron *** gamma rays,commonly referred to as in-beam gamma rays,can induce a non-negligible experimental background in neutron-reaction *** the characteristics of in-beam gamma rays is important for understanding the experimental ***,measuring in-beam gamma rays is challenging because most gamma-ray detectors are sensitive to neutrons;thus,discriminating between neutron-induced signals and those from in-beam gamma rays is *** this study,we propose the use of the black resonance filter method and a CeBr_(3) scintillation detector to measure the characteristics of the in-beam gamma rays of *** types of black resonance filters,^(181)Ta,^(59)Co,^(nat)Ag,and^(nat)Cd,were used in this *** time-of-flight(TOF)technique was used to select the detector signals remaining in the absorption region of the TOF spectra,which were mainly induced by in-beam gamma *** energy distribution and flux of the in-beam gamma rays of Back-n were determined by analyzing the deposited energy spectra of the CeBr_(3) scintillation detector and using Monte Carlo *** on the results of this study,the background contributions from in-beam gamma rays in neutron-reaction measurements at Back-n can be reasonably evaluated,which is beneficial for enhancing both the experimental methodology and data analysis.

~rognostic value of baseline C-reactive protein levels in patients mdergoing coronary revascularization

Background C-reactive protein （CRP） is a lowly expressed marker for inflammatory response. This study aimed to evaluate the prognostic value of baseline CRP levels in patients undergoing coronary revascularization in the context of modern medical treatment. Methods This was a retrospective study in a single center. Four hundred and fourteen patients were enrolled, who underwent coronary revascularization and received adequate medication for secondary prevention of coronary heart disease. The study compared the follow-up clinical outcomes between high level CRP group （CRP 〉5 mg/L） and low level one. The median follow-up time was 551 days. Results Compared with low CRP group, the relative risk （RR） of the major adverse cardiovascular and cerebral events （maCCE） in high CRP group was 5.131 （95% CI： 1.864-14.123, P=0.002）. There were no significant differences in death myocardial infarction and stroke during the follow-up between two groups, but a higher risk of re-revascularization was found in high CRP group （RR 6.008, 95% CI： 1.667-21.665, P=0.006）. Cox regression analysis showed that only CRP level could contribute to maCCE during the follow-up. maCCE-free rate was much lower in high CRP group （Kaplan-Meier log-rank P 〈0.001）. Conclusion In the context of modern medical treatment, the baseline level of CRP is an independent predictor for long-term prognosis in patients with coronary revascularization.

Coronary stenting versus bypass surgery in heart failure patients with preserved ejection fraction

Background The optimal revascularization strategy in patients with heart failure with preserved ejection fraction （HFPEF） remains unclear. The aim of the present study was to compare the effects of percutaneous coronary intervention （PCI） and coronary artery bypass grafting （CABG） in patients with HFPEF. Methods From July 2003 through September 2005, a total of 920 patients with coronary artery disease （CAD） and HFPEF （ejection fraction 〉50%） underwent PCI （n=350） or CABG （n=570）. We compared the groups with respect to the primary outcome of mortality, and the secondary outcomes of main adverse cardiac and cerebral vascular events （maCCE）, including death, myocardial infarction, stroke and repeat revascularization, at a median follow-up of 543 days. Results In-hospital mortality was significantly lower in the PCI group than in the CABG group （0.3% vs. 2.5%, adjusted P=0.016）. during follow-up, there was no significant difference in the two groups with regard to mortality rates （2.3% vs. 3.5%, adjusted P=0.423）. Patients receiving PCI had higher maCCE rates as compared with patients receiving CABG （13.4% vs. 4.0%, adjusted P 〈0.001）, mainly due to higher rate of repeat revascularization （adjusted P 〈0.001）. Independent predictors of mortality were age, New York Heart Association （NYHA） class and chronic total occlusion. Conclusion Among patients with CAD and HFPEF, PCI was shown to be as good as CABG with respect to the mortality rate, although there was a higher rate of repeat revascularization in patients undergoing PCI.

Nondestructive and noninvasive neutron assays are essential applications of neutron *** resonance transmission analysis(NRTA)is a powerful nondestructive method for investigating the elemental composition of an *** back-streaming neutron line(Back-n)is a newly built time-of-flight facility at the China Spallation Neutron Source(CSNS)that provides neutrons in the eV to 300 MeV range.A feasibility study of the NRTA method for nuclide identification was conducted at the CSNS Back-n via two test *** results demonstrate that it is feasible to identify different elements and isotopes in samples using the NRTA method at *** study reveals its potential future applications.
