Clinicopathological significance and immunotherapeutic outcome of claudin 18.2 expression in advanced gastric cancer:A retrospective study

Objective: Immunotherapeutic outcomes and clinical characteristics of claudin 18 isoform 2 positive(CLDN18.2-positive) gastric cancer(GC) vary in different clinical studies, making it difficult to optimize antiCLDN18.2 therapy. We conducted a retrospective analysis to explore the association of CLDN18.2 expression with clinicopathological characteristics and immunotherapeutic outcomes in GC.Methods: A total of 536 advanced GC patients from 2019 to 2021 in the CT041-CG4006 and CT041-ST-01clinical trials were included in the analysis. CLDN18.2 expression on ≥40% of tumor cells(2+, 40%) and CLDN18.2 expression on ≥70% of tumor cells(2+, 70%) were considered the two levels of positively expressed GC. The clinicopathological characteristics and immunotherapy outcomes of GC patients were analyzed according to CLDN18.2 expression status.Results: CLDN18.2 was expressed in 57.6%(cut-off: 2+, 40%) and 48.9%(cut-off: 2+, 70%) of patients.Programmed death-ligand 1(PD-L1) and CLDN18.2 were co-expressed in 19.8% [combined positive score(CPS)≥1, CLDN18.2(cut-off: 2+, 40%)] and 17.2% [CPS≥5, CLDN18.2(cut-off: 2+, 70%)] of patients.CLDN18.2 expression positively correlated with younger age, female sex, non-gastroesophageal junction(nonGEJ), and diffuse phenotype(P<0.001). HER2 and PD-L1 expression were significantly lower in CLDN18.2-positive GC(both P<0.05). Uterine adnexa metastasis(P<0.001) was more frequent and liver metastasis(P<0.001)was less common in CLDN18.2-positive GC. Overall survival and immunotherapy-related progression-free survival(ir PFS) were inferior in the CLDN18.2-positive group.Conclusions: CLDN18.2-positive GC is associated with poor prognosis and worse immunotherapeutic outcomes. The combination of anti-CLDN18.2 therapy, anti-PD-L1/PD-1 therapy, and chemotherapy for GC requires further investigation.

Nanoscale mechanical property variations concerning mineral composition and contact of marine shale

Mechanical properties of shales are key parameters influencing hydrocarbon production – impacting borehole stability, hydraulic fracture extension and microscale variations in in situ stress. We use Ordovician shale(Sichuan Basin, China) as a type-example to characterize variations in mineral particle properties at microscale including particle morphology, form of contact and spatial distribution via mineral liberation analysis(MLA) and scanning electron microscopy(SEM). Deformation-based constitutive models are then built using finite element methods to define the impact of various architectures of fracture and mineral distributions at nanometer scale on the deformation characteristics at macroscale.Relative compositions of siliceous, calcareous and clay mineral particles are shown to be the key factors influencing brittleness. Shales with similar mineral composition show a spectrum of equivalent medium mechanical properties due to differing particle morphology and mineral heterogeneity. The predominance of small particles and/or point-point contacts are conducive to brittle failure, in general, and especially so when quartz-rich. Fracture morphology, length and extent of filling all influence shale deformability. High aspect-ratio fractures concentrate stress at fracture tips and are conducive to extension, as when part-filled by carbonate minerals. As fracture spacing increases, stress transfer between adjacent fractures weakens, stress concentrations are amplified and fracture extension is favored. The higher the fractal dimension of the fracture and heterogeneity of the host the more pervasive the fractures. Moreover, when fractures extend, their potential for intersection and interconnection contributes to a reduction in strength and the promotion of brittle failure. Thus, these results provide important theoretical insights into the role of heterogeneity on the deformability and strength of shale reservoirs with practical implications for their stimulation and in the recovery of hydrocarbons from them.

The heterojunctions between metal and polymer have become the effective ways to produce the lighter,safer and more environmental friendly vehicles for the manufacturing fields of automotive and aerospace.The state-of-the-art frictionbased welding techniques are characterized by low peak temperature,severe plastic deformation,energy efficiency and nonpollution,which can simultaneously realize the mechanical and chemical bonding,improving mechanical performances.In this review,the current progress about friction-based welding techniques is summarized,containing technical development,welding tool design,microstructural characteristic,process optimization,surface modification and joining mechanism.The conclusions and prospects are presented,which focus on the practical implications for the manufacturing sectors and recommendations for further research and development.The purpose of this review is to elucidate the benefits of friction-based welding techniques so that these methods may be better exploited and industrialized.

