Diabetes mellitus in patients with type 1 autoimmune pancreatitis at diagnosis and after corticosteroid therapy

Background:A high prevalence of diabetes mellitus(DM)coexisting with autoimmune pancreatitis(AIP)is ***,evidence on the circumstances under which corticosteroid therapy(CST)for AIP improves or worsens DM is *** study aimed to demonstrate and identify predictors of DM control under the influence of ***:Patients diagnosed with type 1 AIP were enrolled from a prospectively maintained cohort and were classified into three groups according to the chronology in which AIP and DM were diagnosed:pre-existing DM(pDM),concurrent DM(cDM),and non-DM(nDM).The responses of DM to CST were assessed when corticosteroid was ceased or tapered to a maintenance dose and classified as‘improvement’and‘non-improvement’(including‘no change’and‘exacerbation’).Results:Among 101 patients with type 1 AIP,52(51.5%)patients were complicated with DM at the time of AIP diagnosis,with 36 patients in the cDM group and 16 patients in the pDM *** incidences of diffuse pancreatic swelling(72.2%)and pancreatic body/tail involvement(91.7%)were significantly higher in the cDM group than in both the pDM and nDM *** the 52 patients with DM,CST was administered in 48 *** logistic analysis identified that elevated serum gamma-glutamyl transferase(GGT)level at AIP diagnosis[odds ratio(OR)=0.032,95%confidence interval(CI):0.003-0.412,P=0.008]and pancreatic atrophy after CST(OR=0.027,95%CI:0.003-0.295,P=0.003)were negatively associated with DM control ***:Patients with diffuse pancreatic swelling and pancreatic body/tail involvement in pancreatitis tended to be complicated with cDM at AIP *** exerted a beneficial effect on the clinical course of DM in nearly half of the AIP patients complicated with DM at diagnosis,particularly in those without elevated serum GGT levels at diagnosis and who did not experience pancreatic atrophy after CST.

BACKGROUND Autoimmune enteropathy(AIE)is a rare disease whose diagnosis and long-term prognosis remain challenging,especially for adult AIE *** To improve overall understanding of this disease’s diagnosis and *** We retrospectively analyzed the clinical,endoscopic and histopathological characteristics and prognoses of 16 adult AIE patients in our tertiary medical center between 2011 and 2023,whose diagnosis was based on the 2007 diagnostic *** Diarrhea in AIE patients was characterized by secretory *** common endoscopic manifestations were edema,villous blunting and mucosal hyperemia in the duodenum and *** blunting(100%),deep crypt lymphocytic infiltration(67%),apoptotic bodies(50%),and mild intraepithelial lymphocytosis(69%)were observed in the duodenal ***,there were other remarkable abnormalities,including reduced or absent goblet cells(duodenum 94%,ileum 62%),reduced or absent Paneth cells(duodenum 94%,ileum 69%)and neutrophil infiltration(duodenum 100%,ileum 69%).Our patients also fulfilled the 2018 diagnostic criteria but did not match the 2022 diagnostic criteria due to undetectable anti-enterocyte *** patients received glucocorticoid therapy as the initial medication,of which 14/16 patients achieved a clinical response in 5(IQR:3-20)*** were administered to 9 patients with indications of steroid dependence(6/9),steroid refractory status(2/9),or intensified maintenance medication(1/9).During the median of 20.5 months of followup,2 patients died from multiple organ failure,and 1 was diagnosed with non-Hodgkin’s *** cumulative relapse-free survival rates were 62.5%,55.6%and 37.0%at 6 months,12 months and 48 months,*** Certain histopathological findings,including a decrease or disappearance of goblet and Paneth cells in intestinal biopsies,might be potential diagnostic criteria for adult *** long-term prognosis is still unsatisfac

Barrier-focused intervention to increase colonoscopy attendance among nonadherent high-risk populations

