Circumferential decompression via the posterior approach for the surgical treatment of multilevel thoracic ossification of the posterior longitudinal ligaments： a single institution comparative study

Background The treatment strategies for multilevel thoracic ossification of the posterior longitudinal ligaments （T-OPLL） were rarely reported. The aim of this study was to investigate the clinical outcomes and complications of circumferential decompression for multilevel T-OPLL and compare two different methods in the management of the OPLL （resection or floating）. Methods Data of sequentially treated patients who received surgical treatment for thoracic spinal stenosis caused by multilevel T-OPLL from January 2005 to February 2012 were retrospectively reviewed. Based on the surgical approaches applied, the patients were divided into two groups. Group A consisted the patients who received posterior decompression and group B consisted the patients who received circumferential decompression via the posterior approach. Group B was further divided into two subgroups： subgroup 1 （the resection group） where the OPLL was completely resected and subgroup 2 （the floating group） where the OPLL was floated. Results A total of 49 patients were included in the study. Fourteen patients with single posterior decompression were included in group A and 35 patients who received circumferential decompression were included in group B. In group B, 29 patients had complete resection of the ossified posterior longitudinal ligaments, while the other six underwent a flotation procedure. The follow-up data were available in 39 patients. Mean JOA scores improved from 5.4 ± 1.8 to 7.5 ± 2.8 in group A and from 3.7 ± 1.8 to 7.9 ± 2.4 in group B. The main complications included cerebrospinal fluid （CSF） leakage and postoperative neurelogic deterioration （ND）. Twenty-three of the 25 cases with postoperative CSF leakage achieved a complete recovery at the last follow-up and 12 of the 15 cases with ND achieved some neurological improvement at the last follow-up. Vertical transmission of the Yq AZFc microdeletion from father to son over two or three generations in infertile Han Chinese families

This study was carried out to analyze the vertical transmission of Yq AZFc microdeletions from father to son in infertile Han Chinese families to investigate genetic factors and family background affecting fertility *** peripheral blood of infertile males in 19 Han families was extracted and screened with modified multiplex polymerase chain reaction （PCR）. Family trees were drawn according to fertility status and clinical characteristics of the subjects. The vertical transmission of Yq AZFc microdeletions was detected in six cases of 19 investigated families （31.6%,6/19）. Although both fathers and sons showed a similar type of Yq AZFc deletion,the fathers were fertile,whereas the sons were infertile and showed severe oligozoospermia. The vertical transmission of Yq AZFc microdeletion from fertile fathers to infertile sons over generations is not rare. This has different effects on fertility status in fathers and sons in Han Chinese families. Both genetic factors and family background affect spermatogenetic phenotypes.

CT-guided percutaneous radiofrequency ablation of spinal osteoid osteoma

Background This study evaluated the feasibility, efficacy and safety of CT-guided percutaneous radiofrequency ablation in patients with spinal osteoid osteoma. Methods Two patients suffered spinal osteoid osteoma were treated with CT-guided percutaneous radiofrequency ablation under local anesthesia. Lesions located in sacral vertebrae and cervical vertebrae, which were adjacent to nerve root and spinal canal respectively. Tumors were treated under 90℃ radiofrequency temperature lasting 4 minutes by an electrode placement. Visual analog scale was used to evaluate the pain improvement. Results No complications were observed pre- and post-operation. Patients recovered to normal activities immediately and achieved complete pain relief in 24 hours. No symptoms were recurrent in 5 months and 4 months follow up. Mild scoliosis has been recovered in case 2. Conclusions CT-guided percutaneous radiofrequency ablation of spinal osteoid osteoma is safe, effective and has more clinical benefits. The long-term outcome needs further observation.

Toughening ultrastrong low-density steel by textured δ-ferrite lamellas

By both the Charpy V-notched impact and the projectile tests, we here investigated the dynamic fracture behavior of a recently developed ultrastrong lightweight steel comprising a hierarchical martensitic matrix, dispersed ultra-fine-retained austenite grains and oriented δ-ferrite lamellas, the latter being due to high Al and Si contents employed for low-density design. This steel shows a superior combination of specific ultimate tensile strength and impact toughness to other ultrastrong steels and has successfully arrested a real steel-cored bullet shot. These are attributed to the densely textured δ-ferrite lamellas that can deflect the propagating cracks until they are trapped and enclosed besides austenite-to-martensite transformation crack closure, leading to more energy consumed before failure. These results suggest a new pathway for toughening ultrastrong lightweight steels.

Atmospheric transmission algorithm for pulsed X-rays from high-altitude nuclear detonations based on scattering correction

In high-altitude nuclear detonations,the proportion of pulsed X-ray energy can exceed 70%,making it a specific monitoring signal for such *** pulsed X-rays can be captured using a satellite-borne X-ray detector following atmospheric *** quantitatively analyze the effects of different satellite detection altitudes,burst heights,and transmission angles on the physical processes of X-ray transport and energy fluence,we developed an atmospheric transmission algorithm for pulsed X-rays from high-altitude nuclear detonations based on scattering *** proposed method is an improvement over the traditional analytical method that only computes direct-transmission *** traditional analytical method exhibits a maximum relative error of 67.79% compared with the Monte Carlo *** improved method reduces this error to within 10% under the same conditions,even reaching 1% in certain ***,its computation time is 48,000 times faster than that of the Monte Carlo *** results have important theoretical significance and engineering application value for designing satellite-borne nuclear detonation pulsed X-ray detectors,inverting nuclear detonation source terms,and assessing ionospheric effects.

Dozens of genes are associated with idiopathic hypogonadotropic hypogonadism(IHH)and an oligogenic etiology has been ***,the associated genes may account for only approximately 50%*** addition,a genomic systematic pedigree analysis is still ***,we conducted whole exome sequencing(WES)on 18 unrelated men affected by IHH and their corresponding ***,one reported and 10 novel variants in eight known IHH causative genes(AXL,CCDC141,CHD7,DMXL2,FGFR1,PNPLA6,POLR3A,and PR0KR2),nine variants in nine recently reported candidate genes(DCAF17,DCC,EGF,IGSF10,NOTCH1,PDE3A,RELN,SLIT2,and TRAPPC9),and four variants in four novel candidate genes for IHH(CCDC88C,CDON,GADL1,and SPRED3)were identified in 77.8%(14/18)of IHH *** them,eight(8/18,44.4%)cases carried more than one variant in IHH-related genes,supporting the oligogenic ***,we found that those variants tended to be maternally inherited(maternal with n=17 vs paternal with n=7;P=0.028).Our further retrospective investigation of published reports replicated the maternal bias(maternal with n=46 i^s paternal with n=28;P=0.024).Our study extended a variant spectrum for IHH and provided the first evidence that women are probably more tolerant to variants of IHH-related genes than men.

Two new bromoindoles from red alga Laurencia similis

Two new bromoindole alkaloids have been isolated from the ethanolic extract of the red alga Laurencia similis. On the basis of chemical and spectroscopic methods （including 1D and 2D NMR technique）, their structures have been elucidated as 2,2＇,5,5＇,6,6＇- sixibromo-3, 3＇-bi-lH-indole and 3, 5-dibromo-1-methylindole, respectively.
