Advances in management strategies for enteral nutrition-related gastric retention in adult patients with nasogastric tubes

Gastric retention is a common complication in individuals receiving enteral nutrition(EN)via a nasogastric tube,increasing the risk of aspiration pneumonia and causing unnecessary interruptions in nutritional *** its clinical significance,establishing effective,evidence-based,and standardized manage-ment strategies is essential for bettering patient outcomes and mitigating *** review systematically synthesized the diagnostic criteria,assessment methods,influencing factors,management procedures,and intervention strategies for gastric retention in EN *** no universal consensus exists re-garding gastric residual volume(GRV)thresholds,evidence indicates that EN can continue at high GRV levels in the absence of gastrointestinal *** ultrasound emerged as a non-invasive,and precise method GRV assessment,offering potential to standardize clinical *** risk factors for gastric retention include neurological disorders and EN infusion rates exceeding 100 mL/*** management strategies encompass non-pharmacological inter-ventions,pharmacological agents,and traditional Chinese medicine(TCM)*** review underscored the need for integrated,multi-modal manage-ment strategies and recommended the adoption of bedside ultrasound and stan-dardized protocols to optimize EN delivery and improve patient ***-scale,multicenter clinical trials should be a priority for future investigation to verify the effectiveness of TCM therapies and develop personalized intervention plans for high-risk patients.

Confined electric field in nano-sized channels of ionic porous framework towards unique adsorption selectivity

Optimizing the key parameter to accelerate the recovery of AMOC under a rapid increase of greenhouse gas forcing

Atlantic Meridional Overturning Circulation(AMOC)plays a central role in long-term climate variations through its heat and freshwater transports,which can collapse under a rapid increase of greenhouse gas forcing in climate *** studies have suggested that the deviation of model parameters is one of the major factors in inducing inaccurate AMOC *** this work,with a low-resolution earth system model,the authors try to explore whether a reasonable adjustment of the key model parameter can help to re-establish the AMOC after its *** a new optimization strategy,the extra freshwater flux(FWF)parameter is determined to be the dominant one affecting the AMOC’s *** traditional ensemble optimal interpolation(EnOI)data assimilation and new machine learning methods are adopted to optimize the FWF parameter in an abrupt 4×CO_(2) forcing experiment to improve the adaptability of model parameters and accelerate the recovery of *** results show that,under an abrupt 4×CO_(2) forcing in millennial simulations,the AMOC will first collapse and then re-establish by the default FWF parameter ***,during the parameter adjustment process,the saltier and colder sea water over the North Atlantic region are the dominant factors in usefully improving the adaptability of the FWF parameter and accelerating the recovery of AMOC,according to their physical relationship with FWF on the interdecadal timescale.

Forecasting step-like landslide displacement through diverse monitoring frequencies

The precision of landslide displacement prediction is crucial for effective landslide prevention and mitigation ***,the role of surface monitoring frequency in influencing prediction accuracy has been largely *** study examined the effect of varying monitoring frequencies on the accuracy of displacement predictions by using the Baijiabao landslide in the Three Gorges Reservoir Area(TGRA)as a case *** collected surface automatic monitoring data at different intervals,ranging from daily to *** Ensemble Empirical Mode Decomposition(EEMD)algorithm was utilized to dissect the accumulated displacements into periodic and trend components at each monitoring *** fitting was applied to forecast the trend component while the periodic component was predicted with two state-of-the-art neural network models:Long Short-Term Memory(LSTM)and Gated Recurrent Unit(GRU).The predictions from these models were integrated to derive cumulative displacement forecasts,enabling a comparative analysis of prediction accuracy across different monitoring *** results demonstrate that the proposed models achieve high accuracy in landslide displacement forecasting,with optimal performance observed at moderate monitoring ***,the daily mean average error(MAE)decreases sharply with increasing monitoring frequency,reaching a *** findings were corroborated by a parallel analysis of the Bazimen landslide,suggesting that moderate monitoring intervals of approximately 7 to 15 days are most conducive to achieving enhanced prediction accuracy compared to both daily and monthly intervals.

Ribosome profiling and single-cell RNA sequencing identify the unfolded protein response as a key regulator of pigeon lactation

Pigeons and certain other avian species produce a milklike secretion in their crop sacs to nourish offspring, yet the detailed processes involved are not fully elucidated. This study investigated the crop sacs of 225-day-old unpaired non-lactating male pigeons(MN) and males initiating lactation on the first day after incubation(ML). Using RNA sequencing,ribosomeprofiling,andsingle-cell transcriptome sequencing(scRNA-seq), we identified a significant up-regulation of genes associated with ribosome assembly and protein synthesis in ML compared to MN. Results from scRNA-seq analysis identified 12distinct cell types and 22 clusters, with secretory epithelial cells(SECs) exhibiting marked expression of plasma cell markers, including IGLL1 and MZB1. RNA fluorescence in situ hybridization(RNA FISH) and IgY quantification confirmed the critical role of SECs in producing endogenous IgY during lactation. We propose that fibroblast-derived BAFF signals activate SECs, mimicking B cell transformation and enhancing protein production through the unfolded protein response(UPR). These findings shed light on the cellular dynamics of pigeon milk production and contribute to a broader understanding of avian biology.

