Diagnosis and treatment of biliary mucinous cystic neoplasms: A single-center experience

Background: Biliary mucinous cystic neoplasms(BMCNs) are rare hepatobiliary cystic tumors, which can be divided into noninvasive and invasive types. This study aimed to investigate the diagnosis, treatment, and prognosis of BMCNs in a large single center. Methods: We analyzed 49 patients with BMCNs confirmed by postoperative pathology at the First Afflliated Hospital, Zhejiang University School of Medicine between January 2007 and December 2021. Results: Among the 49 patients, 37 were female(75.5%), and the average age was 57.04 years. Common symptoms included abdominal discomfort, jaundice and fever, while 22 patients(44.9%) had no symptoms. Serum carbohydrate antigen(CA) 19-9 and CA125 concentrations were elevated in 34.8% and 19.6% of patients, respectively. Forty-eight patients had tumors in the intrahepatic bile ducts and only one had a tumor in the extrahepatic bile duct. Forty-eight patients with noninvasive intrahepatic BMCNs were further analyzed in terms of pathological features: 34(70.8%) had low-grade intraepithelial neoplasms(LGINs), and 14(29.2%) had high-grade intraepithelial neoplasms(HGINs). The potential immunohistochemical markers of BMCNs were cytokeratin(CK) 19, CK7, estrogen receptor and progesterone receptor. Follow-up data for 37 patients with intrahepatic BMCNs were obtained. The median overall survival(OS) of BMCNs was not reached. The longest survival time was 137 *** 5-and 10-year OS rates were 100% and 85.4%, respectively. The 5-and 10-year recurrence-free survival(RFS) rates were 93.9% and 80.2%, respectively. Conclusions: BMCNs are rare cystic neoplasms that commonly occur in middle-aged females. BMCNs can only be diagnosed and classified by postoperative pathology, as there are no specific clinical presentations, serological indicators or imaging modalities for preoperative diagnosis. Complete surgical resection is necessary for BMCNs, and the postoperative prognosis is favorable.

Shear-thinning Catechol-modified Chitosan Hydrogel Loaded with Silver Nanoparticles for Endoscopic Submucosal Dissection

Shear-thinning hydrogels have emerged for endoscopic submucosal dissection,while wound intervention after surgery has rarely been ***,a catechol-modified chitosa n hydrogel with shear-thinning property was developed for simultaneously facilitating endoscopic submucosal dissection and postoperative wound *** from the shear-thinning and self-healing characteristics,the asprepared hydrogel showed easily endoscopic *** also performed very well as submucosal cushion,which could remain above 70%after injection for 120 min in ex vivo porcine large intestine *** fact,the cushion height of normal saline dramatically decreased to 46%of the initial height at 30 *** nanoparticles encapsulated into the network endowed the hydrogel with almost reached 100%antibacterial effect against *** and *** hemolysis ratio of the hydrogel was calculated to be as low as 0.8%.Combined with good hemocompatibility and cytocompatibility,the as-prepared hydrogel displayed much higher in vivo wound closu re and healing efficacy than normal *** results demonstrated the superiority of the shear-thinning chitosan hydrogel in facilitating clinical endoscopic submucosal dissection surgery.

Thermal stable intermetallic particles are important for the heat resistance of magnesium(Mg)*** this work,many lath-like particles formed in α-Mg grains of a Mg-8Gd-3Sm-0.7Al casting alloy when heat-treated at 873 ***-resolution high-angle annular dark field scanning transmission electron microscopy(HAADF-STEM) characterizations indicate that most of them are Mg-containing Al_(2)(Gd,Sm),with the atomic ratio of Mg:Al:(Gd,Sm) being ~1:1:1;a small part of them with relatively wider thickness are long-period stacking ordered(LPSO) phases simultaneously containing both 14H and 18R *** followed common orientation relationships with Mg matrix as those reported in previous *** addition,many Mg laths were observed in the primary blocky Al_(2)(Gd,Sm) phase at grain boundaries,where the atomic ratio of Al:(Gd,Sm) in the Al_(2)(Gd,Sm) matrix was 2:***,density functional theory(DFT) calculations illustrated the detail structure of the re-constructed Mg/Al_(2)RE interface and simultaneously deduced the underlying reason for the re-dissolution of the newly formed Mg-containing Al_(2)(Gd,Sm) plates in α-Mg matrix.