AIM： To increase attendance tor colonoscopy among nonadherent high-risk individuals for colorectal cancer （CRC） screening in China. METHODS： During the first 12 mo without intervention, only 428 of the 2398 high-risk subjects attended a scheduled colonoscopy examination. The 1970 subjects who did not attend for CRC screening were enrolled in the present study. Prior barrier investigation was performed to ascertain the reasons for nonadherence. A barrier-focused intervention program was then established and implemented among eligible nonadherent subjects by telephone interviews and on-site consultations. The completion rates of colonoscopy during the first 12 mo without intervention and the second 12 mo with intervention were compared. Variations in the effect of the intervention on some high-risk factors and barrier characteristics were analyzed using logistic regression. RESULTS： 540 subjects who were not eligible were excluded from the study. The colonoscopy attendance rate was 23.04% （428/1858） during the first 12 mo without intervention, and 37.69% （539/1430） during the second 12 mo with intervention （P 〈 0.001）. Logistic regression analysis showed that the intervention was more effective among subjects with only objective barriers （OR： 34.590, 95% CI： 23.204-51.563） or subjects with some specific highrisk characteristics： first-degree relatives diagnosed with CRC （OR： 1.778, 95% CI： 1.010-3.131）, personal history of intestinal polyps （OR： 3.815, 95% CI： 1.994-7.300） and positive result for immunochemical fecal occult blood testing （OR： 2.718, 95% CI： 1.479-4.996）. CONCLUSION： The barrier-focused telephone or on- site consultation intervention appears to be a feasible means to improve colonoscopy attendance among nonadherent high-risk subjects for CRC screening in China.

Effect of stress-relief annealing on thermal fatigue cracks for die-casting die steels

The effect of stress-relief annealing at different stages of thermal fatigue tests on crack growth was investigated using a self-built thermal cycle *** results showed that annealing could limit crack expansion *** reduced dislocation density and released the accumulation of residual *** addition,the strain accumulation in carbides was reduced by this *** was also found that double annealing was even more effective at inhibiting crack expansion compared to single stress-relief *** 1000 cycles,the maximum crack length was reduced by 31.1%and 45.2%for the samples after using the optimal single and double annealing processes,*** single stress-relief annealing,earlier annealing provides more benefit in delaying crack ***,effective double stress-relief annealing requires a suitable time interval between the annealing ***,after 800 cycles,surface hardness decreased significantly accompanied by an increase in the size and number of carbides,and cracks expanded predominantly along grain boundaries.

BACKGROUND Preoperative knowledge of mutational status of gastrointestinal stromal tumors(GISTs)is essential to guide the individualized precision *** To develop a combined model that integrates clinical and contrast-enhanced computed tomography(CE-CT)features to predict gastric GISTs with specific genetic mutations,namely KIT exon 11 mutations or KIT exon 11 codons 557-558 *** A total of 231 GIST patients with definitive genetic phenotypes were divided into a training dataset and a validation dataset in a 7:3 *** models were constructed using selected clinical features,conventional CT features,and radiomics features extracted from abdominal CE-CT *** models were developed:ModelCT sign,modelCT sign+rad,and model CTsign+rad+*** diagnostic performance of these models was evaluated using receiver operating characteristic(ROC)curve analysis and the Delong *** The ROC analyses revealed that in the training cohort,the area under the curve(AUC)values for model_(CT sign),model_(CT sign+rad),and modelCT_(sign+rad+clinic)for predicting KIT exon 11 mutation were 0.743,0.818,and 0.915,*** the validation cohort,the AUC values for the same models were 0.670,0.781,and 0.811,*** predicting KIT exon 11 codons 557-558 deletions,the AUC values in the training cohort were 0.667,0.842,and 0.720 for model_(CT sign),model_(CT sign+rad),and modelCT_(sign+rad+clinic),*** the validation cohort,the AUC values for the same models were 0.610,0.782,and 0.795,*** on the decision curve analysis,it was determined that the model_(CT sign+rad+clinic)had clinical significance and *** Our findings demonstrate that the combined modelCT_(sign+rad+clinic)effectively distinguishes GISTs with KIT exon 11 mutation and KIT exon 11 codons 557-558 *** combined model has the potential to be valuable in assessing the genotype of GISTs.

Thermal and hydrological processes in permafrost slope wetlands affect thermosyphon embankment stability

