The learning curve for transurethral enucleation with bipolar energy for benign prostate hyperplasia:a single-surgeon experience of 494 patients

This study was performed to investigate the learning curve of transurethral enucleation with bipolar energy(TUEB)for benignprostatic hyperplasia.The study involved 494 consecutive patients who underwent TUEB for benign prostatic hyperplasia fromAugust 2018 to March 2022 by one surgeon(SJJ,Seoul National University Bundang Hospital,Seongnam,Korea).The patientswere followed up at 1 week,1 month,3 months,and 6 months postoperatively.To evaluate the learning curve of TUEB,perioperativeparameters including the enucleation ratio(enucleated tissue weight/transitional zone volume),TUEB efficiency(enucleatedtissue weight/operation time),and enucleation efficiency(enucleated tissue weight/enucleation time)were analyzed.Functionaloutcomes and postoperative complications were also assessed,including the International Prostate Symptom Score(IPSS),IPSSquality-of-life(QoL)score,and uroflowmetry outcomes.The patients’median age was 72(interquartile range[IQR]:66–78)years,and the estimated prostate volume and transitional zone volume were 63.0(IQR:46.0–90.6)ml and 37.1(IQR:24.0–60.0)ml,respectively.The enucleation ratio,TUEB efficiency,and enucleation efficiency were 0.60(IQR:0.46–0.54)g ml−1,0.33(IQR:0.22–0.46)g min−1,and 0.50(IQR:0.35–0.72)g min−1,respectively,plateauing after 70 cases.The functional outcomes,including total IPSS,IPSS QoL score,and uroflowmetry outcomes,significantly improved at 6 months after TUEB(all P<0.05),but without significant differences over the learning curve.Sixty-five(13.2%)patients developed complications after TUEB,21.5%of whom experienced major complications(Clavien–Dindo grade≥3).The rate of major complications declined as the number ofTUEB cases increased(P=0.013).Our results suggest that the efficiency of TUEB stabilized within 70 procedures.

Elasto-inertial microfluidic separation offers many advantages including high throughput and separation resolution.Even though the separation efficiency highly depends on precise control of the flow conditions,no concrete guidelines have been reported yet in elasto-inertial microfluidics.Here,we propose a dimensionless analysis for precise estimation of the microsphere behaviors across the interface of Newtonian and viscoelastic fluids.Reynolds number,modified Weissenberg number,and modified elastic number are used to investigate the balance between inertial and elastic lift forces.Based on the findings,we introduce a new dimensionless number defined as the width of the Newtonian fluid stream divided by microsphere diameter.The proposed dimensionless analysis allows us to predict whether the microspheres migrate across the co-flow interface.The theoretical estimation is found to be in good agreement with the experimental results using 2.1-and 3.2-μm-diameter polystyrene microspheres in a co-flow of water and polyethylene oxide solution.Based on the theoretical estimation,we also realize submicron separation of the microspheres with 2.1 and 2.5μm in diameter at high throughput,high purity(>95%),and high recovery rate(>97%).The applicability of the proposed method was validated by separation of platelets from similar-sized Escherichia coli(E.coli).

This study investigates the relationship between the persistence and the zonal scale of atmospheric dipolar modes(DMs). Results from the daily data of ERA5 and the long-term output of an idealized atmospheric model show that the atmospheric DMs with a broader(narrower) zonal scale dipolar structure possess a longer(shorter) persistence. A detailed vorticity budget analysis indicates that the persistence of a hemispheric-scale DM(1/1 DM) and a regional or sectoral DM(1/8 DM) in the model both largely rely on the persistence of the nonlinear eddy forcing. Linear terms can indirectly reduce the persistence of the anomalous nonlinear eddy forcing in a 1/8 DM by modifying the baroclinicity via the arousal of anomalous vertical motions. Therefore, the atmospheric DMs with a broader(narrower) zonal scale possess a longer(shorter) persistence because the effects of the linear terms are less(more) pronounced when the atmospheric DMs have better(worse) zonal symmetry. Further analyses show that the positive eddy feedback effect is weak or even absent in a 1/8DM and the high-frequency eddy forcing acts more like a concomitant phenomenon rather than a leading driving factor for a 1/8 DM. Thus, the hemispheric-scale DM and the regional or sectoral DMs are different, not only in their persistence but also in their dynamics.

In this paper,the problem of identifying autoregressive-moving-average systems under random threshold binary-valued output measurements is considered.With the help of stochastic approximation algorithms with expanding truncations,the authors give the recursive estimates for the parameters of both the linear system and the binary sensor.Under reasonable conditions,all constructed estimates are proved to be convergent to the true values with probability one,and the convergence rates are also established.A simulation example is provided to justify the theoretical results.