An early screening model of pulse detection technology for hepatic steatosis

Background The increasing prevalence of hepatic steatosis presents a considerable challenge to public health.There is a critical need for the development of novel preventive and screening strategies for this condition.Thisstudy evaluated the potential applications of wrist pulse detection technology for the early detection of liverdiseases.The pulse time-domain features of a medical exam population with and without hepatic steatosis wereassessed to develop a screening model for this disease.Methods Participants were consecutively recruited from march 2021 to march 2022 in the medical examinationcenters of the Yueyang Hospital of Integrated Traditional Chinese and Western Medicine and the Shanghai Municipal Hospital of Traditional Chinese Medicine.Clinical data from 255 participants,including general information(sex,age,and body mass index),and data related to glucose and blood lipids(fasting plasma glucose,triglyceride,total cholesterol,high-density lipoprotein,and low-density lipoprotein levels)were collected.Wrist pulse signalswere acquired using a pulse detection device,and the pulse time-domain features,including t_(1),t_(4),t_(5),T,w_(1),w_(2),h_(2)/h_(1),h_(3)/h_(1),and h5/h_(1) were extracted.Participants were assigned to hepatic steatosis and non-hepatic steatosisgroups according to their abdominal ultrasound examination results.Their clinical data and pulse time-domainfeatures were compared using chi-square and parametric or non-parametric statistical methods.Three datasetswere used to construct screening models for hepatic steatosis based on the random forest algorithm.The datasetsfor modeling were defined as Dataset 1,containing blood glucose and lipid data and general information;Dataset2,containing time-domain features and general information;Dataset 3,containing time-domain features,bloodglucose and lipid data,and general information.The evaluation metrics,accuracy,precision,recall,F1-score,andareas under the receiver operating characteristic curve(AUC)were compared for each model.Results The time-d

Background The co-occurrence of depression and anxiety among adolescents is typically associated with suicide ideation.Aims The study aimed to investigate the symptom-level relationship between suicide ideation and the comorbidity of depression and anxiety.Methods 1501 adolescents aged 12-19 years were assessed using the Patient Health Questionnaire(PHQ-9)and the Generalized Anxiety Disorder Scale,and 716 adolescents who scored≥5 on both scales were selected as participants.Network analysis was used to identify the network structure of depressive symptoms and anxiety symptoms.Participants were categorised into either the suicide ideation or non-suicide ideation groups based on their scoring on the suicide-related item in PHQ-9.A comparison was made between the depression-anxiety symptom networks of the two groups.Results‘Restlessness’,‘sad mood’and‘trouble relaxing’were the most prominent central symptoms in the depression-anxiety symptom network,and‘restlessness’,‘nervousness’and‘reduced movement’were the bridge symptoms in this network.‘Sad mood’was found to be directly related to‘suicide ideation’with the highest variance.The network structure was significantly different in properties between the suicide ideation group and the non-suicide ideation group,with‘restlessness’and‘sad mood’exhibiting significantly higher influence in the network of the suicide ideation group than that in the non-suicide ideation group.Conclusion Restlessness and sad mood could be targeted for the intervention of depression-anxiety symptoms among adolescents with suicide ideation.

Towards Friction Stir Remanufacturing of High‑Strength Aluminum Components

Volumetric defects in high-strength aluminum alloy components were repaired via friction stir remanufacturing(FSR).Various consumable pins made of AA7075-T6 were designed.Top diameters of the consumable pins afected material fow,which ensured that the materials at the repairing interface were forged to metallurgical bonding.Conical angles determined load transfer besides material fow,which afected the fracture of the pins before the dwelling stage.Sound repaired components were achieved when the conical angle of the consumable pin was 1°larger than that of the volumetric defect.The ultimate tensile strength and elongation of the repaired components reached 445.9 MPa and 9.6%,respectively.The design criteria of the consumable pin in the FSR was established,which provided valuable references to repair volumetric defects in high-strength aluminum components.

碎软低渗煤层在我国普遍发育,制约煤层气单井产气量提高和产业发展。间接压裂是通过在邻近层产生垂直裂缝沟通煤层进而实现煤层有效改造的一种压裂方式,可以有效应对钻井塌陷、煤粉产出、压裂液滤失和煤层厚度薄等不利因素。通过间接压裂物理模拟试验和扩展有限元数值模拟分析,揭示煤层顶板间接压裂裂缝扩展影响因素,明确裂缝扩展机制,以期为间接压裂技术提供指导。通过直接压裂煤层和不同起裂位置、垂向应力和施工排量影响下间接压裂试验表明,起裂压力高,更易产生长裂缝,且受原生裂缝影响程度减小;但起裂点距离煤层越远,起裂所需能量越大,高破裂压力会对煤层造成粉碎性破坏;大施工排量下,起裂压力对应升高,起裂时间变短,原生裂缝影响程度变小。考虑地应力、起裂位置、岩石力学和施工排量等参数的数值模拟结果显示,在模型参数设置条件下,最大水平主应力和垂向应力差在3 MPa、弹性模量差<15 GPa的地层和岩性组合适合间接压裂,起裂位置距离煤层最优距离为<6 m,施工排量需要根据力学性质、断裂能密度等参数确定最优范围。

