Comparison of choroidal thickness in high myopic eyes after FS-LASIK versus implantable collamer lens implantation with swept-source optical coherence tomography

AIM:To investigate the changes in choroidal thickness(CT)in high myopic eyes after femtosecond laser-assisted in situ keratomileusis(FS-LASIK)surgery or central hole implantable collamer lens(ICL V4c)implantation using swept-source optical coherence tomography(SS-OCT).METHODS:We examined the right eyes of 116 patients with high myopia who were candidates for FS-LASIK surgery and ICL implantation.Sixty eyes underwent ICL V4c implantation and 56 eyes were subjected to FS-LASIK surgery.The CT was measured with SS-OCT.All data were recorded preoperatively and 2 h,1 wk,1 and 3 mo postoperatively.Other demographic information was collected,including age,sex,uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),spherical equivalent(SE),intraocular pressure(IOP)and axial length(AL).RESULTS:The UCVA improved in both groups and showed no significant differences between groups.There also were no significant differences between the two groups in postoperative BCVA and SE(P=0.581 and 0.203,respectively).The foveal CTs,inner nasal and outer nasal CTs were significantly thicker at 2 h postoperatively in both groups(P0.05).CONCLUSION:The CTs after ICL implantation and FSLASIK surgery are significantly thicker than those before operation,especially in the foveal and nasal areas,but there is no significant difference between the two methods.

Epidemiologic characteristics and outcomes of open globe injury in Shanghai

AIM： To investigate the epidemiologic characteristics and outcomes of open globe injury in Shanghai. METHODS： A retrospective study was conducted for 148 unilateral open globe injury cases presenting to a tertiary referral hospital of Shanghai. Electronic medical records were reviewed and phone surveys were conducted to collect and analyze 1） background of patient; 2） setting of injury and clinical signs at presentation; 3） treatment procedure and outcome; 4） quality of life after injury. RESULTS： There were more male patients （77.03%） than females （22.97%）, more temporary habitants （79.05%） than residents （20.95%）. The subjects in this study presented a significantly lower constitutional status of education than that of the whole Shanghai population （P〈0.001）. Occupational injury was the first cause of injuries （39.86%）, followed by home accident （20.27%）, road accident （16.80%）, violent behavior （16.89%） and outdoor injury （6,08%）. The 143 subjects （96.62%） were not wearing spectacles at the time of injury. Of all patients, 77 subjects （52.03%） had the outcome of no vision （including enucleation）. The classification and regression tree （CART） prognosis presents 59.58% sensitivity to predict visual survival correctly and 80.19% specificity to predict no vision correctly. The patients whose injured eye had no vision reported more reduction of life quality. CONCLUSION： We found that male subject, temporary habitants, low educational status and no eyewear are risk factors of open globe injury in Shanghai. Occupational injury is the leading cause. CART analysis presents a certain agreement to the actual visual outcome. The injury imposes negative impact on quality of life especially in no vision cases. The education of eye protection may help to avoid the injury.

Smoking and the risk of dry eye: a Meta-analysis

AIM： To collect the evidence to estimate the correlation between smoking and the incidence of dry eye. METHODS： The Pub Med,EMBASE,the Cochrane Central Register of Controlled Trials（CENTRAL,last issue）,CBM（Chinese Bio Medical Literature Database）,and CNKI（China National Knowledge Infrastructure/Chinese Academic Journals full-text Database） were searched for eligible studies published from January1964 to December 2015 to investigate the association of smoking with the risk of dry eye.The odds ratio（OR）and 95% confidence interval（CI） were summarized and calculated.The extracted studies were pooled by the fixed-effects model or a random-effects model. RESULTS： Two cohort studies and eight cross-sectional surveys were included in our Meta-analysis.There was no statistically significant relationship between current（OR=1.32; 95% CI： 0.99-1.76; P=0.055）or ever smoking（OR=1.12; 95% CI： 0.98-1.28; P=0.107）and the risk of dry eye among the studies,even when age and gender were adjusted（OR=1.16; 95% CI： 0.83-1.64; P=0.383）.In the sensitivity analysis in which only general population were included,the association was significant between smoking and dry eye（OR=1.50; 95%CI： 1.08-2.09; P=0.016）.CONCLUSION： This Meta-analysis suggests that smoking may associate with the risk of dry eye in general population.

