Understanding the processes of immune regulation in patients infected with the severe acute respiratory syndrome coronavirus 2.SARS-CoV-2.is crucial for improving ***,we performed longitudinal whole-transcriptome RNA sequencing on peripheral blood mononuclear cell(PBMC)samples from 18 patients with coronavirus disease 2.19(COVID-19)during their treatment,convalescence,and *** analyzing the regulatory networks of differentially expressed messenger RNAs(mRNAs),microRNAs(miRNAs)and long non-coding RNAs(lncRNAs)between the different clinical stages,we found that humoral immunity and type I interferon response were significantly downregulated,while robust T-cell activation and differentiation at the whole transcriptome level constituted the main events that occurred during recovery from *** formation of this T cell immune response might be driven by the activation of activating protein-1(AP-1)related signaling pathway and was weakly affected by other clinical *** findings uncovered the dynamic pattern of immune responses and indicated the key role of T cell immunity in the creation of immune protection against this disease.

目的　研究“清肝活血方”药物血清对体外培养肝星状细胞增殖及胶原生成的影响。方法　原位分离大鼠肝细胞、肝星状细胞 ,3H TdR掺入法测定细胞增殖能力 ,ELISA法测定I型胶原含量。结果　乙醛可以直接促进肝星状细胞增殖和I型胶原的分泌 ,乙醛损伤肝细胞条件培养液培养下的肝星状细胞出现相同的改变 ,不同剂量的清肝活血方药物血清能产生正向调节作用 ,有效地抑制肝星状细胞增殖和I型胶原分泌。结论　清肝活血方既可以通过拮抗乙醛引起的肝脏脂质过氧化损伤 ,还可以直接阻止乙醛引起的肝星状细胞的活化增殖Ⅰ型胶原分泌。

为观察中药活血软肝汤治疗肝炎后肝硬化的临床疗效及其对纤维化标志物的影响 ,随机分为治疗组和对照组各 5 0例 ,分别采用活血软肝汤和人参鳖甲煎丸进行治疗 ,疗程 6个月。结果 :治疗组的症状体征明显改善 ,肝功能恢复等较对照组好 (P <0 .0 5 ) ,血清HA、LN、PⅢP水平降低 ,较对照组明显 (P <0 .0 1)。提示活血软肝汤对肝炎后肝硬化有较好疗效 ,并能明显降低血清肝纤维化标志物水平。

We propose an extended BCS–Hubbard model and investigate its ground state phase diagram in an external magnetic *** mapping the model onto a model of spinless fermions coupled with conserving Z_(2. variables which are mimicked by pseudospins,the model is shown to be exactly solvable along the symmetric lines for an arbitrary on-site Hubbard interaction on the bipartite *** the zero field limit,the ground states exhibit an antiferromagnetic order of *** the large field limit,on the other hand,the pseudospins are fully polarized *** the increase of the applied field,a first-order phase transition occurs between these kinds of phases when the on-site Coulomb interaction is less than a critical value U(c).Above this critical U_(c),a novel intermediate phase emerges between the fully polarized and antiferromagnetic *** ground states in this phase are macroscopically degenerate,like in a spin ice,and the corresponding entropy scales linearly with the lattice size at zero temperature.

Background Hand, foot, and mouth disease (HFMD) is a common infectious disease in childhood caused by an enterovirus (EV), and which is principally seen in children under 5 years of age. To promote diagnostic awareness and effective treat-ments, to further standardize and strengthen the clinical management and to reduce the mortality of HFMD, the guidelines for diagnosis and treatment have been developed. Methods National Health Commission of china assembled an expert committee for a revision of the guidelines. The com-mittee included 33 members who are specialized in diagnosis and treatment of HFMD. Results Early recognition of severe cases is utmost important in diagnosis and treatment of patients with HFMD. The key to diagnosis and treatment of severe cases lies in the timely and accurate recognition of stages 2.and 3 of HFMD, in order to stop progression to stage 4. Clinicians should particularly pay attention to those EV-A71 cases in children aged less than 3 years, and those with disease duration less than 3 days. The following indicators should alert the clinician of possible deterioration and impending critical disease: (1) persistent hyperthermia;(2. involvement of nervous system;(3) worsening respiratory rate and rhythm;(4) circulatory dysfunction;(5) elevated peripheral WBC count;(6) elevated blood glucose and (7) elevated blood lactic acid. For treatment, most mild cases can be treated as outpatients. Patients should be isolated to avoid cross-infection. Intense treatment modalities should be given for those severe cases. Conclusion The guidelines can provide systematic guidance on the diagnosis and management of HFMD.

