BACKGROUND Moyamoya syndrome(MMS)is a group of diseases that involves more than one underlying disease and is accompanied by moyamoya vascular phenomena.Psoriasis is a chronic immune skin disease closely linked to high blood pressure and heart disease.However,psoriasis-related MMS has not been reported.CASE SUMMARY We collected data on patients with stroke due to MMS between January 2017 and December 2019 and identified four cases of psoriasis.Case histories,imaging,and hematological data were collected.The average age of the initial stroke onset was 58.25±11.52 years;three cases of hemorrhagic and one case of ischemic stroke were included.The average duration from psoriasis confirmation to the initial MMS-mediated stroke onset was 17±3.56 years.All MMS-related stenoses involved the bilateral cerebral arteries:Suzuki grade III in one case,grade IV in two cases,and grade V in one case.Abnormally elevated plasma interleukin-6 levels were observed in four patients.Two patients had abnormally elevated immunoglobulin E levels,and two had thrombocytosis.All four patients received medication instead of surgery.With an average follow-up time of 2 years,two causing transient ischemic attacks occurred in two patients,and no hemorrhagic events occurred.CONCLUSION Psoriasis may be a potential risk factor for MMS.Patients with psoriasis should be screened for MMS when they present with neurological symptoms.

Background： G-protein β-polypeptide 3 （GNB3） is a β subunit isoform of G-protein that plays important role in signal transduction of membrane G-protein coupled receptors （GPCRs）. The GNB3 splice variant C825T （rs5443） is associated with risk for essential hypertension （EH） and efficacy of therapeutic drugs targeting GPCRs. It is unknown whether the polymorphism is associated with blood pressure （BP） response to telmisartan or amlodipine, two widely prescribed antihypertensive drugs. Methods： A total of 93 subjects initially diagnosed as EH were recruited and underwent a 4-week treatment with telmisartan （42 patients） or amlodipine （51 patients） monotherapy. Both baseline and after-treatment BP were measured. GNB3 C825T polymorphism was genotyped by polymerase chain reaction-restriction fragment length polymorphism. Results： Baseline systolic BP （SBP） and diastolic BP （DBP） were comparable among C825T genotypes in both telmisartan and amlodipine treatment groups. Patients with the CT or TT genotypes showed significantly lower body mass index （BMI） as compared with CC homozygotes in both groups （P 〈 0.05, respectively）. GNB3 825TT homozygotes showed significantly higher alier-treatment DBP and mean arterial presstire （MAP） than those carrying at least one 825C allele （P 〈 0.01） in the telmisartan treatment group. No difference in after-treatment SBE DBP, and MAP levels among C825T genotypes was observed in the amlodipine treatment group. No significant difference in absolute changes in BP levels was observed among the genotypes in either treatment group. Conchlsion： The GNB3 C825T splice variant is associated with the DBP-lowering effect of telmisartan but not amlodipine in Chinese EH patients.

This study was conducted to compare the feasibility,safety and effectiveness of the combined-laparoscopic splenectomy and esophagogastric devascularization(C-LSED)with open splenectomy and esophagogastric devascularization surgery(OSED)in patients with portal hypertension due to liver cirrhosis.From February 2014 to June 2018,68 patients with portal hypertension were diagnosed as having serious gastroesophageal varices and/or hypersplenism in our center.Thirty patients underwent C-LSED and 38 patients received OSED.Results and outcomes were compared retrospectively.No patients of C-LSED group required an intraoperative conversion to open surgery.Significantly shorter operating time,less blood loss,lower transfusion rates,shorter postoperative hospital stay,lower rates of complications were found in C-LSED group than in C-LSED group(P<0.05).No death and rebleeding were documented in both groups during the follow-up periods of one year Postoperative endoscopy revealed that varices in the patients of both groups were alleviated significantly from severe to mild,and in a part of cases,the varices disappeared.The final results suggest that the C-LSED technique is superior to open procedure,due to slightly invasive,simplified operative procedure,significantly shorter operating time,less intraoperative bleeding and lower post-operative complication rates.And C-LSED offers comparable long-term effects to open surgery.

