Application of psychological intervention in intensive care unit nursing for patients with severe acute pancreatitis

BACKGROUND Severe acute pancreatitis(SAP)is a familiar critical disease in the intensive care unit(ICU)*** staff are important spiritual pillars during the treatment of patients,and in addition to routine nursing,more attention needs be paid to the patient’s psychological *** To investigate the effects of psychological intervention in ICU patients with *** One hundred ICU patients with SAP were hospitalized in the authors’hospital between 2020 and 2023 were selected,and divided into observation and control groups per the hospitalization *** control and observation groups received routine nursing and psychological interventions,*** groups are being compared,using the Self-rating Anxiety Scale(SAS),Self-Determination Scale(SDS),Acute Physiology and Chronic Health Evaluation(APACHE)Ⅱ,and 36-item Short Form Health Survey(SF-36)scores;nursing satisfaction of patients;ICU care duration;length of stay;hospitalization expenses;and the incidence of *** After nursing,the SDS,SAS,and APACHEⅡ scores in the experimental group were significantly lower than in the control group(P<0.05).The SF-36 scores in the observation group were significantly higher than those in the control group(P<0.05).The nursing satisfaction of patients in the experimental group was 94.5%,considerably higher than that of 75.6% in the control group(P<0.05).The ICU care duration,length of stay,and hospitalization expenses in the observation group were significantly lower than those in the control group,and the incidence of complications was lower(P<0.05).CONCLUSION For patients with SAP,the implementation of standardized psychological intervention measures can effectively alleviate adverse psychological conditions.

Prediction of pathological complete response and prognosis in locally advanced rectal cancer

A transient single particle model under FCI conditions

The paper is focused on the coupling effect between film boiling heat transfer and evaporation drag around a hot-particle in cold liquid. Based on the continuity, momentum and energy equations of the vapor film, a transient two-dimensional single particle model has been established. This paper contains a detailed description of HPMC (High-temperature Particle Moving in Coolant) model for studying some aspects of the premixing stage of fuel-coolant interactions (FCIs). The transient process of high-temperature particles moving in coolant can be simu-lated. Comparisons between the experiment results and the calculations using HPMC model demonstrate that HPMC model achieves a good agreement in predicting the time-varying characteristic of high-temperature spheres moving in coolant.

Porcine acute liver failure model established by two-phase surgery and treated with hollow fiber bioartificial liver support system

AIM： To establish a highly reproducible animal model of acute liver failure （ALF）, for assessing the effect of bioartificial liver support system （BALSS）. METHODS： A two-phase complete liver devascularization procedure was performed in eight Ioco-hybrid pigs. Blood biochemical index and liver biopsy were studied every 2 h after surgery, and survival time was recorded. The BALSS constructed with high volume recirculating technique was a hollow fiber circulating system consisting of a hepatocyte reactor-hollow fiber module inoculated with microcarrieradhering hepatocytes, and a double pump, heparinized, thermostabilized, micro-capsulized activated carbonadsorbing plasmapheresis system. Twelve pigs undergoing two-phase surgery were randomized into： control group （perfused without hepatocytes, n = 6） and treatment group （perfused with hepatocytes, n = 6）. Intergroup liver biochemical indexes, survival time, and liver pathological changes were analyzed at regular intervals. RESULTS： Two-phase surgery was performed in all the experimental pigs, and there was no obvious difference between their biochemical indexes. After 3 h of phase II surgery, ammonia （Amm） increased to （269＋37）μmol/L. After 5 h of the surgery, fibrinogen （Fib） decreased to （1.5±0.2） g/L. After 7 h of the surgery, ALT, AST, Tbil and PT were （7.6±1.8） nka/L, （40±5） nka/L, （55±8）μmol/L and （17.5±1.7） nka/L respectively. After 9 h of surgery, ALB and Cr were （27±4） g/L and （87±9）μmol/L. After 13 h of surgery, BUN was （3.5±0.9） μmol/L. All the above values were different from those determined before surgery. Survival time of pigs averaged 13.5±1.4 h. ALF pigs in the other group were treated with BALSS. The comparison analysis between the treated and control animals showed the changes of Tbil, PT, AIb, BUN, Cr, Fib, and Amm （P〈0.01）, but there was no change of ALT and AST. A Parametric Survey of the CME Triggering Process by Numerical Simulations

Observations indicate that solar coronal mass ejections （CMEs） are closely associated with reconnection-favored flux emergence, which was explained in the emerging flux trigger mechanism for CMEs by Chen ＆ Shibata based on numerical simulations. We present a parametric survey of the triggering agent： its polarity orientation, position, and the amount of the unsigned flux. The results suggest that whether a CME can be triggered depends on both the amount and location of the emerging flux, in addition to its polarity orientation. A diagram is presented to show the eruption and non-eruption regimes in the parameter space. The work is aimed at providing useful information for the space weather forecast.

