Natural orifice specimen extraction with laparoscopic radical gastrectomy for distal gastric cancer: A case report

BACKGROUND This article introduces the surgical method and early experience in performing totally laparoscopic radical gastrectomy with transrectal specimen extraction for gastric cancer, and we evaluate the short-term effects and feasibility of this new procedure for gastric cancer in a 64-year-old male patient. This approach may provide new possibilities for gastric natural orifice specimen extraction(NOSE)surgery. In addition, we believe that this new procedure may further relieve pain,speed up recovery, and cause minimal physiological and psychological *** SUMMARY We performed NOSE gastrectomy in a male patient. Tumor resection, digestive tract reconstruction, and lymph node dissection were performed totally laparoscopically;the specimen was extracted from the natural orifice of the rectum-anus. This procedure reduced damage to the abdominal wall and decreased postoperative pain. We successfully performed radical gastrectomy without conversion and complications. Total operative time and blood loss were 176 min and 50 m L, respectively. The patient resumed normal activities of daily living on day 1 without pain, and passed flatus within 48 h. Postoperative hospital stay was 10 d. The number of resected lymph nodes was 0/43. During the follow-up, no stricture or anastomotic leakage was detected. Three months postoperatively, colonoscopy showed full recovery of the rectum without stricture or scar contracture. Computed tomography and laboratory test results showed no signs of tumor recurrence. There was no visible scar on the abdominal *** It is safe and reliable to perform totally laparoscopic radical gastrectomy with transrectal specimen extraction for distal gastric cancer patients.

The advent of immunotherapy and the development of immune checkpoint inhibitors(ICIs)are changing the way we think about cancer *** have shown clinical benefits in a variety of tumor types,and ICI-based immunotherapy has shown effective clinical outcomes in immunologically“hot”***,for immunologically“cold”tumors such as colorectal cancer(CRC),only a limited number of patients are currently benefiting from ICIs due to limitations such as individual differences and low response *** this review,we discuss the classification and differences between hot and cold CRC and the current status of research on cold CRC,and summarize the treatment strategies and challenges of immunotherapy for cold *** also explain the mechanism,biology,and role of immunotherapy for cold CRC,which will help clarify the future development of immunotherapy for cold CRC and discovery of more emerging strategies for the treatment of cold CRC.

Is adjuvant chemotherapy necessary for patients with ypT0–2N0 rectal cancer treated with neoadjuvant chemoradiotherapy and curative surgery?

Background and objective:The benefit from adjuvant chemotherapy for patients treated with neoadjuvant chemoradiotherapy(NCRT)and curative surgery remains controversial,particularly among those responding well to *** retrospective study aimed to clarify the benefits of adjuvant chemotherapy in terms of the oncological outcomes of patients with ypT0–2N0 rectal cancer after NCRT and curative ***:All patients with ypT0–2N0 rectal cancer after NCRT and curative resection between 2005 and 2014 were *** oncological outcomes between patients treated with adjuvant chemotherapy and those without any chemotherapy were ***:The clinicopathological characteristics of 110 patients were reviewed in this study;one patient was excluded due to lack of *** the 109 patients included,58(53.2%)underwent adjuvant chemotherapy(chemo group),whereas the remaining 51(46.8%)did not receive any chemotherapy(non-chemo group).After a median follow-up of 50 months,there were no significant differences in the 5-year overall survival(OS)or recurrence-free survival(RFS)rates between the groups(OS:92.1 vs 86.3%,P=0.375;RFS:80.9 vs 74.7%,P=0.534).Subgroup analysis also demonstrated no significant differences in 5-year OS and RFS rates between patients with ypT0N0 rectal cancer(P=0.712 and P=0.599,respectively)and those with ypT1–2N0 disease(P=0.255 and P=0.278,respectively).Conclusions:These results indicate that patients with ypT0–2N0 rectal cancer after NCRT followed by curative surgery may not derive significant benefit from adjuvant ***,further prospective randomized trials,with larger sample sizes,are warranted to confirm this conclusion.

