高质量专病数据库作为单病种大数据分析与利用的重要支撑,已成为提升临床研究质量和疾病诊疗水平的重要途径。该文以上海交通大学医学院附属第六人民医院专病数据库建设为实例,总结建设过程中的思路及关键技术,梳理专病数据库管理规范,并以糖尿病专病库为例阐述数据库的临床研究应用。从建设、管理和应用三个维度,总结综合性公立医院专病数据库建设路径和管理流程,为我国综合性公立医院建设高质量临床专病数据库和开展临床研究应用提供参考。

Extensive investigation of the passivating agents has been performed to suppress the perovskite defects.However,very few attentions have been paid to rationally design the passivating agents for the balance of the carrier confinement and transfer in quasi-2D perovskites,which is essential to achieve high-performance perovskite LEDs(PeLEDs).In this work,tributylphosphine oxide(TBPO)with moderate carbon chain length is demonstrated as a decent passivator for the quasi-2D perovskites by strengthening the carrier confinement for massive radiative recombination within the perovskites,and more importantly providing efficient carrier transfer in the quasi-2D perovskites.Benefiting from these interesting optoelectronic properties of TBPO-incorporated perovskites,we achieve high-efficient blue PeLEDs with an external quantum efficiency up to 11.5%and operational stability as long as 41.1 min without any shift of the electroluminescence spectra.Consequently,this work contributes an effective approach to promote the carrier confinement and transfer for high-performance and stable blue PeLEDs.

本文研究了增容剂种类和用量对有机硅/聚氨酯热塑性弹性体物理性能及微观形貌的影响。研究发现:不同增容剂均可以有效改善硅橡胶/聚氨酯热塑性弹性体的拉伸强度、断裂伸长率和硅橡胶微球尺寸,但效果具有显著差别。其中,马来酸酐接枝苯乙烯/乙烯/丁二烯/苯乙烯嵌段共聚物对弹性体性能的改善效果最优异,拉伸强度和断裂伸长率增幅分别为41.6%和22.6%,马来酸酐接枝聚烯烃弹性体次之,马来酸酐接枝聚丙烯和乙烯丙烯酸共聚物相近,但也比未添加增容剂的胶料拉伸强度提高24.3%,断裂伸长率略有提高。增容剂的用量对硅橡胶和聚氨酯的界面相互作用和物理性能具有明显影响,其随着增容剂用量的增加呈先增加后不变的现象。

Edge biomarkers for classification and prediction of phenotypes

In general,a disease manifests not from malfunction of individual molecules but from failure of the relevant system or network,which can be considered as a set of interactions or edges among molecules.Thus,instead of individual molecules,networks or edges are stable forms to reliably characterize complex diseases.This paper reviews both traditional node biomarkers and edge biomarkers,which have been newly proposed.These biomarkers are classified in terms of their contained information.In particular,we show that edge and network biomarkers provide novel ways of stably and reliably diagnosing the disease state of a sample.First,we categorize the biomarkers based on the information used in the learning and prediction steps.We then briefly introduce conventional node biomarkers,or molecular biomarkers without network information,and their computational approaches.The main focus of this paper is edge and network biomarkers,which exploit network information to improve the accuracy of diagnosis and prognosis.Moreover,by extracting both network and dynamic information from the data,we can develop dynamical network and edge biomarkers.These biomarkers not only diagnose the immediate pre-disease state but also detect the critical molecules or networks by which the biological system progresses from the healthy to the disease state.The identified critical molecules can be used as drug targets,and the critical state indicates the critical point of disease control.The paper also discusses representative biomarker-based methods.

The global "myopia boom" has raised significant international concerns. Despite a higher myopia prevalence in Asia, previous large-scale genome-wide association studies(GWASs) were mostly based on European descendants. Here, we report a GWAS of spherical equivalent(SE) in 1852 Chinese Han individuals with extreme SE from Guangzhou(631 0.00 D) and Wenzhou(593 -1.75 D), followed by a replication study in two independent cohorts with totaling 3538 East Asian individuals. The discovery GWAS and meta-analysis identify three novel loci, which show genome-wide significant associations with SE, including 1 q25.2 FAM163 A, 10 p11.22 NRP1/PRAD3, and 10 p11.21 ANKRD30 A/MTRNR2 L7, together explaining 3.34% of SE variance. 10 p11.21 is successfully replicated.The allele frequencies of all three loci show significant differences between major continental groups(P < 0.001). The SE reducing(more myopic) allele of rs10913877(1 q25.2 FAM163 A) demonstrates the highest frequency in East Asians and much lower frequencies in Europeans and Africans(EAS = 0.60,EUR = 0.20, and AFR = 0.18). The gene-based analysis additionally identifies three novel genes associated with SE, including EI24, LHX5, and ARPP19. These results provide new insights into myopia pathogenesis and indicate the role of genetic heterogeneity in myopia epidemiology among different ethnicities.

Reduced levels of retinal dopamine,a key regulator of eye development,are associated with experimental myopia in various species,but are not seen in the myopic eyes of C57BL/6 mice,which are deficient in melatonin,a neurohormone having extensive interactions with dopamine.Here,we examined the relationship between form-deprivation myopia(FDM)and retinal dopamine levels in melatonin-proficient CBA/CaJ mice.We found that these mice exhibited a myopic refractive shift in form-deprived eyes,which was accompanied by altered retinal dopamine levels.When melatonin receptors were pharmacologically blocked,FDM could still be induced,but its magnitude was reduced,and retinal dopamine levels were no longer altered in FDM animals,indicating that melatonin-related changes in retinal dopamine levels contribute to FDM.Thus,FDM is mediated by both dopamine level-independent and melatonin-related dopamine level-dependent mechanisms in CBA/CaJ mice.The previously reported unaltered retinal dopamine levels in myopic C57BL/6 mice may be attributed to melatonin deficiency.

Background and Aims:Hepatocellular carcinoma(HCC)is the most common primary hepatic malignancy.This study was designed to investigate the value of computed tomography(CT)spectral imaging in differentiating HCC from hepatic hemangioma(HH)and focal nodular hyperplasia(FNH).Methods:This was a retrospective study of 51 patients who underwent spectral multiple-phase CT at 40–140 keV during the arterial phase(AP)and portal venous phase(PP).Slopes of the spectral curves,iodine density,water density derived from iodine-and water-based material decomposition images,iodine uptake ratio(IUR),normalized iodine concentration,and the ratio of iodine concentration in liver lesions between AP and PP were measured or calculated.Results:As energy level decreased,the CT values of HCC(n=31),HH(n=17),and FNH(n=7)increased in both AP and PP.There were significant differences in IUR in the AP,IUR in the PP,normalized iodine concentration in the AP,slope in the AP,and slope in the PP among HCC,HH,and FNH.The CT values in AP,IUR in the AP and PP,normalized iodine concentration in the AP,slope in the AP and PP had high sensitivity and specificity in differentiating HH and HCC from FNH.Quantitative CT spectral data had higher sensitivity and specificity than conventional qualitative CT image analysis during the combined phases.Conclusions:Mean CT values at low energy(40–90 keV)and quantitative analysis of CT spectral data(IUR in the AP)could be helpful in the differentiation of HCC,HH,and FNH.
