Obstructive sleep apnea is associated with severity and long-term prognosis of acute coronary syndrome

Background Obstructive sleep apnea （OSA） is a common disease in patients with acute coronary syndrome （ACS） and associated with an increased risk of fatal and nonfatal cardiovascular events. However, most patients in previous study were treated with bare metal stents and the sample sizes were relatively low. The goal of this study was to evaluate the influence of OSA on the severity and prognosis of patients admitted for ACS. Methods In this prospective cohort study, we enrolled patients with ACS who were hospitalized for coronary angiogram/percutaneous coronary intervention and undergone polysomnography. We divided the patients into two groups： moderate to severe OSA group [apnea-hypopnea index （AHI）〉 15 events/h] and control group （AHI ≤ 15 events/h）. They were followed up for up 32 months. Then, we compared the ACS severity and long-term major adverse cardiovascular events （MACE） in patients with different severity of OSA. Results Five hundred and twenty nine patients were included in the final analysis, with 76% of them being men and an average age of 59 ＋ 10 years. The overall mean AHI is 29 ± 19 events/h, 70.5% of them （373/529） being with moderate to severe OSA and 29.5% （156/529） assign into control group. Compared with controls, patients with moderate or severe OSA exhibited a higher prevalence of hypertension as well as higher body mass index, SYNTAX score, Epworth score and length of hospitalization. With a median follow-up duration of 30 months, accumulative rate of MACE was also higher in patients with moderate or severe OSA than that in the control group （8.6% vs. 3.2%, P = 0.028）. After adjusting for baseline confounders by cox regression model, moderate to severe OSA was an independent risk factor of long-term MACE （P = 0.047, HR = 1.618, 95% CI： 1.069-3.869）. Neuroinflammation mediated by microglia and oxidative stress play pivotal roles in the development of chronic temporal lobe epilepsy(TLE).We postulated that kainic acid(KA)-Induced status epilepticus triggers microglia-dependent inflammation,leading to neuronal damage,a lowered seizure threshold,and the emergence of spontaneous recurrent seizures(SRS).Extensive evidence from our laboratory suggests that dextromethorphan(DM),even in ultra-low doses,has anti-inflammatory and neuroprotective effects in many animal models of neurodegenerative *** results showed that administration of DM(10 ng/kg per day;subcutaneously via osmotic minipump for 4 weeks)significantly mitigated the residual effects of KA,including the frequency of SRS and seizure *** addition,DM-treated rats showed improved cognitive function and reduced hippocampal neuronal *** found suppressed microglial activation-mediated neuroinflammation and decreased expression of hippocampal gp91^(phox) and p47^(phox) proteins in KA-induced chronic TLE ***,even after discontinuation of DM treatment,ultra-low doses of DM continued to confer long-term anti-seizure and neuroprotective effects,which were attributed to the inhibition of microglial NADPH oxidase 2 as revealed by mechanistic studies.

The Schrodinger cat state produced differently in two directions is anticipated to be a critical quantum resource in quantum information *** exploring the interplay between quantum nonreciprocity and topology in a one-dimensional microcavity array,we obtain the Schrodinger cat state(a pure quantum state)in a chosen direction at the edge cavity,whereas a classical state in the other *** nonreciprocal generation of the cat state originates from the topologically protected chiralitymode excitation in the nontrivial phase,but in the trivial phase,the nonreciprocal generation of cat state ***,our proposal is switchable by tuning the parameters so that a topological phase transition ***,the obtained cat state has nonreciprocal high fidelity,nonclassicality,and quantum coherence,which are sufficient to be used in various one-way quantum technologies,e.g.,invisible quantum sensing,noise-tolerant quantum computing,and chiral quantum *** work provides a general approach to control quantum nonreciprocities with the topological effect,which substantially broadens the fields of nonreciprocal photonics and topological physics.

Elevated IL-37 levels in the plasma of patients with severe coronary arterycalcification

