Akkermansia muciniphila play an important in ameliorating obesity but is not allowed for direct consumption in most countries.To date,microbiota-directed foods selectively promote the targeted human gut microbes,providing a strategy for A.muciniphila enhancement.Multiple studies have indicated the potential regulation of the polyphenol on A.muciniphila.Therefore,a polyphenol screening based on A.muciniphila upregulation was performed in mice.Chlorogenic acid(CGA)exhibited a greater response to A.muciniphila upregulation.Furthermore,we found that CGA did not directly promote A.muciniphila growth or mucin secretion.Microbiome and metabolomics revealed that the increased abundance of A.muciniphila resulted from the inhibition of CGA on Desulfovibrio and Alistipes and the influence of docosahexaenoic acid,β-hydroxybutyrate,and N-acetyl-lactosamine.Finally,to confirm the regulation of CGA on A.muciniphila under disease conditions,high-fat diet-fed mice were established.The results showed CGA promoted A.muciniphila growth,and we expectedly found that CGA suppressed the augment in body weight of mice,significantly attenuated adipose tissue abnormality,provided liver protection and improved gut barrier integrity.These results suggest that CGA inhibits the development of obesity.Overall,our results indicate that microbiota-directed food is a promising approach for the treatment of obesity.

Background Atrial septal defect(ASD)is one of the most common congenital heart diseases.The diagnosis of ASD via transthoracic echocardiography is subjective and time-consuming.Methods The objective of this study was to evaluate the feasibility and accuracy of automatic detection of ASD in children based on color Doppler echocardiographic static images using end-to-end convolutional neural networks.The proposed depthwise separable convolution model identifies ASDs with static color Doppler images in a standard view.Among the standard views,we selected two echocardiographic views,i.e.,the subcostal sagittal view of the atrium septum and the low parasternal four-chamber view.The developed ASD detection system was validated using a training set consisting of 396 echocardiographic images corresponding to 198 cases.Additionally,an independent test dataset of 112 images corresponding to 56 cases was used,including 101 cases with ASDs and 153 cases with normal hearts.Results The average area under the receiver operating characteristic curve,recall,precision,specificity,F1-score,and accuracy of the proposed ASD detection model were 91.99,80.00,82.22,87.50,79.57,and 83.04,respectively.Conclusions The proposed model can accurately and automatically identify ASD,providing a strong foundation for the intelligent diagnosis of congenital heart diseases.

Microneedle permits transdermal biosensing and drug delivery with minor pain.However,accurate microneedle transdermal positioning with minimal skin deformation remains a significant technical challenge due to inhomogeneous skin topology and discontinuous force applied to the microneedle.Here,we introduce bioinspired rotation microneedles for in vivo accurate microneedle positioning as inspired by honeybees’stingers.We demonstrate the benefits of rotation microneedles in alleviating skin resistance through finite element analysis,full-thickness porcine validations,and mathematical derivations of microneedleskin interaction stress fields.The max penetration force was mitigated by up to 45.7%and the force attenuation rate increased to 2.73 times in the holding stage after penetration.A decrease in max skin deflection and a faster deformation recovery introduced by rotation microneedles implied a more precise penetration depth.Furthermore,we applied the rotation microneedles in psoriasis mice,a monogenic disorder animal model,for minimally invasive biological sample extraction and proinflammatory cytokine monitoring.An ultrasensitive detection method is realized by using only one microneedle to achieve cytokine mRNA level determination compared to commonly required biopsies or blood collection.Thus,rotation microneedles permit a simple,rapid,and ultraminimal-invasive method for subcutaneous trace biological sample acquisition and subsequent point-of-care diagnostics with minimal damage to both microneedles and skins.