Antidiastole Value of Three-dimensional Ultrasonography and Power Doppler between Uterine Parenchyma Lumps and Endometrial Cancer:A Retrospective Study

Sometimes endometrial polyps,submucosal myomas,and endometrial cancer show similar findings under *** aim of this study was to assess the antidiastole value of blood flow parameters using three-dimensional(3D)power Doppler ultrasonography angiography(PDA)between endometrial cancer and uterine parenchyma *** data of the blood flow indices in 3D-PDA including the vascularization index(VI),flow index(FI),and vascularization flow index(VFI)in 40 patients with endometrial cancer and 41 patients with uterine parenchyma lumps(endometrial polyps and submucosal myomas)were retrospectively analysed and compared utilizing Virtual Organ Computer-aided AnaLysis(VOCAL)*** results showed that all the blood flow parameters(VI,FI,VFI)were significantly higher in women with endometrial cancer than in those with uterine parenchyma lumps(P<0.001).The area under the curve of ROC of VI,FI,and VFI was 0.98,0.84,and 0.97,***,the best predictor of endometrial carcinoma was VI with a sensitivity of 97.0% and a specificity of 91.0%.The optimal cutoff value of VI was 4.06%.Our data demonstrated that all of the blood flow signal parameters(including VI,FI,and VFI)in 3D power Doppler ultrasonography had significant antidiastole values between endometrial cancer and uterine parenchyma lumps to assist clinicians in properly diagnosing patients.

Genome-Wide Screening of Aberrant Methylation Loci for Nonsyndromic Cleft Lip

Background： The pathogenicity of cleft lip （CL） is pretty complicated since it is influenced by the interaction of environment and genetic factors. The purpose of this study was to conduct a genome-wide screening of aberrant methylation loci in partial lesion tissues of patients with nonsyndromic CL （NSCL） and preliminarily validate candidate dysmethylated genes associated with ***： Fifteen healthy and sixteen NSCL fetal lip tissue samples were collected. The Infinium HumanMethylation450 BeadChip was used to screen aberrant methylation loci in three NSCL and three healthy lip tissues. The differential methylation sites and functions of the annotated genes between NSCL and healthy lip tissues were analyzed using minfi package of R software, cluster analysis, Gene Ontology （GO） annotation, and metabolic pathway annotation. Gene expression was assessed in nine differentially methylated genes by real-time polymerase chain reaction （PCR）. The transcriptions mRNA levels of three out of nine candidate genes were downregulated remarkably in NSCL lip tissues, and these three genes＇ abnormal methylation loci were validated by pyrosequencing in 16 NSCL cases and 15 healthy ***： In total, 4879 sites in the genes of NSCL odinopoeia fetuses showed aberrant methylation when compared with normal lip tissue genome. Among these, 3661 sites were hypermethylated and 1218 sites were hypomethylated as compared to methylation levels in healthy specimens. These aberrant methylation sites involved 2849 genes and were widely distributed among the chromosomes. Most differentially methylated sites were located in cytosine-phosphoric acid-guanine islands. Based on GO analysis, aberrantly methylated genes were involved in 11 cellular components, 13 molecular functions, and a variety of biological processes. Orthogonal metal is a new quantum metallic state that conducts electricity but acquires no Fermi surface(FS)or quasiparticles, and hence orthogonal to the established paradigm of Landau’s Fermi-liquid(FL). Such a state may hold the key of understanding the perplexing experimental observations of quantum metals that are beyond FL, i.e., dubbed non-Fermi-liquid(nFL), ranging from the Cu-and Fe-based oxides, heavy fermion compounds to the recently discovered twisted graphene heterostructures. However, to fully understand such an exotic state of matter, at least theoretically, one would like to construct a lattice model and to solve it with unbiased quantum many-body machinery. Here we achieve this goal by designing a 2D lattice model comprised of fermionic and bosonic matter fields coupled with dynamic Z2 gauge fields, and obtain its exact properties with sign-free quantum Monte Carlo simulations. We find that as the bosonic matter fields become disordered, with the help of deconfinement of the Z2 gauge fields, the system reacts with changing its nature from the conventional normal metal with an FS to an orthogonal metal of n FL without FS and quasiparticles and yet still responds to magnetic probe like an FL. Such a quantum phase transition from a normal metal to an orthogonal metal, with its electronic and magnetic spectral properties revealed, is calling for the establishment of new paradigm of quantum metals and their transition with conventional ones.

Effects of two novel nucleoside analogues on different hepatitis B virus promoters

AIM： To explore the effects of the nucleoside analogues β-L-D4A and β-LPA on hepatitis B virus （HBV） promoters. METHODS： Four HBV promoters were amplified by polymerase chain reaction （PCR） and subcloned into the expression vector pEGFP-1. The four recombinants controlled by HBV promoters were confirmed by restriction analysis and sequencing. Human hepatoma HepG2 cells transfected with the recombinant plasmids were treated with various concentrations of β-L-D4A and β-LPA. Then, enhanced green fluorescent protein （EGFP）-positive cells were detected by fluorescence microscopy and using a fluorescence activated cell sorter RESULTS： Four HBV promoters were separately obtained and successfully cloned into pEGFP-1, Expression of EGFP under the control of the surface promoter （Sp） and the X promoter （Xp） was inhibited by β-L-D4A in a dosedependent manner, while expression of EGFP under the control of the core promoter （Cp） and Xp was inhibited by β-LPA in a dose-dependent manner. CONCLUSION： The two novel nucleoside analogues investigated here can inhibit the activities of HBV promoters in a dose-dependent manner. These findings may explain the mechanisms of action by which these two novel compounds inhibit HBV DNA replication.
