A retrospective study of epidural and intravenous steroids after percutaneous endoscopic lumbar discectomy for large lumbar disc herniation

OBJECTIVE:To assess the early curative effect of epidural or intravenous administration of steroids during a percutaneous endoscopic lumbar discectomy (PELD).;METHODS:28 consecutive patients who underwent PELD due to large lumbar disc herniation between November 2014 and January 2016 were followed up for 6 months. These patients were divided into two groups according to the treatment they received after PELD. 14 patients (Group A) were treated by PELD and epidural steroids, while the other 14 patients (Group B) were treated by PELD and intravenous steroids. We evaluated the effectiveness by the preoperative and postoperative visual analogue scale (VAS) scores for back and leg pain, and the postoperative Oswestry disability index (ODI) at 3 weeks after surgery via the clinical charts and telephone interview. Postoperative hospital stay and time return to work were investigated as well.;RESULTS:There is a significant decrease in VAS (back, leg), ODI, and time return to work (p < 0.05). For VAS (back), Group A showed a significant decrease compared with Group B at 1 day and 1 week after surgery (p = 0.011, p = 0.017). As for VAS (leg), Group A showed a significant decrease compared with Group B at 1 day, 1 week, 3 weeks, and 3 months follow-up examinations (p = 0.002, p = 0.006, p < 0.001, p < 0.001). For ODI, Group A showed a notable decrease compared with Group B (p < 0.001). The postoperative hospital stay in two groups was not statistically different (p = 0.636). But the time return to work in Group A was significantly shorter than that in Group B (p = 0.023).;CONCLUSIONS:Patients who underwent PELD with epidural steroid administration for large lumbar disc herniation showed favorable curative effect compared with those who underwent PELD with intravenous steroid administration.

OBJECTIVE:To investigate the usefulness of three-dimensional (3D) printing in complex spinal surgery.;METHODS:The study was conducted from October 2014 to March 2015 in Shenzhen Second Peoples' Hospital and 4 cases of complex severe spinal disorders were selected from our department. Among them one patient combined with congenital scoliosis, one with atlas neoplasm, one with atlantoaxial dislocation, and the rest one with atlantoaxial fracture-dislocation. The data of the diseased region was collected from computerized tomography scans for 3D digital reconstruction and rapid prototyping to prepare photosensitive resin models, which were applied in the treatment of these cases.;RESULTS:The use of 3D models reduced operating time and intraoperative blood loss as well as the risk of postoperative complications. Furthermore, no pedicle penetrations or screw misplacement occurred according to the postoperative planar radiographic images.;CONCLUSIONS:The tactile models from 3D printing allow direct observation and measurement, helping the orthopedists to have accurate morphometric information to provide personalized surgical planning and better communication with the patient and coworkers. Moreover, the photosensitive resin models can also guide the actual surgery with the drilling of pedicle screws and safe resection of tumor.

Intracranial arterial dissection is an important cause of stroke in young and middle-aged adults, but there are no domestic or globally-accepted criteria for the diagnosis and classification of intracranial arterial dissections. The evidence base for the benefits of diagnostic and interventional angiography techniques is lacking, and the imaging features of intracranial arterial dissections are not yet universally recognized or understood by clinicians. These issues influence the diagnosis and treatment of intracranial arterial dissection. Therefore, a consensus conference was convened by Neurointerventional Expert Committee of Chinese Congress of Neurological Surgeons, Chinese Interventional Neuroradiology Society of Chinese Stroke Association and Young Physicians Committee of Chinese Congress of Neurological Surgeons. The aim of this document is to report an expert consensus on the imaging diagnosis of intracranial arterial dissection. The participating clinical experts utilized their judgment and experience. Every effort was made to achieve consensus among the committee members. The expert consensus was intended to provide guidance, but they are not to be considered prescriptive for any individual patient and cannot replace the judgment of a clinician.

The classical Ambarzumyan’s theorem states that if the Neumann eigenvalues of the Sturm-Liouville operator-d^(2)/dx^(2)+q with an integrable real-valued potential q on[0,π] are {n^(2):n≥0},then q=0 for almost all x∈[0,π].In this work,the classical Ambarzumyan’s theorem is extended to the Dirac operator on equilateral tree graphs.We prove that if the spectrum of the Dirac operator on graphs coincides with the unperturbed case,then the potential is identically zero.

Forward modeling of seismic wave propagation is crucial for the realization of reverse time migration(RTM) and full waveform inversion(FWI) in attenuating transversely isotropic media. To describe the attenuation and anisotropy properties of subsurface media, the pure-viscoacoustic anisotropic wave equations are established for wavefield simulations, because they can provide clear and stable wavefields. However, due to the use of several approximations in deriving the wave equation and the introduction of a fractional Laplacian approximation in solving the derived equation, the wavefields simulated by the previous pure-viscoacoustic tilted transversely isotropic(TTI) wave equations has low accuracy. To accurately simulate wavefields in media with velocity anisotropy and attenuation anisotropy, we first derive a new pure-viscoacoustic TTI wave equation from the exact complex-valued dispersion formula in viscoelastic vertical transversely isotropic(VTI) media. Then, we present the hybrid finite-difference and low-rank decomposition(HFDLRD) method to accurately solve our proposed pure-viscoacoustic TTI wave equation. Theoretical analysis and numerical examples suggest that our pure-viscoacoustic TTI wave equation has higher accuracy than previous pure-viscoacoustic TTI wave equations in describing q P-wave kinematic and attenuation characteristics. Additionally, the numerical experiment in a simple two-layer model shows that the HFDLRD technique outperforms the hybrid finite-difference and pseudo-spectral(HFDPS) method in terms of accuracy of wavefield modeling.

