Trends in mortality of emergency departments patients in China

BACKGROUND: Emergency medical service system (EMSS) in China is becoming more important. However, studies on mortality of emergency departments (EDs) patients in tertiary hospitals and on the trends in mortality of ED patients all over China are stagnant. The objective of this study was to quantify and describe the trends in mortality of ED patients in China. METHODS: Nine tertiary teaching hospitals were selected from tertiary teaching hospitals in different regions. The annual numbers of ED visits and deaths of these hospitals in 2004, 2009 and 2014 were recorded and analyzed. Chi-square test was used to compare the mortality of the EDs’ visits. Moreover, data on the mortality of ED patients in China from 2005 to 2015 were summarized and analyzed from the China Health and Family Planning Statistical Yearbooks (2006–2016). RESULTS: From 2004 to 2014, the overall annual mortalities in EDs increased among the tertiary hospitals (P<0.001). However, the overall annual mortality in EDs all over China decreased from 0.12% in 2005 to 0.08% in 2015. And the mortalities of EDs patients in the eastern, central and western regions of China all decreased. In addition, the average mortality of EDs patients in northern China was obviously higher than that in southern China (P<0.05). CONCLUSION: The ED mortality was increased in tertiary hospitals while decreased all over China during the past decade, which may be partly caused by some critical challenges faced by China’s EMSS, such as overcrowding and long length of stay in EDs of tertiary hospitals.

Primary gastric teratoma on the cardiac orifice in an adult

Gastric teratoma (GT) is a seldom seen congenital abnormality. GT always occurs in children. The greater curvature and posterior wall of the stomach are the most common sites involving GT. We diagnosed a case of GT located on the inferior wall of the cardiac orifice in a 20-year-old man. To the best of our knowledge, this is the first case of GT located on the wall of the cardiac orifice in an adult in the English literature. We report this unusual case as an addition to this rare disease usually found in children. Computed tomography combined with endoscopic ultrasonography can be selected to diagnose GT.

Exploring the performance of large language models on hepatitis B infection-related questions:A comparative study

BACKGROUND Patients with hepatitis B virus(HBV)infection require chronic and personalized care to improve *** language models(LLMs)can potentially provide medical information for *** To examine the performance of three LLMs,ChatGPT-3.5,ChatGPT-4.0,and Google Gemini,in answering HBV-related *** LLMs’responses to HBV-related questions were independently graded by two medical professionals using a four-point accuracy scale,and disagreements were resolved by a third *** question was run three times using three *** was assessed via the Gunning Fog index and Flesch-Kincaid grade *** Overall,all three LLM chatbots achieved high average accuracy scores for subjective questions(ChatGPT-3.5:3.50;ChatGPT-4.0:3.69;Google Gemini:3.53,out of a maximum score of 4).With respect to objective questions,ChatGPT-4.0 achieved an 80.8%accuracy rate,compared with 62.9%for ChatGPT-3.5 and 73.1%for Google *** the six domains,ChatGPT-4.0 performed better in terms of diagnosis,whereas Google Gemini demonstrated excellent clinical ***,in the readability analysis,the mean Gunning Fog index and Flesch-Kincaid grade level scores of the three LLM chatbots were significantly higher than the standard level eight,far exceeding the reading level of the normal *** Our results highlight the potential of LLMs,especially ChatGPT-4.0,for delivering responses to HBV-related *** may be an adjunctive informational tool for patients and physicians to improve ***,current LLMs should not replace personalized treatment recommendations from physicians in the management of HBV infection.

Laparoscopic liver resection utilizing the ventral avascular area of the inferior vena cava:A retrospective cohort study

Effects of Shenqi Xiangyi granules in advanced gastric cancer chemotherapy

BACKGROUND Owing to the absence of specific symptoms in early-stage gastric cancer,most patients are diagnosed at intermediate or advanced *** a result,treatment often shifts from surgery to other therapies,with chemotherapy and targeted therapies being the primary options for advanced gastric cancer treatment.A total of 116 patients with advanced gastric cancer,admitted from January 2021 to December 2023,were selected and divided into two groups of 58 each using the random number table *** control group received FOLFOX4 chemothe-rapy(oxaliplatin+calcium+folinate+5-fluorouracil)combined with intravenous *** observation group received the same treatment as the control group,supplemented by oral administration of Senqi Shiyiwei *** groups underwent treatment cycles of 3 weeks,with a minimum of two *** therapeutic efficacy,immune mechanisms,and treatment-related toxicity and side effects were compared between the *** objective remission rate in the observation group(55.17%)was higher than that of the control group(36.21%)(P0.05).CONCLUSION Adjuvant therapy with Shenqixian granules may enhance the efficacy of simudizumab combined with FOLFOX4 chemotherapy in advanced gastric cancer and the immune function by increasing immune cell counts,making it a valuable option in clinical treatment.

背景:大强度运动可诱导骨骼肌微管蛋白的解聚或降解,通过其与线粒体的密切联系,从而影响线粒体的运动轨道以及分子马达,从而改变线粒体的移动和分布。目的:观察一次大强度运动对骨骼肌α-tubulin蛋白、MAP4蛋白和Miro1蛋白的影响,以及对线粒体超微结构的影响,分析它们之间的时序性变化,探讨微管蛋白的解聚是否可以通过与Miro1蛋白的作用,从而对骨骼肌线粒体的移动和分布产生调控作用。方法:56只SD大鼠经适应训练后分为安静对照组(8只)和运动组(48只),运动模型为一次大强度跑台运动,-16°下坡跑,20 m/min,90 min。将运动组大鼠分别在运动后即刻,6h,12h,24h,48h和72h(每个时间点8只)取比目鱼肌。Western blotting检测α-tubulin、MAP4和Miro1的蛋白表达,透射电镜观察线粒体的超微结构。结果与结论:(1)α-tubulin蛋白表达在运动后6h和12h显著降低;(2)MAP4蛋白表达在运动后6,12,48和72h均显著升高;(3)Miro1蛋白表达在运动后6h和12h有升高趋势,在72h下降;(4)电镜下线粒体在安静状态时成对排列于Z线两侧,肌膜下较少;运动后即刻和6h在肌膜下开始积聚;12h后在肌膜下积聚和损伤最为严重;24h和48h后肌膜下肌膜减弱;72h后已恢复至安静状态。(5)结果表明,一次大强度运动可能诱导骨骼肌微管的解聚,并可能通过对Miro1的作用,从而调节线粒体的移动和分布。