Effect of general anesthesia combined with epidural anesthesia on stress response, inflammatory reaction and coagulation in patients with colon cancer

Objective: To investigate the effects of general anesthesia combined with epidural anesthesia on stress response, inflammation level and coagulation function in patients undergoing laparoscopic colon cancer resection during intraoperative and postoperative. Methods: A total of 98 patients undergoing colon cancer radical surgery in our hospital from August 2.15 to August 2.17 were randomly divided into control group and observation group with 49 cases in each group. In the control group, systemic intravenous anesthesia was used;and in the observation group, a systemic intravenous anesthesia combined epidural anesthesia was used. Serum of venous blood was collected from patients before anaesthesia, 30 min after anesthesia, and two hours after operation to detect stress response indexes, such as adreno-cortico-tropic-hormone (ACTH), adrenaline (AD), norepinephrine (NE) and cortisol (Cor), and inflammatory response indexes, such as tumor necrosis factor-α (TNF-α), procalcitonin (PCT) and C-reaction protein (CRP). Venous blood was collected at the same time points for coagulation testing. Functional indicators such as prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FBG) were detected. Results: There was no significant difference in the level of each index between the two groups before anesthesia. At 30 minutes after anesthesia and at 2.hours after operation, the stress response indicators and inflammatory response indicators in the observation group were lower than those in the control group at the same time points;apart of the blood coagulation function indexes (PT, APTT and TT) in the observation group were higher than the corresponding indicators of the control group at the same time points, but the FBG content in the observation group was lower than that of the control group at the same time points, and these differences were significant. Background This document represents the first evidence-based guidelines to describe best practices in nutrition therapy in critically ill children (>1 month and 2.or 3 days in a Pediatric Intensive Care Unit admitting medical patients domain. Methods A total of 2.,673 articles were scanned for relevance. After careful review, 88 studies appeared to answer the pre-identified questions for the guidelines. We used the grading of recommendations, assessment, development and evaluation criteria to adjust the evidence grade based on the quality of design and execution of each study. Results The guidelines emphasise the importance of nutritional assessment, particularly the detection of malnourished patients. Indirect calorimetry (IC) is recommended to estimate energy expenditure and there is a creative value in energy expenditure, 50 kcal/kg/day for children aged 1–8 years during acute phase if IC is unfeasible. Enteral nutrition (EN) and early enteral nutrition remain the preferred routes for nutrient delivery. A minimum protein intake of 1.5 g/kg/day is sug-gested for this patient population. The role of supplemental parenteral nutrition (PN) has been highlighted in patients with low nutritional risk, and a delayed approach appears to be beneficial in this group of patients. Immune-enhancing cannot be currently recommended neither in EN nor PN. Conclusion Overall, the pediatric critically ill population is heterogeneous, and an individualized nutrition support with the aim of improving clinical outcomes is necessary and important.

Aortico-cameral tunnel is a rare congenital cardiac .anomaly, which arises from the right coronary sinus and enters the left ventricle, occasionally the right ventricle and right atrium. However, more than one tunnel are seldom seen simultaneously in one patient. Here we present a case of multiple aortico-cameral tunnels associated with the bicuspid aortic valve in a 63-year-old man.

Objective: Dermoscopy is a useful technique for improving the diagnostic accuracy of various types of skin disorders. In china, dermoscopy has been widely accepted, and domestic researchers have made tremendous progress in the field of dermoscopy. The main purpose of this review is to summarize the current status of dermoscopy in china and identify its future directions. Data sources: Articles included in this review were obtained by searching the following databases: Wanfang, china National Knowledge Infrastructure, PubMed, and the Web of Science. We focused on research published before 2.19 with keywords including dermoscopy, dermoscopic, dermoscope and trichoscopy. Study selection: A total of 50 studies were selected. Of these studies, 2. studies were in chinese and 30 in English, research samples of all the studies were collected from chinese populations. Results: Since 2.00, more than 380 articles about dermoscopy have been published in domestic or foreign journals. Dermoscopy can improve the diagnostic accuracy of neoplastic diseases, evaluating the therapeutic effect of treatment, and determining the treatment endpoint, and it can also assist in the differential diagnosis of inflammatory diseases and in the assessment of the severity of the disease. In addition, researches about the applications of dermoscopy during surgical treatment have been published. Training courses aiming to improve the diagnostic ability of dermatologists, either face-to-face or online, have been offered. The chinese Skin Image Database, launched in 2.17 as a work platform for dermatologists, has promoted the development of dermoscopy in china. Computer-aided diagnostic systems based on the chinese population are ready for use. In the future, cooperation, resource sharing, talent development, image management, and computer-aided diagnosis will be important directions for the development of dermoscopy in china.