BACKGROUND Multiple myeloma(MM)can be accompanied by amyloidosis,which occurs in a small number of patients and is characterized by deposition of light chains in the joints,leading to multiple myeloma-associated amyloid arthropathy(MAA).As a rare complication of MM,clinical manifestations of MAA are often similar to those of rheumatoid arthritis,and the two are easily confused.CASE SUMMARY In recent years,our center treated two patients of MM with amyloid arthropathy as the first manifestation,both of whom presented with polyarthritis.After treatment for MM,both patients achieved complete remission.However,subsequently,the two patients underwent hip arthroplasty for femoral neck fractures.Congo red staining and immunofluorescence of the joint tissues confirmed MAA after surgery.Eventually,one of the patients died of MM recurrence,while the other survived.CONCLUSION MAA should be regarded as an initial symptom of MM and should be taken seriously.

A New Grasping Mode Based on a Sucked-type Underactuated Hand

Robot hands have been developing during the last few decades. There are many mechanical structures and analyti?cal methods for di erent hands. But many tough problems still limit robot hands to apply in homelike environment. The ability of grasping objects covering a large range of sizes and various shapes is fundamental for a home service robot to serve people better. In this paper, a new grasping mode based on a novel sucked?type underactuated(STU) hand is proposed. By combining the flexibility of soft material and the e ect of suction cups, the STU hand can grasp objects with a wide range of sizes, shapes and materials. Moreover, the new grasping mode is suitable for some situations where the force closure is failure. In this paper, we deduce the e ective range of sizes of objects which our hand using the new grasping mode can grasp. Thanks to the new grasping mode, the ratio of grasping size between the biggest object and the smallest is beyond 40, which makes it possible for our robot hand to grasp diverse objects in our daily life. For example, the STU hand can grasp a soccer(220 mm diameter, 420 g) and a fountain pen(9 mm diameter, 9 g). What’s more, we use the rigid body equilibrium conditions to analysis the force condition. Experiment evaluates the high load capacity, stability of the new grasping mode and displays the versatility of the STU hand. The STU hand has a wide range of applications especially in unstructured environment.

The inhibitory effect of different reperfusion periods 45 min following hepatic ischemia on the expression of cholecystokinin(CCK)and vasoactive intestinal peptide(VIP)in the jejunum and the effect of salvia miltiorrhiza pretreatment were investigated,and the possible mechanism and implications were explored.Eighty rats were randomly divided into four groups:normal control group(CO group),sham-operated group(SO group),ischemia/reperfusion(I/R)injury group(IR group)and salvia miltiorrhiza pretreatment group(SM group).The rat model of I/R was established by using a non-invasive artery clamp to clip(45 min)or relax the hepatic pedicle.In the SM group,saline(40 mL/kg)and salvia miltiorrhiza injection(6 g/kg)were injected via the tail vein 30 min before clipping the hepatic pedicle.In the SO group only the porta hepatis was dissected after laparotomy without clamping the hepatic pedicle.At 0,3,12,24 and 72 h post-reperfusion,respectively,upper jejunum samples were taken for immunohistochemistry of CCK and VIP.It was found that 0 h after I/R,the expression of CCK and VIP in the upper jejunum was upregulated.With prolongation of the reperfusion period,the expression of CCK and VIP was also increased,reached the peak at the 24th h,and gradually returned to the normal level at the 72nd h after reperfusion.The levels of both CCK and VIP in the SM group were lower than those in the IR group.It is suggested that the digestive tract congestion injury caused by liver ischemia can upregulate the expression of CCK and VIP in the jejunum following reperfusion.Salviae pretreatment can partly reduce the increased expression of CCK and VIP in the jejunum in the same period,which might contribute to the early recovery of gastrointestinal motility.

