Minilaparotomy to rectal cancer has higher overall survival rate and earlier short-term recovery

AIM： To report our experience using mini-laparotomy for the resection of rectal cancer using the total meso- rectal excision （TME） technique, METHODS： Consecutive patients with rectal cancer who underwent anal-colorectal surgery at the authors＇ hospital between March 2001 and june 2009 were included, In total, 1415 patients were included in the study, The cases were divided into two surgical proce- dure groups （traditional open laparotomy or mini-lap- arotomy）, The mini-laparotomy group was defined as having an incision length ≤ 12 cm. Every patient un- derwent the TME technique with a standard operation performed by the same clinical team. The multimodal preoperative evaluation system and postoperative fast track were used. To assess the short-term outcomes, data on the postoperative complications and recovery functions of these cases were collected and analysed. The study included a plan for patient follow-up, to ob- tain the long-term outcomes related to 5-year survival and local recurrence. RESULTS： The mini-laparotomy group had 410 pati- ents, and 1015 cases underwent traditional laparoto- my. There were no differences in baseline characteris- tics between the two surgical procedure groups. The overall 5-year survival rate was not different between the rnini-laparotorny and traditional laparotorny groups （80.6% vs 79.4%, P = 0.333）, nor was the 5-year local recurrence （1.4% vs 1.5%, P = 0.544）. However, 1-year mortality was decreased in the rnini-laparotorny group compared with the traditional laparotorny group （0% vs 4.2%, P 〈 0.0001）. Overall 1-year survival rates were 100% for Stage Ⅰ, 98.4% for Stage Ⅱ, 97.1% for Stage Ⅲ, and 86.6% for Stage Ⅳ. Local recurrence did not differ between the surgical groups at 1 or 5 years. Local recurrence at 1 year was 0.5% （2 cases） for mini-laparotorny and 0.5% （5 cases） for traditional laparotorny （P = 0.670）. Background Pediatric antineutrophil cytoplasmic antibody-associated vasculitis(AAV)is a life-threatening systemic vasculitis featured by liability to renal ***,there are few studies on the risk factors and predictive models for renal outcomes of AAV in *** Data from 179 AAV children in multiple centers between January 2012 and March 2020 were collected *** risk factors and predictive model of end-stage renal disease(ESRD)in AAV were *** Renal involvement was the most typical manifestation(95.5%),and the crescent was the predominant pathological lesion(84.9%).The estimated glomerular filtration rate(eGFR)was evaluated in 114 patients,of whom 59.6%developed ESRD,and the median time to ESRD was 3.20 *** eGFR[P=0.006,odds ratio(OR)=0.955,95%confidence interval(CI)=0.924–0.987]and the percentages of global glomerulosclerosis(pGGS;P=0.018,OR=1.060,95%CI=1.010–1.112)were independent risk factors for ESRD of renal *** on the pGGS and eGFR at renal biopsy,we developed three risk grades of ESRD and one predictive *** kaplan‒Meier curve indicated that renal outcomes were significantly different in different risk grades(Pline,the predictive model had higher accuracy(0.86 versus 0.58,P<0.001)and a lower coefficient of variation(0.07 versus 0.92)in external *** Renal involvement is the most common manifestation of pediatric AAV in China,of which more than half deteriorates into *** predictive model based on eGFR at renal biopsy and the pGGS may be stable and accurate in speculating the risk of ESRD in AAV children.

Propensity score matching study of 325 patients with spontaneous rupture of hepatocellular carcinoma

Background:This study aims to find out the possible optimal therapy and assess the prognosis properly for patient with spontaneous rupture of hepatocellular carcinoma(HCC).Methods:Propensity score matching(PSM)analysis was used to study the data from 325 patients with ruptured HCC(RHCC)and 2,291 patients with ***:The incidence and hospital mortality of RHCC were 5.1%and 0.8%respectively,with a median overall survival(OS)time of 17 *** was no difference between ruptured and non-RHCC patients undergoing conservation treatment in terms of ***-arterial embolization(TAE)was carried out in 69(21.2%)cases with RHCC,with a median OS of 7 months,which was no difference from that of non-RHCC(pre-and post-PSM).One hundred and sixty-nine(52.0%)RHCC cases underwent one-stage hepatectomy,with a median OS and disease-free survival(DFS)of 30 and 6 months respectively,which were shorter than that of non-RHCC(post-PSM).TAE plus two-stage hepatectomy was performed in 30 RHCC cases,with a median OS and DFS of 28 and 10 months respectively;these outcomes were better than that from RHCC patients undergoing TAE alone or one-stage hepatectomy(post-PSM),which were no difference from that of non-RHCC patients undergoing *** risk of death for RHCC patient undergoing one-stage hepatectomy is 1.545 times higher than that of one undergoing TAE+two-stage ***:TAE plus two-stage hepatectomy might be the optimal treatment for RHCC *** the premise of the same pathological properties,there is no difference in prognosis between ruptured and non-RHCC patients if the therapy is appropriate.