Background The poultry industry needs effective antibiotic alternatives to control outbreaks of necrotic enteritis(NE)caused by Clostridium perfringens.Methods The aim of this study was to investigate the effects of dietary supplementation with Macleaya cordata extract(MCE)on the immune function and gut microbiota of broilers with NE.A total of 2881-day-old broiler chicks were randomly assigned to a 2×2 factorial arrangement with two concentrations of dietary MCE supplementation(0 or 350 mg/kg of diet)and two disease challenge statuses(control or NE).Results The results revealed that NE significantly increased the feed conversion rate(FCR),mortality,intestinal lesion score,the levels of IL-1β,IL-17 and IFN-γ/IL-4 in serum and IL-17/IL-10 in the jejunal mucosa,m RNA levels of TLR2,IFN-γand p Ig R in the jejunum,and Clostridium perfringens concentrations in the cecum.NE significantly decreased the body weight(BW),body weight gain(BWG),jejunal villus height,V/C,m RNA level of AMPK-α1 in jejunum,IL-4 level in the jejunal mucosa and lactic acid bacteria abundance in the cecum.MCE significantly increased BW,BWG,jejunal villus height,V/C,m RNA levels of occludin,ZO-1 and AMPK-α1 in the jejunum,the levels of Ig A and Ig G in serum and IL-10 in the jejunal mucosa and m RNA levels of NF-κB,IL-10 and MHC-II in the jejunum.Additionally,MCE significantly decreased the FCR,mortality,intestinal lesion score,jejunal crypt depth,the levels of IFN-γand IL-17 in serum and IL-17/IL-10 in the jejunal mucosa,Clostridium perfringens concentrations in the cecum,and m RNA levels of IL-17/IL-10 in the jejunum.Moreover,NE significantly increased the abundance of bacteria that are associated with inflammation,obesity and depression(Alistipes,Barnesiella,Intestinimonas,RF39 and UCG-005)and significantly decreased the abundance of short-chain fatty acid(SCFA)-producing bacteria(Anaerotruncus,Butyricicoccus and Bacteroides)in the cecum.MCE significantly increased the abundance of SCFA-producing bacteria(Streptococcus,Ruminococcus_torques_group

With the rapid development of electronics,electric vehicles,and grid energy storage stations,higher requirements have been put forward for advanced secondary batteries.Liquid metal/alloy electrodes have been considered as a promising development direction to achieve excellent electrochemical performance in metal-ion batteries,due to their specific advantages including the excellent electrode kinetics and self-healing ability against microstructural electrode damage.For conventional liquid batteries,high temperatures are needed to keep electrode liquid and ensure the high conductivity of molten salt electrolytes,which also brings the corrosion and safety issues.Ga-based metal/alloys,which can be operated at or near room temperature,are potential candidates to circumvent the above problems.In this review,the properties and advantages of Ga-based metal/alloys are summarized.Then,Ga-based liquid metal/alloys as anodes in various metal-ion batteries are reviewed in terms of their self-healing ability,battery configurations,working mechanisms,and so on.Furthermore,some views on the future development of Ga-based electrodes in batteries are provided.

家庭育儿支持对实现适度生育水平具有重要意义,但以往研究较少探讨家庭育儿支持的有效性问题。文章基于2021年“低生育率背景下育龄家庭的生育机制与生育支持研究”全国抽样调查数据,探讨当女性面临工作—家庭冲突时,不同家庭育儿支持的效果。研究结果表明:(1)工作—家庭冲突对女性再生育意愿有显著负面影响,家庭成员提供的育儿支持有助于缓解其负面影响;(2)工作—家庭冲突对女性再生育意愿的影响在不同育儿照料模式间存在差异,夫妻合作对缓解工作—家庭冲突的负面影响作用最大;(3)在不同形式的祖辈支持下,工作—家庭冲突对女性再生育意愿的影响存在差异,特别是那些仅获得祖辈经济支持的样本,工作—家庭冲突对再生育意愿的负面影响得到有效缓解;(4)当面临工作—家庭高冲突情境时,新生代育龄女性的再生育意愿难以被代际合作有效提振。文章认为,工作—家庭冲突在现代社会愈发凸显,夫妻合作在未来家庭育儿模式中将占据更加重要的位置。相关部门应当为家庭内部育儿分工平等化提供支持,促进代际和谐,并通过发展普惠性托育服务补充家庭育儿资源的不足。

Carbonate rock chemical weathering carbon sinks reduce the rate of increase of the atmospheric CO_(2) concentration and global warming. However, uncertainty still exists in the estimation results of carbonate rock chemical weathering carbon sink fluxes(CCSF), and the contributions of climate change and ecological restoration to the CCSF are not clear. To this end, we compiled published site data on ion concentrations in different watersheds in China and used a classical thermodynamic dissolution model to reassess the potential and spatial and temporal patterns of the CCSF in China from 1991 to 2020. We quantified the contributions of temperature(MAT), precipitation(MAP), evapotranspiration(ET), soil water(SM), and the normalized difference vegetation index(NDVI) to the CCSF. The results revealed that(1) China's CCSF was 22.76 t CO_(2)km^(-2)yr^(-1), which was higher than the global average(15.77 t CO_(2)km^(-2)yr^(-1)). The total carbonate rock chemical weathering carbon sink(CCS) was 4772.67×10^(4)t CO_(2), contributing 14.91% of the global CCS through a carbonate rock area of 252.98×10^(4)km^(-2).(2) China's CCSF decreased gradually from southeast to northwest, with values of 33.14, 12.93, and7.27 t CO_(2)km^(-2)yr^(-1)in the southern karst, Qinghai-Tibetan karst, and northern karst regions, respectively.(3) The overall CCSF in China exhibited an increasing trend from 1991 to 2020, with a rate of increase of 0.16 t CO_(2)km^(-2)yr^(-1).(4) The contributions of the MAP, MAT, ET, SM, and NDVI to the CCSF were 63.3%, 3.02%, 27.5%, 3.1%, and 3.05%, respectively. Among them, the increase in precipitation was the main contributor to the increase in the CCSF in China over the last 30 years, while the enhancement of ET offset part of the positive contribution of the increase in precipitation to the CCSF. In conclusion, the results of this study provide a systematic quantification of the magnitude, the patterns, and the influencing factors of CCS over a long time series in China.