Clinical Value of Trans-parenchymal Compressing Suture to Decrease the Cutting Surface Related Complications after Non-anatomical Liver Resection

Non-anatomical liver resection with appropriate resection margin is regarded as a potential curative treatment for selected major hepatic carcinoma due to preserving maximal normal liver,especially in cirrhotic patients.But occurrence of cutting surface related complications becomes a main challenge.From June 2010 to June 2016,448 patients with major hepatic carcinoma received non-anatomical liver resection in our liver surgery center.After excluding 66 cases that were incongruent with the purpose of study,235 patients undergoing transparenchymal compressing suture(TCS)to“not good”cutting surface were allocated as study group;147 patients with exposed surface(ES)were matched as control group.The characteristics of postoperative drainage,postoperative hepatic and renal functions,hospital days,and outcomes were collected retrospectively.We further compared cutting surface related complications under different levels of liver cirrhosis between the two groups.Compared with ES group,patients in TCS group had a decreased incidence of cutting surface related complications(14.3%vs.6.8%,P=0.011)and a decreased probability of interventions for cutting surface related complications(8.2%vs.3.4%,P=0.042).TCS application was much more effective to prevent cutting surface related complications in patients with moderate and severe cirrhosis(5.4%vs.15.8%,P=0.003).Postoperative hepatic and renal function,hospita 1 days and mortality did not differ between the two groups.In conclusion,TCS decreases the probability of cutting surface related complications and postoperative interventions for related complications,especially in patients with moderate and severe cirrhosis.

Human genetic prion diseases(gPrDs)are directly associated with mutations and insertions in the PRNP(Prion Protein)gene.We collected and analyzed the data of 218 Chinese gPrD patients identified between Jan 2006 and June 2020.Nineteen different subtypes were identified and gPrDs accounted for 10.9%of all diagnosed PrDs within the same period.Some subtypes of gPrDs showed a degree of geographic association.The age at onset of Chinese gPrDs peaked in the 50–59 year group.Gerstmann–Sträussler–Scheinker syndrome(GSS)and fatal familial insomnia(FFI)cases usually displayed clinical symptoms earlier than genetic Creutzfeldt–Jakob disease(gCJD)patients with point mutations.A family history was more frequently recalled in P105L GSS and D178N FFI patients than T188K and E200K patients.None of the E196A gCJD patients reported a family history.The gCJD cases with point mutations always developed clinical manifestations typical of sporadic CJD(sCJD).EEG examination was not sensitive for gPrDs.sCJD-associated abnormalities on MRI were found in high proportions of GSS and gCJD patients.CSF 14-3-3 positivity was frequently detected in gCJD patients.Increased CSF tau was found in more than half of FFI and T188K gCJD cases,and an even higher proportion of E196A and E200K gCJD patients.63.6%of P105L GSS cases showed a positive reaction in cerebrospinal fluid RT-QuIC.GSS and FFI cases had longer durations than most subtypes of gCJD.This is one of the largest studies of gPrDs in East Asians,and the illness profile of Chinese gPrDs is clearly distinct.Extremely high proportions of T188K and E196A occur among Chinese gPrDs;these mutations are rarely reported in Caucasians and Japanese.

报道1例冠心病合并肥厚型心肌病患者行冠状动脉造影及左心室造影术后40 min突发全面性遗忘,无视力丧失、头痛、精神错乱、癫痫发作、偏瘫、失语、昏迷等其他临床表现。神经系统查体未发现明显阳性体征。急查头颅CT及头颅磁共振成像联合磁共振血管成像均未见缺血性或出血性脑卒中特征性影像。考虑碘对比剂脑病,予地塞米松、依达拉奉、甘露醇及水化治疗,约18 h后患者记忆完全恢复。碘对比剂脑病是一种罕见的并发症,表现为短暂性全面性遗忘的病例更为罕见,鲜有报道。临床医师使用碘对比剂时应警惕诱发脑病的可能性,以防误诊为急性脑卒中导致不必要地额外使用对比剂行进一步检查及侵入性或非侵入性干预。