目的探讨关节镜下肩袖修复术后关节周围鸡尾酒注射镇痛的效果及安全性。方法2020年6月至2021年5月山东大学齐鲁医院(青岛)收治120例肩袖撕裂接受关节镜下肩袖修复术治疗的患者,男45例、女75例,年龄(61.35±5.75)岁(范围57~67岁),左侧肩关节58例、右侧肩关节62例。随机分为鸡尾酒注射组(接受关节周围注射罗哌卡因+吗啡+肾上腺素+复方倍他米松复方制剂)和镇痛泵组(应用镇痛泵),每组60例。围手术期及术后镇痛均采用标准化方案。在手术当日、术后第1天和第2天、出院当日以及术后2周和6周时记录疼痛视觉模拟评分(visual analogue scale,VAS)和肩关节活动范围。并记录术后临时镇痛剂(塞来昔布)的应用次数,以及鸡尾酒注射和镇痛泵应用的不良反应。结果两组患者的年龄、性别、手术时间、体质指数(body mass index,BMI)、美国麻醉医师协会(American Society of Anesthesiologists,ASA)分级、术前VAS评分、手术时间、术后住院时间、肩袖撕裂面积和术中使用锚钉数量的差异均无统计学意义(P>0.05)。鸡尾酒注射组患者术后VAS评分更低、肩关节活动度更大。与镇痛泵组相比,鸡尾酒注射组术后6、12 h和前24 h的临时镇痛剂应用次数减少。术后12 h鸡尾酒注射组VAS评分为3(3,4)分,低于镇痛泵组为5(5,6)分,差异有统计学意义(Z=143.004,P=0.003);术后18 h鸡尾酒注射组VAS评分出现反跳,VAS评分上升至4(3,4)分,但仍低于镇痛泵组的5(4,5)分;术后24~48 h鸡尾酒注射组VAS评分与镇痛泵组的差异无统计学意义(P>0.05)。术后12 h疼痛VAS评分的logistic回归分析显示,应用关节周围鸡尾酒注射是术后12 h较低VAS评分的独立预测因素[OR=4.125,95%CI(2.672,4.328),P=0.015],而患者的年龄、BMI、ASA分级、术前VAS评分、手术时间、肩袖撕裂面积、锚钉使用数量不是术后12 h疼痛VAS评分的独立预测因素(P>0.05)。术后24 h疼痛VAS评分的logistic回归分析结果显示,所有因素均不是术后24 h疼痛VAS评分升高的独立预测因素(P>0.05)。结论关节周围鸡尾酒注射可大幅减少关节镜下肩袖修复术后镇痛药物的用量,并提高患者的满意度。

随着面阵探测器的广泛使用,面向视频卫星的多目标跟踪具有重要意义,但基于图结构的多目标跟踪方法,在图的构建中,大多数从相邻帧提取线索,而忽略了以往帧的线索。针对这个问题,提出了一个端到端的图网络框架,利用从多帧中提取的运动特征、外观特征、拓扑信息等多种线索,对图的节点、边和全局变量进行构建。实现这个统一框架的一个关键原则是为不同的线索和不同的来源(轨迹和检测目标)设计兼容的特征表示和图网络更新机制。该框架以前馈的方式运行,并以在线的方式进行训练。在公共数据集VISO、MOT16、MOT17基准上评测,取得了99.8%、48.8%、51.8%的多目标跟踪精度,优于其他相关多目标跟踪算法,并通过消融试验验证了各个线索对多目标跟踪性能提高的有效性,未来在智慧交通、智慧城市、军事战争等诸多领域具有广泛应用场景。

随着人口老龄化、透析技术的提升,血液透析患者中老年人的比例越来越高,随之伴发的老年综合征在透析患者临床实践管理中需要引起重视。常见的老年综合征,如衰弱、肌少症、营养不良、躯体功能障碍、认知功能障碍及抑郁情绪,在血液透析患者中具有较高的患病率,并且与不良预后相关。对老年血液透析患者进行老年综合评估,能够早期识别高风险患者并进行分层管理,对改善患者预后及提高生活质量具有一定的指导意义。