目的:探讨中药联合甲氨蝶呤治疗类风湿关节炎的临床疗效。方法:搜索检索Pubmed与Embase外文数据库,CNKI及Wanfang中文数据库,纳入中药联合甲氨蝶呤与单纯甲氨蝶呤治疗类风湿关节炎的随机对照试验(RCT)文献,用Cochrane系统评价手册进行质量评分,对总有效率、血沉(ESR)、C反应蛋白(CRP)、类风湿因子(RF)进行Meta分析。结果:共纳入5篇RCT文献进入对照研究,Meta分析显示,中药联合甲氨蝶呤治疗组总疗效优于单纯甲氨蝶呤对照组(P<0.05),并且治疗组降低CRP、ESR、RF效果均优于对照组(P<0.05)。结论:对比单纯使用甲氨蝶呤,联合中药治疗类风湿关节炎有更好的临床疗效,且没有增加不良反应,适合临床推广,但由于纳入文献质量不高,需要进一步开展高质量随机对照试验。

Near-eutectic Al-Si alloys are widely used in automotive manufacturing due to their superior wear resistance and high temperature *** of high Si content,the grain refinement of near-eutectic Al-Si alloy has been a problem for many *** this study,the effect of deep cryogenic treatment(DCT)on the microstructure and mechanical properties of Al-12Si-4Cu-2Ni-Mg alloy with addition of Al-Ti-C-B master alloy was fully *** show that the average grain size of the alloy is greatly reduced from 0.92 mm to 0.50 mm,and the eutectic Si and Al7Cu4Ni precipitates are spheroidized and refined in Al-12Si-4Cu-2Ni-Mg after DCT for 24 h and aging *** these changes of microstructures result in a significant increment of about 22.5%in elongation and a slight enhancement of about 6.8%in tensile ***,the refinement of microstructure also significantly improves the fatigue life of the alloy.

Co-production of hydrogen, oxygen, and electricity via an integrated solar-driven system with decoupled water electrolyzer and Na-Zn ion battery

Combining water electrolysis and rechargeable battery technologies into a single system holds great promise for the co-production of hydrogen (H_(2)) and ***,the design and development of such systems is still in its ***,an integrated hydrogen-oxygen (O_(2))-electricity co-production system featuring a bipolar membrane-assisted decoupled electrolyzer and a Na-Zn ion battery was established with sodium nickelhexacyanoferrate (NaNiHCF) and Zn^(2+)/Zn as dual redox *** decoupled electrolyzer enables to produce H_(2)and O_(2)in different time and space with almost 100%Faradaic efficiency at 100 mA cm^(-2).Then,the charged NaNiHCF and Zn electrodes after the electrolysis processes formed a Na-Zn ion battery,which can generate electricity with an average cell voltage of 1.75 V at 10 m A cm^(-2).By connecting Si photovoltaics with the modular electrochemical device,a well-matched solar driven system was built to convert the intermittent solar energy into hydrogen and electric energy with a solar to hydrogen-electricity efficiency of 16.7%,demonstrating the flexible storage and conversion of renewables.

Influence of Synoptic Pattern on the Spatiotemporal Features and Diurnal Variation of Extremely Persistent Heavy Rainfall over the Sichuan Basin,China

The Sichuan Basin(SCB),China has a high incidence of extremely persistent heavy rainfall(EPHR)*** EPHR events from 2009 to 2019 in the SCB were mainly concentrated over the northern and northwestern windward slopes and the central *** occurred from June to September,but especially in July,and peaked at 0300 ***5 reanalysis data and objective classification were used to investigate the synoptic patterns and their *** were three synoptic patterns during EPHR events,all accompanied by a Southwest *** location and intensity of the Southwest Vortex,thermal forcing of the Tibetan Plateau(TP),and low-level winds can greatly affect the intensity and spatial distribution of *** the Southwest Vortex was located in the western SCB and there were southerly low-level jets(LLJs),convergence and upslope wind would lead to EPHR over the northwestern or northern windward *** there was no LLJ and the whole SCB was under the center of the Southwest Vortex,nocturnal EPHR was controlled by the internal circulation of the Southwest Vortex and the updraft generated by the thermal forcing of the TP,and the rainfall was *** southeastern entrance of the SCB was a key area where the low-level wind dominated the nocturnal peak of *** nocturnal strengthened southeasterly wind in the key area is attributable to inertial oscillation,and the topographic friction plays an essential role in transporting momentum and moisture into the basin by generating easterly and northeasterly ageostrophic winds.