Comprehensive sequential interventional therapy for hepatocellular carcinoma

Background Since the 1980s, various approaches to interventional therapy have been developed, with the development and achievement of medical imaging technology. This study aimed to evaluate the effectiveness of comprehensive sequential interventional therapy especially personal therapeutic plan in 53 radical cure patients with hepatocellular carcinoma （HCC）. Methods From January 2003 to January 2005, a total of 203 patients with HCC received sequential interventional treatment in our hospital. Fifty-three patients achieved radical cure outcomes. Those patients were treated with transcatheter arterial chemoembolization （TACE）, radiofrequency ablation （RFA）, percutaneous ethanol injection （PEI）, or high intensity focused ultrasound （HIFU）, sequentially and in combination depending on their clinical and pathological features. PET-CT was used to evaluate, assess, and guide treatment. Results Based on the imaging and serological data, all the patients had a personal therapeutic plan. The longest follow-up time was 24 months, the shortest was 6 months, and mean survival time was 16.5 months. Conclusion Comprehensive sequential interventional therapy especially personal therapeutic plan for HCC play roles in interventional treatment of HCC in middle or advanced stage.

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;Pkades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

The ＂island of inversion＂ has been known for over a quar- ter century, since Warburton et al. [1] proposed that nuclei with intruder ground states would constitute a 3x3 square with Z=10-12, N=20-22 in 1990. Uncovering the underlying inversion mechanism and exploring the scope of the island have attracted significant theoretical and experimental efforts in the following years. Now it is well known that the reduction of N=20 shell gap, which is likely caused by the strong nucleon-nucleon tensor interaction [2-5], allows the intrusion of neutron orbits from the pf shell into the sd shell and results in the inversion of the 2p-2h intruder and 0p-0h normal con- figurations of the nuclear ground states in this region. Mean- while, the original border of the ＂island of inversion＂ has been extended greatly [6-14].

Nondestructive and noninvasive neutron assays are essential applications of neutron *** resonance transmission analysis(NRTA)is a powerful nondestructive method for investigating the elemental composition of an *** back-streaming neutron line(Back-n)is a newly built time-of-flight facility at the China Spallation Neutron Source(CSNS)that provides neutrons in the eV to 300 MeV range.A feasibility study of the NRTA method for nuclide identification was conducted at the CSNS Back-n via two test *** results demonstrate that it is feasible to identify different elements and isotopes in samples using the NRTA method at *** study reveals its potential future applications.

Neutron-sensitive microchannel plates(nMCPs)have applications in neutron detection,including energy spectrum measurements,neutron-induced cross sections,and neutron imaging.10B-doped MCPs(B-MCPs)have attracted significant attention owing to their potential for exhibiting a high neutron detection efficiency over a large neutron energy *** spatial and temporal resolutions are useful for neutron energy-resolved ***,their practical applications still face many technical *** this study,a B-MCP with 10 mol%10B was tested for its response to wide-energy neutrons from eV to MeV at the Back-n white neutron source at the China Spallation Neutron *** neutron detection efficiency was calibrated at 1 eV,which is approximately 300 times that of an ordinary MCP and indicates the success of 10 B *** factors that caused the reduction in the detection efficiency were simulated and *** neutron energy spectrum obtained using B-MCP was compared with that obtained by other measurement methods,and showed very good consistency for neutron energies below tens of *** response is more complicated at higher neutron energy,at which point the elastic and nonelastic reactions of all nuclides of B-MCP gradually become *** is beneficial for the detection of neutrons,as it compensates for the detection efficiency of B-MCP for high-energy neutrons.
