Prevalence, diagnosis, and treatment of primary hepatic gastrointestinal stromal tumors

Gastrointestinal stromal tumors(GIST), which is the most common mesenchymal tumor of the digestive tract, account for 1%-3% of gastrointestinal tumors. Primary stromal tumors outside the gastrointestinal tract are collectively referred to as extra GISTs, and stromal tumors in different regions often have different prognoses. A primary hepatic GIST is a rare tumor with an unknown origin, which may be related to interstitial Cajal-like cells. Although primary hepatic GIST has certain characteristics on imaging, it lacks specific symptoms and signs;thus, the final diagnosis depends on pathological and genetic evidence. This review summarizes all cases of primary hepatic GIST described in the literature and comprehensively analyzes the detailed clinical data of all patients. In terms of treatment, local resection alone or with adjuvant therapy was the prioritized choice to obtain better disease-free survival and longer survival time. For advanced unresectable cases, imatinib mesylate was applied as the first-line chemotherapy agent. Moreover, transcatheter arterial chemoembolization,/by-nc/4.0/Manuscript source: Unsolicited manuscript radiofrequency ablation, and microwave ablation were shown to improve overall survival for selected patients. Liver transplantation was a final treatment option after resistance to chemotherapy developed.

High-Temperature Stability of Mg-1Al-12Y Alloy Containing LPSO Phase and Mechanism of Its Portevin-Le Chatelier(PLC)Effect

In this study,the high-temperature stability and the generation mechanism of the Portevin-Le Chatelier(PLC)effect in solid-solution Mg-1Al-12Y alloy with different heat treatment processes were investigated by adjusting the content of long-period stacking ordered(LPSO)*** was found that the content of LPSO phases in the alloys differed the most after heat treatment at 530℃for 16 h and 24 h,with values of 13.56%and 3.93%***,high-temperature tensile experiments were conducted on these two alloys at temperatures of 150℃,200℃,250℃,and 300℃.The results showed that both alloys exhibited the PLC effect at temperatures ranging from 150 to 250℃.However,at a temperature 300℃,only the alloy with a greater concentration of LPSO phases exhibited the PLC effect,whereas the alloy with a lower proportion of LPSO phases did not exhibit this ***,both alloys exhibited remarkable high-temperature stability,with the alloy containing a greater percentage of LPSO phases also demonstrating superior *** underlying mechanism for this phenomenon lies in the exceptional high-temperature stability exhibited by the second phase within the ***,the LPSO phase effectively obstructs the movement of dislocations,and it also undergoing kinking to facilitate plastic deformation of the *** results indicate that the PLC effect can be suppressed by reducing dislocation pile-up at grain boundaries,which leads to a decrease in alloy plasticity but an increase in *** presence of the PLC effect in the WA121 alloy is attributed to the abundant dispersed second phase within the alloy,which initially hinders the movement of dislocations,leading to an increase in stress,and subsequently releases the dislocations,allowing them to continue their movement and thereby reducing in stress.

BACKGROUND Sarcomatoid carcinoma of the pancreas(SCP) is a rare and aggressive epithelial tumor that has both epithelial and mesenchymal features. It is characterized by sarcomatous elements with evidence of epithelial differentiation. And the term"sarcomatoid carcinoma" is often confused with "carcinosarcoma".CASE SUMMARY We present a case of SCP with lymph node metastasis in a 59-year-old male patient. He had experienced darkening of the urine, scleral icterus, and fatigue for 4 weeks. Computed tomography and magnetic resonance imaging revealed a mass in the pancreatic head, and laboratory tests revealed elevated serum bilirubin levels. The patient underwent pancreaticoduodenectomy after biliary decompression. Histologically, spindle cells with marked nuclear atypia and brisk mitotic activity arranged in a storiform or fascicular pattern were present in the bulk of the tumor. Immunohistochemical analysis found that the spindle cells exhibited strong diffuse positivity for epithelial markers, indicative of epithelial differentiation. Developing new functional explosives that display high stability,good energy performance,and low sensitivity are one of the key directions of energetic materials *** this work,two-dimensional(2D)Schiff-based energetic covalent organic frameworks(COFs)are prepared based on triaminoguanidine salts with different anions as building *** from the robust covalent bond in 2D extended polygons and strongπ-πinteractions in the eclipsed interlayers,the synthesized energetic COFs showed higher thermal stability and lower mechanical sensitivity than their precursor *** importantly,incorporating triaminoguanidine salts into COFs effectively increase the corrosion resistance to metal under high humidity conditions,which is due to the imine moieties in COFs functioning asπacceptors and offering strong bonding with metallic *** work provides a new pathway for the development of high-performance energetic materials.

Experimental and numerical investigation of the impacts of rotor tip-rake on excitation forces of pump-jet propulsors

The tip-clearance flow in a pump-jet propulsor exerts great impacts on the fluctuating pressures and resultant unsteady forces,which are important sources of structural vibrations and radiated noise *** blade geometry close to the tip is an important factor determining the vortex strength in the tip-clearance *** the open-water condition,the effects of raking the rotor tips on the duct-surface fluctuating pressures and the resultant unsteady forces acting on different components of the propulsor are investigated via physical model experiments and the numerical solution of Reynolds-averaged Navier-Stokes(RANS)equations coupled with the SST k-ωturbulence *** measured and simulated results of hydrodynamic pressures are consistent to each other,and the simulated flows help better understand why the fluctuating pressures change with the tip *** strong fluctuations of duct-surface pressures are caused by intensive tip separation *** duct-surface pressure fluctuations are effectively reduced by using the rake distribution near the tip towards blade back side and,for the combination of the five-bladed rotor and the seven-bladed stator,the resultant unsteady horizontal(and vertical)forces acting on the duct and stator are also reduced;while increasing rake leads to negative effect on pressure fluctuations and unsteady horizontal(and vertical)forces acting on all the components of the propulsor.