To ensure the long-term service performance of infrastructure such as railways,highways,airports and oil pipelines built on permafrost slope wetland sites,it is imperative to systematically uncover the long-term heat-water changes of soil in slope wetlands environment under climate *** specifically,considering valuable field data from 2001 to 2019,the long-term heat and water changes in active layers of the slope wetland site along the Qinghai-Xizang Railway(QXR)are illustrated,the effect of thermosyphon measures in protecting the permafrost environment is evaluated,and the influences of climate warming and hydrological effects on the stability of slope wetland embankments are systematically *** permafrost at the slope wetland site is rapidly degrading,demonstrating a reduction in active layer thickness of>3.7 cm per year and a permafrost temperature warming of>0.006℃ per *** thermosiphon embankment developed by QXR has a specific cooling period;thus,to mitigate the long-term impacts of climate warming on the thermal stability of permafrost foundation,it is essential to implement strengthening measures for the thermosiphon embankment,such as adding a crushed-rock layer or sunshade board on the slope of thermosiphon embankment to creating a composite cooling ***-term seasonal groundwater seepage intensifies frost damage to the slope wetland embankment,while long-term seasonal supra-permafrost water and groundwater seepage exacerbates uneven transverse deformation of slope wetland ***-term climate warming and slope effects have altered the surface water and groundwater hydrological processes of slope wetlands,potentially leading to an increased occurrence of slope embankment *** results are crucial for improving our understanding of heat and water variation processes in the active layer of slope wetland sites located in permafrost regions and ensuring long-term service safety for the QXR.

A rotating packed bed is a typical chemical process enhancement equipment that can strengthen micromixing and mass *** the operation of the rotating packed bed,the nonreactants and products irregularly adhere to the wire mesh packing in the rotor,thus resulting in an imbalance in the vibration of the rotor,which may cause serious damage to the bearing and material *** study proposes a model prediction for estimating the bearing residual life of a rotating packed bed based on rotor imbalance response *** method is used to determine the influence of the mass on the imbalance in the vibration of the rotor on bearing *** major influence on rotor vibration was found to be exerted by the imbalanced mass and its distribution radius,as revealed by the results of orthogonal *** implementing finite element analysis,the imbalance response curve for the rotating packed bed rotor was obtained,and a correlation among rotor imbalance mass,distribution radius of imbalance mass,and bearing residue life was established via data *** predicted value of the bearing life can be used as the reference basis for an early safety warning of a rotating packed bed to effectively avoid accidents.

Experimental and numerical investigations on micromixing performance of multi-orifice cross-flow jet mixers

Multi-orifice cross-flow jet mixers(MOCJMs)are used in various industrial applications due to their excellent mixing efficiency,but few studies have focused on the micromixing performance of ***,the flow characteristics and micromixing performance inside the MOCJM were investigated using experiments and computational fluid dynamics(CFD)simulations based on the Villermaux/Dushman system and the finite-rate/modified eddy-dissipation *** optimal A value was correlated with the characteristic parameters of MOCJMs to develop a CFD calculation method applicable to the study of the micromixing performance of the *** the micromixing efficiency was evaluated using the segregation index XS,and the effects of operational and geometric parameters such as mixing flow Reynolds number(ReM),flow ratio(RF),total jet area(ST),the number of jet orifices(n),and outlet configuration on the micromixing efficiency were *** was found that the intensive turbulent region generated by interactions between jets,as well as between jets and crossflows,facilitated rapid *** decreased with increasing ReM and decreasing ***,MOCJMs with lower ST,four jet orifices,and the narrower outlet configuration demonstrated a better micromixing *** study contributes to a deeper understanding of the micromixing performance of MOCJMs and provides valuable guidance for their design,optimization,and industrial application.

With the continuing boost in the demand for energy storage,there is an increasing requirement for batteries to be capable of operation in extreme environmental ***-ion batteries(SIBs) have emerged as a highly promising energy storage solution due to their promising performance over a wide range of temperatures and the abundance of sodium resources in the earth's *** to lithiumion batteries(LIBs),although sodium ions possess a larger ionic radius,they are more easily desolvated than lithium *** rthermore,SIBs have a smaller Stokes radius than lithium ions,resulting in improved sodium-ion mobility in the ***,SIBs demonstrate a significant decrease in performance at low temperatures(LT),which constrains their operation in harsh weather *** the increasing interest in SIBs,there is a notable scarcity of research focusing specifically on their mechanism under LT *** review explores recent research that considers the thermal tolerance of SIBs from an inner chemistry process perspective,spanning a wide temperature spectrum(-70 to100℃),particularly at LT *** addition,the enhancement of electrochemical performance in LT SIBs is based on improvements in reaction kinetics and cycling stability achieved through the utilization of effective electrode materials and electrolyte ***,the safety concerns associated with SIBs are addressed and effective strategies are proposed for mitigating these ***,prospects conducted to extend the environmental frontiers of commercial SIBs are discussed mainly from three viewpoints including innovations in materials,development and research of relevant theoretical mechanisms,and intelligent safety management system establishment for larger-scale energy storage SIBs.