Prognostic scoring system for synchronous brain metastasis at diagnosis of colorectal cancer: A population-based study

Micro RNAs in colorectal cancer:Role in metastasis and clinical perspectives

Colorectal cancer (CRC) is the third most common malignancy and the third leading cause of cancer related deaths in the United States. Almost 90% of the patients diagnosed with CRC die due to metastases. MicroRNAs (miRNAs) are evolutionarily conserved molecules that modulate the expression of their target genes post-transcriptionally, and they may participate in various physiological and pathological processes including CRC metastasis by influencing various factors in the human body. Recently, the role miRNAs play throughout the CRC metastatic cascade has gain attention. Many studies have been published to link them with CRC metastasis. In this review, we will briefly discuss metastatic steps in the light of miRNAs, along with their target genes. We will discuss how the aberration in the expression of miRNAs leads to the formation of CRC by effecting the regulation of their target genes. As miRNAs are being exploited for diagnosis, prognosis, and monitoring of cancer and other diseases, their high tissue specificity and critical role in oncogenesis make them new biomarkers for the diagnosis and classification of cancer as well as for predicting patients’ outcome. MiRNA signatures have been identified for many human tumors including CRC, and miRNA-based therapies to treat cancer have been emphasized lately. These will also be discussed in this review.

Intra-corporeal delta-shaped anastomosis in laparoscopic right hemicolectomy for right colon cancer:a safe and effective technique

Background and objective:Intra-corporeal delta-shaped anastomosis(IDA)is an important development in laparoscopic digestive-tract *** applied it in laparoscopic right hemicolectomy for right colon cancer and compared the short-term outcomes between the patients treated with IDA and conventional extracorporeal anastomosis(EA).Methods:Between 1 January 2016 and 1 October 2017,36 and 50 patients who underwent IDA and EA,respectively,were *** on clinicopathological characteristics,surgical outcomes,post-operative recovery and complications were collected and compared between the two ***:Surgical outcomes and clinicopathological characteristics were similar between the two groups except the length of incision,which was significantly shorter in the IDA group than in the EA group(4.660.6 vs 5.660.7 cm,P<0.001).The time to ground activities,fluid diet intake and post-operative hospitalization did not differ between the groups;however,the time to first flatus was significantly shorter in the IDA group than in the EA group(2.860.5 vs 3.260.8 days,P=0.004).The post-operative visual analogue scale for pain was lower in the IDA group than in the EA group on post-operative Day 1(4.060.7 vs 4.561.0,P=0.002)and post-operative Day 3(2.760.6 vs 3.460.6,P<0.001).The surgical complication rates were 8.3 and 16.0%in the IDA and EA groups(P=0.470),*** complications such as anastomotic bleeding,stenosis and leakage occurred in any ***:IDA is safe and feasible and shows more satisfactory short-term outcomes than EA.

Surface functionalization of Cu-based catalysts has demonstrated promising potential for enhancing the electrochemical CO_(2)reduction reaction(CO_(2)RR)toward multi-carbon(C2+)products,primarily by suppressing the parasitic hydrogen evolution reaction and facilitating a localized CO_(2)/CO concentration at the *** upon this approach,we developed surface-functionalized catalysts with exceptional activity and selectivity for electrocatalytic CO_(2)RR to C_(2+)in a neutral *** CuO nanoparticles coated with hexaethynylbenzene organic molecules(HEB-CuO NPs),a remarkable C_(2+)Faradaic efficiency of nearly 90%was achieved at an unprecedented current density of 300 mA cm^(-2),and a high FE(>80%)was maintained at a wide range of current densities(100-600 mA cm^(-2))in neutral environments using a flow ***,in a membrane electrode assembly(MEA)electrolyzer,86.14%FEC2+was achieved at a partial current density of 387.6 mA cm^(-2)while maintaining continuous operation for over 50 h at a current density of 200 mA cm^(-2).In-situ spectroscopy studies and molecular dynamics simulations reveal that reducing the coverage of coordinated K⋅H2O water increased the probability of intermediate reactants(CO)interacting with the surface,thereby promoting efficient C-C coupling and enhancing the yield of C_(2+)*** advancement offers significant potential for optimizing local micro-environments for sustainable and highly efficient C_(2+)production.
