Effect of small bowel preparation with simethicone on capsule endoscopy

Background： Capsule endoscopy is a novel non-invasive method for visualization of the entire small bowel. The diagnostic yield of capsule endoscopy depends on the quality of visualization of the small bowel mucosa and its complete passage through the small bowel. To date, there is no standardized protocol for bowel preparation before capsule endoscopy. The addition of simethicone in the bowel preparation for the purpose of reducing air bubbles in the intestinal lumen had only been studied by a few investigators. Methods： Sixty-four participants were randomly divided into two groups to receive a bowel preparation of polyethylene glycol （PEG） solution （Group 1） and both PEG solution and simethicone （Group 2）. The PEG solution and sime- thicone were taken the night before and 20 rain prior to capsule endoscopy, respectively. Frames taken in the small intestine were examined and scored for luminal bubbles by two professional capsule endoscopists. Gastric emptying time and small bowel transit time were also recorded. Results： Simethicone significantly reduced luminal bubbles both in the proximal and distal small intes- tines. The mean time proportions with slight bubbles in the proximal and distal intestines in Group 2 were 97.1% and 99.0%, respectively, compared with 67.2% （P〈0.001） and 68.8% （P〈0.001） in Group 1. Simethicone had no effect on mean gastric emptying time, 32.08 rain in Group 2 compared with 30.88 min in Group 1 （P=0.868）, but it did increase mean small intestinal transit time from 227.28 to 281.84 rnin （P=0.003）. Conclusion： Bowel preparation with both PEG and simethicone significantly reduced bubbles in the intestinal lumen and improved the visualization of the small bowel by capsule endoscopy without any side effects observed.

Objective： To assess the diagnostic efficiency of OMOM capsule endoscopy （CE） in a group of patients with different indications. Methods： Data from 89 consecutive patients （49 males, 40 females） with suspected small bowel disease who underwent OMOM CE （Jinshan Science and Technology Company, review. The patients＇ indications of the disease consisted of the Chongqing, China） examination were obtained by retrospective following： obscure gastrointestinal bleeding （OGIB）, abdominal pain or diarrhea, partial intestinal obstruction, suspected inflammatory bowel disease, tumor of unknown origin, hypoproteinemia, constipation, weight loss, and elevated tumor markers. Results： CE failed in one patient. Visualization of the entire small bowel was achieved in 75.0%. Capsules were naturally excreted by all patients. The detection rate of abnormalities was 70.5% for patients with suspected small bowel disease, and the diagnostic yield for patients with OGIB was higher than that for patients with abdominal pain or diarrhea （85.7% vs 53.3%, P〈0.005）. Angiodysplasia was the most common small bowel finding. Active bleeding sites were noted in the small intestine in 11 cases. Conclusion： OMOM CE is a useful diagnostic tool for the diagnosis of variably suspected small bowel disease, whose diagnostic efficiency is similar to that of the Pillcam SB （small bowel） CE （Given Imaging, Yoqneam, Israel）.

Frontotemporal lobar degeneration(FTLD)represents a group of clinically,neuropathologically and genetically heterogeneous disorders with plenty of overlaps between the neurodegenerative mechanism and the clinical *** is pathologically characterized by the frontal and temporal lobar *** dementia(FTD)clinically presents with abnormalities of behavior and personality and language impairments *** clinical spectrum of FTD encompasses distinct canonical syndromes:behavioural variant of FTD(bvFTD)and primary progressive *** later includes nonfluent/agrammatic variant PPA(nfvPPA or PNFA),semantic variant PPA(svPPA or SD)and logopenic variant PPA(lvPPA).In addition,there is also overlap of FTD with motor neuron disease(FTD-MND or FTD-ALS),as well as the parkinsonian syndromes,progressive supranuclear palsy(PSP)and corticobasal syndrome(CBS).The FTLD spectrum disorders are based upon the predominant neuropathological proteins(containing inclusions of hyperphosphorylated tau or ubiquitin protein,e.g transactive response(TAR)DNA-binding protein 43 kDa(TDP-43)and fusedin-sarcoma protein in neurons and glial cells)into three main categories:(1)microtubule-associated protein tau(FTLD-Tau);(2)TAR DNA-binding protein-43(FTLD-TDP);and(3)fused in sarcoma protein(FTLD-FUS).There are five main genes mutations leading clinical and pathological variants in FTLD that identified by molecular genetic studies,which are chromosome 9 open reading frame 72(C9ORF72)gene,granulin(GRN)gene,microtubule associated protein tau gene(MAPT),the gene encoding valosin-containing protein(VCP)and the charged multivesicular body protein 2B(CHMP2B).In this review,recent advances on the different clinic variants,neuroimaging,genetics,pathological subtypes and clinicopathological associations of FTD will be discussed.