目的为探究松软煤层大采高工作面煤壁片帮机理,并对片帮严重的松软煤层大采高工作面煤壁进行有效控制,方法利用材料力学理论得到煤壁片帮的频发位置,引入煤壁剪切破坏准则,得出松软煤体煤壁片帮的主控因素。并以李村煤矿1305工作面为例,通过FLAC3D建立数值模型,以煤壁最大位移、煤体破坏深度、支承压力大小和支承压力峰值位置4个因素作为参考比较对象,通过引入正交试验法,研究了煤层埋深、内摩擦角、黏聚力、抗拉强度以及支护强度对煤壁片帮的影响程度。结果结果表明:煤体自身的物理力学性质(内摩擦角和黏聚力)是影响煤壁片帮的主要因素,其次煤体的赋存条件(埋深)也是一个较为关键的影响因素。结论工业性试验基于正交试验结果提出了大采高综采工作面深浅孔交替超前预注浆加固技术,有效控制了李村煤矿1305工作面煤壁片帮严重情况,为其他类似地质条件大采高综采工作面煤壁片帮控制提供了借鉴。

Background:Immunosuppressive medication reduces the immunogenicity of the coronavirus disease 2019(COVID-19)vaccines in patients with rheumatic and musculoskeletal diseases(RMDs).However,the underlying mechanism remains unclear.The primary aim of our study was to dissect the impact of immunosuppressive medication on cellular and humoral immune responses in RMD patients receiving the inactivated COVID-19 vaccine.Methods:A total of 28 RMD patients and five healthy controls(HCs)receiving two doses of the inactivated COVID-19 vaccine(Sinovac-CoronaVac)were prospectively enrolled.Blood samples were collected before the primary vaccination(Week 0)and one week after the second vaccination(Week 5).Neutralizing antibody(nAb)titers and autoantibody titers were measured by a pseudovirus-based neutralization assay and enzyme-linked immunosorbent assay,respectively.CD4^(+)T-cell and CD19^(+)B-cell subsets and serum cytokines were analyzed by flow cytometry.Results:The inactivated COVID-19 vaccine was immunogenic in RMD patients and HCs after the second vaccination,but the nAb titers were lower in RMD patients than those in HCs.Only patients with systemic lupus erythematosus(SLE)had notably increased nAb titers.Remarkably,IgG^(+)CD27^(+),IgG^(+)IgG1^(+),and IgG^(+)IgG1^(−)B cells were reduced,whereas IgG−IgG1^(+)B cells,and total IgA and IgG titers were markedly increased.However,Tfh cell and Tfr cell subsets and cytokines produced by Tfh cells were not increased.The flare rate was low in RMD patients with comparable autoantibody titers,unchanged CD4^(+)T cell subsets and serum proinflammatory cytokines(interleukin[IL]-6,IL-17,interferon-γ,and tumor necrosis factor-α)after the second vaccination.Conclusions:Immunosuppressive therapy decreased the immunogenicity of the vaccine and maintained a low flare rate by selectively modulating B cell but not CD4^(+)T cell responses in RMD patients receiving the inactivated COVID-19 vaccine.Optimization of the treatment regimen might ensure a durable and robust COVID-19 vaccination respon

目的总结抗黑色素瘤分化相关基因5抗体(MDA5)相关幼年型皮肌炎(JDM)的临床特征,研究其治疗及评估。方法回顾性分析2019年1月至2022年5月住院的5例抗MDA5相关JDM患儿的基本信息、临床表现、实验室及影像学检查、治疗及预后等临床资料。结果 5例患儿中男3例、女2例,5例均有典型的高春疹/征,3例有皮肤溃疡表现,2例出现皮肤钙化,1例有指端坏死。5例患儿肌力均正常,但肌肉核磁共振均异常。肌炎特异性抗体均为抗MDA5抗体阳性,3例合并抗Ro-52阳性。5例高分辨CT均示间质性肺病,只有1例出现气促,2例涎液化糖链抗原(KL-6)升高;2例有消化道受累。5例均使用激素,4例使用环磷酰胺作为最初免疫抑制剂,2月内均可部分缓解。结论抗MDA 5相关JDM起病年龄偏小,临床表现以典型皮疹为主,肌肉受累表现不明显,以临床无肌病性皮肌炎为主,并高发间质性肺病;抗MDA5滴度、高分辨CT、KL-6检查对于诊断及疾病疗效评估尤为重要;治疗主要甲基泼尼松龙联合免疫抑制剂,联合托法替布者治疗效果良好。
