目的探讨巨大高颈段颈内动脉动脉瘤的治疗策略。方法回顾性分析1例巨大高颈段颈内动脉动脉瘤患者的临床资料、手术策略及随访,回顾相关文献。结果1例52岁男性,因“间断呛咳1年,加重伴头痛2个月”入院,DSA提示右侧巨大高颈段颈内动脉动脉瘤,行颈外动脉-桡动脉-颈内动脉岩骨段搭桥并动脉瘤孤立术,术后造影显示动脉瘤消失,桥血管通畅。术后无新发脑缺血或脑梗死,无新发神经功能障碍,原有症状显著改善。结论对于高颈段的巨大颈内动脉动脉瘤的治疗,脑血管搭桥手术仍然是有效治疗方案之一,颈外动脉-桡动脉-岩骨段颈内动脉搭桥可以很好地处理此类病变,是理想的治疗手段。

目的探讨鼻咽癌放疗后颈内动脉破裂的治疗策略。方法回顾性分析2020年3月—2023年3月收治的7例鼻咽癌放疗后颈内动脉破裂出血患者的临床资料。结果7例患者中4例发生颅底骨坏死伴感染,3例肿瘤复发伴感染。DSA造影提示6例颈内动脉破裂位于颈内动脉岩骨段,1例位于斜坡旁段。6例患者行球囊闭塞试验(BOT),通过3例,未通过3例。血管治疗方案:颈内动脉长程栓塞4例,假性动脉瘤栓塞1例(再出血),覆膜支架1例(再出血),手术中肌肉压迫1例,再出血的患者行高流量搭桥。栓塞后脑梗死3例,经过治疗无后遗症,90 d内无死亡患者。出血控制后3例颅底坏死感染者均行手术治疗清除坏死骨质,并组织瓣修复,1例复发患者行伽马刀及靶向治疗,1例行免疫及手术治疗,1例免疫及靶向治疗。结论鼻咽癌放疗后颈内动脉爆裂与肿瘤侵犯和放疗后的损伤和局部组织感染密切相关。对于肿瘤侵犯导致的,建议牺牲责任血管;对于感染造成的破裂出血,建议保留血管,并尽早手术。紧急血管闭塞仍是挽救生命的选择。

Oil and gas are the major energy resources all over the world but are not renewable. According to their present reserves, the resource volumes yet to be found and the large amount of non-conventional oil and gas resources, there is still great potential in oil and gas production. The proportion ofoil and gas in energy structure will be influenced by four major factors： 1） Potential of the world oil and gas resources; 2） Technological progress of oil and gas exploration and development; 3） Speed of the development of substitute energy resources; and 4） Variation ofoil price. It is estimated that, oil and gas will still retain an important proportion in energy structure by the first half of the 21st century.

原位间置搭桥术治疗颅内复杂动脉瘤18例

目的:探讨原位间置搭桥术在颅内复杂动脉瘤中的应用。方法:回顾性分析2015年7月至2022年9月,在天津医科大学神经内外科及神经康复临床学院神经外科用原位间置搭桥术治疗的18例颅内动脉瘤患者的临床资料。动脉瘤分别位于大脑中动脉(MCA)14例,大脑前动脉(ACA)2例,小脑后下动脉(PICA)1例,大脑后动脉(PCA)1例;间置移植血管分别取自桡动脉(RA)14例,颞浅动脉(STA)3例,枕动脉(OA)1例;所有患者均行动脉瘤切除后端-端吻合原位间置搭桥,其中间置血管形态为I型13例,V型3例,Y型2例。术后均行数字减影血管造影(DSA)或CTA复查,采用格拉斯哥结果量表(GOS)评估患者预后,并定期采用门诊、电话或网络随访。结果:2例患者术后出现偏瘫;1例出现短暂的失语,3个月后完全恢复;其余15例患者未出现新的神经功能障碍。术后DSA检查结果显示,1个吻合口术后重度狭窄,其余吻合口及移植血管均通畅,所有动脉瘤均未显影;随访3~89个月,无动脉瘤复发,GOS评分5分16例,3分2例。结论:原位间置搭桥技术是治疗部分颅内复杂动脉瘤的一种安全有效的选择。

Forecast and analysis of the "roof effect" of world net oil-exporting capacity

Oil is extremely crucial to the development of the modern economy. It is important to forecast the oil supply capacity due to its scarcity and non-renewability. This paper attempts to forecast and analyze thirty-five current and potential net oil-exporting countries. Integrating both qualitative and quantitative methods, the oil production and consumption are predicted based on historical data, so that the world net oil-exporting capacity can be obtained. The results show that the ＂roof effect＂ of the world net oil-exporting capacity may appear before 2030. Unconventional oil will play an important role in the future world oil market. The competition and cooperation relationships between OPEC and non-OPEC will last for a long time.

