Efficacy of Intragastric Balloons in the Markers of Metabolic Dysfunction-associated Fatty Liver Disease:Results from Meta-analyses

Background and Aims:Nonalcoholic fatty liver disease,now renamed metabolic dysfunction-associated fatty liver disease(MAFLD),is common in obese *** balloon(IGB),an obesity management tool with low complication risk,might be used in MAFLD treatment but there is still unexplained heterogeneity in results across ***:We conducted a systematic search of 152 citations published up to September ***-analyses,stratified analyses,and meta-regression were performed to evaluate the efficacy of IGB on homeostasis model assessment of insulin resistance(HOMA-IR),alanine aminotransferase(ALT),aspartate aminotransferase(AST),and gamma-glutamyl transpeptidase(GGT),and to identify patients most appropriate for IGB ***:Thirteen observational studies and one randomized controlled trial met the inclusion criteria(624 participants in total).In the overall estimate,IGB therapy significantly improved the serum markers change from baseline to follow-up[HOMA-IR:1.56,95%confidence interval(CI)=1.16–1.95;ALT:11.53 U/L,95%CI=7.10–15.96;AST:6.79 U/L,95%CI=1.69–11.90;GGT:10.54 U/L,95%CI=6.32–14.75].In the stratified analysis,there were trends among participants with advanced age having less change in HOMA-IR(1.07 vs.1.82).The improvement of insulin resistance and liver biochemistries with swallowable IGB therapy was no worse than that with endoscopic *** meta-regression analyses showed that greater HOMA-IR loss was predicted by younger age(p=0.0107).Furthermore,effectiveness on ALT and GGT was predicted by basal ALT(p=0.0004)and GGT(p=0.0026),***:IGB is effective among the serum markers of *** patients had a greater decrease of HOMA-IR after IGB therapy.

Outcomes of CAG Regimen for Refractory Biphenotypic Acute Leukemia Patients

Objective To evaluated the efficiency of low-dose cytosine arabinoside plus aclarubicin with concurrent administration of granulocyte colony-stimulating factor （CAG） regimen for refractory biphenotypic acute leukemia （BAL）. Methods We treated 5 refractory BAL patients by CAG regimen （10 mg.m 2 cytosine arabinoside subcutaneously administrated every 12 hours, day 1-14; 5-7 mg·m^-2 aclarubicin intravenously administrated daily, day 1-8; and concurrently used 200 μg·m^-2·d^-1 granulocyte colony-stimulating factor subcutaneously） from November 2002 to April 2007. The efficacy of the regimen was evaluated by response rate, and the side effects were also measured. Results The complete remission rate was 80%, median duration of absolute neutrophil count〈5.0×10^8/L and platelet count〈2.0×10^10/L was day 13 and day 1, respectively; and the infection rate was low （Ⅲ-Ⅳ infection rate, 20.00%）.

Monte Carlo study on abnormal growth of Goss grains in Fe-3%Si steel induced by second-phase particles

The selective abnormal growth of Goss grains in magnetic sheets of Fe-3%Si （grade Hi-B） induced by second-phase particles （AlN and MnS） was studied using a modified Monte Carlo Ports model. The starting microstructures for the simulations were generated from electron backscatter diffraction （EBSD） orientation imaging maps of recrystallized samples. In the simulation, second-phase particles were assumed to be randomly distributed in the initial microstructures and the Zener drag effect of particles on Goss grain boundaries was assumed to be selectively invalid because of the unique properties of Goss grain boundaries. The simulation results suggest that normal growth of the matrix grains stagnates because of the pinning effect of particles on their boundaries. During the onset of abnormal grain growth, some Goss grains with concave boundaries in the initial microstructure grow fast abnormally and other Goss grains with convex boundaries shrink and eventually disappear.

Optimal Glycemic Control for Patients with Pancreatitis in the Intensive Care Unit: A Retrospective Study Based on a Large Intensive Care Unit Database

Objectives: Stress hyperglycemia is a common complication of pancreatitis. This study aimed to determine optimal blood glucose control goals for patients with pancreatitis. Methods: We conducted a screening of adult patients diagnosed with pancreatitis who were admitted to the ICU using data from the Medical Information Marketplace for Intensive Care IV (MIMIC-IV) database. In total, 574 cases were enrolled in this study and were divided into four groups based on blood glucose levels. We investigated the effects of four groups on the prognosis of patients with pancreatitis. Results: The survival group exhibited lower Glu_mean and Glu_cv values, but there was no significant effect on mortality. Glu_cv and frequency of daily blood glucose monitoring were significantly correlated with length of hospital stay, but had no significant effect on length of ICU stay. Subgroup analysis showed that the mortality rate was the highest when the mean blood glucose was 7.8 - 10.0 mmol/L, and the lowest when Glu_mean was 6.1 - 7.8 mmol/L.

The mid-infrared absorption spectra of geometrically frustrated hydroxyl cobalt halogenides Co2 （OH）3 CI and Co2 （OH）3Br are measured by FTIR spectrometers, and the stretching vibrational modes of hydroxyl groups are found to be 3549 cm-1 and 3524 cm-1 respectively. Through finding their true terminal O II group stretching vibration frequencies, we obtain 107cm-1 and 99cm-1 red shift caused by the corresponding O-H...Cl and 0 H...Br hydrogen bonds. Rarely reported trimeric hydrogen bonds （Co3≡O H）3….C1/Br are pointed out to demonstrate the relative weakness of this kind of hydrogen bond which may have a critical effect on the lattice symmetry and magnetic structures.