Sulfide layer with certain thickness was made on the nitrided surface of 16Mn steel by means of plasma S-C-N composite treatment. Under half lubricated condition, friction coefficient and wear loss of LY12 aluminum alloy were measured in sliding against samples of 16Mn treated by cubrizing and sulphiding respectively; In order to avoid the transfer of aluminum to the steel that lead to the inaccurate measurement of wear loss of carburized samples, Grl5 steel was adopted as counterpart face to measure the wear loss of them. SEM and EDAX were used for the morphological and chemical characterization of the wear surface and longitudinal cross-sections beneath sliding surface of LY12 aluminum alloy cirque and the wear tacks of the carburized samples and sulphided samples. Results show: The surface roughness, wear rate, average friction coefficient and magnitude of friction fluctuations of LY12 aluminum alloy cirque sliding against sulphided sample were all lower than sliding against carburized sample; Compared with carburized layer, sulfide layer of 16Mn steel can not only efficiently prevent the occurrence of adhesion when sliding against LY12 aluminum alloy, but also greatly lower the wear loss when sliding against Grl5 steel.

BACKGROUND Oncocytic adrenocortical tumor(OACT)is rare,with few cases reported in the *** more than 20 cases in children have been *** clinical characteristics,diagnosis,treatment and prognosis of children with OACT are summarized based on a literature review,in order to improve the understanding of OACT in *** SUMMARY We report a case of a 17-mo-old patient who was admitted to our hospital due to symptoms of odynuria and fever,which are clinical features consistent with a functional adrenocortical *** patient was diagnosed with OACT of uncertain malignant *** tomography indicated a soft tissue giant tumor in the right adrenal region,approximately 4.3 cm×5.5 cm in *** nodular and speckled calcifications were observed in the *** patient received robot-assisted laparoscopic right adrenal tumor *** pathological results were consistent with OACT,and immunohistochemical results showed cytokeratin+/-,chromogranin A+,synaptophysin-,neuronspecific enolase-,S100-,Ki67 about 10%,CD34-and D2-40-.After surgery,urinary tract ultrasonography was reviewed monthly,catecholamine hormone and sex hormone levels were examined every 2 mo and computed tomography was performed every 6 *** date,no tumor metastasis or recurrence has been identified in this *** levels of sex hormones and catecholamine hormones decreased to normal 1 mo after *** OACT is rare in the pediatric population,with few cases reported in the *** most pediatric OACTs are benign,malignant cases have been *** resection is the preferred option in most patients.

BACKGROUND Aorto-esophageal injury is a rare but life-threatening complication of esophageal foreign bodies,which typically requires open *** best way to treat patients with this condition remains *** date,few reports have described an aortic wall directly penetrated by a sharp foreign ***,we present a rare case of a fishbone completely embedded in the esophageal muscularis propria and directly piercing the aorta,which was successfully treated by endoscopy and thoracic endovascular aortic repair(TEVAR).CASE SUMMARY We report the case of a 71-year-old man with a 1-d history of retrosternal pain after eating *** abnormal findings were observed by the emergency *** tomography showed a fishbone that had completely pierced through the esophageal mucosa and into the *** patient refused to undergo surgery for personal reasons and was *** days after the onset of illness,he was readmitted to our *** examination showed a nodule with a smooth surface in the middle of the *** ultrasonography confirmed a fishbone under the *** performing TEVAR,we incised the esophageal mucosa under an endoscope and successfully removed the *** patient has remained in good condition for 1 *** Incising the esophageal wall under endoscope and extracting a foreign body after TEVAR may be a feasible option for cases such as ours.

Objective:Chronic levodopa(L-dopa)treatment in Parkinson’s disease(PD)is often associated with the development of motor complications,but the corresponding epidemiological data is rare in Chinese PD *** present survey was to investigate the prevalence rate of wearing-off(WO)and dyskinesia among the patients with PD in ***:From May 2012 to October 2012,a 3-step registry survey for wearing off(WO)and dyskinesia patients with PD receiving levodopa therapy was performed simultaneously at 28 movement disorders clinics in ***:There were 1,558 PD patients fulfilling the inclusion *** them,1,051 had at least one positive response of 9-item wearing off questionnaire(WOQ-9),724 and 160 patients were finally diagnosed with WO and dyskinesia by movement disorders specialists,*** overall prevalence rates of WO and dyskinesia were 46.5%(95%CI 44.0%-48.9%)and 10.3%(95%CI 8.8%-11.8%),*** mean score of WOQ-9 for those with WO was 3.8(SD=1.8),with movement slowness being the most common motor symptoms and pain/aching being the most common non-motor *** improvement of motor symptoms(n=354,87.8%)and long-term disease control and drug selection(n=288,71.5%)were the two most frequently considered factors when movement disorders specialists adjusted therapeutic strategies for patients with ***:This survey provided the first multi-center epidemiological data of motor complications among PD patients on L-dopa therapy from China's *** prevalence rate among Chinese PD patients was in line with,while dyskinesia prevalence rate was lower than previous reports from other Countries.