BACKGROUND The advanced first-line regimen for advanced gastric cancer is based on a combination of fluoropyrimidine and platinum and/or paclitaxel(PTX),forming a two-or three-drug regimen.Compared to conventional PTX,nanoparticle albumin-bound PTX(Nab-PTX)has better therapeutic effects and fewer adverse effects reported in studies.Nab-PTX is a great option for patients presenting with advanced gastric cancer.Herein,we highlight an adverse event(hemorrhagic cystitis)of Nab-PTX in advanced gastric cancer.CASE SUMMARY A 55-year-old male was diagnosed with lymph node metastasis after a laparo-scopic-assisted radical gastrectomy for gastric cancer that was treated by Nab-PTX and S-1(AS).On the 15th day after treatment with AS,he was diagnosed with hemorrhagic cystitis.CONCLUSION Physicians should be aware that hemorrhagic cystitis is a potential adverse event associated with Nab-PTX treatment.

According to the report from the Chinese Center for Disease Control and Prevention,the prevalence of human immunodeficiency virus(HIV)infection exceeded 1.2 million individuals by the year 2022,with an annual increase of about 80000 cases.The overall prevalence of hepatitis B surface antigen among individuals co-infected with HIV reached 13.7%,almost twice the rate of the general population in China.In addition to the well-documented susceptibility to opportunistic infections and new malignancies,HIV infected patients frequently experience liver-related organ damage,with the liver and kidneys being the most commonly affected.This often leads to the development of end-stage liver and kidney diseases.Therefore,organ transplantation has emerged as an important part of active treatment for HIV infected patients.However,the curative effect is not satisfactory.HIV infection has been considered a contraindication for organ transplantation.Until the emergence of highly active anti-retroviral therapy in 1996,the once intractable replication of retrovirus was effectively inhibited.With prolonged survival,the failure of important organs has become the main cause of death among HIV patients.Therefore,transplant centers worldwide have resu-med exploration of organ transplantation for HIV-infected individuals and reached a positive conclusion.This study provides an overview of the current landscape of HIV-positive patients receiving liver transplantation(LT)in main-land China.To date,our transplant center has conducted LT for eight end-stage liver disease patients co-infected with HIV,and all but one,who died two months postoperatively due to sepsis and progressive multi-organ failure,have survived.Comparative analysis with hepatitis B virus-infected patients during the same period revealed no statistically significant differences in acute rejection reactions,cytomegalovirus infection,bacteremia,pulmonary infections,acute kidney injury,new-onset cancers,or vascular and biliary complications.

Long-wavelength(>500 km)magnetic anomalies originating in the lithosphere were first found in satellite magnetic surveys.Compared to the striking magnetic anomalies around the world,the long-wavelength magnetic anomalies in China and surrounding regions are relatively weak.Specialized research on each of these anomalies has been quite inadequate;their geological origins remain unclear,in particular their connection to tectonic activity in the Chinese and surrounding regions.We focus on six magnetic high anomalies over the(1)Tarim Basin,(2)Sichuan Basin(3)Great xing’an Range,(4)Barmer Basin,(5)Central Myanmar Basin,and(6)Sunda and Banda Arcs,and a striking magnetic low anomaly along the southern part of the Himalayan-Tibetan Plateau.We have analyzed their geological origins by reviewing related research and by detailed comparison with geological results.The tectonic backgrounds for these anomalies belong to two cases:either ancient basin basement,or subduction-collision zone.However,the geological origins of large-scale regional magnetic anomalies are always subject to dispute,mainly because of limited surface exposure of sources,later tectonic destruction,and superposition of multi-phase events.

The welding arc,as a carrier for the conversion of electrical energy to thermal energy,has a direct impact on the quality of welding by its properties and states.In the tungsten inert gas(TIG)welding process under the condition of Ar-He alternating gas supply,the arc is alternately converted between Ar arc and He arc with an alternating gas supply cycle,which has obvious arc change characteristics.The FLUENT software was used to numerically simulate the characteristics of the TIG arc under the condition of alternating gas supply,and the arc temperature field,arc pressure,electric potential and current density distribution under the condition of alternating gas supply were obtained.Combined with the real-time data of arc pressure measured by the water-cooled copper plate with holes,it is proved that the TIG arc has obvious dynamic characteristics under the condition of Ar-He alternating gas supply.This unique dynamic TIG arc acts on the 5A06 aluminum alloy weld,causing the molten pool to stir,resulting in uniform microstructure and grain refinement at the weld,and thereby improving the mechanical properties of the welded joint.