Background Coronary artery calcification （CAC） is a predictor of cardiovascular events and plaque burden and is closely associatedwith chronic inflammation. Intedeukin （IL）-37 is a newly discovered member of the IL-1 family and is considered an anti-inflammatorycytokine. Our recent study on mice indicated that IL-37 could attenuate atherosclerosis and vascular calcification, which suggests that IL-37could be associated with the development of atherosclerosis and related diseases. The aim of this study was to investigate if IL-37 plays arole in the progression of CAC in patients. Methods Two hundred participants with suspected cardiovascular disease were recruited. Thelevels of plasma IL-37, osteoprotegerin （OPG）, hypersensitive C-reactive protein （hsCRP） together with other biochemical parameters weremeasured, and a coronary calcium assessment was carried out by multi-detector row CT. A score of 〈 10 AU （Agatston units） denotes anabsence of CAC, a score of 11-100 AU denotes mild CAC, 101-400 denotes moderate CAC, and 〉 400 AU denotes severe CAC. ResultsOur initial data showed that there were no apparent differences in plasma IL-37 levels among patients with or without mild or moderate ***, IL-37 levels were significantly increased in patients with severe CAC （P 〈 0.001）. Similar results were observed for plasma OPGand hsCRP levels. When IL-37 levels in patients with severe calcification were compared with that in all of the other non-severe CAC groups,it became apparent that there was a significant positive correlation between IL-37 level and severe CAC （r = 0.360, P 〈 0.001; OR = 1.033）using Spearrnan＇s correlation and binary logistic regression analysis. Conclusions This study demonstrates that the anti-inflammatory cy-tokine IL-37 is associated with high coronary calcium levels, suggesting that IL-37 expression may be caused by the activation ofinf/amma-tion and that IL-37 might become a predictor of severe CAC in the future, which requires further invest

Limits of Uneven Battery Power in Modular Multilevel Converter-based Battery Energy Storage Systems

For modular multilevel converter-based battery energy storage systems (MMC-BESS), uneven power among batteries of SMs will be deduced by battery aging, battery fault, etc., which will degrade performance and even lead to system failure. For maintaining the balance of capacitor voltage, this paper analyzes the limits of the uneven power of batteries, which are presented as the current limits in this paper. According to analysis, an analytical method is proposed that only the dc and fundamental frequency components of the arm current are used to calculate current limits. With the current limits it is able to evaluate the reasonability of power distribution among batteries. Meanwhile, increase of fundamental frequency component will enhance the current limits, and the dc component determines the size relationship between the absolute values of the upper and lower current limits. Finally, simulation model and experiment prototype are built for verifying the theoretical analysis and current limits calculation method, and satisfactory results are provided.

A study on carbon emission calculation in operation stage of residential buildings based on micro electricity usage behavior:Three case studies in China

Along with the improvement of social productivity and living standard,residential buildings generate a growing portion of carbon emissions,especially during the operation ***,energy use behaviors are usually ignored in carbon emission *** study focuses on calculating carbon emissions during the operation stage for residential buildings based on the characteristics of energy use behaviors in different ***,we investigated energy use behaviors in dwellings across three cities in China:Xi'an,Shanghai and ***,we established calibrated carbon emission models and optimization models with different green building measures for residential *** results of this research reveal a significant disparity between the energy usage habits of residents in different climate *** carbon emissions of residential electricity bills in Xi'an,Shanghai and Fuzhou are 13.6 kgCO_(2)/(m^(2)·a)(excluding central heating),29.3 kgCO_(2)/(m^(2)·a)and 17.2 kgCO_(2)/(m^(2)·a),*** carbon emissions account for 32.2%-64.1% of the *** comparison to the model based on internal standard setting,the accuracy of the models using actual internal has improved by 25.9%-37.4%.The three-star green building methods have the highest carbon reduction rate among different star buildings,the emission reduction rates are around 30%.This study's findings are useful for carbon emission calculation and green building design of residential buildings in the future.

Integrated Frequency-constrained Scheduling Considering Coordination of Frequency Regulation Capabilities from Multi-source Converters

As the proportion of renewable energy(RE)increases,the inertia and the primary frequency regulation(FR)capability of the power system ***,ensuring frequency security in the scheduling model has become a new technical requirement in power systems with a high share of *** to a shortage of conventional synchronous generators,the frequency support of multi-source converters has become an indispensable part of the system frequency resources,especially variable-speed wind turbine generation(WTG)and battery energy storage(BES).Quantitative expression of the FR capability of multi-source converters is necessary to construct frequency-constrained scheduling ***,the frequency support performance of these converter-interfaced devices is related to their working states,operation modes,and parameters,and the complex coupling of these factors has not been fully exploited in existing *** this study,we propose an integrated frequency-constrained scheduling model considering the coordination of FR capabilities from multi-source *** FR control strategies and variable FR parameters for WTG with or without reserved power are modeled,and multi-target allocation of BES capacity between tracking dispatch instruction and emergency FR is ***,the variable FR capabilities of WTG and BES are embedded into the integrated frequency-constrained scheduling *** nonlinear constraints for frequency security are precisely linearized through an improved iteration-based *** effectiveness of the proposed model is verified in a modified IEEE 24-bus standard *** results suggest that the coordinated participation of BES and WTG in FR can effectively reduce the cost of the scheduling model while meeting frequency security constraints.