Application of side-to-side anastomosis of the lesser curvature of stomach and jejunum in gastric bypass

AIM To evaluate the feasibility of side-to-side anastomosis of the lesser curvature of stomach and jejunum in laparoscopic Roux-en-Y gastric bypass(LRYGB).METHODS Seventy-seven patients received side-to-side anastomosis of the lesser curvature of stomach and jejunum by utilization of linear stapler in LRYGB from April 2012 to July 2015 were retrospectively *** All patients were successfully completed laparoscopic gastric bypass with the side-to-side anastomosis of the lesser curvature of stomach and jejunum. No patient was switched to laparotomy during operation. No early complications including gastrointestinal anastomotic bleeding, fistula, obstruction, deep vein thrombosis, incision infections, intra-abdominal hernia complications were found. One patient complicated with stricture of gastrojejunal anastomosis(1.3%) and six patients complicated with incomplete intestinal obstruction(7.8%). BMI and Hb A1 c determined at 3, 6, 12, 24 mo during follow up period were significantly reduced compared with preoperative baselines respectively. The percentage of patients who maintain HbA 1c(%) < 6.5% without taking antidiabetic drugs reached to 61.0%, 63.6%, 75.0%, and 63.6% respectively. The outcome parameters of concomitant diseases were significantly improved *** Present surgery is a safety and feasibility procedure. It is effective to lighten the body weight of patients and improve type 2 diabetes and related complications.

Food allergy(FA)is one of the global human health problems,affecting about 1 in 12 children and 1 in 10 adults *** have alleviating effects on FA,but the mechanisms have not been fully *** sought to explore the therapeutic potential of probiotics Bifidobacterium longum CECT7894(*** CECT7894)on anaphylaxis in the ovalbumin(OVA)-induced FA model using an integrated multiomics *** results revealed treatment with *** CECT7894 relieved OVA-induced allergic symptoms such as patho-logical changes in small intestine to some ***,probiotics utilization also reduced IgE *** transcriptomic data demonstrated that the expressions of genes associated with sphingolipid metabolism were altered in the group received *** CECT7894 compared to the sensitized group,including Sgpl1,Trpc1 and *** addition,the abundances of Sphingobacterium at the genus level,Sphingobacterium sp.21 and *** at the species level,which might related with sphingolipid metabolism,were significantly altered by *** CECT7894 when compared to the OVA *** the metabolomics analysis,phytosphingosine,C16 sphinganine,sphinganine and sphingosine were identified as significantly changed metabolites and sphingolipid metabolism pathway was significantly enriched by pathway enrichment *** Spearman’s correlation analysis further demonstrated that there were strong correlations between serum biochemical indicators,tran-scripts,gut microbiota and *** on the findings described above,from the perspective of genes,gut flora and metabolites changes,it can be concluded that *** CECT7894 might suppress OVA-induced FA by regulating the sphingolipid metabolism pathway.

Experimental analysis of additional aerodynamic effects caused by wind-driven rain on bridge main girder

To study the additional aerodynamic effect on a bridge girder under the action of wind-driven rain, the rainfall similarity considering raindrop impact and surface water is first given. Then, the dynamic characteristics and the process of vortex and flutter generation of the segment models under different rain intensities and angles of attack are tested by considering several typical main girder sections as examples. The test results indicate that the start and end wind speeds,interval length and number of vortex vibrations remain unchanged when it is raining, rainfall will reduce the windinduced vortex response. When test rain intensity is large, the decrease of amplitude is obvious. However, after considering the rain intensity similarity in this study, all of actual maximum rain intensities after conversion approach the domestic extreme rain intensity of approximately 709 mm/h. It can be observed that rainfall has a limited influence on the dynamic characteristics of the structure and vortex vibration response. When the test rain intensity is 120 mm/h, the critical wind speed of the model flutter increases by 20%-30%. However, after considering the rain intensity similarity ratio, the influence of rainfall on the wind-induced flutter instability of the bridge girder may be ignored.

Behavior recognition and fuel consumption prediction of tractor sowing operations using smartphone

In order to qualitatively recognize the behaviors and investigate the relationship between fuel consumption and machinery driving modes of the tractor in a low-cost approach,this study proposed a method for behavior recognition and fuel consumption prediction of tractor sowing operations using a ***,three driving modes were developed for maize sowing scenarios:manual driving assisted driving and unmanned *** sowing,smartphone software and CAN(Controller Area Network)storage devices collected both positional data and engine operating ***,the tractor trajectory points were divided into kinematic sequences,with six driving cycle indicators built in each series based on the time *** on the semantic information of the kinematic sequences,the three operations of sowing,seeds filling,and turning round were well ***,a model for maize sowing fuel consumption forecast was advanced using the principal component analyses and random forest algorithm,regarding three factors:driving cycles,operating behaviors,and driving *** compared to the traditional K-means algorithm,the results demonstrated that the harmonic mean of the precision and recall(F1 score)of sowing behavior recognition,seeds filling behavior recognition,and turning behavior recognition were enhanced by 2.06%,8.99%,and 21.79%,*** terms of the impacts of driving modes and operating behaviors on fuel consumption,assisted driving mode had the lowest fuel usage for both sowing and turning ***,assisted driving is the most fuel-efficient mode for maize *** the three driving modes,the relative error of the fuel consumption prediction model was 0.11 L/h,with the manual driving mode having the lowest relative error at 0.09 L/*** research method lays the foundation for the optimization of tractor operation behavior,the selection of tractor driving mode,and the fine management of tractor fuel consumption.