Research on the quasi-isentropic driving model of aluminized explosives in the detonation wave propagation direction

Taking CL-20(Hexanitrohexaazaisowurtzitane)-based aluminized explosives with high gurney energy as the research object, this research experimentally investigates the work capability of different aluminized explosive formulations when driving metal flyer plates in the denotation wave propagation *** research results showed that the formulations with 43 μm aluminum(Al) powder particles(The particle sizes of Al powder were in the range of 2~43 μm) exhibited the optimal performance in driving flyer plates along the denotation wave propagation direction. Compared to the formulations with Al powder 13 μm, the formulations with Al powder 2 μm delivered better performance in accelerating metal flyer plates in the early stage, which, however, turned to be poor in the later stage. The CL-20-based explosives containing 25% Al far under-performed those containing 15% Al. Based on the proposed quasi-isentropic hypothesis, relevant isentropy theories, and the functional relationship between detonation parameters and entropy as well as Al reaction degree, the characteristic lines of aluminized explosives in accelerating flyer plates were theoretically studied, a quasi-isentropic theoretical model for the aluminized explosive driving the flyer plate was built and the calculation methods for the variations of flyer plate velocity, Al reaction degree, and detonation product parameters with time and axial positions were developed. The theoretical model built is verified by the experimental results of the CL-20-based aluminized explosive driving flyer plate. It was found that the model built could accurately calculate the variations of flyer plate velocity and Al reaction degree over time. In addition, how physical parameters including detonation product pressure and temperature varied with time and axial positions was identified. Silicon carbide fiber reinforced silicon carbide matrix(SiC_(f)/SiC)composite is the key cladding material of nuclear fuel,which determines the safety and reliability of nuclear fuel storage and *** replacement of its storage and transportation scenario needs to be completed by the manipulator,but the application of SiC_(f)/SiC wear,fracture,and nuclear leakage in the snatching process of brittle-flexible-rigid contact in the irradiation environment has been seriously restricted due to unclear understanding of the damage ***,the effects of irradiation dose and clamping load on the friction characteristics of the contact interface between SiC_(f)/SiC clad tube are studied in this paper,and the effects of irradiation parameters and clamping force on the static friction coefficient of the contact interface between the clad tube and flexible nitrile are *** on the Greenwood-Williamson tribological model,a numerical model of the shape and structure of the contact micro-convex at the micro-scale of the clamping interface is constructed by introducing the multi-surface integral,and finally verified by *** research results show that there is a unique“Irradiation suppression zone”under the clamping condition of SiC_(f)/SiC cladding tube under the nuclear irradiation environment,and the growth of static friction coefficient slows down until stagnates after irradiation reaches a certain extent(600 kGy),and there will be a decline when the irradiation dose continues to increase,among which the clamping force of 15.2 N within the irradiation interval of 1,000 kGy can meet the safety of nuclear environment *** results of this paper can provide an important theoretical basis and application guidance for the safe operation of SiC_(f)/SiC cladding tubes in the storage and transportation clamping process.

Clinical value of oral contrast-enhanced ultrasonography in diagnosis of gastric tumors

BACKGROUND The incidence of gastric cancer remains high,and it is the sixth most common cancer and the fourth leading cause of cancer deaths *** contrastenhanced ultrasonography is a simple,non-invasive,and painless method for the diagnosis of gastric *** To explore the diagnostic value of oral contrast-enhanced ultrasonography for the detection of gastric *** The screening results based on oral contrast-enhanced ultrasonography and electronic gastroscopy were compared with those of the postoperative pathological *** Among 42 patients with gastric tumors enrolled in the study,the diagnostic accordance rate was 95.2%for oral contrast-enhanced ultrasonography(n=40)and 90.5%for electronic gastroscopy(n=38)compared with postoperative pathological *** Kappa value of consistency test with pathological findings was 0.812 for oral contrast-enhanced ultrasonography and 0.718 for electronic gastroscopy,and there was no significant difference between them(P=0.397).For the TNM staging of gastric tumors,the accuracy rate of oral contrast enhanced ultrasonography was 81.9%for the overall T staging and 50%,77.8%,100%,and 100%for T1,T2,T3,and T4 staging,*** sensitivity and specificity were both 100%for stages T3 and *** diagnostic accuracy rate of oral contrast-enhanced ultrasonography was 93.8%,80%,100%,and 100%for stages N0,N1-N3,M0,and M1,*** The accordance rate of qualitative diagnosis by oral contrast-enhanced ultrasonography is comparable to that of gastroscopy,and it could be used as the preferred method for the early screening of gastric tumors.