AIM： To explore the effects of conditioned media on the proliferation of corneal endothelial cells （CECa） and to compare the efficiency of different conditioned media （CM）. METHODS： Rat CECs, corneal stromal cells （CSCs）, bone marrow -derived endothelial progenitor cells （BEPCs）, and bone marrow-derived mesenchymal stem cells （BMSCs） were isolated and cultured in vitra CM was collected from CSCs, BEPCs, and BMSCSo CECs were cultivated in different culture media. Cell morphology was recorded, and gene and protein expression were analyzed.~ RESULTS： After grown in CM for 5d, CECs in each experimental group remained polygonal, in a cobblestone- like monolayer arrangement. Immunocytofluorescence revealed positive expression of Na＋/K＋-ATP, aquaporin 1 （AQP1）, and zonula occludens 1 （ZO-1）. Based on quantitative polymerase chain reaction （qPCR） analysis, Na ＋/K ＋-ATP expression in CSC-CM was notably upregulated by 1.3-fold （＋0.036）（P〈0.05, n=3）. The expression levels of ZO-1, neuron specific enolase （NSE）, Vimentin, paired homebox 6 （PAX6）, and procollagen type VII （COL8A1） were notably upregulated in each experimental group. Each CM had a positive effect on CEC proliferation, and CSC-CM had the strongest effect on proliferation.~ CONCLUSION： CSC-CM, BEPC-CM, and BMSC-CM not only stimulated the proliferation of CECs, but also maintained the characteristic differentiated phenotypes necessary for endothelial functions. People in elevators are at risk of respiratory infection because the elevator cabin is crowded and has poor ventilation.The exhaled particles may be inhaled by the susceptible person,deposited on the surface and suspended in the elevator,which can result in direct and indirect transmission.However,whether the air vent designs adopted in the elevator can effectively reduce the transmission risk of respiratory particles remains unknown.In this study,the dispersion of particles under four common ventilation strategies used in the commercial elevator was investigated by proven computational fluid dynamics(CFD)simulations.The flow field was simulated with the RNG κ-ξ turbulence model and the Lagrangian method was adopted to track particle trajectories.The effects of air vent layout and airflow rate on particle transmission were analyzed.We found that more than 50% of exhaled particles(average value)were suspended in the cabin and difficult to discharge under the investigated ventilation strategies.The deposited fraction of particles on the susceptible person reached up to 39.14% for infiltration ventilation,which led to a high risk of contact infection.Increasing the ventilation rate could not significantly reduce the inhalation proportion of particles due to the poor airflow distribution inside the elevator.A more proper ventilation strategy should be explored for the elevator to control transmission risk.

BACKGROUND Hepatocellular carcinoma(HCC),a major contributor to cancer-related deaths,is particularly prevalent in Asia,largely due to hepatitis B virus infection.Its prognosis is generally poor.This case report contributes to the medical literature by detailing a unique approach in treating a large HCC through multidisciplinary collaboration,particularly in patients with massive HCC complicated by ruptured bleeding,a scenario not extensively documented previously.CASE SUMMARY The patient presented with large HCC complicated by intratumoral bleeding.Treatment involved a multidisciplinary approach,providing individualized care.The strategy included drug-eluting bead transarterial chemoembolization,sorafenib-targeted therapy,laparoscopic partial hepatectomy,and standardized sintilimab monoclonal antibody therapy.Six months after treatment,the patient achieved complete radiological remission,with significant symptom relief.Imaging studies showed no lesions or recurrence,and clinical assessments confirmed complete remission.This report is notable as possibly the first docu-mented case of successfully treating such complex HCC conditions through integrated multidisciplinary efforts,offering new insights and a reference for future similar cases.CONCLUSION This study demonstrated effective multidisciplinary treatment for massive HCC with intratumoral bleeding,providing insights for future similar cases.

Since the demands for lithium have been in explosive growth recently and several great clay lithium resources were founded in China,the development and utilization of clay-type lithium resources become a hot topic.In order to provide a guidance for the concentration and extracting lithium of clay-type lithium resources,this review systematically summarized the main types and characteristics of clay-type lithium resources and the research status of lithium extraction from clay lithium resources.Meanwhile,the flotation research status of the main mineral of clay lithium ore was also summarized.Furthermore,we discussed the difficulties in the development and utilization of clay-type lithium ore and put forward the subsequent research orientation.This paper can provide a guidance for the con-centration and extracting lithium of clay-type lithium resources in the future.

Lipid and sugar homeostasis is critical for insect development and survival.In this study,we characterized an acetyl coenzyme A carboxylase gene in Blattella germanica(BgACC)that is involved in both lipogenesis and sugar homeostasis.We found that BgACC was dominantly expressed in the fat body and integument,and was significantly upregulated after molting.Knockdown of BgACC in 5th-instar nymphs did not affect their normal molting to the next nymphal stage,but it caused a lethal phenotype during adult emergence.BgACC-RNA interference(RNAi)significantly downregulated total free fatty acid(FFA)and triacylglycerol(TAG)levels,and also caused a significant decrease of cuticular hydrocarbons(CHCs).Repression of BgACC in adult females affected the development of oocytes and resulted in sterile females,but BgACC-RNAi did not affect the reproductive ability of males.Interestingly,knockdown of BgACC also changed the expression of insulin-like peptide genes(BglLPs),which mimicked a physiological state of high sugar uptake.In addition,BgACC was upregulated when B.germanica were fed on a high sucrose diet,and repression of BgACC upregulated the expression of the glycogen synthase gene(BgGlyS).Moreover,BgACC-RNAi increased the circulating sugar levels and glycogen storage,and a longevity assay suggested that BgACC was important for the survival of B.germanica under conditions of high sucrose uptake.Our results confirm that BgACC is involved in multiple lipid biogenesis and sugar homeostasis processes,which further modulates insect reproduction and sugar tolerance.This study benefits our understanding of the crosstalk between lipid and sugar metabolism.