Based on the compilation and analysis of the lithofacies and paleogeography distribution maps at present and paleoplate locations during six key geological periods of the Mesozoic and Cenozoic,the lithofacies and paleogeography features and their development laws were *** on our previous research results on lithofacies and paleogeography from Precambrian to Paleozoic,we systematically studied the features and evolution laws of global lithofacies and paleogeography from the Precambrian and their effects on the formation of source rocks,reservoirs,cap rocks and the distribution of oil and gas *** results show that since Precambrian,the distribution areas of uplift erosion and terrestrial clastic deposition tended to increase gradually,and increased significantly during the period of continental *** scale of coastal and shallow marine facies area had three distinct cycles,namely,from Precambrian to Devonian,from Carboniferous to Triassic,and from Jurassic to ***,the development of shallow carbonate platform also showed three cycles;the lacustrine facies onshore was relatively developed in Mesozoic and Cenozoic;the sabkha was mainly developed in the Devonian,Permian and *** Cretaceous is the most important source rock layers in the world,followed by the Jurassic and Paleogene source rocks;the clastic reservoirs have more oil and gas than the carbonate reservoirs;the basins with shale caprocks have the widest distribution,the most abundant reserves of oil and gas,and the evaporite caprocks have the strongest sealing capacity,which can seal some huge oil and gas fields.

目的探讨以脑缺血为首发症状的前循环动脉瘤临床特点及治疗策略。方法回顾性分析2016年9月至2021年12月收治的10例以脑缺血为首发症状前循环动脉瘤患者的临床资料。结果10例患者中,以短暂性脑缺血发作(transient ischemic attack,TIA)为首发症状者4例,以缺血性卒中为首发症状者6例;颈内动脉动脉瘤3例,大脑中动脉瘤7例;巨大动脉瘤5例,大动脉瘤4例,小动脉瘤1例;单纯囊状动脉瘤3例,囊状动脉瘤合并血栓形成3例,夹层动脉瘤4例;治疗方式上,支架辅助弹簧圈栓塞1例,显微手术夹闭1例,动脉瘤切除3例,其中1例动脉瘤切除后原位吻合,2例辅以颞浅动脉-大脑中动脉(STA-MCA)搭桥术,动脉瘤孤立联合STA-MCA搭桥术3例,近端阻断联合颈外动脉-桡动脉-大脑中动脉(ECA-RA-MCA)高流量搭桥1例,颈内动脉缩窄联合STA-MCA搭桥1例。术后除1例患者出现同侧基底节小范围梗死致对侧肌力下降外,其他患者未出现新的神经功能障碍。术后复查8例动脉瘤消失,2例动脉瘤缩小。术后随访9~72个月,平均35.3个月,随访期间无蛛网膜下腔出血或新发脑梗死,无动脉瘤复发、扩大。结论前循环单纯宽颈或合并血栓形成的囊状动脉瘤、夹层动脉瘤,均可能出现远端血管栓塞致脑缺血发作,明确诊断后通过对动脉瘤及血流的细致分析,制定个性化的手术方案,可取得良好的治疗效果。

Effects of rafting on the meso deformation and fracture behaviour of a Ni-based single crystal superalloy at room temperature: In-situ observation and simulation

Microstructural rafting of Ni-based single-crystal(SC) superalloys is inevitable at elevated temperatures during long-term service with mechanical loading, which significantly affects the mechanical behaviour of the material. In this study, the effects of rafting on the mesodeformation and fracture behaviour of a Ni-based SC superalloy under cyclic and tensile loads were investigated using in situ scanning electron microscopy(SEM), digital image correlation(DIC), and crystal plasticity finite element method(CP-FEM) simulations. The results indicated that the tensile strength decreased significantly in the rafted specimens. In the cyclic tests, both the virgin and rafted specimens showed an increase in the maximum shear strain with cycle number. The interaction of cross-slip bands was captured by in situ SEM-DIC around the micro-notch of the virgin specimens during the tensile test, while a more homogeneous local deformation field was observed in the rafting specimens. In addition,the fracture behaviour was strongly influenced by the rafting morphology. The crack exhibited instantaneous and long-range fracture features along the octahedral plane as it propagated in the rafting specimen, whereas it deflected over a short distance between the crystallographic planes at an early stage in the virgin specimen, which is consistent with the CP-FEM ***, the CP-FEM results for the crack initiation direction on(111) dominant slip plane were consistent with the in situ SEM observations.
